ABSTRACT Objective: Data regarding rare FPAs from India, a resource limited setting, are limited. We describe a case series of rare FPAs from a single center in western India. Materials and methods: This was a retrospective case record review of patients diagnosed between January 2010 and July 2022. The diagnosis was based on biochemical (inappropriately elevated serum FSH/LH) and pathologic (positive immunostaining for FSH/LH) features in patients with FGA, and elevated serum thyroid hormones and normal/elevated TSH in patients with TSHomas. Results: We identified 11 patients with a total of six FGAs (median age 43.5 years, five men, one FGA cosecreting TSH, median largest dimension 40 mm, range 33-60 mm) and six TSHomas (median age 34.5 years, four women, two TSHomas cosecreting GH, median largest dimension 42.5 mm, range 13-60 mm). Symptoms of sellar mass effects led to pituitary imaging in most patients with FGA. Patients with TSHomas had symptoms of excess hormone secretion (GH/TSH) or sellar mass effects. The TSHomas that cosecreted GH/FSH were larger than those secreting only TSH. Transsphenoidal resection was the most common first-line therapy but significant residual disease was frequent (3 out of 6 FGAs and 4 out of 5 TSHomas). Conclusion: This is the first and second case series of FGAs and TSHomas, respectively, from India. In this study, TSHomas presented at younger age, were larger and had low surgical cure rates.

ABSTRACT Burosumab, a monoclonal antibody directed against the fibroblast growth factor 23 (FGF23), has been approved for the treatment of X-linked hypophosphatemia (XLH). We conducted a systematic review to compare the efficacy and safety of burosumab versus conventional therapy (phosphorus and calcitriol) on XLH treatment. After a comprehensive literature search on MEDLINE/PubMed and Embase, we found nine studies for inclusion in the analysis. Risk of bias was assessed, and a random-effects model was used to determine the effect size. Clinical, biochemical, and radiological parameters of disease severity before and after treatment were analyzed and expressed in standardized mean difference (SMD). Burosumab resulted in normalization of phosphate homeostasis with an increase in renal tubular phosphate reabsorption and significant resolution of skeletal lesions (change in Thacher's total rickets severity score SMD: −1.46, 95% confidence interval [CI]: −1.76 to −1.17, p < 0.001, improvement in deformities, and decline in serum alkaline phosphatase levels [SMD: 130.68, 95% CI: 125.26-136.1, p < 0.001)]. Conventional therapy led to similar improvements in all these parameters but to a lower degree. In adults, burosumab normalized phosphorus levels (SMD: 1.23, 95% CI: 0.98-1.47, p < 0.001) with resultant clinical improvement. Burosumab treatment was well tolerated, with only mild treatment-related adverse effects. The present review indicates a potential role for burosumab in improving rickets, deformities, and growth in children with XLH. Given its superior efficacy and safety profile, burosumab could be an effective therapeutic option in children. We suggest further studies comparing burosumab versus conventional therapy in children and adults with XLH.

Introducción. El brote de COVID-19 generó cambios significativos en los hábitos alimentarios asociados a las medidas de confinamiento y distanciamiento social. Objetivo. Evaluar los hábitos alimentarios asociados a la emergencia sanitaria COVID-19 en adultos chilenos durante el año 2020. Materiales y Métodos. Se realizó un estudio observacional, descriptivo y transversal en adultos chilenos seleccionados mediante muestreo no probabilístico por conveniencia. Se utilizó una encuesta validada por juicio de expertos, que incluyó preguntas sobre características sociodemográficas, ingesta habitual de alimentos y percepción de cambios de hábitos alimentarios durante el confinamiento. Resultados. Participaron 4.346 adultos chilenos con una edad promedio de 35,8 ± 13,9 años, predominantemente residentes urbanos (94,3%) y mujeres (75,1%). Se observó un aumento en el abastecimiento de alimentos en los hogares durante el confinamiento. La mayoría de los participantes manifestó que mantuvo o aumentó el consumo de verduras, frutas, lácteos, pescado y legumbres. Sin embargo, la ingesta de alimentos saludables no alcanzó las recomendaciones nutricionales establecidas. Adicionalmente, se encontró un aumento en el consumo de dulces. Conclusiones. Este estudio revela los efectos del confinamiento por la pandemia de COVID-19 en los hábitos alimentarios de los adultos chilenos, donde un porcentaje considerable de la población no cumplió con las recomendaciones de una dieta equilibrada según las guías alimentarias chilenas. El aumento en el consumo de algunos alimentos poco saludables puede tener implicaciones en la salud y el estado nutricional. Es necesario fomentar una alimentación equilibrada durante situaciones de crisis como esta pandemia(AU)


Introduction. The COVID-19 outbreak has led to significant changes in dietary habits due to confinement and social distancing measures. Objective. To assess the dietary habits associated with the COVID-19 health emergency among Chilean adults during the year 2020. Materials and methods. An observational, descriptive, and cross- sectional study was conducted on Chilean adults selected through non-probabilistic convenience sampling. A survey validated by expert judgment was used, including questions on sociodemographic characteristics, usual food intake, and perceived changes in dietary habits during confinement. Results. A total of 4,346 Chilean adults participated in the study, with an average age of 35.8 ± 13.9 years, predominantly urban residents (94.3%), and female (75.1%). An increase in household food supplies during confinement was observed. Most participants reported maintaining or increasing their consumption of vegetables, fruits, dairy, fish, and legumes. However, the intake of healthy foods did not meet the established nutritional recommendations. Additionally, there was an increase in the consumption of sweets. Conclusions. This study reveals the effects of COVID-19 confinement on the dietary habits of Chilean adults, where a considerable percentage of the population did not comply with the recommended balanced diet according to Chilean dietary guidelines. The increased consumption of some unhealthy foods may have implications for health and nutritional status. It is essential to promote balanced eating during crisis situations like this pandemic(AU)

Resumen: Introducción: los aneurismas son dilataciones vasculares localizadas y permanentes de una arteria; en los pseudoaneurismas, las capas normales del vaso sanguíneo son reemplazadas por tejido fibroso. Debido a su baja incidencia, así como el desafío diagnóstico y terapéutico que representan; nuestro objetivo es presentar el caso clínico de un pseudoaneurisma de una arteria digital de la mano y realizar una revisión sistemática sobre dicha patología. Material y métodos: búsqueda bibliográfica en Medline, utilizando los términos «arteria digital¼ y «aneurisma¼. Se incorporaron estudios de patología de dilatación vascular que afecte la mano y los dedos. Se excluyeron trabajos con patología de afección proximal de la mano. Presentación de caso: paciente femenino de 79 años de edad, que posterior a herida cortante de quinto dedo de mano izquierda, desarrolla tumoración necrótica de rápido crecimiento. Contaba con ecografía y angiografía que sugerían hematoma. Se decidió manejo quirúrgico, durante el cual se observó que la tumoración involucraba arteria digital colateral cubital del quinto dedo. Se resecó lesión y segmento arterial involucrado. Cursó postquirúrgico sin complicaciones. Se confirmó el diagnóstico histopatológico de pseudoaneurisma de la lesión. Discusión: la etiología traumática es la causa más frecuente de los aneurismas digitales. Los factores de riesgo para los pseudoaneurismas incluyen lesiones penetrantes y alteraciones de la cascada de coagulación, como en el caso presentado. Conclusión: el pseudoaneurisma de una arteria digital es una patología rara y con gran variabilidad de manejo terapéutico. La resección quirúrgica de la lesión con la reconstrucción del flujo vascular, es el tratamiento recomendado.


Abstract: Introduction: aneurysms are focal and permanent dilations of an artery; in pseudoaneurysms, the normal layers of the blood vessel are replaced by fibrous tissue. Due to their low incidence, as well as the diagnostic and therapeutic challenge they represent; our objective is to present the clinical case of a pseudoaneurysm of a digital artery of the hand and to carry out a systematic review of this pathology. Material and methods: literature search in Medline, using the terms "digital artery" and "aneurysm." Studies of vascular dilation pathology affecting the hand and fingers were incorporated. Studies with pathology of proximal involvement of the hand were excluded. Case presentation: a 79-year-old female patient who, after a sharp force trauma to the fifth finger of the left hand, develops a rapidly growing necrotic tumor. She had ultrasound and angiography that suggested hematoma. Surgical management was decided, during which it was observed that the tumor involved ulnar collateral digital artery of the fifth finger. The lesion and the arterial segment involved were resected. Post-surgical course without complications. The histopathological diagnosis of pseudoaneurysm of the lesion was confirmed. Discussion: traumatic etiology is the most frequent cause of digital aneurysms. Risk factors for pseudoaneurysms include sharp force trauma and alterations of the coagulation pathways, as in the case presented. Conclusion: the pseudoaneurysm of a digital artery is a rare pathology with great variability of therapeutic management. Surgical resection of the lesion with vascular flow reconstruction is the recommended treatment.

Objective: To describe the characteristics of gonadotropin-dependent precocious puberty (GDPP) in Indian children. Methods: Clinical profiles of GDPP (n=78, 61 females) and premature thelarche (n=12) from a single center in Western India were retrospectively studied. Results: Pubertal onset was earlier in boys than girls (29 vs 75 months, respectively; P=0.008). The basal luteinizing hormone (LH) was ?0.3 mIU/mL, except 18% of GDPP girls. At 60 minutes after GnRHa-stimulation, all patients (except one girl) had LH ?5 mIU/mL. The GnRHa-stimulated LH/FSH ratio was ?0.34 at 60 minutes in girls with GDPP unlike premature thelarche. Only one girl had an allergic reaction to long-acting GnRH agonist. Among GnRH agonist-treated girls (n=24), the predicted final adult height was -1.67±1.5 SDS, whereas the attained final height was -0.25±1.48 SDS. Conclusion: We establish the safety and efficacy of long acting GnRH agonist therapy in Indian children with GDPP. The 60-minute stimulated serum LH/FSH of ?0.34 differentiated GDPP from premature thelarche.

Introducción En comparación con ciencias clínicas básicas o aplicadas, la bioética es considerada una disciplina secundaria y subutilizada en la práctica diaria. Sin embargo, el razonamiento ético es indispensable para la calidad del cuidado. Existen pocos estudios sobre bioética en unidades de emergencia pediátrica. Nuestro objetivo fue evaluar la percepción sobre la importancia y la suficiencia del conocimiento teórico adquirido y la aplicación de los principios bioéticos en casos estandarizados. Métodos Realizamos un estudio descriptivo transversal en profesionales médicos y de enfermería que se desempeñan en unidades de emergencia pediátrica de Puerto Montt. Mediante una encuesta, evaluamos la percepción de la importancia y suficiencia del conocimiento bioético obtenido y la aplicación de los principios bioéticos en casos hipotéticos, pero probables, en la atención de urgencias pediátricas. Resultados De una población total de 50 médicos y 53 enfermeras, participaron en nuestro estudio 30 médicos (60%) y 20 enfermeras (37,7%). La mayoría reportó formación ética en pregrado (84%). Una minoría reportó formación durante la práctica (20%). Sin embargo, sólo 60% percibía tener conocimientos suficientes de bioética y 72% la consideraba importante para la práctica diaria. Además, al aplicar los principios de Beauchamp y Childress a casos clínicos estandarizados, el 82,7% no reconoció el principio de justicia y solo 50% reconoció los principios de autonomía y no maleficencia. Conclusión Aunque la mayoría de los profesionales de la salud tienen formación en bioética, el aprendizaje muchas veces se considera insuficiente y no se incorpora a la práctica diaria en las unidades de emergencia pediátrica.


Background Compared to basic or applied clinical sciences, bioethics is frequently considered as a secondary discipline and underutilized in daily practice. However, ethical reasoning is indispensable for the quality of care. There are few studies on bioethics in pediatric emergency units. Our objective was to evaluate the perception of the acquired bioethical knowledge and the application of bioethical principles in standardized cases. Methods We conducted a cross-sectional descriptive study in medical and nursing professionals working at pediatric emergency units in Puerto Montt. Through a survey, we assessed the perception of the sufficiency of the acquired bioethics knowledge and the application of bioethical principles on hypothetical, but probable cases in emergency pediatric care. Results Of a total population of 50 physicians and 53 nurses, 30 physicians (60.0%) and 20 nurses (38.7%) participated in our study. The majority reported ethics training in undergraduate education: 84%. A minority reported training during practice: 20%. However, only 60.0% perceived having sufficient knowledge of bioethics and 72.0% considered it important for daily practice. Further, when applying the principles of Beauchamp and Childress to standardized clinical cases, 82.7% did not recognize the justice principle and only 50.00% the principles of autonomy and nonmaleficence. Conclusion Although most health professionals undergo bioethics training, learning is often considered insufficient and not incorporated into daily practice at pediatric emergency units.

Abstract: Scaphoid fractures are a common wrist injury accounting for 2-7% of all adult fractures. Nonunion is described in 5-12% of cases leading to osteoarthritis. Several classifications have been developed focused on this pathology and its complication. We present a case of a 28 years old male patient with a scaphoid fracture and nonunion who spontaneously consolidates without treatment. We performed a literature review to recognize this pathology, its common evolution and possible treatment options.


Resumen: Las fracturas de escafoides son una lesión frecuente de la muñeca y representan de 2-7% de todas las fracturas en adultos. La no unión se describe en 5-12% de los casos y conduce a la osteoartritis. Se han desarrollado varias clasificaciones centradas en esta patología y su complicación. Presentamos el caso de un paciente varón de 28 años con fractura y no unión de escafoides que consolida espontáneamente sin tratamiento. Realizamos una revisión bibliográfica para reconocer esta patología, su evolución habitual y las posibles opciones de tratamiento.

RESUMO Objetivo: demonstrar a aplicabilidade da ferramenta Healthcare Failure Mode and Effect Analysis para analisar, preventivamente, os riscos relativos ao processo de inserção, manutenção e retirada de cateter intravenoso periférico. Método: estudo teórico, realizado de agosto a novembro de 2022, em São Paulo-SP, Brasil, cujo processo foi mapeado em etapas/atividades, detalhando-se os modos de falha, com o uso da ferramenta. Calculou-se o Risk Priority Number, elaborou-se a matriz de severidade e probabilidade, adaptada à saúde por DeRosier e colaboradores, e propuseram-se as ações para reduzir os modos de falha. Resultados: identificaram-se como maiores riscos: "realizar antissepsia da área a ser puncionada com swab de álcool" e "desinfecção do conector com swab de álcool" sendo recomendados treinamentos e uso de kit de materiais como principais estratégias de mitigação. Conclusão: conhecer os riscos associados ao cateter intravenoso periférico, fundamenta a implementação de estratégias preventivas, minimizando a ocorrência de danos e os custos assistenciais deles decorrentes.


ABSTRACT Objective: to demonstrate the applicability of the Healthcare Failure Mode and Effect Analysis tool to analyze, preventively, the risks related to the process of insertion, maintenance, and removal of peripheral intravenous catheters. Method: theoretical study, conducted from August to November 2022, in São Paulo-SP, Brazil, whose process was mapped in stages/activities, detailing the failure modes, using the tool. The Risk Priority Number was calculated, the severity and probability matrix was elaborated, adapted to health by DeRosier and collaborators, and actions were proposed to reduce failure modes. Results: The major risks identified were: "perform antisepsis of the area to be punctured with an alcohol swab" and "disinfect the connector with an alcohol swab", and were recommended training and use of kit materials as the main mitigation strategies. Conclusion: knowing the risks associated with peripheral intravenous catheters is the basis for the implementation of preventive strategies, minimizing the occurrence of damage and the associated healthcare costs.


RESUMEN Objetivo: demostrar la aplicabilidad de la herramienta Healthcare Failure Mode and Effect Analysis para analizar, de forma preventiva, los riesgos relacionados con el proceso de inserción, mantenimiento y retirada de catéteres intravenosos periféricos. Método: estudio teórico, realizado de agosto a noviembre de 2022, en São Paulo-SP, Brasil, cuyo proceso fue mapeado en etapas/actividades, detallando los modos de falla, utilizando la herramienta. Se calculó el Número de Prioridad de Riesgo, se elaboró la matriz de severidad y probabilidad, adaptada a la salud por DeRosier y colaboradores, y se propusieron acciones para reducir los modos de falla. Resultados: Los principales riesgos identificados fueron: "realizar la antisepsia de la zona a puncionar con un bastoncillo con alcohol" y "desinfectar el conector con un bastoncillo con alcohol", recomendándose como principales estrategias de mitigación la formación y el uso de kits de materiales. Conclusión: Conocer los riesgos asociados al catéter intravenoso periférico sienta las bases para la aplicación de estrategias preventivas, minimizando la aparición de daños y los costes sanitarios derivados de los mismos.

The severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) emerged in December 2019 in Wuhan city, Hubei province, China. The patients with comorbid conditions like diabetes mellitus, lung disease are prone to a wide range of viral, bacterial and fungal infections. Aims And Objectives 1. To know the frequency of mucormycosis in Covid-19 patients. 2. To describe histomorphological pattern of mucormycosis and other fungal infections in Covid-19 patients. Materials And Methods: A prospective study was conducted from May 2021 to August 2021 for four month period with a sample size of 57 at a tertiary care hospital in south India. All the patients who were diagnosed with and or recovered from COVID-19 infection and came back with the fungal infection symptoms were subjected to surgical debridement were included in the study. All the demographic and clinical data were obtained from requests. In the presentResults: study we got 57 cases of covid-19 patient with suspected fungal infections.We examined the biopsy taken from different sites of rhinoorbital area with H&E and GMS stain. Out of 57 cases 36 cases were found to be having mucormycosis. Conclusion: Since Covid-19 is a risk for many opportunistic infections, its association with invasive fungal infection is dangerous and must be given serious consideration

El nervio mediano desciende por el brazo y, en el codo, comienza a atravesar estructuras que pueden generar compresión, como el ligamento de Struthers, el lacertus fibrosus, el pronador redondo, el flexor superficial de los dedos. Finalmente, en la muñeca, se encuentra otro sitio de compresión producido por el ligamento transverso del carpo. Todas estas estructuras pueden provocar signos y síntomas de atrapamiento nervioso y favorecer el deterioro funcional del nervio. Nuestro objetivo es dar a conocer una actualización sobre estos sitios de atrapamiento del nervio mediano, y cómo realizar un diagnóstico preciso e indicar un trata-miento adecuado. The median nerve is a nervous structure that begins to cross structures at the level of the elbow that might cause compression. The Struthers ligament, lacertus fibrosus, pronator teres, and flexor digitorum superficialis are among them. Finally, the transverse carpal ligament creates another compression site in the wrist. All these structures can develop pathological signs and symptoms of nerve entrapment, which favors nerve functional degradation. Our objective is to provide an update on these median nerve entrap-ment sites, as well as information on how to establish an accurate diagnosis and provide adequate treatment. Level of Evidence: IV

ABSTRACT Infectious keratitis is a sight-threatening condition that is usually an ocular emergency. The visual outcome depends on prompt and accurate clinical management as well as geographic and epidemiological awareness. We conducted a retrospective observational study to define the epidemiological and laboratory profile, as well as the clinical course of bacterial keratitis in a tertiary hospital in São Paulo over 21 years. Information about age, sex, predisposing factors, topical and surgical treatment, visual acuity, ulcers' classification, bacterioscopy, culture, and antibiotic sensitivity tests were collected. This study included 160 patients. The mean age was 65.1 ± 18.4 years and risk factors were identified in 83.1 % of the patients. Empirical topical fortified cephalosporin with an aminoglycoside or fourth-generation fluoroquinolone was curative for 66.2 % of the cases. The mean treatment duration was 22.5 ± 9 days. The mean variation of visual acuity was −0.25 logMAR, p < 0.001. Culture revealed 64 % of Gram-positive bacteria. All Gram-positive bacteria were sensitive to cephalothin, vancomycin, and quinolones. All Gram-negative bacteria were sensitive to gentamicin, tobramycin, amikacin, and ciprofloxacin. These findings reinforce the importance of prompt empirical treatment of severe corneal ulcers with a fortified cephalosporin and aminoglycoside or a fourth-generation fluoroquinolone as there are equally effective. Collected data was insufficient to evaluate resistance of ocular infections over time in this population.

Silent or subclinical inflammatory bowel diseases (IBD) is a relatively new term that has been used to describe individuals with asymptomatic active mucosal bowel inflammation, often unaware of their disease due to either the lack of or mild inflammatory symptoms. These patients are at risk for gastrointestinal and extra-gastrointestinal manifestations, with more advanced complications. In this article we intend to describe a case report of a patient with chronic history of many organ involvements including ocular, skin, and musculoskeletal, which was later placed under the umbrella of silent ulcerative colitis. ABSTRACT The tendency to be permissive in face of a discriminatory situation is called collusion. The present study aimed to define and characterize collusion, and identify the variables connected to it, considering the perspective of different identity groups. Participants were 31 individuals divided in seven focus groups. The analysis indicated four categories connected to collusion: a) Close Relations: valuable interactions through which individuals learn behavioral patterns that lead to permissiveness; b) Group Identity: social identities, and intergroup relations patterns; c) Situation: characteristics of the situation in which discrimination is observed; and d) Cost-Effective Balance: perception the individuals have regarding the impact of their actions on the context and the cost attached to it.


RESUMO A tendência das pessoas de serem permissivas diante de uma situação discriminatória é chamada colusão. O presente estudo teve como objetivo definir e caracterizar colusão e identificar as variáveis a ela associadas, a partir da perspectiva de diferentes grupos identitários. Os participantes foram 31 sujeitos divididos em sete grupos focais. Abstract Objectives BDNF has been implicated in the pathophysiology of systemic lupus erythematosus (SLE), especially its neuropsychiatric symptoms. The purpose of this study was to investigate the profile of blood BDNF levels in patients with SLE. Methods We searched PubMed, EMBASE, and the Cochrane Library for papers that compared BDNF levels in SLE patients and healthy controls (HCs). The Newcastle-Ottawa scale was used to assess the quality of the included publications, and statistical analyses were carried out using R 4.0.4. Results The final analysis included eight studies totaling 323 healthy controls and 658 SLE patients. Meta-analysis did not show statistically significant differences in blood BDNF concentrations in SLE patients compared to HCs (SMD 0.08, 95% CI [− 1.15; 1.32], P value = 0.89). After removing outliers, there was no significant change in the results: SMD -0.3868 (95% CI [− 1.17; 0.39], P value = 0.33. Univariate meta-regression analysis revealed that sample size, number of males, NOS score, and mean age of the SLE participants accounted for the heterogeneity of the studies (R2 were 26.89%, 16.53%, 18.8%, and 49.96%, respectively). Conclusion In conclusion, our meta-analysis found no significant association between blood BDNF levels and SLE. The potential role and relevance of BDNF in SLE need to be further examined in higher quality studies.

As parasitoses intestinais ainda constituem um problema de saúde pública no Brasil, especialmente nas regiões onde o saneamento básico e condições socioeconômicas são frágeis, como na região Nordeste. São um conjunto de doenças causadas por parasitas helmínticos ou protozoários, que acometem o intestino humano, causando sintomas, como diarreia, dor abdominal, astenia, náuseas, vômitos, entre outros. Seu diagnóstico é feito pelo parasitológico de fezes e o tratamento é baseado no uso de anti-protozoários ou anti-helmínticos. Objetivo: analisar os principais aspectos das parasitoses intestinais presentes no Nordeste, identificando aquelas mais prevalentes e as populações de maiores riscos. Metodologia: o estudo foi do tipo descritivo, utilizando-se uma revisão integrativa de literatura do período de 2012 a 2021, nas bases de dados eletrônicas Scielo, BVS e Google Acadêmico. Para o processo de busca e análise dos artigos, foi utilizado o diagrama flow. Todos os dados da pesquisa foram analisados no Microsoft Word e, posteriormente os resultados foram transformados em quadros e tabelas para melhor organização e interpretação das informações colhidas. Resultados: observou-se, a partir da análise dos 10 artigos, uma variação de 26,00% a 92,85% na taxa de infecções por parasitas intestinais na região Nordeste, em diversas populações, desde pré-escolares, escolares e adultos. Os fatores de risco que estiveram associados ao desenvolvimento, foram: falta de higiene pessoal e dos alimentos, ausência ou precariedade dos serviços de saneamento básico, baixa escolaridade e renda dos pais, qualidade da água de beber e práticas de andar descalço por parte das crianças. Considerações finais: Os resultados apontam para a necessidade de medidas preventivas e tratamento das enteroparasitoses, como mudanças de hábitos de higiene e melhorias nas condições de vida, como o acesso à água tratada e saneamento básico adequado. Além disso, é importante que surjam mais estudos epidemiológicos para subsidiar medidas de controle e prevenção adequadas e eficazes.


Intestinal parasites are still a public health problem in Brazil, especially in regions where basic sanitation and socioeconomic conditions are fragile, as in the Northeast region. They are a set of diseases caused by helminthic or protozoan parasites, which affect the human intestine, causing symptoms such as diarrhea, abdominal pain, asthenia, nausea, vomiting, among others. Its diagnosis is made by fecal parasitology and treatment is based on the use of anti-protozoal or anthelmintic agents. Objective: to analyze the main aspects of intestinal parasites present in the Northeast, identifying the most prevalent ones and the populations at greatest risk. Methodology: the study was descriptive, using an integrative literature review from 2012 to 2021, in the electronic databases Scielo, BVS and Google Scholar. For the process of searching and analyzing the articles, the flow diagram was used. All survey data were analyzed in Microsoft Excel and, later, the results were transformed into graphs and tables for better interpretation of the collected information. RESULTS: from the analysis of the 10 articles, a variation from 26.00% to 92.85% was observed in the rate of infections by intestinal parasites in the Northeast region, in different populations, from preschoolers, schoolchildren and adults. The risk factors that were associated with development were: lack of personal hygiene and food, absence or precariousness of basic sanitation services, low parental education and income, quality of drinking water and practices of walking barefoot by children. Final Considerations: The results point to the need for preventive measures and treatment of intestinal parasites, such as changes in hygiene habits and improvements in living conditions, such as access to treated water and adequate basic sanitation. In addition, it is important that more epidemiological studies appear to support appropriate and effective control and prevention measures.


Las parasitosis intestinales siguen siendo un problema de salud pública en Brasil, especialmente en regiones donde el saneamiento básico y las condiciones socioeconómicas son frágiles, como en la región Nordeste. Son un conjunto de enfermedades causadas por parásitos helmínticos o protozoarios, que afectan el intestino humano, causando síntomas como diarrea, dolor abdominal, astenia, náuseas, vómitos, entre otros. Su diagnóstico se realiza mediante parasitología fecal y el tratamiento se basa en el uso de agentes antiprotozoarios o antihelmínticos. Objetivo: analizar los principales aspectos de los parásitos intestinales presentes en el Nordeste, identificando los más prevalentes y las poblaciones de mayor riesgo. Metodología: el estudio fue descriptivo, utilizando una revisión bibliográfica integradora de 2012 a 2021, en las bases de datos electrónicas Scielo, BVS y Google Scholar. Para el proceso de búsqueda y análisis de los artículos, se utilizó el diagrama de flujo. Todos los datos de la encuesta fueron analizados en Microsoft Excel y, posteriormente, los resultados fueron transformados en gráficos y tablas para una mejor interpretación de la información recogida. Resultados: a partir del análisis de los 10 artículos, se observó una variación de 26,00% a 92,85% en la tasa de infecciones por parásitos intestinales en la región Nordeste, en diferentes poblaciones, desde preescolares, escolares y adultos. Los factores de riesgo que se asociaron al desarrollo fueron: falta de higiene personal y alimentaria, ausencia o precariedad de servicios de saneamiento básico, baja escolaridad e ingresos de los padres, calidad del agua de consumo y prácticas de caminar descalzo por parte de los niños. Consideraciones Finales: Los resultados apuntan a la necesidad de medidas preventivas y de tratamiento de los parásitos intestinales, como cambios en los hábitos de higiene y mejoras en las condiciones de vida, como el acceso a agua tratada y a un saneamiento básico adecuado. Además, es importante que aparezcan más estudios epidemiológicos que respalden medidas de control y prevención adecuadas y eficaces.

Objectives: This study aimed to examine possible associations between previously undiagnosed subclinical hypothyroidism and short-term outcomes and mortality in a sample of Iraqi patients undergoing primary percutaneous coronary intervention for ST-segment elevation myocardial infarction. Design: This is a prospective observational cohort study. Setting: The study was conducted in a single tertiary referral centre in Baghdad, Iraq. Participants: Thyroid-stimulating hormone and free T4 levels were measured in 257 patients hospitalised with STelevation myocardial infarction who underwent primary percutaneous coronary intervention between January 2020 and March 2022. Main outcome measures: Adverse cardiovascular and renal events during hospitalisation and 30-day mortality were observed. Results: Previously undiagnosed subclinical hypothyroidism was detected in 36/257 (14%) ST-elevation myocardial infarction patients and observed more commonly in females than males. Patients with subclinical hypothyroidism had significantly worse short-term outcomes, including higher rates of suboptimal TIMI Flow (< III) (p =0.014), left ventricular ejection fraction ≤ 40% (p=0.035), Killip class >I (p=0.042), cardiogenic shock (p =0.016), cardiac arrest in the hospital (p= 0.01), and acute kidney injury (p= 0.044). Additionally, 30-day mortality was significantly higher in patients with subclinical hypothyroidism (p= 0.029). Conclusion: Subclinical hypothyroidism previously undiagnosed and untreated had a significant association with adverse short-term outcomes and higher short-term mortality within 30 days compared to euthyroid patients undergoing primary percutaneous coronary intervention. Routine thyroid function testing during these patients' hospitalisation may be warranted.

Lespedeza cuneata (LC) is a perennial plant used in herbal medicine to treat numerous diseases, including prostatic hyperplasia, diabetes, early atherosclerosis, and hematuria. Reference collections of bioactive compounds of LC are crucial for the determination of their pharmacological properties. However, little is known regarding its anti-oxidative and anti-inflammatory effects in alveolar macrophage (MH-S) cells. This study examined whether LC can inhibit reactive oxygen species and Coal fly ash (CFA) induced inflammation in MH-S cells. The anti-oxidative effects of LC were evaluated using 1,1-diphenyl-2-picrylhydrazyl (DPPH) and 2,2'-azino-bis(3-ethylbenzothiazoline-6-sulfonic acid) (ABTS) assays, anti-inflammatory effects were examined using nitric oxide (NO) assay, and cytotoxicity was analyzed using methyl thiazolyl tetrazolium assay. The expression of inflammatory cytokine genes was assessed through a reverse-transcription polymerase chain reaction. Our results revealed that LC exhibited high radical scavenging activity and a dose-dependent (7.8–1,000 μg/mL) inhibition of oxidation as compared to ascorbic acid and Trolox. It also inhibited CFA-induced NO production in MH-S cells. Moreover, it suppressed the CFA exposure-mediated expression of pro-inflammatory mediators and cytokines, including inducible nitric oxide synthase (iNOS), cyclooxygenase-2 (COX-2), interleukin (IL)-1β, IL-6, and tumor necrosis factor-α. These results suggest that LC is a potent antioxidant and anti-inflammatory agent that can be useful as a nutraceutical product.

Cardiovascular disease (CVD) related mortality and morbidity heavily strain society. The relationship between external risk factors and our genetics have not been well established.It is widely acknowledged that environmental influence and individual behaviours play a significant role in CVD vulnerability, leading to the development of polygenic risk scores (PRS). We employed the PRISMA search method to locate pertinent research and literature to extensively review artificial intelligence (AI)-based PRS models for CVD risk prediction.Furthermore, we analyzed and compared conventional vs. AI-based solutions for PRS. We summarized the recent advances in our understanding of the use of AI-based PRS for risk prediction of CVD. Our study proposes three hypotheses: i) Multiple genetic variations and risk factors can be incorporated into AI-based PRS to improve the accuracy of CVD risk predicting. ii) AI-based PRS for CVD circumvents the drawbacks of conventional PRS calculators by incorporating a larger variety of genetic and non-genetic components, allowing for more precise and individualised risk estimations. iii) Using AI approaches, it is possible to significantly reduce the dimensionality of huge genomic datasets, resulting in more accurate and effective disease risk prediction models. Our study highlighted that the AI-PRS model outperformed traditional PRS calculators in predicting CVD risk. Furthermore, using AI-based methods to calculate PRS may increase the precision of risk predictions for CVD and have significant ramifications for individualized prevention and treatment plans.
