RESUMEN

Opuntia ficus-indica has traditionally been used in prevention and treatment of various diseases such as diabetes mellitus. The current study was performed to determine whether Opuntia ficus-indica is associated with diabetes. Diabetic rat models were induced with streptozotocin (STZ). This study divided rats into 1 day (short-term) and 4 consecutive weeks (long-terms) of daily administration. These groups were subdivided into four groups each other for assessment of blood glucose level as follows: Group 1, untreated rats given distilled water; Group 2, untreated rats given Opuntia ficus-indica; Group 3, STZ-induced diabetic rats given distilled water; Group 4, STZ-induced diabetic rats given Opuntia ficus-indica. Blood glucose level was measured for one day and four weeks. In addition, serum markers of alanine aminotransferase (ALT), aspartate transaminase (AST), cholesterol, and creatinine were determined, and total protein triglycerides were measured at four weeks. Blood glucose level was highest in both groups (Group 3 and Group 4) at 30 minutes and two weeks and gradually decreased in a time-dependent manner. The difference in blood glucose among the four groups was significant (p < 0.05). Additionally, the levels of ALT, AST and triglycerides were significantly decreased by Opuntia ficus-indica.

BACKGROUND AND OBJECTIVES: The aim of the present study was to assess the feasibility of a PC-based facial asymmetry assessment program (PC-FAAP) and to compare the results of PC-FAAP with subjective regional scoring by raters in acute unilateral peripheral facial nerve paralysis (FNP). SUBJECTS AND METHODS: Participants were divided into 3 groups with 8 participants per group: group I, normal; group II, mild to moderate FNP; and group III, severe FNP. Using the PC-FAAP, the mouth asymmetry ratio (MAR), eyebrow asymmetry ratio (EAR), and complete eye closure asymmetry ratio (CAR) were calculated by comparing the movement of tracking points on both sides. The FNP grading scale (FGS) integrated each score, and the scores were weighted with a ratio of 5:3:2 (MAR:CAR:EAR). Subjective regional scoring was measured on a 0-100 scale score by three otologists. PC-FAAP and subjective scoring were compared in each group regarding the consistency of the results. RESULTS: The mean scores of the MAR, EAR, CAR, and FGS of each group were significantly different. PC-FAAP showed significant differences between the three groups in terms of MAR, EAC, CAR, and FGS. PC-FAAP showed more consistent results than subjective assessment (p < 0.001). The PC-FAAP was significantly more consistent in group I and group III (p < 0.001 and p=0.002, respectively). FGS in group III was the only parameter that showed a more consistent result in PC-FAAP than the subjective scoring (p=0.008). CONCLUSIONS: An FNP grading system using a PC-based program may provide more consistent results, especially for severe forms.

BACKGROUND/AIMS: Leptin is associated with metabolic disorders, which predispose one to non-alcoholic fatty liver disease (NAFLD). The role of leptin in NAFLD pathogenesis is not fully understood. We aim to investigate the association between serum leptin level and severity of NAFLD using U.S. nationally representative data. METHODS: Data were obtained from the United States Third National Health and Nutrition Examination Survey. NAFLD was defined by ultrasound detection and severity of hepatic steatosis in the absence of other liver diseases. The severity of hepatic fibrosis was determined by NAFLD fibrosis score (NFS). We used multivariate survey-weighted generalized logistic regression to evaluate the association between leptin level and the degree of NAFLD. We also performed subgroup analyses by body mass index (lean vs. classic NAFLD). RESULTS: Among 4,571 people, 1,610 (35%) had NAFLD. By ultrasound findings, there were 621 people with mild, 664 with moderate, and 325 with severe steatosis. There were 885 people with low NFS ( 0.676, advanced fibrosis). Leptin levels for normal, mild, moderate and severe steatosis were 10.7±0.3 ng/mL, 12.1±0.7 ng/mL, 15.6±0.8 ng/mL, 16±1.0 ng/mL, respectively (trend P-value < 0.001). Leptin levels for low, intermediate, and high NFS were 11.8±0.5 ng/mL, 15.6±0.8 ng/mL, 28.5±3.5ng/mL, respectively (trend P-value < 0.001). This association remained significant even after adjusting for known demographic and metabolic risk factors. In the subgroup analysis, this association was only prominent in classic NAFLD, but not in lean NAFLD. CONCLUSIONS: Serum leptin level is associated with the severity of NAFLD, especially in classic NAFLD patients.

RESUMEN

Headaches are one of the most common conditions associated with temporomandibular disorder (TMD). In the present paper, we evaluated the relationship between headache and TMD, determined whether headache influences the symptoms of TMD, and reported two cases of TMD accompanied by headache. Our practical experience and a review of the literature suggested that headache increases the frequency and intensity of pain parameters, thus complicating dysfunctional diseases in both diagnostic and treatment phases. Therefore, early and multidisciplinary treatment of TMD is necessary to avoid the overlap of painful events that could result in pain chronicity.

RESUMEN

Since the first human cancer cell line, HeLa, was established in the early 1950s, there has been a steady increase in the number and tumor type of available cancer cell line models. Cancer cell lines have made significant contributions to the development of various chemotherapeutic agents. Recent advances in multi-omics technologies have facilitated detailed characterizations of the genomic, transcriptomic, proteomic, and epigenomic profiles of these cancer cell lines. An increasing number of studies employ the power of a cancer cell line panel to provide predictive biomarkers for targeted and cytotoxic agents, including those that are already used in clinical practice. Different types of statistical and machine learning algorithms have been developed to analyze the large-scale data sets that have been produced. However, much work remains to address the discrepancies in drug assay results from different platforms and the frequent failures to translate discoveries from cell line models to the clinic. Nevertheless, continuous expansion of cancer cell line panels should provide unprecedented opportunities to identify new candidate targeted therapies, particularly for the so-called "dark matter" group of cancers, for which pharmacologically tractable driver mutations have not been identified.

BACKGROUND AND OBJECTIVES: This study aimed to review the characteristics of microbiology of deep neck abscess and to recommend adequate empirical antibiotics. SUBJECTS AND METHOD: Medical records of the 71 patients who underwent surgical drainage for deep neck abscess were reviewed retrospectively. We analyzed correlations between age, gender, etiology and diabetes mellitus and the cultured organism with respect to susceptibility of antibiotics. RESULTS: The leading pathogens were Streptococcus viridians group (18.3%), Klebsiella pneumonia (K. pneumonia, 15.5%), Staphylococcus aureus (S. aureus, 15.5%) and alpha-hemolytic Streptococcus (12.7%). The most common source of infection had a dental origin. Streptococcus viridians group was associated with infections of dental origin and K. pneumonia infection was over-represented in the diabetic group compared to the non-diabetic group. Antibiotics susceptibility test showed that four leading pathogens were susceptible to ampicillin/sulbactam. The rate of methicillin-resistant S. aureus accounting for S. aureus infections was only 18.2%. CONCLUSION: Only ampicillin/sulbactam without other antibiotics was sufficient to treat deep neck abscess empirically. Thus we suggest that other antibiotics that target anaerobes or 3rd generation cephalosporin are not necessary for the empirical treatments of deep neck abscess.

RESUMEN

A 73-year-old woman who presented with chest discomfort visited the emergency room. The 12-lead electrocardiography showed ST-segment elevation in II, III, and lead augmented vector foot. Emergent coronary angiography revealed a thrombus in the distal right coronary artery. Percutaneous coronary angioplasty with a stent was performed. After the procedure, persistent dyspnea occurred. Her chest computed tomography (CT) showed occlusion of the bilateral pulmonary artery. We could not find a source for the embolization or a hypercoagulable state. We started a course of dual antiplatelets with oral anticoagulants. Pulmonary embolism was resolved at follow-up chest CT scan. Aspirin, clopidogrel and warfarin were given for 6 months. The patient is doing well now 1 year after the episode.

Growing numbers of studies employ cell line-based systematic short interfering RNA (siRNA) screens to study gene functions and to identify drug targets. As multiple sources of variations that are unique to siRNA screens exist, there is a growing demand for a computational tool that generates normalized values and standardized scores. However, only a few tools have been available so far with limited usability. Here, we present siMacro, a fast and easy-to-use Microsoft Office Excel-based tool with a graphic user interface, designed to process single-condition or two-condition synthetic screen datasets. siMacro normalizes position and batch effects, censors outlier samples, and calculates Z-scores and robust Z-scores, with a spreadsheet output of >120,000 samples in under 1 minute.

Nitric oxide (NO) regulates proliferation, differentiation and survival of neurons. Although NO is reported to involve in NGF-induced differentiation of PC12 cells, the role of NO has not been characterized in primary neuron cells. Therefore, we investigated the role of NO in neuronal differentiation of primary cortical neuron cells. Primary cortical neuron cells were prepared from rat embryos of embryonic day 18 and treated with NMMA (NOS inhibitor) or PTIO (NO scavenger). Neurite outgrowth of neuron cells was counted and the mRNA levels of p21, p27, c-jun and c-myc were measured by RT-PCR. Neurite outgrowth of primary cortical neuron cells was inhibited a little by NOS inhibitor and completely by NO scavenger. The mRNA levels of p21 and p27, differentiation-induced growth arrest genes were increased during differentiation, but they were decreased by NOS inhibitor or NO scavenger. On the other hand, the level of c-jun mRNA was not changed and the level of c-myc mRNA was increased during differentiation differently from previously reported. The levels of these mRNA were reversed in NOS inhibitor- or NO scavenger-treated cells. The level of nNOS protein was not changed but NOS activity was inhibited largely by NOS inhibitor or NO scavenger. These results suggest that NO is an essential mediator for neuronal differentiation of primary cortical neuron cells.

OBJECTIVE: To find out whether the footprint test was significant as a screening test in hallux valgus (HV) patients. METHOD: The standing AP radiography and the static footprint were performed on 26 general patients, 52 feet total, ranging in age from 15~70 years. As markers of the footprint test, the angles that we want to measure were designated as H and H' angles. RESULTS: The HV angle on plain radiography correlated significantly to the H angle of the footprint test (r=0.75), and presented as HV angle=1.012H+2.723. The HV angle on plain radiography inversely correlated significantly to the H' angle (r=-0.56) and presented as HV angle=-0.731H' +90.953. CONCLUSION: The footprint test could be used as a screening test for hallux valgus prior to plain radiography. HV angle in standing AP radiograph could be predicted by measuring the H and H' angle on the static footprint. Supplement studies on variable factors such as age, gender, etc. are required for further study.

RESUMEN

Early reduction and stabilization of the pelvic bone after traumatic unstable pelvic bone fracture is the most effective method to control life-threatening bleeding and reduce late complication. In this study, we compared the effect of the non-invasive pelvic belt on various positions, and studied the effects of weight bearing and shifting during walking on pelvic belt position. Case 1) separation of symphysis pubis; Case 2) fracture of the superior and inferior ramus of the right pubis; Case 3) fractures of the superior and inferior ramus of both pubis and separation of symphysis pubis. Pelvic belt was prescribed in all three cases. Pelvic belt reduction reduced the distance between fracture line by 40~60% at all levels. Therefore, pelvic belt reduction is recommended as a non-invasive, safe and effective rehabilitative treatment for pelvic bone fracture patients to restore function without pain or secondary injury.

BACKGROUND: Many countries have been legally prescribing graphic health warning labels on cigarette package as a part of their national policy of smoking cessation. This study was designed to evaluate the effectiveness and appropriateness of graphic health warning labels of Canada, Singapore and European union, in Korean for smoking cessation. METHODS: From February to July in 2005, we surveyed 110 people who were high school students and college students. After showing them the 64 graphic health warning labels (16 from Canada, 6 from Singapore and 42 from European union), the self recorded questionnaires were collected. RESULTS: The effectiveness for smoking cessation was related to the arousal levels of visual effects and it had the same result for each country in which we researched. The high arousal loss-framed graphic health warning labels were more effective than the gain-framed low arousal ones. CONCLUSION: To quit smoking, it can be reasonably concluded that high and negative images of health that were shown on warning labels of cigarette packages were effective. Hereafter, it is recommended more useful and proper designs of graphic warning labels be developed and applied.

Ascorbic acid has been reported to extend replicative life span of human embryonic fibroblast (HEF). Since the detailed molecular mechanism of this phenomenon has not been investigated, we attempted to elucidate. Continuous treatment of HEF cells with ascorbic acid (at 200 micrometer) from 40 population doubling (PD) increased maximum PD numbers by 18% and lowered SA-beta-gal positive staining, an aging marker, by 2.3 folds, indicating that ascorbic acid extends replicative life span of HEF cells. Ascorbic acid treatment lowered DCFH by about 7 folds and Rho123 by about 70%, suggesting that ascorbic acid dramatically decreased ROS formation. Ascorbic acid also increased aconitase activity, a marker of mitochondrial aging, by 41%, indicating that ascorbic acid treatment restores age-related decline of mitochondrial function. Cell cycle analysis by flow cytometry revealed that ascorbic acid treatment decreased G1 population up to 12%. Further western blot analysis showed that ascorbic acid treatment decreased levels of p53, phospho-p53 at ser 15, and p21, indicating that ascorbic acid relieved senescence-related G1 arrest. Analysis of AP (apurinic/apyrimidinic) sites showed that ascorbic acid treatment decreased AP site formation by 35%. We also tested the effect of hydrogen peroxide treatment, as an additional oxidative stress. Continuous treatment of 20 micrometer of hydrogen peroxide from PD 40 of HEF cells resulted in premature senescence due to increased ROS level, and increased AP sites. Taken together, the results suggest that ascorbic acid extends replicative life span of HEF cells by reducing mitochondrial and DNA damages through lowering cellular ROS.

RESUMEN

Multiple sclerosis (MS) is a demyelinating disorder that affects discrete areas of the CNS, including the optic nerves, in a quite variable relapsing-remitting fashion over a prolonged period of time. Although MS is usually considered to be a disease that affects peoples in early to middle adulthood, children do develop multiple sclerosis. The frequency of MS onset before the age of 15 years is 2.7-5% of all cases, while MS onset during infancy and early childhood was observed to be 0.2- 0.7% of all cases. We report here on a Korean case of a relapsing-remitting MS female child who was treated with four rounds of intravenous methylpredinsolone pulse therapy and preventive Interferon-beta-1b (Betaferon(R)).

RESUMEN

Perforation of the appendix into the neighboring viscera is an uncommon sequel to acute appendicitis, and the persistence of the fistula between the appendix and a viscus is even rarer. Although a variety of causes such as foreign body ingestion and regional enteritis may be responsible for persistent appendiceal enteric fistula, the cause for these unusual lesions in our experience appeared to be antecedent appendicitis. Generally, the incidence of persistent appendiceal enteric fistula is very rare and the progress of disease is quite different from the so-called 'usual' acute appendicitis. The diagnosis might lean the wrong way such as towards colitis or cystitis, and so surgical intervention is delayed. Treatments of perforation are diversion (colostomy), as is shown above or conservative treatment such as drainage only.

PURPOSE: Because voiding cystourethrography(VCUG) is an invasive method, we studied whether VCUG could be postponed through evaluation of alternative non-invasive tests including renal ultrasonography and (99m)Tc-DMSA renal scan. METHODS: We reviewed the medical records of 175 patients initially diagnosed with febrile urinary tract infection during the one year period of 1999, and compared 3-tests:renal ultrasongraphy, (99m)Tc-DMSA renal scan, and VCUG. RESULTS: Renal ultrasonography didn't contribute to the prognostication of pyelonephritis(photopenic areas) or vesicoureteral reflux(VUR). Presentation of photopenic areas in (99m)Tc-DMSA renal scan was related to VUR. If both findings of renal ultrasonography and (99m)Tc-DMSA renal scans were normal, this condition was closely related to normal results in VCUG. And if both examinations were abnormal, the condition was closely related to VUR. But this state could not always guarantee the normal result from VCUG because of low sensitivity in finding VUR. CONCLUSION: In cases in which acute phyelonephritis is demonstrated by (99m)Tc-DMSA renal scan, VCUG is required. In addition to this, if the conditions of hydronephrosis, vesicoureteral dilatation, increases of renal volume, and changes of echogenesity are shown by renal ultrasonography, VCUG should be performed. If a patient has difficulty undergoing VCUG, temporary postponement of VCUG can be taken into consideration, but only in cases where both examinations of renal ultrasonography and (99m)Tc-DMSA renal scan are normal. Nevertheless, close observation is be advised even in this case.

RESUMEN

Primary torsion of the greater omentum is a rare cause of an acute abdomen in children, which often mimic other acute abdominal conditions. Resection of the infarcted segment is the treatment of choice. Here, a case of torsion of the greater omentum, diagnosed with preoperative computed tomography and subsequently managed by laparoscopic surgery, is reported. The patient was discharged three day after surgery without any complication. Omental torsion can be accurately diagnosed and safely treated with a laparoscopy.

RESUMEN

Total auricular reconstruction with autogenous tissue remains one of the greatest technical challenges for reconstructive plastic surgeons because of the ear's complex morphology with delicately convoluted cartilages and very thin skin. In a successfully created ear, a natural three-dimensional illusion visualized from the patient's profile, frontal, and posterior views is crucial. Accordingly ear framework should have adequate lateral aspect as well as suitable frontal aspect even before being lifted to this purpose. For this goal, rib cartilage should be harvested from three-dimensionally adequate area. It is the most essential point in framework fabrication that the baseblock should have semi-cup curvature via multiple parallel cuts made on its medial surface. Between January 1999 and May 2003, we performed 29 cases of total ear reconstruction with autogenous rib cartilage graft using this scoring technique and obtained satisfactory results, which showed more natural appearance visualized from the patient's profile, frontal, and posterior views.

PROPOSE: In this study, we evaluated whether powder on surgical gloves is a cause of postpuncture backpain in children. MOTHODS: In 164 children with meningitis between July and September 1997, we did not remove powder from surgical gloves. However, in 149 children with menigitis between May and October 2001 the powder was removed from the surgical gloves. RESULTS: Out of the 164 patients in 1997, 41 cases(25.00%) were found to have postdural puncture backpain. On the other hand, out of 149 patients in 2001, with whom we used gloves from which the powder was removed, we found only 8 patients(5.36%) with postdural puncture backpain. CONCLUSION: We conclude that the powder on surgical gloves is one of the main causes of postdural puncture backpain in children.
