RESUMEN

BCR-ABLT315I mutation is the main mechanism of resistance to the first and second generation tyrosine kinase inhibitor (TKI) for patients with chronic myeloid leukemia (CML). Ponatinib as the third generation TKI has been found that can significantly improve the prognosis of CML patients with T315I mutation. However, the latest report has discovered that the T315I compound mutant is even resistant to ponatinib, which aroused the enthusiasm of research on the mechanism of CML resistance and targeted therapy once again. Previous studies have shown that TKI combined with other targeted drugs is effective to CML patients with drug resistance or relapse due to T315I mutation. The latest research has found that the allosteric inhibitor asciminib combined with TKI therapy is equally effective to CML patients with T315I compound mutant, but the specific mechanism is not yet clarified. This review will focus on the latest research progress of therapy for CML with BCR-ABLT315I mutation, hoping to provide reference for researching new drugs and improve therapy for treating CML with T315I mutation.

Diabetes is one of the most common chronic non-communicable diseases. The incidence rate is on the increase year by year. As the disease progresses, it can cause damages to many tissues and organs, such as eyes, kidneys, nerves, heart and blood vessels, which seriously endangers human health. Traditional Chinese medicine (TCM) has outstanding clinical advantages in the prevention and treatment of diabetes, with distinctive characteristics of diagnosis and treatment. TCM has a long history of understanding diabetes. In recent years, researches on etiology, pathogenesis and treatment have gradually deepened and made certain progress. As early as in the Huangdi Neijing, there is a record of "If people’s lung, spleen viscera (stomach), liver, kidney are weak, they can easily get diabetes". This paper collected and summarized ancient literatures relating to diabetes. According to the basic construction principle of "integration-authenticity-innovation" in the theoretical framework of TCM,the causes of the disease include liver weakness and failure in dredging and regulation,emotional disorder, endogenous heat and dryness, heart weakness, heart Yin depletion, endogenous dryness and heat, spleen deficiency and failure in transportation and transformation, lung injury caused by dryness and heat, dysfunction of body fluid distribution, kidney Yin deficiency, astringent weakness, insufficient Qi transformation for maintaining body fluid metabolism balance. In general, Yin deficiency is the root cause, and dryness and heat are the appearance. A framework for the treatment of diabetes was built with five organs as spatial dimension. This paper summarizes the etiology and pathogenesis of diabetes caused by viscera deficiency, with the treatment of diabetes with the five internal organs as the basic framework. The main methods of treating diabetes are relieving Qi stagnancy in liver, regulating the mood,clearing the heart fire, Yin-enriching and dryness-moistening, reinforcing spleen Yang, invigorating spleen and removing dampness, clearing lung and moistening dryness, nourishing Yin, kidney Yin, warming kidney and promoting Yang. A clear and hierarchical theoretical framework was built as a bridge between the theory of TCM and clinical practice of TCM, in the expectation to provide new ideas for the study of ancient TCM literatures and the treatment of diabetes mellitus with TCM, and contribute to the inheritance and development of TCM theory.

Atractylodes macrocephala, belonging to Compositae family, whose dry rhizome as its medicinal parts, is a famous traditional Chinese medicine, with the function of tonifying spleen, aesthetic improvements and so on. Its main medicinal ingredient is volatile oil, which has obviously effect of antitumor. Recently, the market demand of this herb gradually increased, while its quality has not been guaranteed. This paper concluded the process of biosynthesis and transformation of sesquiterpenoids, an important secondary metabolite in violate oil; And firstly reviewed the factors affecting the accumulation of volatile oil in A. macrocephala in the aspects of inheritance and ecology. It is expected to provide a reference for the further study of the biosynthesis and biotransformation of sesquiterpenoids in A. macrocephala and help to improve the yield and quality of the volatile oil.

It is recognized that prenatal care plays an important role in reducing adverse birth.Chinese pregnant women with medical condition were required to seek additional health care based on the recommended at least 5 times health care visits.This study was to estimate the association between prenatal care utilization (PCU) and preterm birth (PTB),and to investigate if medical conditions during pregnancy modified the association.This population-based case control study sampled women with PTB as cases;one control for each case was randomly selected from women with term births.The Electronic Perinatal Health Care Information System (EPHCIS) and a questionnaire were used for data collection.The PCU was measured by a renewed Prenatal Care Utilization (APNCU) index.Logistic regression models were used to estimate odds ratios (OR) and the 95% confidence interval (95% CI).Totally,2393 women with PTBs and 4263 women with term births were collected.In this study,695 (10.5%) women experienced inadequate prenatal care,and 5131 (77.1%) received adequate plus prenatal care.Inadequate PCU was associated with PTB (adjusted OR:1.41,95% CI:1.32-1.84);the similar positive association was found between adequate plus PCU and PTB.Among women with medical conditions,these associations still existed;but among women without medical conditions,the association between inadequate PCU and PTB disappeared.Our data suggests that women receiving inappropriate PCU are at an increased risk of having PTB,but it does depend on whether the woman has a medical condition during pregnancy.

RESUMEN

<p><b>OBJECTIVE</b>To compare the difference of the therapeutic effect of acupuncture and western medicine on the patients with vertebral-basilar insufficiency (VBI), and investigate its mechanism.</p><p><b>METHODS</b>According to the different velocity of blood flow recorded by Transcranial Doppler (TCD), sixty patients with VBI were divided into an acupunture group (31 cases) and a western medicine group (29 cases). In the acupuncture group, the patients were punctured at Fengchi (GB 20) and "Gongxue" (Extra). In the western medicine group, the patients were orally given the Flunarizine hydrochlorid capsules. Before and after two-week treatment, the clinical symptoms and related data of TCD were compared, analyzed and evaluated.</p><p><b>RESULTS</b>The clinical symptoms were obviously improved in the acupuncture group, which was better than the western medicine group (P < 0.05). Acupuncture not only could up-regulate the velocity of vertebral-basilar blood flow (VBF) on the patients with slower VBF of peak-systolic phase (Vs), end-diastolic phase (Vd) and mean value (Vm), but also reduced the VBF on the patients with faster Vs, Vd and Vm. In comparison of the data before and after treatment, there were significantly differences in two groups (P < 0.05), however, there were no difference between the two groups (all P > 0.05). On the index of vascular pulsation (PI), resistance index (RI) and Vs/Vd, there were no difference in both groups before and after treatment (all P > 0.05).</p><p><b>CONCLUSION</b>Acupuncturing at Fengchi (GB 20) and "Gongxue" has bidirectional and beneficial function of regulation on the VBF, but no active role on the compatibility of vertebral-basilar blood vessel.</p>
