RESUMEN

PURPOSE: HLA (human leukocyte antigen)-class I genes are highly polymorphic, play many roles in organ and bone marrow transplantation. HLA-B is the most polymorphic class I locus with 414 alleles. HLA-class I typing, which is based on serologic method, has been used until recently. The development of molecular biological techniques make it possible to define the genotypes of HLA genes. METHODS: Analyses of HLA-B genotyping on 1,000 UCB (Umbilical Cord Blood) samples which were considered to be HLA-B homozygote or blank were performed by ARMS-PCR (Amplification Refractory Mutation System-PCR) method and direct sequencing. RESULTS: We could identify HLA-B*5001 which was known to be absent in Koreans. CONCLUSION: It is strongly suggested that HLA-B homozygote should be confirmed to the DNA level especially in cases of donor selection for the unrelated bone marrow transplantation.

RESUMEN

BACKGROUND: Postoperative vomiting is a troublesome problem in pediatric patients undergoing strabismus surgery. We compared the effect of clonidine and midazolam to prevent vomiting in a randomized, single-blind, placebo-controlled study of 60 healthy children undergoing surgical correction of strabismus. METHODS: The children, aged 3~15 years, were randomly allocated into three groups. The children in group 1 received clonidine 4 microgram/kg per oral about 90 min before induction of anaesthesia. Those in group 2 received midazolam 75 microgram/kg intravenously after induction before surgery. Those in group 3 received a placebo. Anaesthesia consisted of enflurane, nitrous oxide in oxygen, vecuroniun and thiopental. RESULTS: There was no difference among the groups with respect to age, gender, weight, duration of anaesthesia and recovery room time. The data showed that clonidine 4 microgram/kg per oral before induction of anaesthesia was significantly effective in preventing vomiting (P<0.017). But there was no significant difference between the midazolam group and the placebo group. CONCLUSION: We recommend the prophylactic administration of clonidine 4 microgram/kg per oral to children 3~15 years old because it decreases postoperative vomiting. It also provides preoperative sedation, postoperative analagesia, perioperative hemodynamic stability and a reduction in the volatile anesthetic requirement.

RESUMEN

Renal cell carcinoma is associated with inferior vena cava tumor spread in 4~10% of cases and with extension of the tumor thrombus into the right atrium in less than 1% of cases. Because inferior vena caval involvement does not affect the ultimate survival in patients with nonmetastatic renal cell carcinoma, aggressive surgical resection is indicated. We experienced a case of complete tumor excision with radical nephrectomy and inferior vena caval and right atrial thrombectomy using adjunctive cardiopulmonary bypass(CPB) and deep hypothermic circulatory arrest(DHCA). During total circulatory arrest(TCA), we protected brain from ischemic insult using deep hypothermia, retrograde cerebral perfusion, thiopental, and high dose steroid. The patient recovered uneventfully except minor neuropsychiatric symptom for 3 weeks after operation.

RESUMEN

Flexor hallucis longus tendinitis is a common overuse syndrome in classic ballet dancers. To describe the clinical symptoms, diagnosis and non-operative and operative treatment, we evaluated twenty ballet dancers from march 1994 to August 1995. For the diagnosis, Tomasen test was helpful clinically and MRI was essential for surgical intervetion to find a tendon degeneration. Patients were classified into three groups by symptoms and clinical stages. All of the Group 1 and 2 were recovered by means of the conservative treatment. Two dancers belongs to Group 3 showed a severe degeneration of tendon on the MRI scan, finally was taken the an operation. Release of the tarsal tunnel were performed. Six months after operation, they could return to dancing.

RESUMEN

To investigate the changing patterns of microfilament and microtubule arrangement and influence of myocardial cells and colchicines to microfilament and microtubule formation in cardiac endothelial cells the authors carried out indirect immunofluorescence stain for actin and tubulin with supernatant monoclonal antibodies. Secondary antibodies were IgG FITC conjugate. The results were summarized as follows. Fiberform reactions were stronger in the cells with many processes and spread cytoplasm and they became weaker after the endothelial cells formed monolayer. In the endothelial cells cocultured with myocardial cells the fiberform of the microtubule became less visible compared to control group but fiberform of the microtubule maintained strong intensity as endothelial cells formed monolayer. In the group treated with colchicines, there were no visible differences in microfilaments compared to control group but fiberform of microtubule revealed weaker intensity after colchicines treatment. The intensity of microtubule fiberform returned to control level after 2 days.

RESUMEN

The accuracy of prediction of postoperative refractive error was evaluated in 113 patients with extracapsular cataract extraction and posterior chamber intraocular lens. The method of calculating the IOL power was using the SRK formula. The refraction was done on 113 cases which could be followed up for more than 6 months. The results were as follows: 1. The best corrected and naked visual acuity of 0.5 or better was 98.2%, 77.9% respectively. 2. The mean spherical equivalent of postoperative corrective lens power was -0.86 +/- 41D. 3. Error of predicted required spectacle lens power was -0.67+/-0.4D. in relative average. With the use of the standard formula described by Hoffer, out results for the accuracy of intraocular lens calculation was 70.7% +/- 1.0D / 94.6% +/- 2.0D./ +2.51D. to -4.0D. 4. The mean axial length, keratametric power, anterior chamber depth, and predictable lens power of 113 cases was 23.85 +/- 16mm, 42.79 +/- 23D., 3.40 +/- 12mm, 18.53 +/- 67D. respectively.

RESUMEN

The depth of the anterior chamber decreases with aging, secondary to lens growth. However, short-term changes in the depth of the anterior chamber have been reported in recent articles. The authors measured the depth of the anterior chamber in normal population and investigated the diurnal variation in the depth of the anterior chamber and correlation between the depth of the anterior chamber and intraocular pressure. The results were as the followings; 1. The depth of the anterior chamber decreases with age. 2. The depth of the anterior chamber in the morning is 3.48 +/- 0.31 mm, while the depth of the anterior chamber in the evening is 3.37 +/- 0.30mm. It is statistically significant in diurnal variation of the depth of the anterior chamber. 3. The depth of the anterior chamber does not correlate with the intraocular pressure.
