RESUMEN

Objective:To observe the efficacy of Yuebi Jia Banxiatang on old patients with community acquired pneumonia (CAP) combined with syndrome of phlegm heat damming lung, and observe effect on inflammatory markers and prognosis. Method:A total of 120 patients with CAP were randomly divided into control group (60 cases) and observation group (60 cases) by random number table. In control group, 55 patients finished the therapy (3 patients fell off or were lost to follow-up, 2 were eliminated), 55 patients in observation group completed the therapy (5 patients fell off or were lost to follow-up). Both groups' patients got anti-infection and other comprehensive therapies. Patients in control group got Feilike mixture, 20 mL/time, 3 times/day. Patients in observation group obtained Yuebi Jia Banxiatang, 1 dose/day. The course of treatment for the two groups continued for 10 days. Antipyretic time and antipyretic rate at the 5th day after treatment, relief time of cough, expectoration and lung rale were recorded. And clinical pulmonary infection score (CPSI), time (CPSI<6) and time to stop antibiotics were recorded. Before and after treatment, syndrome of phlegm heat damming lung was scored, and failure of initial treatment was also recorded. Levels of procalcitonin (PCT), red blood cell volume distribution wid (RDW), D-dimer (D-D) and synpeptin, high sensitivity C-reactive protein (hs-CRP), tumor necrosis factor-α (TNF-α), interleukin (IL-1β) and IL-6 were measured. Recurrence rate of chest radiograph was recorded, and safety was evaluated. Result:Relief time of antipyretic, cough, expectoration and lung rale in observation group were less than those in control group (P<0.01). Antipyretic rate at the 5th day after treatment, antibiotic discontinuation rate, recurrence rate of chest radiograph were 85.45% (47/55), 94.55% (52/55) and 90.91% (50/55), which were higher than 65.45% (36/55), 81.82% (45/55) and 74.55% (45/55) in control group. And initial treatment failure rate was 5.54% (3/55), which was lower than 20.00% (11/55) in control group (P<0.05). Scores of CPSI and syndrome of phlegm heat damming lung were less than those in control group (P<0.01). And levels of PCT, RDW, D-D, peptide, hs-CRP, TNF-α, IL-1β and IL-6 were all below the level in control group (P<0.01). Total effective rate of traditional Chinese medicine (TCM) syndrome was 96.36% (53/55), which was higher than 83.64% (46/55) in control group (χ2=4.949, P<0.05). And there was no adverse reactions relating to traditional Chinese medicine. Conclusion:In addition to anti-infection and other comprehensive therapies, Yuebi Jia Banxiatang can control the clinical symptoms, reduce the degree of pulmonary infection and disease, control the inflammatory reaction, shorten the course of disease and improve the prognosis, with a significant effect and safety in clinical use.

Objective To investigate the effect of microecological therapy on rotavirus ( RV)-induced diarrhea and infants’ immunologic function in infants. Methods 150 infants with RV diarrhea were randomly divided into the ex-perimental group and the control group. The control group was given the treatment of montmorillonite powder. The experimental group was given live combined bifidobacterium and lactobacillus tablets in addition to montmorillonite powder. The clinical efficacy and the improvement of clinical symptoms were compared between the two groups. RV antigen and immunologic function were detected with colloidal gold method. Immune function was detected by flow cytometry and ELISA. Results The total effective rate of the experimental group was 90.7%,and it was significant-ly higher than that of the control group which was 74.7%(P<0.05). Compared with the control group, the antidi-arrheal time, fever clearance time and disappearance time of vomiting symptoms in the experimental group were shorter ( P<0.05) . The negative conversion rate of RV virus antigen in the experimental group after 1 and 2 weeks treatment was significantly higher than that in the control group ( P<0.05) . After 3 weeks treatment, RV virus antigen in the experimental group all turned negative, but the control group still had a positive rate of 12.0% ( P<0.05) . The peripheral blood CD3+, CD4+and the ratio of CD4+/CD8+in the experimental group after treatment were significantly higher than those before treatment and those in the control group after treatment ( P<0.05) , the same phenomenon was found in serum immunoglobulin A ( IgA) and IgM ( P<0.05) . No adverse events occurred during the treatment period in the two groups. Conclusions Microecological therapy has an exact curative effect and high safety in the treatment of infantile RV diarrhea, it could effectively improve the immunologic function of the infants.

Objective :To explore correlation between diabetic peripheral neuropathy (DPN) and cardiovascular disea-ses (CVD) in patients with type 2 diabetes mellitus (T2DM).Methods :A total of 375 T2DM patients ,who were treated in our community health service center from Jan 2014 to Jul 2016 ,were selected .Demographic characteris-tics ,systolic blood pressure (SBP) ,blood lipids ,blood glucose ,electromyogram and CVD were collected in all sub-jects using questionnaire .According to complicated CVD or not ,patients were divided into CVD group (n=54) and non-CVD group (n=321).Correlation among all indexes and CVD risk were analyzed .Multifactor Logistic regres-sion analysis was used to analyze independent risk factors for CVD risk .Results :Among the 375 T2DM patients ,54 cases (14. 4%) were complicated with CVD .Compared with non-CVD group ,there were significant rise in age≥65 years ,abnormal fasting blood glucose (FBG) , abnormal 2h postprandial blood glucose (2hPG) , abnormal glycosy-lated hemoglobin A1c (HbA1c) and SBP in CVD group , P<0.05 or <0.01 .Spearman correlation analysis indica-ted that age ,FBG ,2hPG ,DPN ,SBP and HbA1c were significant positively correlated with CVD risk (r=0.612~0.735 , P=0.001 all).Multifactor Logistic regression analysis indicated that DPN , abnormal FBG and abnormal 2hPG were independent risk factors for CVD (OR=1.248~2.023 , P<0.05 all).Conclusion : Diabetic peripheral neuropathy is one of the independent risk factors for CVD in T2DM patients .Attention should be paid to preven-tion ,screening and early treatment of CVD in these patients in clinic .

Objective To observe the effects of Ligustrazine on proliferation of human retinal vascular endothelial cells (HRCECs) induced by high glucose.Methods HRCECs cells were divided into control (saline) group,model (25 mmol/L glucose) group,ligustrazine low,medium,high concentration (50,100 and 200 mol/L) group,cultured for 48 h.Cell proliferation was detected by MTT cytotoxicity assay.Flow cytometry was used to detect cell cycle,and the expression of VEGF was detected by ELISA analysis.Results Compared with control group,HRCECs cell proliferation,cell division (M) phase ratio and supematant of VEGF concentration in model group increased significantly (P < 0.05,0.01).Compared with model group,cell proliferation,M phase ratio and supematant of VEGF in Ligustrazine low,medium and high concentration groups decreased significantly (P < 0.05,0.01),and showed a concentration dependence.Conclusion Ligustrazine can inhibit the expression of VEGF in HRCECs cells induced by high glucose to block cell cycle and inhibit the proliferation of HRCECs cells.

Objective To investigate the role of SDF-1/CXCR4 axis on the apoptosis of human degenerative nucleus pulposus cells (NPCs) and its potential molecular mechanism .Methods The intervertebral disces tissues from clinical discectomy were divided into normal group and intervertebral disc degeneration ( IVD) group according to Pfirrmann classification.The different expression of SDF 1 and CXCR4 in human IVDs was tested by immunohistochemistry, quantify polymerase chain reaction (q-PCR) and Western blot.The primary degenerative NPCs were primary cultured.The generation Ⅲ~Ⅴ NPCs was treated with 10 ng/mL SDF-1, in the presence of or in the absence of CXCR4 siRNA transfection and 20 μmol/L NF-κB inhibitor (pyrrolidine dithiocar bamate,PDTC).The transfection efficiency and target protein of signal pathway were verified by Western blot , the apoptosis of NPCs were tested by Annexin V /PI, the nucleus transferlocation of P65 from NF-κB were tested by immunofluorescent method.Results SDF-1and CXCR4 were both expressed in all donor tissues, however, there was a significantly increased in the degenerative IVDs .The apoptosis of degenerative NPCs was expedited by SDF -1 stimulation,which was significantly suppressed by CXCR 4 silencing by siRNA (P<0.05).Furthermore, with SDF-1 stimulation,the expressions of phosphorylated P 65 was significantly increased and the P65 perssad transferred to the nucleuses,which could be suppressed by the NF-κB inhibitor, PDTC(P<0.05).Conclusions The expression levels of SDF-1 and CXCR4 are increased in degenerative NP tissue.The SDF-1/CXCR4 axis is considered to induce apoptotic of human degenerative NPCs via the NF-κB signaling pathway.

BACKGROUND: The relationship between obesity and allergic diseases in children of China is unclear. OBJECTIVE: To analysis the relationship between obesity and overweight and the prevalence of allergic diseases and the impact of gender. METHODS: Questionnaire based on those used in National Study of Asthma and Allergies in Childhood in China. The study included 3,327 participants (23.7% aged 2–6 years, 65.8% aged 7–12 years, 10.5% aged 13–14 years) in Wuhan City. Allergic diseases were determined by physicians. RESULTS: Overweight was found in 35.68% of participants (8.96% of 2–6 years old, 32.83% of 7–12 years old, and 48.57% of 13–14 years old), obesity in 12.53% (4.18%, 12.01%, and 4.29%, respectively). Obesity (odds ratio [OR], 1.33) increased the prevalence of allergic rhinitis and atopic dermatitis. Obesity (OR, 1.48) affected the incidence of allergic rhinitis in girls. There was no relationship between body mass index (BMI) and asthma in child from Wuhan City. Obesity and overweight did not affect the frequency of asthma, food allergy, and drug allergy. CONCLUSION: Obesity increased the prevalence of allergic rhinitis and atopic dermatitis in child. Higher BMI was no relationship with the prevalence of asthma, food allergy, and drug allergy.

Objective:To analyze the curative effect of tripterygium on NOD mice and the possible mechanisms.Methods:NOD mice were divided into 2 groups,Group A:tripterygium treatment(0.07 mg/kg,intraperitoneal injection,12 weeks);Group B:saline control.BALB/c mice were enrolled as control group( Group C).Results:After experiment,Group A had lower salivary flow rate than these of Group C,but higher than these of Group B at 12 and 20 weeks old( P<0.05).Group A had higher rate of inflammatory cells apoptosis than these of Group B and Group C(P<0.05).Group A mice had lower levels of TNF-α,IL-6 and IL-1βthan these of Group B(P<0.05),but higher than these of Group C(P<0.05).Group A mice had a higher level of SHIP-1 but a lower level of Mir-155 than these of Group B mice(P<0.05).Group A mice had a better neuroelectrophysiological outcomes than these of Group B mice ( P<0.05).Conclusion:Tripterygium can meliorate the sailoadentitis of NOD mice,which may though activating the SHIP-1/Mir-155 signaling pathway.

RESUMEN

Non-contact measurement is an effective method of long time measurement of human electrocardiograph (ECG) signal. Because the relative position between measuring electrode and human body is not fixed, this method could result in constant changes of ECG signal collection. It often appears ECG signal distorting and weakened in filtering. This paper, using the principal component analysis (PCA) basic theory, proposes a fast adaptive PCA denoising algorithm which can automatically adjust the parameters according to the changes of ECG signal. The experiment proved that PCA denoising could be barely impacted by signal changes and can disposably remove interference signal on the premise of keeping the main features of ECG signal and can prevent ECG signal from being weakened in filtering at the same time.

Objectives To study the age and gender distribution of clinical and non-clinical depressive symptoms in chil-dren, and the potential link between non-clinical and clinical depressive symptoms. Methods 4520 children aged 8-15 years old were screened for mental disorders by Strength and Dififculties Questionnaire (SDQ) and the Development and Well Being Assessment (DAWBA), and further assessed by the Depression Self-Rating Scale for Children (DSSC). Fifty-three children diagnosed as depressive disorder were included in clinical group and 1504 children without depression disorders whose DSSC scores≥14 were included in non-clinical group. The relationship between gender, age and DSSC scores was analyzed. Results Interaction effect on DSSC scores between gender and age was found in clinical group (F=4.40, P=0.049), but not found in non-clinical group (F=1.04, P=0.409). DSSC scores of girls were higher than that of boys in clinical group (t=2.16, P=0.048). There was no statistically signiifcant main effect of gender in non-clinical group (F=0.21, P=0.646). No signiifcant difference in DSSC scores existed among different age groups in clinical group (F=0.82, P=0.573). There was signiifcant main effect of age in non-clinical group (F=1.96, P=0.048). Conclusions There were different age and gender distribution of clinical and non-clinical depressive symptoms in children.

Objective To investigate domestic child abuse within one year before the survey being conducted in Grades 4-6 preliminary school students and explore the relationship between domestic child abuse and mental health problems including prevalence of mental disorders,anxiety,depression as well as children's selfawareness in this sample.Methods Screen Questionnaire of Child Abuse was used to screen all the students participated in the survey.For those students whose screen results were positive,further face to face interview was needed to verify the abuse history.Once a child was identified as abused one,a student without abuse history would be matched by sex and the same age.Both groups with abused history and without abuse history over past year were evaluated by the two child psychiatrists with diagnostic an statistical manual of mental disorder (DSM-Ⅳ) and mental health scales.Results 1500 10-13 year-old children received child abuse screening.23.73% students surveyed over the past year suffered child abuse in the family.The proportion of boys who were victims of domestic child abuse was 30.44% while 17.44% to girls (x2 =5.95,P < 0.01).The proportion of male students who were victims of domestic abuse was two times that of female students.For all participants,14.6% had been suffered one-way abuse,5% for two-way abuse,and 3% for three-way abuse.Prevalence of anxiety disorder in abused students was higher than in non-abused ones (9.26% vs 3.45%,x2 =3.19,P=0.0015).Similarly,the rate of disruptive behavior disorder in abused students was higher than in non-abused ones(18.53%vs 9.22% x2 =3.61,P=0.0001).The total scores of The Screen for Child Anxiety Related Emotional Disorder was higher in abused students group than in non-abused students group while the total scores of Piers-Harris Children's self-concept Scale was lower in abused student group than in non-abused student group.Conclusion Domestic child abuse in 10-13 old-age students is a widespread phenomenon and abuse history over last year is associated with current mental health level.
