RESUMEN

Prostatic abscess is seen infrequently. Furthermore small abscess or microabscess accompanied with BPH or chronic prostatitis is likely to be overlooked and very difficult to diagnose with history and physical examination due to obscurity of its clinical symptoms. We have experienced frequent pus discharge from resected area of prostate during transurethral resection in patients with BPH and/or chronic prostatitis. And through clinical observation of 32 patients with BPH and 7 patients with chronic prostatitis whom we had treated by transurethral resection of prostate following results were obtained. 1. Prostatic abscess was found in 26 cases of 39 patients with BPH and/or chronic prostatitis during transurethral resection of prostate. 2. Transvesical and transrectal ultrasonography of prostate may increase the chances to diagnose small abscess with sensitivity of 46%. 3. Urinary tract infection and concomitant prostatic calculi with lower urinary tract obstruction were important predisposing factors in formation of prostatic abscess. 4. Diabetes mellitus was accompanied in 5 cases of 26 patients of prostatic abscess. 5. Definite drainage with transurethral resection of the gland and the use of broad spectrum antibiotics brought good results.

Uninfected prostatic calculi cause no symptoms, however, when infected, it serves as a source of resistance against antibiotic therapy. We have performed transabdominal and transrectal prostatic ultrasonography in 64 cases of chronic bacterial prostatitis, and these patients were divided into two groups ; calculi group(42 cases) and non-calculi group(22 cases). All patients of non-calculi group and 37 cases of calculi group were treated with oral antibiotics, and 5 cases of calculi group who had concomitant benign prostatic hyperplasia were treated with transurethral resection. The cure rate of antibiotic therapy in non-calculi group(63. %) was statistically higher than that in calculi group(3S.7%) (p=0.005). Five out of 17 cases in calculi group(42 cases) which had concomitant benign prostatic hyperplasia got transurethral resection and 4 of 5 cases became free from prostatitis after transurethral resection of prostate. Judging from the above results, the prostatic calculi can be easily detected by prostatic ultrasonography in patient with chronic bacterial prostatitis. By knowing the presence of prostatic calculi, the treatment modality can be chosen and the prognosis estimated. Furthermore, transurethral resection of prostate may be useful in the treatment of chronic bacterial prostatitis in old aged patients with benign prostatic hyperplasia.

RESUMEN

Possible role of chlamydia trachomatis (C. trachomatis) in non-gonococcal Urethritis and chronic prostatitis was investigated by enzyme immunoassay technique in a study of 85 cases of non-gonococcal urethritis and 23 chronic prostatitis cases. 1. C. trachomatis was detected from 21 (24.7%) of 85 non-gonococcal urethritis patients and third decade represented the highest positive rate. 2. C. trachomatis was detected from 5 cases (21.7%) of 23 chronic prostatitis patients. 3. A significantly higher positive C. trachomatis detection rate of 60. 7% was obtained from those with a profuse urethral discharge compared with those with a scanty discharge. 4. A significantly higher positive C. trachomatis detection rate of 52.3% was obtained from cases with no previous history of urethritis compared with those who had previous history of urethritis in 85 cases of non-gonococcal urethritis.

RESUMEN

A clinical study on therapeutic effects and toxicity of Netilmicin(Neticin(R)) in urinary tract infection was performed in 47 patients with various degrees of impairment of renal function and the following results were obtained. 1. Among 45 patients with various urinary tract infection, Netilmicin brought total elimination of the bacteria in 38 patients, persistent infection(one Serratia, one Pseudomonas fluoresce.) in 2 patients superinfection with a resistant strain of Psuedomonas fluoresces and a Netilmicin-sensitive

RESUMEN

Rhabdomyosarcoma of the kidney is a highly malignant and very rare tumor. Only a few cases were reported previously. A 54-years-old male was admitted to the Korea University, Haewha hospital with gross, painless hematuria and palpable mass in RUQ of abdomen. Herein, we are presenting a case of renal rhabdomyosarcoma with the review of literatures.

RESUMEN

Benign polyps with prostatic type epithelium (BPPE) in the male urethra is known as a rare case. The common signs of reported cases are painless gross hematuria, hemospermia, microscopic hematuria, dysuria, frequency and postatism in young adult. We are reporting three cases of benign polyps with prostatic type epithelium of the verumontanum and the clinicopathological findings of these cases with the references.

RESUMEN

Leydig cell tumors of the testis is only 1 to 3% of all testicular tumors. Although they are usually clinically benign, about 1O% of the reported cases has been associated with a malignant course. Malignant testicular leydig cell tumors occur exclusively in adult. Early metastasis is usually seen and there is no proved sensitivity to radiation or chemotherapeutic regimens in patients with disseminated tumor with poor prognosis. A-70-year-old man was admitted to Korea University Haewha Hospital with rapid growing, nontender testicular mass. On physical examination, right testicular hard mass measuring 9 x 6 x 5cm in size showed negative transilumination test. Under the impression of testicular tumor, radical orchiectomy was performed. Histologic and electron-microscopic features revealed malignant leydig cell tumor and the literatures are reviewed.

RESUMEN

It is well known fact that dry ejaculation is commonly found following simple prostatectomy including transurethral resection of hypertrophied prostate. To preserve normal ejaculatory function following transurethral resection in benign prostatic hypertrophy patients, we have modified the technique of transurethral resection to give less injury to the bladder neck, ejaculatory duct, urethral mucosa and branches of nerves by preserving smaller lateral lobe in cases one lateral lobe is much smaller than the other, or preserving lower one third of a lateral lobe in cases of big bilateral lobes, resecting all other lobes(B-type), and this technique was applied in 66 cases. Control group was 24 cases in which all lobes were resected like ordinary transurethral resection technique(A-type). Ninety cases, ranged in age from 42 to 79 years (mean; 68.0 years), were followed by interviewing with the questionaionaires concerning sexual desire, erection, vaginal penetration, orgasm and ejaculation. Loss of ejaculatory function was noted preoperatively in 4 cases of the group A and in 6 cases of the group B. Four out of 60 cases had dry ejaculation in the group in which B-type of transurethral resection of prostate was applied, while 15 out of 20 cases had dry ejaculation or anejaculation in the group A. There were 3 patients who had undergone B-type operation first and one to four years later A-type operation was also performed, and they complained of no ejaculatory disturbance after B-type operation. However, dry ejaculation did occur after A-type operation in all three patients. All 90 cases were able to achieve an erection with sexual desire preoperatively and postoperatively Seven cases of decreased orgasm were noted in the group B, while 8 cases complained of decreased orgasm in the group A. Judging from above results this modified technique of transurethral resection of prostate is a useful method to preserve normal ejaculatory function.

RESUMEN

Primary Signet-ring-cell adenocarcinoma of the urinary bladder is an extremely rare variant that promotes considerable interest, 21 cases reported as of 1981. Morphologically, this tumor is identical to signet-ring-cell carcinomas of the gastrointestinal tract, breast, lung and nasal cavity, and differentiation must be carried out with great care. Characteristically, primary signet-ring-cell adenocarcinoma carries a poor prognosis with early penetration of the mucosa, submucosa, and muscularis. This tumor is sessile, infiltrates early, extends locally to adjacent organs, and metastasized late. Many patients reported died of local extension with ureteral obstruction and uremia rather than wide spread metastasis. We are reporting two cases of primary signet-ring-cell adenocarcinoma of the urinary bladder treated with radical cystectomy and transurethral resection with radiotherapy, which died of local extension with ureteral obstruction and uremia, with pathological findings and review of the literatures.

RESUMEN

Retrocaval or postcaval ureter is a well-recognized venous congenital anomaly commonly causing ureteric obstruction. A 42 year-old male was admitted to Korea University Hospital because of right flank pain. The right AGP reveals the S-shaped coures of ureter and hydronephrosis. The ureteral and to end anastomosis was done with double J ureteral stent. Herein we present a case of retrocaval ureter with brief review of literatures.

RESUMEN

Pertutaueous transluminal angioplasty is the treatment of choice for renovascular hypertension in cases due to the renal vascular occlusion which is not severe enough to pass dilatation catheter. The advantages of percutaneous transluminal angioplasty are the use of local anesthesia, its noninvasiveness without surgical risk, feasibility of repetition and shorter hospital days compared with surgical methods. We are presenting a case of renovascular hypertension due to atherosclerosis cured by percutaneous transluminal angioplasty with the review of literatures.

RESUMEN

Priapism is a relatively rare condition consisted of persistent painful erection, unrelated to sexual activity. The management is often delayed or ineffective, resulting in the development erectile bodies into impotence. The most common form in adult is idiopathic but in the childfood sickle cell disease is more common. Initial therapy consist of Analgesic, Antibiotic, Sedatives, Ice water enema, and Anticoagulant irrigation of corpus cavernosum. We used Hemo-Vac drainage which was continuous drainage from corpus cavernosum and obtained a good results.

RESUMEN

A case of ovarian dermoid cyst invading the posterior bladder wall with pilimiction and recurrent bladder stone was presented. Fifty-four years old female patient had a symptoms of frequency, dysuria, hematuria and pilimiction intermittently She had received cystolithotomies and litholapaxy due to recurrent bladder stones for 3 times in other hospitals Cystoscopic examination revealed elevated flat mass on the base of bladder and hairs outgrowing a from the surface of mass. Also two-large bladder stones were seen at cystoscopy. So, recurrent bladder stones caused by the hairs growing from invading dermoid cyst was diagnosed. Removal of bladder stone, partial cystectomy and left oophorectomy were performed.

RESUMEN

A clinical observation was made on 239 cases of benign prostatic hypertrophy, admitted to the Department of Urology, Korea University College of Medicine during the period of 9 years and 8 months from Jan. 1972 to Aug. 1981. Following results were obtained. 1. Amount of residual urine from 10 ml to 1,500 ml with average 304 ml. 2. Hypertension in 35 cases (14.8%)and pulmonary diseases in 29 cases (12.1%)were frequently associated with B.P.H. 3. E.K.G. was performed in 171 cases. Abnormal findings included L.V.H. in 38 cases (22.2%), arrhythmia in 11 cases (6.4%), but normal in 118 cases (69%). 4. In blood chemistry, blood urea nitrogen was elevated in 75 cases (31.4%)and creatinine was elevated in 69 cases (28.9%). 5. The most common endoscopic findings were trabeculation in 214 cases (89.5%), inflammation in 192 cases (80.3%), kissing of lateral lobes in 138 cases (58.7%)and elevated interureteric ridge in 71 cases (29.7%). 6. Methods of operative treatment were T.U.R. in 184 cases, retropubic prostatectomy in 7 cases and suprapubic prostatectomy in 3 cases.

RESUMEN

Xanthogranulomatous pyelonephritis is an atypical form of severe renal parenchymal infection, characterized by yellow lobulated masses macroscopically and massive inflammatory cell, foamy histiocyte microscopically. Preoperative diagnosis confused with hypernephroma, pyonephrosis with stone, other renal inflammatory disease. We report two cases of xanthogranulomatous pyelonephritis, diagnosed by microscopical finding and literature has been reviewed briefly.

Renal angiography is a highly valuable diagnostic procedure for the detection and exact evaluation of various renal disease, especially differentiation of benign and malignant lesions. During the period of January 1978 to December 1983, 39patients who had been taken renal angiography were analysed and following results were obtained. 1. Male to female ratio of performing renal angiography was approximately 1:1 and peak incidence was fifth decade. 2. Indication of renal angiography was renal mass, abnormal I.V.P. and R.G.P., renal bleeding, kidney donor, hypertension, in order of its frequency. 3. The most frequent diagnosis encountered was renal cell carcinoma (15 cases), diagnostic accuracy of renal angiography was 93.3%. 4. Arterial embolization of renal cell carcinoma was tried in 2 cases preoperatively, and it was found that this was a valuable method in reducing blood loss, in performing operation.

RESUMEN

Ceftizoxime was administered to 30 cases with urinary tract infection for 7 days in the most cases at a daily dose of 1-2 gm intravenously or intramuscularly. The clinical efficacy of Ceftizoxime was as follows: Excellent results were obtained in 13 cases, good result in 14 cases and poor result in 3 cases, the overall effectiveness rate was 90% and no adverse reaction was noted in all cases. bacteriologically, 13 strains were isolated in 26 cases and 4 cases were bacteriologically unknown, and 2 strains in 3 cases, which were a case of proteus morganii and two cases of Pseudomonas fluoresces, were failed in treatment. Judging from the response to Ceftizoxime as shown in al Tables, Ceftizoxime is considered to be a very useful antibiotic in the treatment of urinary tract infection.

RESUMEN

A 26-year-old-male with a foreign body stone of the bladder caused by gold thread acupuncture, which was inserted 10 years ago for the treatment of lumbago and migrated spontaneously from the right renal area to the bladder through upper urinary tract, was presented. He was managed by extraction of foreign body stone with foreign body forceps and endoscopic manipulation. Authors report this unusual case with review of some literatures.
