ABSTRACT

Objective: To investigate the expression of MSI1 and HER2 in mammary Paget's disease, and the correlation between the expression levels of MSI1 and HER2 and the clinicopathologic characteristics and prognosis of the patients. Methods: Clinical data and paraffin-embedded specimens of 34 pairs of mammary Paget's disease and underlying breast cancer were collected at the Department of Pathology, Affiliated Lianyungang Oriental Hospital of Xuzhou Medical University from March 2011 to December 2019. Immunohistochemistry was used to detect the expression of MSI1 and HER2 in mammary Paget's disease and the accompanying breast cancer, and to analyze the correlation between the expression levels of MSI1 and HER2 and their clinicopathologic features, as well as their influence on prognosis. Results: In mammary Paget's disease, the positive rate of MSI1 was 91.2% (31/34) and the positive rate of HER2 was 88.2% (30/34); the expression of MSI1 and HER2 was positively correlated (P=0.001, r=0.530). The expression of MSI1 was positively correlated with menopausal status (r=0.372, P=0.030) and lymph node metastasis (r=0.450, P=0.008). HER2 expression was positively correlated with menopausal status (r=0.436, P=0.010), and negatively correlated with ER expression (r=-0.365, P=0.034). The co-expression of MSI1 and HER2 was positively correlated with age (r=0.347, P=0.044) and menopausal status (r=0.496, P=0.003), and negatively correlated with ER expression (r=-0.461, P=0.006). Conclusions: MSI1 and HER2 are highly expressed in mammary Paget's disease and their expression levels are positively correlated. The correlation analysis between clinicopathological features and prognosis suggests that both of them may be involved in the occurrence and development of mammary Paget's disease and are potential therapeutic targets for mammary Paget's disease.

ABSTRACT

Objective: To investigate the clinicopathological factors and prognostic status of young Mammary Paget's disease (MPD) patients with invasive ductal carcinoma (IDC). Methods: In this study, we defined the age at diagnosis below 40 years old as young patients, and retrospectively analyzed data from 123 MPD-IDC patients who were admitted at the Cancer Hospital Chinese Academy of Medical Sciences from June 2002 to February 2019. Patients were divided into the young group (≤40 years old, 15 cases) and the old group (>40 years old, 108 cases) according to the age of onset, and the clinicopathological characteristics and prognosis of the two groups were compared. Cox regression model analysis was used to analyze the prognosis influencing factors. Results: The proportions of patients in the young group with non-menopausal, axillary lymph node metastasis, and Ki-67 index ≥15% were 93.3% (14/15), 73.3% (11/15), and 86.7% (13/15), respectively, which were higher than those in the old group [45.4% (49/108), 39.8%(43/108), and 60.2% (65/108), respectively] , with statistically significant differences (P<0.05). At an average follow-up of 63.2 months, patients in the young group had a significantly shorter disease-free survival (DFS) compared with that of the old group (P=0.012), while the difference in overall survival (OS) between the two groups was not statistically significant (P=0.161). Multifactorial Cox regression analysis showed that axillary lymph node status was an independent influencing factor on OS (HR=3.339, 95% CI: 1.121-9.943) in patients with MPD-IDC, while age was not. Conclusion: Compared with the old group, young patients with MPD-IDC have a higher incidence of axillary lymph node metastasis, high Ki-67 expression, and a shorter DFS, but age is not an independent influencing factor on DFS or OS in patients with MPD-IDC.

Resumen La enfermedad de Paget mamaria es una neoplasia infrecuente de la mama, muchas veces de difícil diagnóstico y mal pronóstico. El objetivo es revisar los aspectos fundamentales clínicos, diagnósticos, así como los avances más recientes en su tratamiento. Se realizó una búsqueda bibliográfica a través de PUBMED, GOOGLE SCHOLAR y UPTODATE, de los últimos 20 años. Fueron seleccionados e incluidos en la revisión 38 artículos. La enfermedad de Paget mamaria es una neoplasia maligna infrecuente del complejo areola pezón de características inflamatorias al examen físico, asociado en un 85% de los casos a un cáncer de mama subyacente, en el 50% de los casos no identificable al examen físico, de difícil diagnóstico, cuya sospecha clínica se confirma con imágenes, biopsia e inmunohistoquímica. El tratamiento es mastectomía versus cirugía conservadora y radioterapia. Un mal enfoque clínico, retrasa el diagnóstico y empeora la sobrevida. MÉD.UIS.2021;34(1): 45-53.


Abstract Paget's disease of the breast is an uncommon neoplasm of the breast, many times difficult to diagnose and of poor prognosis. The objective is to review the fundamental clinical and diagnostic aspects, as well as the most recent advances in its treatment. A bibliographic search of the last 20 years was carried out through PUBMED, GOOGLE SCHOLAR and UPTODATE. 38 articles were selected and included in the review. Mammary paget disease is a rare malignant neoplasm of the nipple areola complex with inflammatory features on physical examination, associated in 85% of cases with underlying breast cancer, in 50% of cases not recognizable on physical exam, difficult to diagnose, whose clinical suspicion is confirmed by imaging, biopsy and immunohistochemistry. Treatment is mastectomy versus conservative surgery and radiotherapy. A poor clinical approach delays the diagnosis and worsens survival rate. MÉD.UIS.2021;34(1): 45-53.

The authors present an atypical, exuberant case that had evolved over eight years, emphasizing the importance of early diagnosis.

ABSTRACT

INTRODUCCIÓN: La enfermedad de Paget del pezón es infrecuente y representa del 1 al 3 % de las neoplasias de mama. Está asociada con un carcinoma invasivo o in situ en el 82 al 94 % de los casos. Se presenta entre los 55 a 64 años de edad. CASO CLÍNICO: Se presenta el caso de una mujer de 58 años con zona de descamación de 1 cm2 indolora en pezón derecho y tumor de 4 cm2 en mama izquierda, ecografía y mamografía BI-RADS III. Biopsia incisional de pezón derecho que reportó enfermedad de Paget, con ampliación de márgenes. Se completó estudios de extensión con resonancia magnética que reportó lesión retro-areolar sospechosa en mama derecha y lesión nodular maligna en mama izquierda. EVOLUCIÓN: Se realizó cuadrantectomía central más neo-pezón en mama derecha, y en mama izquierda cuadrantectomía superoexterna con vaciamiento ganglionar axilar bilateral, el reporte de patología fue mama derecha negativa para malignidad, mama izquierda hiperplasia intraductal atípica con focos de carcinoma intraductal. Se administró 25 sesiones radioterapia, tras un año de seguimiento se mantiene sin recurrencia. CONCLUSIÓN: La enfermedad de Paget es poco frecuente, pero debe considerarse como diagnóstico diferencial en pacientes sobre los 55 años, su tratamiento estandarizado es quirúrgico, la adyuvancia con radioterapia aún es motivo de discusión. El autoexamen de mama, el acudir a controles periódicos a partir de los 40 años además de realizar una ecografía y mamografía nos lleva a diagnósticos tempranos con un elevado porcentaje de cirugías más conservadoras.(au)


BACKGROUND: Paget's disease of the nipple is uncommon and represents 1 to 3% of breast malignancies. It is associated with invasive or in situ carcinoma in 82 to 94% of cases. It occurs between 55 to 64 years old. CASE REPORT: We present the case of a 58 year old woman with a painless 1cm2 scaling skin lesion in the right nipple and a 4cm2 tumor in the left breast, ultrasound and mammograghy classified as BIRADS III. Incisional biopsy of the right nipple that reported Paget's¬ disease, with extension of surgical margins of resection. MRI extension studies were made; they reported suspicious retroaereolar lesion in the right breast and malignant nodular lesion in the left breast. EVOLUTION: We performed central quadrantectomy with nipple reconstruction in the right breast, and superoexternal quadrantectomy in the left breast, with bilateral axillary lymph node dissection, the pathology report was negative for malignancy in the right breast, in the left breast atypical intraductal hiperplasia with foci of intraductal carcinoma. The patient received 25 radiotherapy sessions. After one year of follow-up there is no recurrence. CONCLUSIONS: Paget's disease is rare, but should be considered as a differential diagnosis in patients over 55 years, its standardized treatment is surgery, adjuvant treatment with radiotherapy is still a matter of discussion. Breast self-exam, regular checkups in people over 40 years old, ultrasound and mammogram lead to early diagnosis with a higher percentage of conservative surgeries.(au)

RESUMEN Objetivo: presentar un caso de enfermedad de Paget (EP) de la mama, sin carcinoma ductal asociado, y realizar una revisión de literatura respecto a su diagnóstico y tratamiento. Materiales y métodos: mujer de 59 años, con lesión pruriginosa, eritematosa y ulcerada crónica, en pezón izquierdo, de 2 años de evolución, quien recibió inicialmente manejo dermatológico con corticoides tópicos sin mejoría clínica, que acude finalmente a una institución de atención primaria en Tunja (Colombia) para su diagnóstico, el cual se realizó por biopsia mediante inmunohistoquímica y posterior manejo por cuadrantectomía central con patología posquirúrgica negativa para malignidad, sin recurrencia al año de seguimiento. Se realizó una búsqueda en las bases de datos Medline vía PubMed, SciELO, ProQuest, ScienceDirect y Google Académico con los términos "Paget´s Disease", "Breast Neoplasms", "Mammography" "Ultrasonography Mammary", "Biopsy" y "Mastectomy", rastreando artículos de revisión bibliográfica, editoriales, reportes y series de casos clínicos en inglés y español, sin límite de tiempo. Resultados: se incluyeron 11 publicaciones correspondientes a 5 reportes de caso, 3 series de casos, 1 revisión de la literatura a partir de un caso clínico, y 2 cartas al editor que describieron casos relacionados con enfermedad de Paget del pezón sin carcinoma asociado. El diagnóstico se fundamentó en la sospecha clínica de la enfermedad con la posterior realización de biopsia de la lesión; la inmunohistoquímica fue útil en el diagnóstico diferencial. El tratamiento varió desde mastectomía hasta resección de complejo areola-pezón. La radioterapia también ha sido utilizada. Conclusiones: el diagnóstico de la enfermedad de Paget sin carcinoma ductal asociado se fundamenta en una sospecha clínica temprana y en el estudio histopatológico del complejo areola-pezón. No hay consenso sobre el manejo quirúrgico de esta lesión. Se requieren estudios de seguimiento de este subgrupo de pacientes tratado con radioterapia o cirugía conservadora.


ABSTRACT Objective: To present one case of Paget's disease (PD) of the breast without associated ductal carcinoma, and to conduct a review of the literature on its diagnosis and treatment. Materials and methods: A 59-year-old woman with a two-year history of a chronic erythematous, ulcerated, itchy lesion in the left nipple, initially treated dermatologically with topical steroids with no improvement, finally visits a primary healthcare institution in the city of Tunja, Colombia for diagnosis. A biopsy and histochemical testing were performed to make the diagnosis and the lesion was then treated with central quadrantectomy. The post-operative pathology testing was negative for malignancy, without recurrence at one-year follow-up. A search was conducted in the PubMed, SciELO, ProQuest, ScienceDirect and Google Scholar databases using the terms "Paget´s Disease," "Breast Neoplasms," "Mammography," "Ultrasonography," Mammary," "Biopsy," and "Mastectomy," tracking bibliographic review articles, editorials, reports and clinical case series in English and Spanish, without time limitation. Results: Overall, 11 publications were included consisting of 5 case reports, 3 case series, 1 review of the literature based on a clinical case, and 2 letters to the editor describing cases related to Paget's disease of the nipple with no associated carcinoma. The diagnosis was based on the clinical suspicion of the disease and a biopsy of the lesion. Histochemistry was useful for the differential diagnosis: treatment options ranged from mastectomy to nipple-areolar complex resection. Radiotherapy has also been used. Conclusions: The diagnosis of Paget's disease with no associated ductal carcinoma is based on early clinical suspicion and histopathology of the nipple-areolar complex. There is no consensus regarding surgical management of this lesion. Follow-up studies of this subgroup of patients treated with radiotherapy or conservative surgery are required.

ABSTRACT

Mammary Paget's disease is clinically defined as skin inflammation of the nipple area and is an adenocarcinoma of the epidermis of the nipple. The pathogenesis of mammary Paget's disease is relatively unknown; nonetheless, there are two popular theories that support the underlying carcinoma and de novo carcinogenesis. For the attending medical practitioner, mammary Paget's disease poses a diagnostic and therapeutic dilemma, especially in the absence of a clinically palpable breast mass. We report a rare case of a 48-year-old Malay woman who presented at Hospital Universiti Sains Malaysia, Kelantan, Malaysia with the symptom of skin erosion on the left nipple and unresponsiveness to multiple topical treatments. A full evaluation and assessment of the patient were conducted, and mammary Paget's disease was diagnosed.

ABSTRACT

Mammary Paget's disease (MPD) is usually accompanied by underlying breast malignancy; however, a few cases have been reported as only skin lesions without any evidence of malignancy of the breast on imaging tests and microscopic examination of surgical specimen. Here, we describe a 47-year-old woman who visited our hospital who had an eczematous lesion on right nipple and areola for over 10 years. The lesion was diagnosed as Paget's disease by punch biopsy; however, imaging studies demonstrated no breast malignancy or lymph node metastasis. The patient underwent surgery of on the nipple and areola including underlying breast tissue. No underlying malignancy was found upon microscopic examination, except for Paget's disease. Immunohistochemical stains revealed that the tumor cells were positive for cytokeratin 7, and negativity for p63, cytokeratin 5/6, estrogen receptor, progesterone receptor, and human epidermal growth factor receptor 2. We report a case of MPD without underlying malignancy. To the best of our knowledge, this is the third case reported in Korea.

Se describe el caso clínico de una paciente de 63 años de edad, quien acudió a la consulta de Oncología del Hospital Gubernamental de Mbabane, en Suazilandia, por presentar una lesión ulcerada en el pezón de la mama izquierda desde hacía 1 año. Luego de realizarle una biopsia por escisión, que reveló la presencia de la enfermedad de Paget, clasificada en el estadio 0 según los resultados de los exámenes complementarios, se decidió remitirla al Servicio de Cirugía donde le practicaron una mastectomía simple. La paciente mostró buena evolución clínica y se mantenía estable hasta la última consulta en que fuera asistida


The case report of a 63 years patient who went to the Oncology Service of the Government Hospital of Mbabane, in Swaziland is described, due to an ulcerated lesion in the nipple of the left breast for 1 year. After carrying out an excision biopsy that revealed the presence of the Paget´s disease, classified in the stage 0 according to the results of the complementary tests, she was referred to the Surgery Service where a simple mastectomy was carried out. The patient showed good clinical course and she remained stable until the last appointment when she was assisted

ABSTRACT

Paget's disease, described by Sir James Paget in 1874, is classified as mammary and extramammary. The mammary type is rare and often associated with intraductal cancer (93-100% of cases). It is more prevalent in postmenopausal women and it appears as an eczematoid, erythematous, moist or crusted lesion, with or without fine scaling, infiltration and inversion of the nipple. It must be distinguished from erosive adenomatosis of the nipple, cutaneous extension of breast carcinoma, psoriasis, atopic dermatitis, contact dermatitis, chronic eczema, lactiferous ducts ectasia, Bowen's disease, basal cell carcinoma, melanoma and intraductal papilloma. Diagnosis is histological and prognosis and treatment depend on the type of underlying breast cancer. Extramammary Paget's disease is considered an adenocarcinoma originating from the skin or skin appendages in areas with apocrine glands. The primary location is the vulvar area, followed by the perianal region, scrotum, penis and axillae. It starts as an erythematous plaque of indolent growth, with well-defined edges, fine scaling, excoriations, exulcerations and lichenification. In most cases it is not associated with cancer, although there are publications linking it to tumors of the vulva, vagina, cervix and corpus uteri, bladder, ovary, gallbladder, liver, breast, colon and rectum. Differential diagnoses are candidiasis, psoriasis and chronic lichen simplex. Histopathology confirms the diagnosis. Before treatment begins, associated malignancies should be investigated. Surgical excision and micrographic surgery are the best treatment options, although recurrences are frequent.

Pigmented mammary Paget disease is a very rare clinicopathologic variant of mammary Paget disease. Diagnosis is often difficult because its clinical and histological features are very similar to those of malignant melanoma. Herein, we report a case of pigmented mammary Paget disease misdiagnosed as malignant melanoma.

La enfermedad de Paget de la mama es una manifestación cutánea infrecuente de cáncer de mama que tiene mejor pronóstico cuando se detecta de manera temprana. Se presenta el caso de una mujer de 56 años, que asistió a consulta en cuidado primario en la ciudad de Cali para la evaluación del resultado de una citología cervicouterina. Se le realizó examen mamario preventivo y se encontró una lesión en areola-pezón de características eritematodescamativas de dos años de evolución. Los estudios anatomopatológicos y de inmunotipificación confirmaron enfermedad de Paget. El reporte histológico de la pieza quirúrgica extraída durante la cuadrantectomía con resección del complejo areola-pezón, diagnosticó un carcinoma intraductal subyacente. La utilización del método clínico en cuidado primario, en una paciente sin lesión palpable y con mamografías negativas para malignidad, fue la clave del éxito en la detección temprana y recuperación de la paciente que ha evolucionado de manera favorable. Paget's disease of the breast is a rare cutaneous manifestation of breast cancer which has a better prognosis when detected early. We report the case of a 56 year-old woman who attended consultation in primary care in the city of Cali to evaluate the result of uterine cervical cytology. She underwent preventive breast exam and a descamative erythematous nipple-areola lesion of two years of evolution was found. Pathological and immunotyping studies confirmed Paget's disease. The histological report of the surgical specimen removed during lumpectomy with resection of the nipple-areola complex, diagnosed an underlying intraductal carcinoma. The use of the clinical method in primary care in a patient without palpable lesion and malignancy negative mammograms, was the key to success in early detection and recovery of the patient who has performed well. Mammary Paget's disease is a rare intraepithelial adenocarcinoma, located on the nipple/areola complex, highly associated with breast cancer. Although the international literature emphasizes the dermatoscopic pattern of mammary Paget's disease pigmented variant, the authors describe the dermoscopic findings of classical Paget's disease and demonstrate the presence of chrysalis-like structures, criteria recently described in the literature and not yet reported in Paget's disease. .


Doença de Paget mamária é considerada um adenocarcinoma intra-epitelial raro, localizado no complexo mamilo-aréola,com alta associação ao câncer de mama. Apesar da literatura mundial realçar o padrão dermatoscópico da doença de Paget mamária variante pigmentada os autores descrevem os achados dermatoscópicos da doença de Paget clássica realçando a presença das estruturas crisálida-símiles,critério recentemente descrito na literatura mundial e ainda não relatado na Doença de Paget. .

Los carcinomas in situ de la mama son el primer estado de cáncer de mama morfológicamente reconocible y pueden evolucionar a un carcinoma de mama invasivo, aunque se desconoce la tasa precisa de progresión. Debido a que el carcinoma ductal in situ es un predictor de carcinoma ductal invasivo en el futuro, es importante la participación de la paciente en la toma de decisiones sobre el tratamiento; actualmente, existen indicaciones precisas para realizar tratamiento conservador o radical, pero cada caso se debe analizar en forma independiente. Por su parte, el carcinoma lobulillar in situ es solo un marcador de riesgo de carcinoma invasivo, muchas de las pacientes no requieren tratamiento y se pueden dejar bajo observación. En este módulo se presentan los avances sobre el carcinoma in situ de mama en sus dos variedades, ductal y lobulillar, en lo relacionado con las manifestaciones clínicas, las alteraciones genéticas, el diagnóstico, el pronóstico, el tratamiento y el seguimiento. De igual forma, se hará una actualización de la enfermedad de Paget.


Carcinoma in situ of the breast is the first morphologically recognizable stage of breast cancer; it can progress to invasive disease; however, the precise rate of progression to invasive cancer is yet unknown. As ductal carcinoma in situ is a predictor of invasive ductal carcinoma, the decision-making process for treatment should be achieved with the patient. Currently, there are particular indications on which cases require conservative treatment and which require a radical treatment; nevertheless, each case must be considered independently. Regarding lobular carcinoma in situ, it is only considered a risk factor to develop invasive cancer, many of the patients do not need treatment and a close follow-up is suggested. In this review article, an update in signs and symptoms of ductal carcinoma in situ and lobular carcinoma in situ is included, as well as an update of their genetic abnormalities, diagnosis, prognosis, treatment and follow-up. In addition, an update in Paget disease is included.

ABSTRACT

Erosive adenomatosis of the nipple is a rare, benign neoplasm, currently thought to originate from the terminal lactiferous ducts and subareolar breast tissue. Clinically, erosive adenomatosis of the nipple is often indistinguishable from that of mammary Paget's disease, and it could be misdiagnosed as intraductal carcinoma as it has a similar histological finding. We present the case of a 36-year-old woman who had a unilateral erythematous induration of the nipple that contained a palpable, firm nodule. It had first been noticed 2 years earlier. The surface was occasionally crusted, and there was a bloody discharge. Histopathological examination showed irregular, dilated tubular structures, which were lined by a peripheral layer of cuboidal cells and a luminal layer of the columnar cells that demonstrated decapitation secretion at their luminal border.

Objectifs : La maladie de Paget mammaire est une manifestation rare du cancer du sein, caractérisée par une infiltration cancéreuse du mamelon se manifestant par une éruption eczématiforme. L'objectif de cette étude était de décrire les aspects épidémiologiques, diagnostiques et thérapeutiques de cette pathologie à l'Institut du Cancer de Dakar et d'en évaluer la survie.Matériel et Méthode : Il s'agissait d'une étude rétrospective de 7 maladies de Paget mammaire colligés à l'Institut du Cancer de Dakar de Janvier 2000 à Décembre 2008. Nous avons inclus les patientes qui présentaient une lésion évocatrice de Paget mammaire confirmé par un examen anatomo-pathologique. Nous avons réalisé une étude descriptive des paramètres épidémiologiques, diagnostiques et thérapeutiques et évalué le suivi de ces malades.Résultats : La maladie de Paget mammaire représentait 0,7% des cancers mammaires à l'Institut du Cancer de Dakar. L'âge moyen des malades était de 55 ans. Toutes les patientes étaient de sexe féminin, multipares dont trois étaient ménopausées. Le délai moyen de consultation était de 60 mois. Le motif de consultation était un eczéma mammaire dans 4 cas et une masse mammaire dans 2 cas. Au terme du bilan d'extension 4 malades étaient classés stade 0, 2 au stade IIIB et 1 au stade IV. Six patientes ont bénéficié d'une mastectomie associée à une chimiothérapie et une radiothérapie adjuvantes. Le suivi a montré que 6 patientes étaient vivantes sans récidive ni métastases. Conclusion : La maladie de Paget mammaire est une manifestation rare du cancer du sein. La mastectomie est préconisée alors que le traitement conservateur n'est indiqué qu'en l'absence de masse mammaire sous-jacente
