RESUMEN

BACKGROUND: Procambarus clarkii produces high-quality, delicious meat that is high in protein, low in fat, and rich in calcium and phosphorus. It has become an important aquatic resource in China. Our objectives are (i) to analyze the level of genetic diversity of P. clarkii populations; (ii) to explore the genetic differentiation (Gst); and (iii) to propose appropriate strategies for the conservation. RESULTS: In this study, Shannon's index (I) and Nei's gene diversity index (H) for P. clarkii were high (I = 0.3462 and H = 0.2325 on average and I = 0.6264, H = 0.4377 at the species level) based on the SSR markers. The expected heterozygosity value of 17 microsatellite loci in 25 crayfish populations was 0.9317, the observed heterozygosity value was 0.9121, and the observed number of alleles per locus was 2.000; and the effective number of alleles per locus was 1.8075. Among the P. clarkii populations, the inbreeding coefficient within populations (Fis) was 0.2315, overall inbreeding coefficient (Fit) was 0.4438, genetic differentiation coefficient among populations (Fst) was 0.3145 and gene differentiation (Gst) was 0.4785 based on SSR analyses. The cluster analysis results obtained by unweighted pair-group method with arithmetic mean (UPGMA) analysis, principal coordinate analysis (PCoA) and STRUCTURE analysis were similar. A mantel test showed that the isolation-by-distance pattern was not significant. CONCLUSIONS: The high Gst among P. clarkii populations is attributed to genetic drift and geographic isolation. The results indicated that more P. clarkii populations should be collected when formulating conservation and aquaculture strategies.

Objective :To explore relationship among levels of C reactive protein (CRP) ,mean platelet volume (MPV) and occurrence of no-reflow (NR) during primary percutaneous coronary intervention (PCI) in patients with acute ST elevation myocardial infarction (STEMI).Methods :Clinical and imaging data of 156 STEMI patients ,who received primary PCI from Jan 2015 to Dec 2016 ,were retrospectively analyzed .According to presence of NR during PCI ,patients were divided into NR group (n=54) and no NR control group (n=102).Multi-factor Logistic regression analysis was used to analyze influencing factors of NR occurrence .Results :Compared with no NR control group ,there were significant rise in percent-age of diabetes mellitus (23.5% vs.42.6%) ,levels of CRP [(31.7 ± 5.3)mg/Lvs.(64.5 ± 9.7)mg/L]and MPV [(9.31 ± 1.05)fl vs.(11.29 ± 0.98)fl] ,number of stents [(1.9 ± 0.4) vs.(2.1 ± 0.6)] and symptom-onset-to-balloon [SOTB , (254 ± 96)min vs.(315 ± 108)min] in NR group , P<0.05 or <0.01. Multi-factor Logistic regression analysis indicated that diabetes mellitus ,SOTB ,levels of CRP and MPV were independent risk factors for NR occurrence (OR=2.335~3.670 ,P<0.05 or <0.01).Conclusion :Rise of CRP and MPV levels are independent risk factors for no-reflow during primary PCI in STEMI patients ,which possess certain predictive value for no-reflow occurrence and worth extending .

Arthritis is a common chronic disease characterized by the destruction of joint cartilage and inflammation in the sur-rounding tissues. Although it is known that the pathogenesis of arthritis is influenced by a series of factors, the underlying mechanisms remain unclarified. Recently, increasing attention has been paid to the increase of matrix metalloproteinases (MMPs) in articular cartilage,resulting in an inevitable degra-dation of cartilage and extracellular matrix (ECM). MMP-13, the major functioning enzyme during arthritis development,plays a vital role in the cartilage destruction, thus contributing to the decomposition of type Ⅱ collagen irreversibly. A variety of cellu-lar cytokines such as IL-1β and TNF-α,and Runx2 are assumed to affect the expression of MMP-13 in chondrocytes. The hypom-ethylation of the promoter region of the MMP-13 may induce its expression, while it can be reduced by inhibiting histone acety-lation. Meanwhile, microRNA can reduce the expression of MMP-13. In conclusion, MMP-13 can be used as an important therapeutic target in arthritis. In this review, we focus on the role of MMP-13 in arthritis and its underlying regulatory mecha-nisms.

Bladder cancer is one of the malignant tumors that seriously endanger human health, but its diagnosis and treatment progress are relatively slow. With the rapid development of molecular biology and the continuous emergence of biological detection technology, the molecular typing of bladder cancer through gene analysis is expected to become an important means to improve the diagnosis and treatment of bladder cancer. In this paper, we summarized the recent advances in the molecular typing of bladder cancer and its clinical significance in predicting drug responsiveness and judging prognosis.

Objective To study the asymmetry of medulla on the sectional plane to support the way of localizing gyris by the neurapophysis,and to enrich the anatomical knowledge about the telencephalic sectional plane.Methods Slected 30 adult normal head specimens.Head specimens saw cutting were made through the planum sphenoidale till the surface of the brain were marked.Colored the different gyris with different colors so as to distinguish.Then cut the brain parallel to the marked line to make brain slices of 6 mm.Observed the corresponding neural process by different staining of brain slices.The number and direction of neurapophysis branches of the corresponding neurapophysis were observed.And these data were statistically analyzed and processed.Results On the telencephalic sectional planes,most of the corresponding neurapophysis of gyrus were symmertrical in the number and direction of neurapophysis branches.Conclusion It is reliable and practical to localize gyris with neurapophysis on telencephalic sectional planes.

RESUMEN

<p><b>OBJECTIVE</b>To observe the early cardiovascular complications of patients underwent lung transplantation.</p><p><b>METHODS</b>The clinical records of 73 patients who underwent lung transplantation in Wuxi People's Hospital from September 2002 to September of 2010 were retrospectively analyzed. All patients were transferred to intensive care unit (ICU) after lung transplantation, received invasive monitoring (PICCO), mechanical ventilation, immunosuppressive therapy and measures to prevent ischemic reperfusion injury. The early cardiovascular complications after lung transplantation up to discharge from hospital were observed.</p><p><b>RESULTS</b>The postoperative mortality was 20.5% (15/73) within 30 days after surgery. Five patients died of cardiovascular reasons including 2 cases of pulmonary embolism and 3 cases of ventricular fibrillation. Cardiovascular complications during the early post-operation period included: paroxysmal atrial fibrillation (19 cases, 26.0%) and persist atrial fibrillation (1 case, 1.4%); atrial fibrillation and atrial flutter in 3 cases and persistent atrial flutter in 1 patient; ventricular fibrillation (3 cases, 4.1%); paroxysmal supraventricular tachycardia (3 cases, 4.1%); ventricular tachycardia (2 cases, 2.7%); bundle branch block (8 cases, 11.0%); intraventricular block(4 cases, 5.5%); left ventricular heart failure(4 cases, 5.5%), right heart failure(6 cases, 8.2%); pulmonary embolism (2 cases, 2.7%), deep venous thrombosis (1 case, 1.4%).</p><p><b>CONCLUSIONS</b>Atrial fibrillation is the most common cardiovascular complication post lung transplantation. Pulmonary embolism and ventricular fibrillation are not common but related with high mortality rate post lung transplantation.</p>

RESUMEN

Physician payment system (PPS) is a principal incentive system to motivate doctors to provide excellent care for patients. During the past decade, physician remuneration in China has not been in proportional to physician's average work load and massive responsibilities. This paper reviewed the constitution of the PPS in China, and further discussed the problems and issues to be addressed with respect to pay for performance. Our study indicated that the lower basic salary and bonus distribution tied to "profits" was the major contributor to the physician's profit-driven incentive and the potential cause for the speedy growth of health expenditures. We recommend that government funding to hospitals should be increased to fully cover physicians' basic salary, a flexible human resource and talent management mechanism needs to be established that severs personal interest between physicians and hospitals, and modern performance assessment and multiplexed payment systems should be piloted to encourage physicians to get the more legitimate compensation.

RESUMEN

In 2005, an avian influenza virus stain was isolated from Parrot in Guangdong, which was then genotyped as H5N2 subtype and designated as A/Parrot/Guangdong/268/2005. According to the current OIE definition on the low-pathogenicity of avian influenza virus, the strain was recognized as a low pathogenic avian influenza virus due to the presence of one basic amino acid residue at the HA cleavage site. Some molecular characteristics of the virus, such as potential glycosylation sites in HA and NA, receptor binding sites of HA, and drug resistance site of NA, showed no variations. To analyze molecular evolution of this strain, we selected the sequences of H5N2 subtype AIVs from GenBank and established the phylogenetic trees. Our results indicated that this strain shared the highest homologies with the H5N2 LPAI isolate A/Pheasant/NJ/1355/1998-like. Phylogenic analysis revealed the isolate, together with A/Chicken/Pennsylvania/1/1983 (H5N2), belonged to America lineages and clustered with A/Pheasant/NJ/1355/1998-like.

Objective To evaluate the effect of three diamension-digital subtraction angiography (3D-DSA) or computed tomography angiography (CTA) on the patients with ruptured multiple intmcranial aneurysm (MIA). Methods A retrospective study on 21 patients with MIA was performed.After scanning with 3D-DSA or 3D-CTA, three-dimensional reconstruction of MIA was carried out by 3D workstation,then the diagnosis was decided and the treatment plan (endovascular treatment or microsurgery) was selected according to stereoscopic image of MIA. Results (1) 3D-DSA or CTA was performed in 21 patients with subarachnoid hemorrhage (SAH),it was revealed these patients carried with 48 aneurysms,including 35 small aneurysms (25 mm).Not only miero-aneurysms and small aneurysms could be precisely showed,also the size of aneurysmal neck,the relationship of the aneurysm and the parent vessel and contiguous branches by stereoscopic image.(2) According to the standard of classification,9 patients with MIA for gradeⅠ(42.9%),10 for gradeⅡ(47.6%),2 for gradeⅢ(9.5%),0 for gradeⅣ.Endovascular treatment was selected prior to microsargery for those high grade patients.In this group,17 patients with 40 aneurysms underwent endovascular embolotherapy with GDC coils.Twenty four anemysms were completely occlusioned,12 beyond 90%,4 were left without treatment because of their small size.In microsurgery group,3 aneurysrus were totally clipped,1 could not be found during operation.No any treatment was accepted in 2 patients with 4 aneurysms. Conclusions 3D-DSA or CTA,which is very useful for the diagnosis and treatment of MIA,could improve the accuracy of diagnosis of MIA and clearly show the stereoscopic image of MIA,also the relation of sac and parent artery.For those patients with high grade MIA,endovascular treatment was selected prior to microsurgery,pro re nata,used to combine with mierosurgery.

Objective To explore the syndrome of vital energy deficiency and blood stasis of CHF rat model. Methods The Wistar male rats were deligated the main stem of left coronary artery. The succeeded twenty rats were cut the diet half from the second week, and swam exhausted every day, compared with normal group. After four weeks, the general condition, heart function, heart construction and NE, PRA, AngⅡ,TXB2, PAI-1 were observed.
