ABSTRACT

Introducción. La infección por COVID-19 afectó drásticamente la atención en salud a nivel mundial, generando retos para la atención primaria. En orden de mitigar y manejar el contagio, la telemedicina se convirtió en una modalidad emergente y efectiva en varias especialidades médicas, incluida la cirugía de cabeza y cuello. Métodos. Estudio de corte transversal con análisis retrospectivo de pacientes atendidos en la consulta virtual durante 18 meses. Se estimaron frecuencias absolutas y relativas, y bivariado con regresión logística binaria. Se incluyeron las variables de diagnóstico primario, poder resolutivo de la consulta, necesidad de cita presencial, plataforma y dificultades de la misma. Resultados. Se incluyeron 2485 pacientes provenientes de 11 departamentos. La patología tiroidea fue la más frecuente (62,9 %), seguida de la aerodigestiva (10,9 %). La consulta fue eficiente en el 99 % de los casos, con una capacidad resolutiva del 96,4 %. El 1,4 % tuvo dificultades en la plataforma y el 8,3 % de los pacientes requirió cita presencial. Cuando hubo dificultad para la revisión de exámenes o una inadecuada inspección funcional, fue 30 veces más probable no poder resolver eficientemente la consulta. Conclusión. La telemedicina provee una alternativa eficiente de atención en cirugía de cabeza y cuello, especialmente en los controles de patología tiroidea, evitando desplazamientos innecesarios. En el tracto aerodigestivo, donde el examen físico es primordial, su utilidad está limitada a la posibilidad de realizar un examen endoscópico posterior que permita una adecuada estadificación y facilite la valoración presencial.


Introduction. The COVID-19 infection drastically affected health care worldwide, creating challenges for primary care. In order to mitigate and manage infection, telemedicine has become an emerging and effective modality in several medical specialties, including head and neck surgery. Methods. Retrospective cross-sectional analysis of patients seen in virtual consultation over 18 months. Absolute and relative frequencies were estimated, univariate analysis was done with chi-square, and bivariate analysis with binary logistic regression. Variables such as primary diagnosis, the resolution power of the consultation, the need for an in-person appointment, the platform, and its difficulties were included. Results. 2485 patients from 11 departments were included. Thyroid pathology was the most frequent (62.9%), followed by aerodigestive tract pathology (10.8%). The consultation was efficient in 99% of cases, with a resolution capacity of 96.4%. 1.4% had difficulties on the platform and 8.3% of patients required an in-person. When there was difficulty in reviewing exams or an inadequate functional inspection, it was 30 times more likely to not be able to efficiently be resolved. Conclusion. Telemedicine provides an efficient alternative for care in head and neck surgery, especially in thyroid pathology controls, avoiding unnecessary travel. In the aerodigestive tract, where the physical examination is essential, its usefulness is limited to the possibility of performing a subsequent endoscopic examination that allows adequate staging and facilitates in-person assessment.

ABSTRACT

The aim of this article was to analyze the literature on technological development in telemedicine through bibliometrics, by identifying the state of the art, research gaps, and trends in the literature. The analysis covers a total of 67 articles related to the field of study, published between 2010-2020 in the Springer Link, Science Direct, Wiley Online Library, Web of Science, and Scopus databases. The data was processed using the software StArt, Excel, IBM SPSS Statistics, and Iramuteq. The results presented bibliometric analysis of the articles, classified into the areas of Management (52.2%), IT (25.4%), and Medicine (22.4%), along with a Table of 34 suggestions for future research. Literature trends encompassed six study clusters (health, study, service, technology, patient, and telemedicine), which further subdivided into nine research themes (digital platform, telemedicine service management, telemedicine service operation, end-user perception, business opportunities, healthcare professional perception, covid-19, regulation, and robotics). An observed outcome was a significant increase in the number of publications in the area due to covid-19.


O objetivo deste artigo foi analisar a literatura acerca do desenvolvimento tecnológico na telemedicina, por meio da bibliometria, ao identificar o estado da arte, lacunas de pesquisa e tendências na literatura. Analisou-se 67 artigos relacionados ao campo de estudo, publicados entre 2010-2020 nas bases de dados Springer Link, Science Direct, Wiley Online Library, Web of Science e Scopus. O tratamento dos dados se deu por meio dos softwares StArt, Excel, IBM SPSS Statistics e Iramuteq. Os resultados apresentaram a análise bibliométrica dos artigos, classificados nas áreas de Gestão (52,2%), TI (25,4%) e Medicina (22,4%), e uma tabela com 34 sugestões para pesquisas futuras. As tendências da literatura envolveram seis classes de estudo (saúde, estudo, serviço, tecnologia, paciente e telemedicina), que se subdividiram em nove temas de pesquisa (plataforma digital, gestão do serviço de telemedicina, operação do serviço de telemedicina, percepção do usuário final, oportunidades de negócios, percepção de profissionais de saúde, covid-19, regulamentação e robótica). Observou-se aumento significativo no número de publicações na área devido à covid-19.


El objetivo de este artículo fue analizar la literatura sobre el desarrollo tecnológico en la telemedicina me-diante bibliometría, identificando el estado del arte, las lagunas de investigación y las tendencias en la literatura. Se analizaron un total de 67 artículos relacionados con el campo de estudio, publicados entre 2010-2020 en las bases de datos de Springer Link, Science Direct, Wiley Online Library, Web of Science y Scopus. Los datos fueron procesados utilizando los programas StArt, Excel, IBM SPSS Statistics e Iramuteq. Los resultados presentaron un análisis bibliométrico de los artículos, clasificados en las áreas de Gestión (52,2%), TI (25,4%) y Medicina (22,4%), junto con una tabla de 34 sugerencias para futuras investiga-ciones. Las tendencias en la literatura abarcaron seis clases de estudio (salud, estudio, servicio, tecnología, paciente y telemedicina), que se subdividieron en nueve temas de investigación (plataforma digital, gestión del servicio de telemedicina, operación del servicio de telemedicina, percepción del usuario final, oportuni-dades de negocio, percepción de los profesionales de la salud, covid-19, regulación y robótica). Un resultado observado fue un aumento significativo en el número de publicaciones en el área debido al covid-19.

ABSTRACT

La Organización Mundial de la Salud define la salud digital como la incorporación de tecnologías de información y comunicación para mejorar la salud. En los últimos años, se vio una fuerte aceleración en la adopción de estas herramientas digitales, lo que impactó de lleno en los modelos asistenciales tradicionales. Actualmente, estamos observando el surgimiento de un gran entorno virtual inmersivo llamado metaverso. Su aparición genera nuevas y desafiantes oportunidades en la salud. En este artículo se exploran algunos conceptos relacionados con este campo, se dan ejemplos concretos de su aplicación en pediatría, se mencionan algunas experiencias en el ámbito hospitalario para finalmente adentrarse en los desafíos y oportunidades que emergen.


The World Health Organization has defined "digital health" as the use of information and communication technologies to improve health. In recent years, there has been a strong acceleration in the adoption of these digital tools, which has had a major impact on traditional healthcare models. We are currently witnessing the emergence of a large immersive virtual environment called the "metaverse." Its emergence creates new and challenging opportunities in health care. This article explores some metaverse-related concepts, provides specific examples of its use in pediatrics, describes experiences in the hospital setting, and finally delves into the resulting challenges and opportunities.

ABSTRACT

La adolescencia constituye una etapa atravesada por cambios físicos, emocionales y sociales significativos, que incluyen la adquisición de múltiples habilidades. Aumentan los riesgos de sufrir alteraciones mentales, consumo de sustancias, embarazo no deseado, trastornos alimentarios, y se evidencian los efectos negativos del uso inapropiado de las redes sociales. El bienestar digital implica el uso saludable de la tecnología, la protección de la privacidad y la seguridad en línea. Los adolescentes utilizan las redes con la finalidad de identificarse, relacionarse, entretenerse y buscar información. La exposición no regulada conlleva riesgos: acceso a contenido inapropiado, ciberacoso, consumo problemático y fraudes. Desde el ámbito pediátrico, se debe acompañar a los adolescentes para que optimicen el uso de la tecnología y, para ello, es fundamental implementar estrategias multisectoriales para minimizar los riesgos y promover el bienestar de los adolescentes en línea, así como garantizar la alfabetización digital y el acceso equitativo a recursos tecnológicos de calidad y telesalud.


Adolescence is a period characterized by significant physical, emotional, and social changes, including the acquisition of multiple skills. It is also a time when the risks of mental disorders, substance use, unwanted pregnancy, eating disorders, and negative effects of inappropriate social media use manifest greatly. Digital well-being implies the healthy use of technology, the protection of privacy and security online. Adolescents use social media to identify themselves, interact with others, entertain themselves, and seek information. However, risks are present, including access to inappropriate content, cyberbullying, problematic consumption, and fraud. The pediatric field must support adolescents in optimizing their online use. Implementing multisectoral strategies can minimize risks, promote digital literacy, and ensure equitable access to quality technological resources and telehealth, thereby promoting adolescent online well-being.

ABSTRACT

OBJETIVO: Investigar os aplicativos móveis que foram desenvolvidos com a finalidade de orientar os profissionais, os pacientes e seus acompanhantes quanto às ações de segurança do paciente no contexto de hospitalização. MÉTODO: Prospecção tecnológica realizada nas lojas virtuais App Store e Google Play e na ferramenta de busca da Google, cujo critério de inclusão foi aplicativos voltados para segurança do paciente no contexto hospitalar sem limitação de recorte temporal. RESULTADOS: Foram incluídos e analisados no estudo 25 aplicativos. A maioria dos aplicativos eram móveis, desenvolvido em inglês, com ênfase para os profissionais de saúde. Ademais, tiveram como limitação, fragilidades relacionadas ao design e características lúdicas. CONCLUSÃO: O estudo pode contribuir para desenvolvimento de novos aplicativos para educação sobre a segurança do paciente, promovendo o aumento do engajamento nas ações de prevenção de erros.


OBJECTIVE: To examine mobile applications developed to guide healthcare professionals, patients, and their caregivers regarding patient safety interventions in the context of hospitalization. METHODS: A technological prospection was conducted in the virtual stores App Store and Google Play and through the Google search tool. Inclusion criteria included applications focused on patient safety in the context of hospitalization without temporal limitations. RESULTS: 25 applications were included in the study and analyzed. The Most applications were mobile, developed in English, and focused on healthcare professionals. In addition, they had limitations and weaknesses related to design and playful features. CONCLUSIONS: The study may contribute to developing of new patient safety education applications that promote increased engagement in error prevention interventions.

Background and Objective@#Pre-pandemic, various healthcare settings were not used to seeing patients virtually. The unprecedented need to adopt virtual care during the COVID-19 pandemic may have caught physical therapists (PTs) unready for it. This study aimed to determine the telerehabilitation knowledge, attitude, and practice of PTs in the Philippines during the COVID-19 pandemic and determine the association between demographic and study outcome variables.@*Methods@#This is an analytical cross-sectional study among members of the Philippine Physical Therapy Association, Inc. (PPTA) practicing in the Philippines. Purposive sampling (total enumeration) was employed. All PPTA members were invited to the study through e-mail and official social media group chats. A self-administered questionnaire was used to obtain data on telerehabilitation knowledge (through test questions on various theoretical aspects), attitude, and practice.@*Results@#The questionnaire items had a content validity index of >0.80. The study yielded a 40% response rate. Most respondents were practicing clinicians in urban-based, private rehabilitation centers. Approximately half had average telerehabilitation knowledge, while the majority had agreeable telerehabilitation attitudes across different constructs. Among the respondents, 15.9% used telerehabilitation pre-pandemic, while 64.8% used it during the pandemic. Hybrid (synchronous and asynchronous) telerehabilitation sessions usually lasted one hour per patient, mostly using Facebook Messenger. @*Conclusion@#Telerehabilitation was not widely practiced locally pre-pandemic, which may explain their average telerehabilitation knowledge. The positive telerehabilitation attitudes may represent a small group of PTs favoring telerehabilitation, while information from the larger population remains unknown. Early adopters of telerehabilitation may help introduce virtual care to colleagues and guide them in developing relevant knowledge and skills amid and beyond the enduring COVID-19 crisis.

ABSTRACT

Objective@#This paper aims to provide a better understanding of the different engagement, cost, and resource considerations in developing and implementing mHealth solutions in the Philippines during the COVID-19 pandemic. @*Methods@#First, six participants completed a form to document the estimated costs of developing a pseudo mobile application with features to mitigate the pandemic. Second, ten key informant interviews determined the facilitators, barriers, and resource requirements in developing mHealth tools. @*Results@#The average cost estimate to develop and roll out a mobile application with public health and epidemiology features is Php 4,018,907 (US $78,650). The analysis of the interviews resulted in 12 themes organized in three domains: 1) facilitators and barriers in developing and sustaining mHealth solutions; 2) costs of sustaining mHealth technologies; and 3) factors affecting the costs of development and maintenance of mHealth technologies. @*Conclusion@#While differences in the cost estimates are evident, it provides a ballpark figure and the different factors that implementers need to sustain and maintain an mHealth solution. This paper hopes to inform policies and practices in engaging technology solution partners and in scaling up mHealth technologies.

ABSTRACT

@#Disruption of the healthcare system was more pronounced in low- to middle-income countries than in developed countries during the period of the COVID-19 pandemic.1 A reduction of about a third of the patient cases was observed, especially for the mild to moderate conditions.2 Considering that non-severe cases are most amenable to remote management through telehealth, the latter was a popular alternative to going to a health facility. Among its benefits include connecting patients with health providers at their convenient time and place, avoiding exposure to infection, saving time, responding early to medical emergencies, providing e-prescriptions, accessing even remote areas, among others.

ABSTRACT

Introduction@#Patients with diabetes require regular follow-ups to achieve optimal glycemic targets. The coronavirus- 19 (COVID-19) pandemic resulted in interruptions in healthcare delivery placing greater importance on patient’s self-management of their condition. Telemedicine bridged the gap between the physician and patient that was created by community quarantines. @*Objective@#To determine if there is a difference in patient’s self-care activities before and after using telemedicine using the Diabetes Self-Management Questionnaire (DSMQ). @*Methodology@#A descriptive repeated cross-sectional study of patients with type 2 diabetes mellitus at the University of Santo Tomas Hospital who consulted via telemedicine using different available platforms were included. Self-care was measured using the DSMQ. Patient satisfaction with telemedicine was also assessed using a patient satisfaction survey. @*Results@#An improvement in self-care practices was seen as significantly higher mean DSMQ scores after telemedicine consultations (6.79 ± 1.33 to 7.32 ± 1.21, p = 0.0015), with the highest scores on dietary control and physical activity. There was a statistically significant reduction in HbA1c on follow up (8.37 ± 2.31 to 7.31 ± 1.36; p<0.00001). Those with well-controlled diabetes (n = 14) at baseline remained to have good control while the proportion of those with poorly controlled diabetes (n = 34) showed improved glycemic control on follow up (p = 0.0045). Most patients were highly satisfied with telemedicine. @*Conclusion@#The use of telemedicine by patients with diabetes showed numerical improvement in both self-care practices and glycemic control. These findings imply that telemedicine may be mainstreamed as part of diabetes care among Filipinos.

ABSTRACT Objective: To evaluate questions concerning oral medicineand pharmacology-related specialties of asynchronous dental teleconsulting sessions of the Telehealth Brazil Networks Program. Material and Methods: Data were collected from secondary databases of asynchronous dental teleconsulting sessions of the telehealth centers of Minas Gerais from July 2015 to July 2017. The variables for dental underlying fields and the types of questions were evaluated. Descriptive analysis was performed with the SPSS v.22.0 program. Results: 3,920 teleconsulting sessions were referred to the telehealth centers of Minas Gerais during the study period. Regarding oral medicine-related questions (n=745), most (n=469; 62.95%) addressed diagnosis, whereas the underlying field questions mostly regarded fungal, viral, and bacterial infections (17.3%), biopsies (16.4%), developmental defects and dental abnormalities (9.9%), and soft tissue tumors (9.4%). Pharmacology-related questions (n=738) mostly addressed general approaches (n=672; 91.06%), and the most common questions were about underlying fields' prescriptions (44.7%), anesthetics (17.6%), adverse effects of medications and anesthetics (10.2%), and selection of anesthetics for patients with systemic conditions (9.8%). Conclusion: Most teleconsulting sessions regarded conditions or procedures common in primary health care and essential for diagnosis and treatment planning at all care levels, which suggests a need for more academic learning processes for healthcare professionals, especially in dentistry primary fields.

ABSTRACT

Objetivo: evaluar las evidencias sobre estrategias de telesalud en la atención de personas con enfermedad renal crónica. Método: revisión integradora de la literatura. La búsqueda de estudios primarios se realizó en seis bases de datos: PubMed/MEDLINE, Web of Science, EMBASE, CINAHL, LILACS y Scopus. La muestra estuvo compuesta por 48 artículos publicados entre 2000 y 2021, la estrategia de telesalud fue aplicada mediante equipo multidisciplinario, médico, enfermero, farmacéutico, nutricionista y trabajador social. De los artículos se extrajo el tipo de estudio, país, estrategia aplicada, escenario, población y profesional. Los estudios fueron seleccionados mediante la lectura de título y resumen (fase 1) y, posteriormente, mediante la lectura completa (fase 2), se los categorizó por estrategia de telesalud. El resumen de resultados se presentó de forma descriptiva y los estudios se clasificaron según el nivel de evidencia. Resultados: el domicilio fue el más representativo en las terapias de diálisis y tratamiento conservador. Se identificaron seis categorías de estrategias de telesalud: dispositivos de monitoreo remoto, teleconsulta, plataforma digital, aplicaciones, estrategias multimodales y contacto telefónico. Conclusión: el uso de esas estrategias para la atención de personas con enfermedad renal crónica tiene diferentes formatos e implementaciones, las mismas son factibles para dicha población en cualquier etapa de la enfermedad y pueden ser aplicadas por diferentes profesionales de la salud especialmente en el ámbito domiciliario. La evidencia ha demostrado que la telesalud favorece la reducción de costos, la accesibilidad a ubicaciones remotas, un mejor seguimiento de la diálisis con resultados positivos sobre el control de los síntomas, la reducción de riesgos y capacitación del paciente.


Objective: to evaluate the evidence about telehealth strategies in caring for people with chronic kidney disease. Method: integrative literature review. The search for primary studies was carried out in six databases: PubMed/MEDLINE, Web of Science, EMBASE, CINAHL, LILACS, and Scopus. The sample consisted of 48 articles published between 2000 and 2021. The telehealth strategy was applied by a multidisciplinary team of doctors, nurses, pharmacists, nutritionis, and social workers. The type of study, country, strategy applied, setting, population, and professional were extracted from the articles. The studies were selected by reading the title and abstract (phase 1) and then reading them in full (phase 2), categorizing them by telehealth strategy. The results were summarized descriptively and the studies were classified according to their level of evidence. Results: the home was the most representative in dialysis and conservative treatment. Six categories of telehealth strategies were identified: remote monitoring devices, teleconsultation, digital platforms, apps, multimodality strategies, and telephone contact. Conclusion: using these strategies for the care of people with chronic kidney disease presents different forms and implementations, being feasible for the renal population at any stage of the disease and applicable by different health professionals with an emphasis on the home environment. The evidence shows that telehealth favors lower cost, accessibility to remote locations, and better monitoring of dialysis with positive resul in symptom control, risk reduction, and patient training.

A busca pelos estudos primários foi realizada em seis bases de dados: PubMed/MEDLINE, Web of Science, EMBASE, CINAHL, LILACS e Scopus. A amostra foi composta por 48 artigos publicados entre 2000 e 2021, a estratégia de telessaúde foi aplicada por equipe multidisciplinar, médico, enfermeiro, farmacêutico, nutricionista e assistente social. Foram extraídos dos artigos o tipo de estudo, país, estratégia aplicada, cenário, população e profissional. Os estudos foram selecionados por leitura de título e resumo (fase 1) e, após, por leitura completa (fase 2), com sua categorização por estratégia de telessaúde. A síntese dos resultados foi apresentada de forma descritiva e os estudos classificados de acordo com o nível de evidência. Resultados: o domicílio foi o de maior representatividade nas terapias dialíticas e tratamento conservador. Foram identificadas seis categorias de estratégias de telessaúde: dispositivos de monitoramento remoto, teleconsulta, plataforma digital, aplicativos, estratégias multimodalidades e contato telefônico. Conclusão: a utilização dessas estratégias para o atendimento de pessoas com doença renal crônica apresenta diferentes formatos e implementações, sendo viável à população renal em quaisquer fases da doença e aplicável por diferentes profissionais de saúde com ênfase no ambiente domiciliar. As evidências apontaram que a telessaúde favorece a diminuição de custos, acessibilidade aos locais afastados, melhor monitoramento da diálise com resultados positivos no controle dos sintomas, redução dos riscos e treinamento do paciente.

Introducción: el uso de cuestionarios electrónicos se ha convertido en algo común, sin embargo, depende de diversos factores (como la edad y las habilidades digitales), por lo que es necesario determinar la confiabilidad de estos instrumentos para su aplicación. Objetivo: determinar la concordancia y reproducibilidad de los cuestionarios Calidad de vida breve de la OMS (WHOQoL-B) y Actitudes autoevaluadas hacia la vejez (AAV) en formato electrónico para su aplicación vía remota a mujeres de edad madura y adulta mayor. Metodología: se realizó un estudio instrumental con el método test-retest con 35 mujeres (40-69 años). Se transfirieron ambos cuestionarios a un formulario de Google y se enviaron vía WhatsAppen dos ocasiones, con una diferencia mayor a 15 días entre la aplicación. Se calcularon los parámetros de confiabilidad por estabilidad temporal y concordancia estadística. Resultados: el WHOQoL-B obtuvo a y CCI = 0.880 y r = 0.785 en la calificación total; para sus dimensiones a > 0.740 y r > 0.590. La calificación total del AAV mostró a y CCI = 0.708 y r = 0.552 y sus dimensiones a > 0.710 y r de 0.295 a 0.508. Conclusiones: los cuestionarios WHOQoL-B y AAV en formato electrónico pueden ser utilizados para aplicación vía remota, aunque el AAV es menos consistente...(AU)


Abstract Introduction: The electronic tests have been used on a regular basis, although their use depends on several factors such as age and digital ability, therefore it is necessary to determine the confidence of these instruments for their application. Objective: To determine the agreement and reproducibility of the WHO Quality of Life-Bref (WHOQoL-B) and the Self-Assessed Attitudes Towards Old Age (SATO) questionnaires in electronic format for application in middle-aged and older women. Methods: An instrumental study with a test-retest method was carried out with 35 women (40-69 years). Both questionnaires were transferred to a Google form and sent to the women by WhatsApp twice with a difference of more than 15 days between applications. We calculate the confidence by temporal stability and the statistical agreement. Results: For the WHOQoL-B, an a and ICC= 0.880, and r = 0.785 were obtained in the total rating; for their dimensions the result for a was > 0.740, and r > 0.590. The total rating of SATO shows an a and ICC= 0.708, and r = 0.552; for their dimensions, the a was > 0.710, and r from 0.295 to 0.508. Conclusions: The electronic tests WHOQoL and SATO can be used for remote application, although SATO is less consistent.

ABSTRACT

O objetivo deste artigo é refletir sobre o papel da saúde digital no acesso à saúde pública no contexto do Sistema Único de Saúde durante a pandemia de covid-19 em Palmas ­ TO. Para tanto, adotou-se como referencial teórico os pressupostos do campo Comunicação & Saúde e, como forma de análise, o método denominado Discurso do Sujeito Coletivo, que busca compreender os modos de pensar de um grupo social sobre dado tema. Neste caso, trata-se de moradores do condomínio Residencial Parque da Praia, situado em Palmas. Também se procurou entender em que medida essas pessoas estão inseridas no mundo tecnológico ou são excluídas dele, e qual a percepção delas sobre saúde digital, Sistema Único de Saúde, direito à saúde e à comunicação. O grupo se considera relativamente informado sobre esses temas e a maioria faz uso, no cotidiano, das Tecnologias da Informação e Comunicação e da internet. Entretanto, observou-se que a saúde digital é uma realidade ainda distante para essa comunidade


The purpose of this article is to reflect on the role of digital health in the access to public health in the context of the Unified Health System during the covid-19 pandemic in Palmas ­ TO. In order to do this, the assumptions of the Communication & Health field were adopted as a theoretical framework and the method called Collective Subject Discourse was used as a form of analysis. This method seeks to understand the ways of thinking of a social group on a given topic. In the case examined here, the group is constituted by residents of the Residencial Parque da Praia condominium, situated in Palmas. We also sought to understand to what extent these people are included in or excluded from the technological world and their perception of digital health, the Unified Health System, the right to health and communication. The group considers itself relatively informed about these topics and the majority of those residents use Information and Communication Technologies as well as the internet in their daily live. However, it was observed that digital health is still a distant reality for this community


El objetivo de este artículo es reflexionar sobre el papel de la salud digital en el acceso a la salud pública en el contexto del Sistema Único de Salud durante la pandemia de covid-19 en Palmas ­ TO. Para hacer eso, se adoptó como marco teórico los presupuestos del campo de Comunicación y Salud y, como forma de análisis, el método denominado Discurso del Sujeto Colectivo, que busca comprender las formas de pensar de un grupo social acerca de un tema determinado. En el caso abordado en este artículo, el grupo es constituido por residentes del condominio Residencial Parque da Praia, situado en Palmas. También buscamos comprender en qué medida estas personas están incluidas en el mundo tecnológico o son excluidas de él, y su percepción sobre la salud digital, el SUS, el derecho a la salud y a la comunicación. El grupo se considera relativamente informado sobre esos temas y su mayoría utiliza las Tecnologías de la Información y las Comunicaciones y la internet en su vida diaria. Sin embargo, se observó que la salud digital es aún una realidad lejana para esta comunidad

ABSTRACT

Low user engagement in m-Health applications has been driving the use of retention techniques that aim to ensure a satisfactory long-term user experience. The aim was to understand the experience of hypertensive patients interacting with a mobile health application for 12 months. A qualitative/exploratory study was conducted after 12 months, with the same experimental group of participants as the non-randomized con-trolled clinical trial conducted in 2019. The mean age of the 16 participants was 57 years (SD=8), of which11 were female. All had low socioeconomic and educational levels. Content analysis showed no engagement with the m-health app over time. The main factors contributing to the lack of engagement were; inability of the user to use the app, lack of support and technical problems. When designing m-Health interventions, it is important to understand users' behavioral characteristics, motivations for treatment, level of involve-ment in health care, and ability to use technology


O baixo envolvimento dos usuários em aplicativos m-Health vem impulsionando o uso de técnicas de retenção que visam garantir uma experiência do usuário (UX) satisfatória a longo prazo. Objetivou-se compreender a experiência de pacientes hipertensos interagindo com um aplicativo de saúde móvel (m-Health) durante 12 meses. Realizou-se estudo qualitativo/exploratório após 12 meses, com o mesmo grupo experimental de participantes do ensaio-clínico controlado e não-randomizado realizado em 2019. A idade média dos 16 participantes foi de 57 anos (DP=8); 11 eram do sexo feminino, com baixos níveis socioeconômicos e educacionais. A análise de conteúdo não mostrou nenhum envolvimento ao longo do tempo usando o aplicativo m-Health. Os principais fatores que contribuíram para a falta de envolvimento foram: incapacidade do usuário de usar o aplicativo, falta de suporte e problemas técnicos. Ao propor intervenções que utilizem m-Health, é essencial conhecer características comportamentais dos usuários, motivações para o tratamento, nível de envolvimento nos cuidados com a saúde e sua capacidade de usar tecnologias


La baja participación de los usuarios en las aplicaciones de m-Health ha impulsado el uso de técnicas de retención que tienen como objetivo garantizar una experiencia de usuario satisfactoria a largo plazo. Se pretendió comprender la experiencia de pacientes hipertensos interactuando con una aplicación de salud móvil durante 12 meses. Se realizó un estudio cualitativo/exploratorio después de 12 meses, utilizando los participantes del ensayo clínico controlado no aleatorizado que se llevó a cabo en 2019. La edad media de los 16 participantes fue 57 años (SD=8); 11 mujeres, con niveles socioeconómicos y educativos bajos. El análisis de contenido no mostró compromiso a lo largo del tiempo utilizando la aplicación m-Health. Los principales factores contribuyentes fueron: incapacidad del usuario para utilizar la aplicación, falta de apoyo y problemas técnicos. Cuando se proponen intervenciones con m-Health, es esencial conocer las características de comportamiento de los usuarios, sus motivaciones para el tratamiento, nivel de implicación en el cuidado de la salud y capacidad para utilizar tecnologías

ABSTRACT

Apresentamos e discutimos a construção de um instrumento de compliance para tratamento de dados pessoais e dados pessoais sensíveis do Núcleo Telessaúde UFSC, com base na Lei Geral de Proteção de Dados Pessoais. Trata-se da elaboração de diretrizes para um Relatório de Impacto à Proteção de Dados Pessoais visando à preservação da dimensão humana do dado e à preservação de direitos. A pesquisa foi qualitativa e exploratória, tendo o relato de experiência como metodologia. O levantamento bibliográfico e a análise documental permitiram a investigação, realizada em 2022, de processos, etapas e fluxos do tratamento dos dados. A análise dos dados foi qualitativa, por comparação dos resultados com a legislação vigente e com a adequação ao princípio da autodeterminação informativa. Os resultados demonstraram que as propostas para o relatório contribuíram para um tratamento de dados mais adequado ao ordenamento jurídico e, consequentemente, mais humanizado


We present and discuss the construction of a compliance instrument for the processing of personal data and sensitive personal data of the Telessaúde UFSC Center, based on the General Data Protection Law. It is a model for the elaboration of guidelines for the construction of an Impact Report on the Protection of Personal Data that collaborates to the preservation of the human dimension of data and the preserva-tion of rights. The research was qualitative and exploratory, adopting experience report as methodology. Bibliographical research and documental analysis enabled the investigation of the processes, stages and flows of data processing, carried out in 2022. Data analysis was carried out qualitatively, comparing the results with current legislation and adequacy to the principle of informative self-determination. Results showed that the proposed guidelines for the report contributed to a data processing more appropriate to the legal system and, consequently, more humanized


Presentamos y discutimos la construcción de un instrumento de cumplimiento para el tratamiento de datos personales y datos personales sensibles del Núcleo Telessaúde UFSC, con base en la Ley General de Pro-tección de Datos. Es la preparación de directrices para un Informe de Impacto de Protección de Datos Per-sonales para preservación de la dimensión humana de los datos y preservar los derechos. La investigación cualitativa y exploratoria adoptó un relato de experiencia como metodología. El levantamiento bibliográfico y el análisis documental permitieron la indagación de procesos, etapas y flujos de procesamiento de datos, realizados en 2022. El análisis de datos fue cualitativa, mediante la comparación de los resultados con la legislación vigente y la adecuación al principio de autodeterminación informativa. ABSTRACT

O objetivo do presente estudo foi identificar elementos estruturantes do processo de trabalho das redes educacionais colaborativas em saúde mediadas por tecnologias de informação e comunicação, assinalando suas características-chave e sua dinâmica de funcionamento. Trata-se de uma revisão de escopo, realiza-da nas bases de dados: Medline, Lilacs e no portal Oasisbr. Foram encontrados 1.499 estudos, dos quais, oito atenderam aos critérios de inclusão. Os resultados da investigação apontam as práticas educacionais colaborativas em saúde mediados por essas tecnologias como processos amplos e complexos, envolvendo a associação de diversos fatores. Destacam-se a dimensão técnico-pedagógica, a gestão estratégica, o gerenciamento do fluxo de informações, os aspectos relacionais, as competências de cada membro e a mediação do processo colaborativo. A integração dos diferentes aspectos da rede traduz um conjunto de desafios para a estruturação dos processos colaborativos que não se limita à pura acumulação cognitiva, mas vincula-se a processos de aprendizagem e desenvolvimento dos sujeitos


The aim of the present study was to identify structural elements of the work process of collaborative educational networks in health, mediated by information and communication technologies, by pointing out its key characteristics and its operating dynamics. This is a scope review, accomplished in the following databases Medline and Lilacs and in the Oasisbr portal. A total of 1.499 studies were found, and eight of them met the inclusion criteria. The search results indicate the collaborative educational practices in health mediated by these technologies as broad and complex processes, involving the association of several factors. The technical-pedagogical dimension, the strategic management, the management of the information flow, the relational aspects, the competencies of each member, and the mediation of the collaborative process are highlighted. The integration of the different aspects of the network translates a set of challenges for the structuring of collaborative processes, which is not limited to pure cognitive accumulation, but is bound to learning processes and to the subjects' development.


El objetivo del presente estudio es identificar elementos estructurantes del proceso de trabajo de las redes educativas colaborativas en salud mediadas por tecnologías de la información y comunicación, señalando sus características claves y su dinámica de funcionamiento. El trabajo presenta una revisión de escopo, realizada en las bases de datos: Medline, Lilacs y en el portal Oasisbr. Fueron encontrados 1.499 estudios, de los cuales ocho cumplieron los criterios de inclusión. Los resultados de la investigación apuntan a las prácticas educativas colaborativas en salud mediadas por estas tecnologías como procesos amplios y com-plejos, que abarcan la asociación de diversos factores. Se destacan la dimensión técnico-pedagógica, la gestión estratégica, el gerenciamiento del flujo de información, los aspectos relacionales, las competencias de cada miembro y la mediación del proceso colaborativo. La integración de los diferentes aspectos de la red traduce un conjunto de desafíos para la estructuración de los procesos colaborativos, que no se limitan a la acumulación cognitiva, sino que se vinculan a los procesos de aprendizaje y desarrollo de los sujetos

ABSTRACT

Objetivo: verificar asociaciones entre variables sociodemográficas y factores que facilitan y dificultan la transición de la atención psicológica presencial a la modalidad remota en el primer año de la pandemia de COVID-19. Método: se trata de un estudio analítico, cuantitativo y de corte transversal. Después de la aprobación del Comité de Ética en Investigación, la recolección de datos se realizó mediante la aplicación de un formulario en línea que consta de 55 preguntas. Los datos fueron analizados utilizando técnicas de estadística descriptiva e inferencial. Resultados: la muestra de conveniencia estuvo compuesta por 385 psicólogos brasileños, en su mayoría mujeres (67,01%), jóvenes profesionales con hasta cinco años de actuación después de la graduación (44,16%) y predominio de actividades en la clínica privada. Se encontró que el tiempo de formación entre cinco y 10 años se asoció con una mayor percepción de dificultades y que la experiencia previa con la atención a distancia facilitó la adaptación en la transición de una modalidad a otra. Conclusión: considerando que la teleasistencia puede ser una poderosa herramienta en el escenario de la salud, se sugiere que los temas de la teleasistencia sean incluidos en la agenda de investigación y los contenidos programáticos en los currículos de los cursos de formación en salud.


Objective: to verify associations between sociodemographic variables and factors that facilitate and hinder the transition from face-to-face psychological care to remote mode in the first year of the COVID-19 pandemic. Method: this is an analytical, quantitative, cross-sectional study. After approval by the Research Ethics Committee, data collection was performed by applying an online form consisting of 55 questions. Data were analyzed using descriptive and inferential statistics techniques. Results: the intentional sampling consisted of a total of 385 Brazilian psychologists, mostly women (67.01%), young professionals with up to five years of graduation (44.16%) most of activities in the private clinic. It was found that training time between five and 10 years was associated with a greater perception of difficulties and that previous experience with remote care facilitated adaptation in the transition from one modality to another. Conclusion: considering that call center can be a powerful tool in the health scenario, it is suggested the inclusion of remote care issues in the research agenda and syllabus in the curricula of health training courses.

Conclusão: considerando que o teleatendimento pode ser uma ferramenta potente no cenário da saúde, sugere-se a inclusão das questões do atendimento remoto na agenda de pesquisa e conteúdos programáticos das grades curriculares dos cursos de formação em saúde.

Objective: This study aimed to assess the satisfaction of patients or their caregivers and medical staff with the telemedicine strategies adopted at HC Criança, a Brazilian tertiary-care hospital, during the COVID-19 pandemic. Methods: An electronic survey was sent to each group (patients/caregivers and staff) with questions on their experiences with telemedicine answered as Likert scales. Results: 81 answers were obtained from medical staff and 1,721 from patients/caregivers. Among patients/caregivers, 892 (52%) reported they did not have any appointment by telemedicine. Most patients/caregivers who experienced telemedicine at least once reported they were not trained to use telemedicine (77%), while overall satisfaction was 56%. Most (78%) believed telemedicine could be adopted after the pandemic. Among the medical staff, 77% reported they were not trained to use telemedicine, 56% rated their experience with telemedicine as satisfactory, and 78% believed telemedicine could be adopted in the future. Conclusion: Satisfaction with telemedicine of patients, their caregivers, and medical staff at HC Criança was good during the COVID-19 pandemic, even without proper training, and they think telemedicine could be adopted after the pandemic (AU).


Objetivo: Este estudo teve como objetivo avaliar a satisfação dos pacientes ou seus responsáveis e dos profis-sionais de saúde em relação à telemedicina implantada no HC-Criança, um hospital pediátrico terciário, durante a pandemia da COVID-19. Métodos: Para cada grupo (pacientes/cuidadores e médicos) foi enviado um questionário eletrônico contendo perguntas que foram respondidas em escala de Likert sobre suas experiências com o uso da telemedicina. Resultados: Foram obtidas 81 respostas válidas de médicos e 1.721 de pacientes ou responsáveis. Entre pacientes ou cuidadores, 892 (52%) relataram não ter recebido nenhum atendimento por telemedicina. A maioria dos pacientes/cuidadores que receberam atendimento por telemedicina pelo menos uma vez relatou não ter sido treinada para usar a telemedicina (77%), enquanto a satisfação geral foi de 56%. A maioria (78%) acredita que a telemedicina pode ser adotada após a pandemia. Entre os médicos, 77% relataram não terem sido treinados para usar a telemedicina, 56% avaliaram sua experiência com a telemedicina como satisfatória, e 78% acreditam que a telemedicina pode ser adotada no futuro. Conclusão: A satisfação com a telemedicina de pacientes, seus cuidadores e profissionais de saúde do HC Criança foi boa durante a pandemia da COVID-19, mesmo sem receberem treinamento adequado, e eles acreditam que a telemedicina pode ser adotada após a pandemia (AU).

ABSTRACT

A telemedicina, permitida em caráter emergencial durante a covid-19, foi autorizada e regulamentada pela Lei nº 14.510/2022. Reconhecida como serviço imprescindível para a garantia da equidade em saúde, na telemedicina veiculam-se dados considerados sensíveis pela Lei Geral de Proteção de Dados. Este ensaio apresenta uma discussão a respeito de tais dados, os quais detêm relação intrínseca com direitos da personalidade e que devem ser reconhecidos como sigilosos, a fim de garantir o direito à privacidade dos titulares, bem como o respeito ao sigilo médico. Conclui-se que eventual violação dos dados sensíveis pode ensejar sanções administrativas aos agentes de tratamento, mas há divergência doutrinária a respeito do regime de responsabilidade adotado pela Lei Geral de Proteção de Dados, com três possíveis interpretações: responsabilidade objetiva, responsabilidade subjetiva e responsabilidade ativa


Telemedicine, which had been allowed on an emergency basis during covid-19, was authorized and regulat-ed by Law nº 14.510/2022. Recognized as an essential service in guaranteeing equity in health, in telemedi-cine, data considered sensitive by the General Data Protection Law is transmitted. This essay elaborates on a discussion regarding such data, which are intrinsically related to personal rights and must be recognized as confidential in order to ensure the right to privacy of the data subjects, as well as respect for medical confidentiality. It is concluded that any violation of sensitive data may result in administrative sanctions for treatment agents. Still, doctrinal divergence exists regarding the liability regime adopted by the law, with three possible interpretations: strict liability, fault liability, and active liability


La télémédecine, qui avait été permise en urgence pendant le covid-19, a été autorisée et réglementée par la Loi nº 2022-14510. Reconnue comme un service essentiel pour garantir l'équité en santé, en télémédecine, des données considérées comme sensibles par la Loi Générale sur la Protection des Données sont transmises. Cet essai développe une discussion concernant de telles données, qui sont intrinsèquement liées aux droits personnels et doivent être reconnues comme confidentielles afin de garantir le droit à la vie privée des sujets de données, ainsi que le respect de la confidentialité médicale. On en conclut que la violation éventuelle de données sensibles peut entraîner des sanctions administratives pour les agents de traitement. Néanmoins, des divergences doctrinales existent quant au régime de responsabilité adopté par la loi, avec trois interprétations possibles: la responsabilité stricte, la responsabilité pour faute et la responsabilité active

ABSTRACT

O objetivo foi analisar o perfil das pessoas com diabetes que utilizaram a telefarmácia na pandemia de covid-19. Trata-se de um estudo transversal com dados oriundos da websurvey DIABETESvid que ocorreu nos meses de setembro e outubro de 2020. Verificou-se que 45 participantes recorreram à telefarmácia, sendo que 55,6% eram do sexo masculino, 42,2% tinham idade compreendida entre 18 e 34 anos e 46,7% estudaram 12 ou mais anos. Além disso, 48,9% autorreferiram diabetes mellitus tipo 1. A maioria usava insulina (55,6%) e obteve os medicamentos para o diabetes em farmácias do sistema público de saúde (60%). Ademais, as pessoas que tiveram o diagnóstico de covid-19 buscaram mais pela telefarmácia (RP=2,20; IC95% 1,23­3,94). Sabe-se que os medicamentos são essenciais para o tratamento do diabetes. Logo, no período estudado em que se preconizava o distanciamento físico, os participantes apropriaram-se da telefarmácia a fim de obter a integralidade do cuidado


The objective was to analyze the profile of people with diabetes who used telepharmacy during the covid-19 pandemic. This is a cross-sectional study with data from the DIABETESvid websurvey that took place in September and October 2020. It was found that 45 participants used telepharmacy, 55.6% of whom were male, 42.2% were between 18 and 34 years-old and 46.7% studied 12 or more years. In addition, 48.9% self-reported type 1 diabetes mellitus. Most used insulin (55.6%) and obtained their diabetes medication from pharmacies in the public health system (60%). Also, people diagnosed with covid-19 used telephar-macy more often (PR=2.20; 95%CI 1.23­3.94). It is known that drugs are essential for the treatment of diabetes. Therefore, in the period studied in which physical distancing was advocated, the participants used telepharmacy to obtain comprehensive care


El objetivo fue analizar el perfil de las personas con diabetes que utilizaron la telefarmacia durante la pandemia del covid-19. Estudio transversal con datos de la encuesta por internet DIABETESvid realizada en septiembre y octubre de 2020. Se encontró que 45 participantes recurrieron a la telefarmacia, de los cuales el 55,6% eran hombres, el 42,2% tenían entre 18 y 34 años y el 46,7% estudiaban 12 o más años. Además, el 48,9% se autorrefirió diabetes mellitus tipo 1. La mayoría utilizaba insulina (55,6%) y recibía su medicación antidiabética en farmacias del sistema público (60%). Además, las personas diagnosticadas con covid-19 buscaron telefarmacia con más frecuencia (RP = 2,20; IC 95% 1,23­3,94). Los medicamentos son esenciales para el tratamiento de la diabetes. Por lo tanto, en el período estudiado en el que se propugnaba el distanciamiento físico, los participantes se apropiaron de la telefarmacia para la atención integral
