ABSTRACT

Atualmente, o Gothenburg Trismus Questionnaire (GTQ) é o único questionário de qualidade de vida específico sobre trismo. A afecção, definida como restrição à abertura da boca, gera prejuízo a atividades habituais como comer, engolir, falar e fazer a higiene oral, trazendo grande desconforto aos pacientes. A tradução de questionários de qualidade de vida desempenha um importante papel no conhecimento da saúde das populações nos diferentes países. O objetivo do presente estudo é apresentar a validação do GTQ para a língua portuguesa, a fim de permitir sua aplicação efetiva nas populações de idioma português. O GTQ foi validado com sucesso para a língua portuguesa conforme os seguintes passos: Tradução, Retradução (back translation), Adaptação cultural e Revalidação.


Currently, the Gothenburg Trismus Questionnaire (GTQ) is the only quality-of-life questionnaire specific for the assessment of trismus. The disease, characterized by limited mouth opening, impairs usual activities such as eating, swallowing, talking, and performing oral hygiene, causing great discomfort to patients. Translation of quality-of-life questionnaires plays an important role in promoting health awareness in the populations of different countries. This study aimed to validate the Portuguese version of the GTQ to allow its effective application in Portuguesespeaking populations. The Portuguese version of the GTQ was successfully validated through the following steps: translation, back translation, cultural adaptation, and revalidation.

ABSTRACT

Fusion of the zygomatic bone to the coronoid process of the mandible is a rare phenomenon reported in the literature and commonly called as zygomaticocoronoid ankylosis. It can be sequel of trauma or infection in the midfacial region, mimicking a wide range of problems including the common temporomandibular joint ankylosis and dysfunction. Maxillofacial trauma involving the displaced fracture of zygoma can obstruct the movement of coronoid and if not treated can cause ankylosis between both bones. It is very diffi cult to identify zygomaticocoronoid ankylosis on conventional radiographs and requires the through clinical and advanced radiological evaluation like cone-beam computed tomographic (CBCT) to diagnose it. CBCT can be a great help to identify the size and extension of ankylotic mass and decide the approach to remove it. Zygomaticocoronoid ankylosis can be approached intraorally by Keen’s incision, and extraorally through a hemicoronal approach we have approached intraorally. Here, we present surgical management of post-traumatic zygomaticocoronoid ankylosis in 42-year-old male patient who had trismus for 18 years.

ABSTRACT

Introdução: O câncer de cabeça e pescoço acomete diversas estruturas do trato aerodigestivo superior e seu tratamento pode ser multimodal ou exclusivo envolvendo cirurgia (CR), radioterapia (RDT) e quimioterapia (QT). O câncer assim como seu tratamento pode levar a diversas alterações funcionais, entre elas a limitação na abertura da boca, o trismo. Esta complicação geralmente afeta hábitos de vida diária, como alimentação e higiene oral. Assim, pode haver significativo impacto na qualidade de vida. Objetivos: Avaliar pacientes com câncer de cabeça e pescoço submetidos a tratamento cirúrgico e/ou radioquimioterápico quanto à presença de trismo, identificar a prevalência de trismo e características comuns para o seu desenvolvimento nestes pacientes, além do seu impacto na qualidade de vida. Material e Método: Estudo descritivo que avaliou pacientes do A.C. Camargo Cancer Center, tratados por câncer de cabeça e pescoço. A avaliação foi composta de anamnese, medida de máxima abertura vertical (MAV) bucal com um paquímetro, avaliação da dor pela Escala Visual Analógica (EVA) e avaliação da qualidade de vida pelo questionário de qualidade de vida da Universidade de Washington. Foram incluídos no estudo 313 participantes entre 19 e 89 anos, de ambos os sexos, diagnosticados e tratados por câncer de boca, orofaringe, seios paranasais ou nasofaringe. Foram excluídos aqueles que apresentavam recidivas ou comorbidades não controladas. Resultados: A prevalência do trismo na amostra estudada foi de 30%. O maior número de indivíduos com trismo esteve presente nos grupos que apresentavam tumores em estádio clínico mais avançado (T3, 37%; T4, 55%) que realizaram tratamento curativo combinado (CR+RDT+QT, 43%; CR+RDT, 39%; RDT+QT, 39%), e que necessitaram de sonda de alimentação enteral por mais tempo (Com trismo, Média = 71 dias; Sem trismo, Média = 44 dias). Introduction: Head and neck cancer (HNC) affects several structures the upper aerodigestive tract and its treatment can be uni or multimodal. It may involve surgery (S), radiotherapy (RT) and chemotherapy (CT). Cancer and its treatment can lead to several functional changes, including limitation of mouth opening, diagnosed as trismus. This complication usually affects daily living habits, such as food intake and oral hygiene. Thus, there may be a significant impact on quality of life. Purpose: To evaluate patients with HNC who underwent surgery and / or chemoradiotherapy for the presence of trismus, indentify characteristics associated with this complication and its impact on quality of life. Materials and Methods: This is a descriptive study assessing trismus in HNC patients treated at A. C. Camargo Cancer Center. The evaluations consisted of an interview, measurement of mouth opening with a caliper, pain assessment by visual analogue scale (VAS) and quality of life evaluation with the University of Washington Quality of Life Questionnaire. The study included 313 participants aged 19-89 years, of both genders, diagnosed and treated for cancer of the mouth, oropharynx, paranasal sinuses or nasopharynx. Patients who had relapses or uncontrolled comorbidities were not considered eligible for the study. Results: The prevalence of trismus in the sample was 30%. Trismus was more frequent in the groups that had more advanced clinical stage (37%, T3; 55%, T4), who underwent combined curative treatment (S + RT + CT, 43%; S + RT, 39%; RT + CT, 39%), and those who required enteral feeding tube for longer periods (with trismus, Mean = 71 days; No trismus, Mean = 44 days). These data were confirmed by multivariate analysis. No relationship was found between the presence of trismus and age (p = 0.26) or gender (p = 0.13), tumor location (p = 0.139), total dose (p = 0.92) and modality of RT (p = 0.078). ABSTRACT

Objetivo: Avaliar por meio da Tomografia Computadorizada singleslice, alterações nas estruturas do espaço mastigador, por disseminação loco-regional das neoplasias malignas originárias da loja tonsilar, trígono retromolar, seio maxilar e nasofaringe; correlacionar a presença de trismo com os achados tomográficos e dimensões do tumor. Material e métodos: foram selecionados prontuários de pacientes portadores de tumores malignos envolvendo as 4 regiões a serem estudadas, junto aos arquivos do Hospital Heliópolis. Foram excluídos os pacientes submetidos a terapêutica antineoplásica prévia ou com doenças inflamatórias ou infeciosas locais. Os prontuários foram revisados por um único examinador, onde foram coletadas informações relativas a idade, gênero, localização do tumor primário, tempo de evolução da doença, estadiamento do tumor e presença ou ausência de trismo. Após a administração do meio de contraste iodado, o protocolo de aquisição de imagens tomográficas foi de cortes axiais de 3mm de espessura, aquisições coronais foram feitas quando necessário. A análise das imagens foi feita por dois examinadores médicos, residentes do 3° de Radiologia médica, em momentos distintos, sem o conhecimento prévio das informações clínicas. Na avaliação do EM, os achados foram transcritos para uma planilha específica, considerando a presença ou ausência dos seguintes itens: simetria com o EM contralateral, obliteração do plano gorduroso, do trígono retromolar e do espaço faríngeo, edema e/ou atrofia dos músculos pterigoideos medial e lateral, destruição do ramo da mandíbula. Resultados:


O trismo esteve presente em 10 pacientes estudados. Na associação entre dimensão do tumor e trismo, observamos que 90% dos pacientes apresentam tamanho T4, enquanto essa porcentagem é de 43% nos pacientes sem trismo. Analisando-se em termos de razão de chances, verificou-se que pacientes com tumores T4 apresentam uma chance de trismo de 11,6 vezes maior que as demais dimensões (T0 a T3). As neoplasias da loja tonsilar e trígono retromolar perfizeram 95% das neoplasias avaliadas. Foram encontrados apenas 3 casos de neoplasia da nasofaringe, e em nenhum deles observamos a presença de trismo, e não foi encontrado nenhum caso de neoplasia de seio maxilar. Relacionando os achados tomográficos com o grupo de pacientes com trismo, observamos que 60% dos pacientes com trismo apresentaram edema e/ou atrofia dos músculos pterigoideos na TC, enquanto que essa porcentagem foi de 21,8% nos pacientes sem trismo. Em relação às dimensões do tumor e sua relação com os achados das imagens observamos associação apenas entre tumores T4 e detecção de edema e/ou atrofia dos músculos pterigoideos, neste item pacientes com edema e/ou atrofia dos músculos pterigoideos apresentam 5,4 vezes ais chance de apresentarem trismo. Conclusões: Observamos na TC as seguintes alterações no EM: obliteração do plano gorduroso em 69,2 % dos pacientes, assimetria em 27,7%, edema/atrofia em 26,2 % dos pacientes e destruição óssea em 20%. Os graus de concordância inter examinadores foram variáveis. Encontramos associação apenas entre pacientes com trismo e edema/atrofia dos músculos pterigoideos. Neste mesmo item foi observado associação com tumores de dimensão T4. Não encontramos associação do trismo com os demais itens avaliados na TC.


Objective: Evaluate by computed tomography (CT) singleslice changes in the structures of the masticator space, for loco-regional dissemination of malignancies originating in the tonsil, retromolar area, maxillary sinus and nasopharynx to correlate the CT findings with trismus and the tumor dimensions. Material and Methods: The medical records of patients with malignant tumors involving the four regions to be studied were selected from the archives of the Heliopolis Hospital. Patients submited previously to antineoplastic therapy or with local inflammatory or infectious diseases were excluded. The medical records were reviewed by a single examiner, where information was collected on age, gender, tumor location, disease evolution, tumor stage, and the presence or absence of trismus. After administration of iodinated contrast media, the protocol the image acquisition was axial slices 3mm, coronal acquisitions were made when necessary. The image analysis of the MS by two medical examiner residents in their 3rd year of Medical Radiology, at distinct moments , without prior knowledge of the clinical information. In the evaluation of the MS, the findings were transcribed to a specific worksheet, considering the presence or absence of the following items: symmetry with the contralateral MS, obliteration of the fat plane of the retromolar trigone and the pharyngeal space, edema, and/or the atrophy of the medial and lateral pterygoid muscles, and destruction of the mandibular ramus. Results: According to the adopted criteria for trismus in this study, the presence of trismus was observed in 10 patients. The association between tumor size and trismus shows that 90% of patients present size T4, while this percentage is 43% in patients without trismus. Analyzing in terms of odds ratios, the study showed that patients with T4 sized tumors had a chance of developing trismus 11.6 times greater than patients with T0 to T3 sized tumors.


The neoplasms of tonsillar crypts and retromolar trigone aggregated 95% of the neoplasms evaluated. Only 5% of the cases accounted for neoplasms of nasopharynx, in which none of them presented the existence of trismus. No cases of neoplasm of maxillary sinus was found. Correlating the CT findings with the group of patients with trismus, the observation was made that 60% of patients with trismus showed edema and/or pterygoid muscle atrophy on CT, whereas this percentage was 21.8% in patients without trismus. ABSTRACT

Securing an airway in children with trismus is challenging and dangerous. Sound clinical judgment is critical for timing and for selecting the method for airway intervention. We present two pediatric cases of Submandibular abscess with difficult oral intubation who underwent incision and drainage. Large facial [jaw] swelling, trismus-limited mouth opening, edema, protruding teeth, and altered airway anatomy makes airway management more difficult. Chances of rupture of abscess intraorally and aspiration under General Anesthesia [GA] is a major threat. Loss of airway under muscle relaxation, difficult to ventilate, difficult to intubate and unwillingness for awake intubation in the pediatric age group makes these cases most challenging. On the basis of our experience, both cases were successfully intubated in anaesthetized, spontaneously breathing children with visual-guided fibreoptic intubation

ABSTRACT

The peroperative management of a 4 1/2 yr old child having bilateral recurrent temporomandibular joint (TMJ) ankylosis and obstructive sleep apnoea (OSA) is presented here. From the anaesthesiologist's point of view, the patient demands special considerations as regards difficulty in intubation, high sensitivity to all central depressant drugs, awake extubation, and post-operative maintenance of a clear airway. It is to be concluded that these conditions call for strict adherence to the basic principles of airway management and close observation extending into the post-operative period.

ABSTRACT

Se reviso las historias clínicas de 76 pacientes con tétanos del adulto. Los factores asociados significativamente a los casos severos o muy severos fueron: una edad mayor o igual a 70 años, un periodo de incubación menor o igual a 7 dias, un tiempo de enfermedad menor igual a 4 dias y una puerta de entrada quirúrgica. se evaluo tres clasificaciones de pronóstico diferentes. Debido al tamaño pequeño de la muestra, los resultados de letalidad encontrados tienen intervalos de confianza muy amplios, lo que no permite detectar diferencias significativas en la letalidad entre las tres clasificaciones. Consideramos que la clasificación que toma en cuenta el tiempo de enfermedad y la presencia de espasmos, es la mas simple y facil de reproducir en el medio urbano y urbano-marginal

ABSTRACT

A case of Wells' syndrome with lock jaw in a 26-year-old Thai female is presented. The clinical features, the course of disease, the histological findings of tissue eosinophilia and flame figures and also significant peripheral blood eosinophilia support the diagnosis of Wells' syndrome in this patient. To the best of our knowledge this is the first case reported in Thailand. The association of lock jaw and Wells' syndrome has never been published in the literature.
