RESUMEN

ObjectiveTo study the mechanism of astragaloside Ⅳ (AS Ⅳ) on db/db mice with type 2 diabetes mellitus (T2DM) and non-alcoholic fatty liver disease (NAFLD) based on network pharmacology and experimental validation. MethodA total of 24 db/db mice were randomly divided into four groups: model group, metformin group, and low-dose and high-dose AS Ⅳ groups. Six C57 mice were used as the blank group. The low-dose and high-dose AS Ⅳ groups were given AS Ⅳ of 0.015 and 0.030 g·kg-1 by gavage, and the metformin group was given 0.067 g·kg-1 by gavage. The blank and model groups were given equal volumes of distilled water by gavage. After intragastric administration, fasting blood glucose (FBG) was detected, and an oral glucose tolerance test was performed. Serum lipid level and liver histopathology were detected. The target and enrichment pathway of AS Ⅳ for treating T2DM and NAFLD were predicted by network pharmacology, and the main enrichment pathway was verified by molecular biology techniques. The protein expressions of AMPK, p-AMPK, sterol regulatory element-binding protein-1 (SREBP-1), and fatty acid synthetase (FAS) in liver tissue were detected by Western blot. ResultCompared with the blank group, the levels of body mass, liver weight coefficient, fasting blood glucose, serum total cholesterol, triglyceride, and low-density lipoprotein cholesterol in mice treated with AS Ⅳ were decreased (P<0.05, P<0.01). The pathology of liver tissue showed significant improvement in lipid accumulation, and imaging results showed that the degree of fatty liver was reduced after AS Ⅳ therapy. Network pharmacological prediction results showed that vascular endothelial growth factor α (VEGFA), galactoagglutinin 3 (LGALS3), serine/threonine kinase B2 (Akt2), RHO-associated coiled-coil protein kinase 1 (ROCK1), serine/threonine kinase B1 (Akt1), signaling and transcriptional activator protein (STAT3), and messtimal epidermal transformation factor (MET) were key targets in "drug-disease" network. The results from the Kyoto encyclopedia of genes and genomes (KEGG) enrichment showed that the AMP-dependent protein kinase (AMPK) signaling pathway was strongly associated with T2DM and NAFLD. Western blot results showed that compared with the blank group, the expression levels of p-AMPK/AMPK in the model group were significantly down-regulated, while those of SREBP-1 and FAS proteins were significantly up-regulated (P<0.01). Compared with the model group, the expression levels of p-AMPK/AMPK in the metformin group and high-dose AS Ⅳ group were significantly up-regulated, while those of SREBP-1 and FAS proteins were significantly down-regulated (P<0.05, P<0.01). ConclusionAS Ⅳ regulates the expression of lipid proteins by activating the AMPK signaling pathway, thereby improving lipid metabolism.

RESUMEN

Moderate and deep sedation can effectively relieve or eliminate the pain and body discomfort during wound dressing change in pediatric burn patients, relieve anxiety, agitation, and even delirium of the children, reduce the metabolic rate of the children, make them in a quiet, comfortable, and cooperative state, which is conducive to the smooth completion of dressing change. This paper summarized the three aspects of moderate and deep sedation in pediatric burn patients, including the overview, main points of implementation, and effects, and further introduced the moderate and deep sedation medication regimens for different routes of administration, as well as the content of evaluation and monitoring. Suggestions on the prevention and management of related complications and the management of moderate and deep sedation implementation procedures were put forward, in order to provide references for the development of moderate and deep sedation for wound dressing change in pediatric burn patients in China.

RESUMEN

This study aimed to investigate the effect and mechanism of Zuogui Jiangtang Qinggan Formula(ZGJTQG) on the glucolipid metabolism of type 2 diabetes mellitus(T2DM) complicated with non-alcoholic fatty liver disease(NAFLD). NAFLD was induced by a high-fat diet(HFD) in MKR mice(T2DM mice), and a model of T2DM combined with NAFLD was established. Forty mice were randomly divided into a model group, a metformin group(0.067 g·kg~(-1)), and high-and low-dose ZGJTQG groups(29.64 and 14.82 g·kg~(-1)), with 10 mice in each group. Ten FVB mice of the same age were assigned to the normal group. Serum and liver tissue specimens were collected from mice except for those in the normal and model groups after four weeks of drug administration by gavage, and fasting blood glucose(FBG) and fasting insulin(FINS) levels were measured. The levels of total cholesterol(TC), triglyceride(TG), and low-density lipoprotein(LDL) were detected by the single reagent GPO-PAP method. Very low-density lipoprotein(VLDL) was detected by enzyme-linked immunosorbent assay(ELISA). Alanine aminotransferase(ALT) and aspartate ami-notransferase(AST) were determined by the Reitman-Frankel assay. The pathological changes in the liver were observed by hematoxylin-eosin(HE) staining and oil red O staining. Real-time fluorescence-based quantitative polymerase chain reaction(real-time PCR) and Western blot were adopted to detect the mRNA and protein expression of forkhead transcription factor O1(FoxO1), microsomal triglyceride transfer protein(MTP), and apolipoprotein B(APOB) in the liver. The results showed that high-dose ZGJTQG could signi-ficantly reduce the FBG and FINS levels(P<0.05, P<0.01), improve glucose tolerance and insulin resistance(P<0.05, P<0.01), alleviate the liver damage caused by HFD which was reflected in improving liver steatosis, and reduce the serum levels of TC, TG, LDL, VLDL, ALT, and AST(P<0.05, P<0.01) in T2DM mice combined with NAFLD. The findings also revealed that the mRNA and protein expression of FoxO1, MTP, and APOB in the liver was significantly down-regulated after the intervention of high-dose ZGJTQG(P<0.05, P<0.01). The above study showed that ZGJTQG could effectively improve glucolipid metabolism in T2DM combined with NAFLD, and the mechanism was closely related to the regulation of the FoxO1/MTP/APOB signaling pathway.

RESUMEN

ObjectiveTo explore the effects of Baihu Jia Renshen Tang (BHRS) on the related molecules on the phosphatidylinositol-3-kinase/protein kinase B(PI3K/Akt)signaling pathway in the liver of MKR diabetic model mice. MethodThirty 6-week-old MKR mice were selected and fed on a high-fat diet for four weeks, followed by intraperitoneal injection of streptozotocin(STZ)for the diabetes model establishment. The model was properly induced in the case of the fasting blood glucose (FBG) of ≥11.1 mmol·L-1. After modeling, the mice were randomly divided into a model group,a BHRS group (12.09 g·kg-1·d-1),and a metformin group (0.065 g·kg-1·d-1),with 10 mice in each group. Ten FVB mice were assigned to the control group. The mice in the groups with drug intervention were continuously administered correspondingly for 28 days. After administration, the mice were sacrificed, followed by the oral glucose tolerance test (OGTT) and FBG detection. Serum very low-density lipoprotein(VLDL)content was determined by semi-quantitative enzyme-linked immunosorbent assay (ELISA). Four indexes related to blood lipid were determined by the biochemistry analyzer. Liver tissues were subjected to pathological examination by hematoxylin-eosin(HE)staining. Western blot was used to detect the protein expression of PI3K,Akt,phosphorylated(p)-PI3K,p-Akt,forkhead box protein O1 (FoxO1),insulin receptor(InsR),and insulin receptor substrate-2(IRS-2) in liver tissues of mice. Real-time polymerase chain reaction(Real-time PCR) was used to detect the mRNA expression of PI3K,Akt,FoxO1,InsR,and IRS-2 in liver tissues of mice. ResultCompared with the control group, the model group showed poor general conditions, abnormal glucose tolerance (P<0.05),increased FBG (P<0.01),abnormal blood lipid metabolism, increased serum total cholesterol (TC),triglyceride(TG),low-density lipoprotein cholesterol(LDL-C),and VLDL (P<0.05),decreased level of high-density lipoprotein cholesterol(HDL-C)(P<0.05),fatty degeneration and obvious pathological changes of liver cells, reduced protein expression of PI3K,Akt,p-PI3K/PI3K,p-Akt/Akt,IRS-2,and InsR in liver tissues(P<0.05),increased protein expression of FoxO1(P<0.05),decreased mRNA expression of PI3K,Akt,IRS-2,and InsR in liver tissues (P<0.05),and increased FoxO1 mRNA expression(P<0.05). Compared with the model group, the BHRS group showed improved general conditions and glucose and lipid metabolism (P<0.05),improved pathological state of liver cells, increased protein expression of PI3K,Akt,p-PI3K/PI3K,p-Akt/Akt,IRS-2,and InsR in liver tissues(P<0.05),decreased protein expression of FoxO1(P<0.05),increased mRNA expression of PI3K,Akt,IRS-2,and InsR in liver tissues (P<0.05),and reduced FoxO1 mRNA expression(P<0.05). ConclusionBHRS can effectively reduce blood glucose, regulate blood lipid metabolism, and improve the pathological state of the liver in MKR diabetic mice, and its mechanism of action may be related to the regulation of the activity of molecules on the PI3K/Akt signaling pathway.

RESUMEN

ObjectiveTo investigate the mechanism of Zuogui Jiangtang Qinggan prescription (ZJQP) in improving glucose and lipid metabolism in loss of skeletalmuscle-specific insulin-like growth factor-1 receptor function (MKR) mice with type 2 diabetes mellitus (T2DM) and non-alcoholic fatty liver disease (NAFLD). MethodNAFLD was induced by high-fat diet feeding for 8 weeks in MKR mice, which were randomly divided into model group, metformin group (0.067 g·kg-1), and ZJQP high and low-dose groups(14.8, 7.4 g·kg-1). Ten FVB mice of the same age were used as the normal group. After 8 weeks of drug treatment, the oral glucose tolerance test (OGTT) was performed, the serum was taken to detect triacylglycerol (TG) and total cholesterol (TC), and the wet weight of the mouse liver was weighed. Haematoxylin-eosin (HE) staining and oil red O staining were performed to assess histopathology of liver. The mRNA expression and protein expression of Fork head box protein O1 (FoxO1), phosphoenolpyruvate carboxykinase (PEPCK), glucose-6-phosphatase (G6Pase), and apolipoprotein C3 (ApoC-Ⅲ) in liver tissues were detected by real-time fluorescent quantitative PCR (Real-time PCR) and Western blot, respectively. ResultAs compared with the normal group, the levels of fasting blood glucose, liver index, serum TG, TC, and OGTT of mice in the model group increased significantly (P<0.01). As compared with model group, the fasting blood glucose and liver index of the mice in the metformin group and the ZJQP group decreased significantly (P<0.01), the serum levels of TG and TC in the high-dose ZJQP group decreased significantly (P<0.05,P<0.01), and the OGTT of mice in the metformin group and the high-dose ZJQP group improved (P<0.05). In histopathology, as compared with the normal group, mice in the model group showed decreased lipid droplets and vacuoles in hepatocytes, and their volumes became larger. Compared with the model group, the ZJQP group and metformin group showed that the lipid droplets in liver tissues were reduced, the vacuoles in liver cells were reduced, and the volume was smaller. At the molecular level, as compared with the normal group, the mRNA and protein levels of FoxO1, PEPCK, G6Pase, and ApoC-Ⅲ in liver tissues of mice in the model group were significantly up-regulated (P<0.01). As compared with the model group, the mRNA and protein levels of FoxO1, PEPCK, G6Pase, and ApoC-Ⅲ in the ZJQP group was significantly decreased (P<0.01). ConclusionZJQP can improve the glucose and lipid metabolism of T2DM with NAFLD and repair the pathological damage of liver, which may be through regulating the expression of FoxO1, PEPCK, G6Pase, ApoC-Ⅲ-related proteins in liver tissues to achieve the effects of regulating lipid, lowering glucose, and delaying hepatic steatosis.

RESUMEN

ObjectiveTo investigate the mechanism of Huangqi Guizhi Wuwutang (HQGZWWT) in the treatment of diabetic peripheral neuropathy (DPN) in MKR mice via regulating endoplasmic reticulum (ER) stress. MethodThirty-two 8-week-old MKR mice (half were male and half were female) were fed with a high-fat diet for four weeks, and then 1% streptozotocin (STZ) was injected intraperitoneally for five days. After the blood glucose was stabilized, the mice were housed in the cage covered with ice bags for another one hour stimulation per day for four weeks. Mice with fasting blood glucose (FBG) value ≥11.1 mmol·L-1 were randomly divided into model group , Huangqi Guizhi Wuwutang in original dosage group (30 g·kg-1·d-1), Huangqi Guizhi Wuwutang in formula dosage group (6.25 g·kg-1·d-1), and positive drug group (mecobalamin tablets, 0.17 mg·kg-1·d-1). Another eight MKR mice of the same age were set as blank group and eight FVB mice were normal group. After four weeks of intragastric administration in each group, the change in FBG was tested, and hematoxylin and eosin (HE) staining and transmission electron microscope were used for observing the morphology of sciatic nerve tissue. In addition, the expression of c-Jun N-terminal kinase (JNK), phosphorylated c-Jun N-terminal kinase (p-JNK) and inositol requiring enzyme 1α (IRE1α) proteins was determined by immunohistochemical test and Western blot (WB). ResultCompared with the conditions in the normal group and blank group, the time of paw withdrawal, paw licking and tail flick in the model group was shortened (P<0.01), and the conduction velocity of sciatic nerve was decreased (P<0.01). Compared with the conditions in the model group, the behavioral and functional indicators were improved by HQGZWWT (P<0.05,P<0.01). The immunohistochemical test revealed the JNK expression was elevated in the model group compared with the conditions in the normal group and blank group (P<0.05), while that was lowered by HQGZWWT compared with the condition in the model group (P<0.05). However, there was no difference among the treatment groups. According to the WB, the expression of IRE1α and p-JNK in the model group was enhanced compared with the conditions in the normal group and blank group (P<0.05,P<0.01), while that was decreased by HQGZWWT compared with the condition in the model group (P<0.05,P<0.01). No difference was observed between the HQGZWWTO and HQGZWWTF groups. ConclusionHQGZWWT can improve the neurophysiological function and pathological damage of sciatic nerve, which may be related to its delaying the ER stress response of sciatic nerve.

RESUMEN

The Leguminosae perennial vines of Callerya and Millettia have many species and wide distribution, not only can be used for medicines, but also they have ornamental and insecticidal effects. With increasing demand for Spatholobi Caulis, and the reserves of wild medicinal materials are on the verge of exhaustion, resulting in the increasing number of mixtures and substitutes in the market, which makes it urgent to study the origin of Spatholobi Caulis. By referring to related literature, there are three major origins of Spatholobi Caulis, including Callerya, Millettia and Spatholobus. Callerya is separated from Millettia, they are divided and united for many times, now the official website of Flora of China has accepted the revision of them as two genera. This paper intends to compare the chemical components and pharmacodynamic effects of Callerya and Millettia, aiming to explore the similarities and differences between the two genera, so as to determine the rationality and necessity of separating Callerya from Millettia. After comparing, it was found that the chemical composition and pharmacodynamic effects of the two genera were different, which supported the separation of Callerya from Millettia, and it was not recommended to mix use of them.

RESUMEN

Objective:To investigate the action mechanism of Yinchenhao Tang against type 2 diabetes mellitus (T2DM) complicated with non-alcoholic fatty liver disease (NAFLD) in MKR mice. Method:Forty eight-week-old MKR mice were fed a high-fat diet for eight weeks and then divided into the model group, original Yinchenhao Tang (17.16 g·kg-1) group,Yinchenhao Tang group at a specified dose (4.68 g·kg-1) in teaching materials, and positive drug [metformin + simvastatin, (65+2.6)×10-3 g·kg-1] group. Another 10 MKR mice of the same age were classified into the blank group and 10 FVB mice into the normal group. After eight weeks of intragastric administration in each group, the liver wet weight, oral glucose tolerance test (OGTT),serum inflammatory factors interleukin-6 (IL-6) and tumor necrosis factor-α (TNF-α),and changes in blood lipid and liver function were determined. Hematoxylin-eosin (HE) staining was conducted for observing the morphological changes in liver tissue under a transmission electron microscope, followed by the detection of Toll-like receptor 4 (TLR4),myeloid differentiation factor 88 (MyD88),and nuclear transcription factor-κB (NF-κB) protein expression by Western blot. Result:Compared with the model group, the medication groups exhibited significantly reduced liver wet weight index (P<0.01),improved OGTT result (P<0.05),and down-regulated serum IL-6 and TNF-α levels (P<0.01). In terms of morphological changes,Yinchenhao Tang protected the hepatocyte structure and alleviated hepatocyte steatosis. Moreover, Yinchenhao Tang obviously down-regulated the protein expression levels of TLR4,MyD88,and NF-κB in liver tissue of MKR mice with T2DM combined with NAFLD (P<0.05),and the down-regulation of TLR4 and NF-κB in the original Yinchenhao Tang group was better than that in the Yinchenhao Tang group at a specified dose in teaching materials (P<0.05). Conclusion:Yinchenhao Tang is able to reduce inflammatory factor levels and down-regulate TLR4,MyD88,and NF-κB expression in liver tissue to relieve the pathological liver injury and interfere with T2DM combined with NAFLD of MKR mice. It exerts a certain liver-protective effect by lowering the blood lipids and delaying the hepatic inflammation.

RESUMEN

Yemen has been faced with the worst cholera epidemic of modern times, with more than 1 million suspected cases and 3000 deaths at the time of writing. This problem is largely due to the longstanding civil war between pro-government forces and the Houthi armed movement, which has severely damaged already vulnerable sanitation and healthcare facilities and systems in the country. It is further compounded by a dire lack of basic amenities, chronic malnutrition, and unfavourable weather conditions. Another contributory component may be aerial transfer by cholera-infected chironomid insects. To contain the spread of cholera in Yemen, a nation-wide armistice should be negotiated, and national and local committees must be convened to coordinate efforts on the ground. Community isolation facilities with proper sanitation, reliable disposal systems, and a clean water supply should be set up to isolate and treat sick patients. The continuity of vaccination programmes should be ensured. Public health campaigns to educate local communities about good hygiene practices and nutrition are also necessary. The One Health paradigm emphasizes a multi-sectoral and transdisciplinary understanding and approach to prevent and mitigate the threat of communicable diseases. This paradigm is highly applicable to the ongoing cholera crisis in Yemen, as it demands a holistic and whole-of-society approach at the local, regional, and national levels. The key stakeholders and warring parties in Yemen must work towards a lasting ceasefire during these trying times, especially given the extra burden from the mounting severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) outbreak worldwide.

RESUMEN

Objective To investigate the effects of resveratrol pretreatment on activation and inflammation of astrocytes after oxygen-glucose deprivation/ reoxygenation (OGD/ R) injury in vitro. Methods Twenty newborn SD rats within 24 hours were recruited, and their primary cultured astrocytes were deprived of oxygen glucose for 150 minutes and reoxygenation for 24 hours. The experiment was divided into the normal (Nor), the control (Ctrl) and the resveratrol pretreatment (Res) groups. Cell viability was detected with CCK-8 assay. Cell proliferation was measured with 5-ethynyl-2’- deoxyuridine (EdU). The expressions of glial fibrillary acidic protein (GFAP) and S100β proteins were determined with immunofluorescence. The expressions of GFAP, S100β, vimentin, interleukin (IL)-10, β-interferon (IFN-β), tumor necrosis factor α (TNF-α) and IL-1 β proteins were detected with Western blotting. The levels of IL-10, IFN-β, TNF-α and IL-1β proteins in the supernatant were measured by ELISA. Results CCK-8 assay showed that with the increase of resveratrol concentration (5, 20 and 40 μmol/ L), the cell viability increased gradually, which was significantly lower than that of the control group (P<0. 01). After that, with the increase of resveratrol concentration (80 and 100 μmol/ L), the cell viability decreased significantly, and the cell viability of 100 μmol/ L resveratrol group was significantly lower than in the control group (P<0. 02). EdU testing showed that after OGD/ R injury, the percentage of EdU positive cells in the control and resveratrol group was significantly higher than that of in the normal group (P<0. 01), while it was significantly lower in the resveratrol pretreatment group than that of the control group (P<0. 01).Immunofluorescence, Western blotting and ELISA method showed that the expressions or levels of GFAP, S100β, vimentin, IL-10, IFN-β, TNF-α, IL-1β proteins in the control and resveratrol group were significantly higher than that of the normal group (P<0. 05), but the expressions or levels of IL-10 and IFN-β proteins in the resveratrol group were higher than those of the control group, and the others were lower than the control group (P<0. 05).However,the expressions or levels of IL-10 and IFN-β proteins in the resveratrol group were higher than those of the control group, and the others in the resveratrol group were lower than those of the control group (P<0. 05). Conclusion Resveratrol pretreatment can inhibit the activation of astrocytes and reduce the inflammation after OGD/ R injury in vitro.

RESUMEN

Objective To investigate the effect of resveratrol on microglia polarization after oxygen glucose deprivation / reoxygenation (OGD / R) in vitro. Methods N9 microglias were cultured under oxygen-glucose deprivation for 150 minutes and reoxygenation for 24 hours. The experiment was divided into normal, control and resveratrol pretreatment groups. Cell viability was measured by cell counting kit-8(CCK-8) assay. Malondialdehyde (MDA) levels and superoxide dismutase (SOD) activity in the supernatant were detected by thiobarbituric acid(TBA) and water soluble terazolium salt (WST-1) respectively. Nuclear factor E2-related factor 2 (Nrf2) nuclear translocation was detected by immunofluorescence. The levels of CD206, inducible nitric oxide synthase (iNOS), Nrf2, heme oxygenase-1 (HO-1), NAD(P) H: quinone oxidoreductase 1(NQO1) protein were measured by Western blotting. The mRNA levels of CD206, iNOS were detected by Real-time PCR. Results After OGD / R injury, cell viability, SOD activity, CD206, Nrf2, HO-1, NQO1 protein or mRNA expression in resveratrol groups were significantly higher than those in the control group (P<0. 05, n = 3), while MDA content and iNOS protein and mRNA expression were significantly lower than those in the control group (P<0. 05, n = 3) . Nrf2 protein in resveratrol group was significantly transferred to the nucleus compared with the control group. Conclusion Resveratrol pretreatment may regulate M1 / M2 polarization of microglia and reduce oxidative stress after OGD / R via strengthening activation of Nrf2.

Objective To observe the influence of Chinese medicine skin dialysis on traditional Chinese medicine (TCM) syndrome scores and microinflammation in patients with chronic renal failure(CRF). Methods A total of 180 patients with CRF were divided into groups A, B and C, 60 cases in each group. All of the 3 groups were given conventional treatment, and additionally, group A was given Chinese medicine skin dialysis, and group B was given pure water dialysis. Meanwhile, 60 healthy volunteers served as normal control group (group D), and had no any medication. The course of treatment lasted for one month. Before and after treatment, we examined the scores of TCM syndromes and the levels of 24-hour urinary protein, serum creatinine (SCr), blood urea nitrogen (BUN), estimated glomerular filtration rate (eGFR), albumin (ALB), immunoglobulin A(IgA), immunoglobulin G (IgG) , immunoglobulin M(IgM), interleukin 6 (IL-6) , interleukin 1(IL-1), hyper-sensitive C reactive protein (hs-CRP), and tumor necrosis factor alpha (TNF-α). Results After treatment, the scores of TCM syndromes and the levels of 24-hour urinary protein, SCr, BUN, eGFR, IL-6, IL-1, hs-CRP, and TNF-α in the 3 groups were decreased (P < 0.05 compared with those before treatment), and were markedly lower in group A than those in group B and group C (P<0.05); serum levels of IgA, IgG, IgM and ALB in the 3 groups were increased(P < 0.05 compared with those before treatment), and the levels in group A were significantly higher than those in group B and group C (P < 0.05). However, the levels of observation indexes did not arrive to the levels of group D(P<0.05). Conclusion Chinese medicine skin dialysis is effective on relieving the symptoms of CRF patients and enhancing the therapeutic effect through decreasing inflammatory mediators, inhibiting micro-inflammation and improving nutritional status.

RESUMEN

Objective To establish HPLC fingerprints of Jinzepaishi Granules from ten different batches and a preliminary method for its anti-inflammatory biological activity titer detection, and construct a dose-effect relationship between cyclooxygenase-2 (COX-2) and Jinzepaishi Granules for scientific evaluation and effective control of granules quality. Methods The HPLC method was used to analyze the similarity of common peaks in different batches of granules and assigned 19 common peaks to herbs. The fingerprints were further analyzed by the chemometric method of hierarchical clustering analysis and principal component analysis (PCA). The EIA method was used to determine the half-inhibitory concentration (IC50) of Jinzepaishi Granules. Results The similarity of HPLC fingerprints of all the ten batches was ≥ 0.955, which were consistent with the results of cluster analysis and PCA. Jinzepaishi Granules had a significant inhibitory effects on COX-2 with a IC50 of 10.272 mg/mL, which demonstrated a significant dose-effect relationship. Conclusion The multiple quality control system of chemical fingerprinting and biological activity detection has the characteristics of high efficiency, rapidness, accuracy, sensitivity, and good reproducibility. It can effectively evaluate the anti-inflammatory biological activity of Jinzepaishi Granules and achieve more scientific and reasonable control of its quality. It also provides a reference for the establishment of more scientific quality control for other Chinese materia medica preparations.

RESUMEN

<p><b>INTRODUCTION</b>Advance care planning (ACP) is an important aspect of end-of-life care that has been shown to improve patient autonomy in decision-making and reduce stress for surviving family members. Given the rapidly ageing population in Singapore, a greater emphasis on end-of-life care planning is needed. This study therefore sought to examine the awareness and attitudes of the general Singaporean community towards participating in ACP, which are not known hitherto.</p><p><b>MATERIALS AND METHODS</b>A 24-item interviewer-administered questionnaire was constructed and administered via door-to-door survey amongst community-dwelling residents living in Housing and Development Board (HDB) flats across Singapore, selected via a two-stage stratified random sampling.</p><p><b>RESULTS</b>Of the 406 completed surveys, 14.4% of respondents had heard of ACP (n = 58), mostly through the media (67.9%), from family and friends (21.4%) and healthcare providers (21.4%). Only 26.8% of those who had previously heard of ACP knew how to begin an ACP discussion and 12.5% of them had a prior ACP discussion. After education, the majority of respondents were willing to begin an ACP discussion (n = 236, 60.1%). Being of an older age, having a life threatening illness, and having more knowledge about ACP were significant factors associated with willingness to have an ACP discussion. Barriers included perceiving oneself as still healthy and preferring the family to make decisions instead.</p><p><b>CONCLUSION</b>There is a low awareness but high expressed willingness to engage in an ACP discussion amongst the Singaporean community. More efforts are needed to educate the public about ACP, engage the family unit and correct the present misconceptions.</p>
