RESUMEN

Aging is a major risk factor for many human diseases, including cognitive impairment, which affects a large population of the elderly. In the past few decades, our understanding of the molecular and cellular mechanisms underlying the changes associated with aging and age-related diseases has expanded greatly, shedding light on the potential role of these changes in cognitive impairment. In this article, we review recent advances in understanding of the mechanisms underlying brain aging under normal and pathological conditions, compare their similarities and differences, discuss the causative and adaptive mechanisms of brain aging, and finally attempt to find some rules to guide us on how to promote healthy aging and prevent age-related diseases.

Abstract Objective: This study aimed to examine changes in miRNAs expression profile of COPD patients. Methods: Thirty-six COPD patients as well as thirty-three healthy volunteers were recruited. Total RNAs were collected from the plasma of each participant. The differentially expressed miRNAs in COPD were screened from the GEO database. RT-qPCR was carried out to detect miRNA expression. Results: In total, 9 out of 55 miRNAs were expressed differentially in COPD patients. Confirmed by RT-qPCR validation, 6 miRNAs increased while 3 miRNAs decreased. Further analysis of miR-423-5p, which has not been reported in COPD, showed that AUC for the diagnosis of COPD was 0.9651, and miR-423-5p levels was inversely correlated with the duration of smoking. Conclusion: The present study demonstrates that miR-423-5p is a potential marker for identifying COPD patients.

@#Abstract: Objective To analyze the impact of the flood disaster on the distribution of Oncomelania snails in 2020 in Wuhu City, to provide scientific basis for formulating the "14th Five-Year Plan" for schistosomiasis control and precise prevention and control measures in Wuhu City. Methods Systematic sampling combined with environmental spot check was used to investigate the distribution of snails in the historical snail's environment, existing snail's environment and suspected snail's environment in Wuhu City. The collected snails were crushed and examined by microscope to understand the death and life of snails and the infection of Schistosoma japonicum, and the density of live snails and other indicators were counted. Results The historical area of snails was 14 475.24 hm2 in Wuhu City, and the existing area of snails was 4 588.72 hm2, including 4 210.32 hm2 for lake type and 378.40 hm2 for hill type snails. The average occurrence rate of live snails was 14.59%, and the average density of live snails was 0.50 snails/0.1 m2 in Wuhu City. There were 2 745 snail breeding environments, of which 491 were lake type and 2 254 are hill type, accounting for 17.89% and 82.11% respectively. The existing snail area was mainly distributed in the beaches and ditches, accounting for 92.51% and 6.29% of the existing snail area respectively. Some snails were distributed in ponds, paddy fields and other environments. Among all the historical snail habitats, the majority were class Ⅱ and class Ⅲ environments (which still have the basic conditions for snail breeding at present), with a total of 1 739 sites (blocks), accounting for 63.35% of the total environment. No schistosomiasis infected snails have been found, and the newly discovered and recovered snails cover an area of 268.21 hm2 in Wuhu City. Conclusions The distribution of snails is extensive in Wuhu City, and snails are mostly distributed in beaches, which are vulnerable to the impact of flood disasters. The spread of Oncomelania snails is found in 30 marshlands in 3 counties in this survey. It is necessary to continue to strengthen snail monitoring after disasters, and take class Ⅱ and class Ⅲ environments as key areas for snail monitoring, so as to find, identify and evaluate the risk of schistosomiasis transmission as soon as possible, to avoid or reduce the impact of flood disasters on the process of schistosomiasis control, and promote the process of schistosomiasis elimination.

Objective To investigate the prevalence of Schistosoma japonicum infection in wild mice in Shitai County, Anhui Province, so as to provide insights into precise control of the source of S. japonicum infections. Methods Wild mice were captured using the trapping method for three successive nights at snail-infested settings from Jitan Village of Jitan Township, and Shiquan Village and Xibai Village of Dingxiang Township, Shitai County, Anhui Province in June and October, 2018. All trapped wild mice were sacrificed and liver and mesenteric vein specimens were collected for detection of S. japonicum eggs using microscopy, while the fecal samples in mouse intestines were collected for identification of S. japonicum infections using Kato-Katz technique. In addition, the population density of trapped wild mice was estimated and the prevalence of S. japonicum infection was calculated in trapped wild mice. Results A total of 376 wild mice were trapped from three villages in Shitai County. The population density of trapped wild mice was 9.1% (376/4 124), and the prevalence of S. japonicum infection was 24.2% (91/376) in trapped wild mice. The highest prevalence of S. japonicum infection was detected in Shiquan Village of Dingxiang Township (30.1%), and the lowest prevalence was seen in Xibai Village of Dingxiang Township; however, there was no significant difference in the prevalence of S. japonicum infection in trapped wild mice among three villages (χ2= 4.111, P > 0.05). In addition, there was no significant difference in the prevalence of S. japonicum infection in wild mice captured between on June (26.8%, 34/127) and October (22.9%, 57/249) (χ2 = 0.690, P = 0.406). The trapped wild mice included 6 species, including Rattus norvegicus, Niviventer niviventer, R. losea, Apodemus agrarius, Mus musculus and N. coning, and the two highest prevalence of S. japonicum infection was detected in R. losea (34.9%, 22/63) and R. norvegicus (31.2%, 44/141). Conclusions The prevalence of S. japonicum infections is high in wild mice in Shitai County, and there is a natural focus of schistosomiasis transmission in Shitai County.

OBJECTIVE@#To analyze the spatial-temporal distribution characteristics of Oncomelania hupensis snails in Anhui Province from 2011 to 2020, to provide insights into precision control of O. hupensis snails in Anhui Province.@*METHODS@#O. hupensis snail distribution data were collected in Anhui Province from 2011 to 2020 and descriptively analyzed, including actual area of snail habitats, area of emerging snail habitats and area of Schistosoma japonicum-infected snails. The actual area of snail habitats and area of emerging snail habitats were subjected to spatial autocorrelation analysis, hotspot analysis, standard deviation ellipse analysis and space-time scanning analysis, and the clusters of snail distribution and settings at high risk of snail spread were identified in Anhui Province from 2011 to 2020.@*RESULTS@#The actual area of snail habitats gradually decreased in Anhui Province from 2011 to 2020. The actual area of snail habitats were 26 238.85 hm2 in Anhui Province in 2020, which were mainly distributed in marshland and lake regions. There was a large fluctuation in the area of emerging snail habitats in Anhui Province during the period from 2011 to 2020, with the largest area seen in 2016 (1 287.65 hm2), and 1.96 hm2 emerging infected snail habitats were detected in Guichi District, Chizhou City in 2020. Spatial autocorrelation and hotspot analyses showed spatial clusters in the distribution of actual areas of snail habitats in Anhui Province from 2011 to 2020 (Z = 3.00 to 3.43, all P values < 0.01), and the hotspots were mainly concentrated in the marshland and lake regions and distributed along the south side of the Yangtze River, while the cold spots were mainly concentrated in the mountainous regions of southern Anhui Province. There were no overall spatial clusters in the distribution of areas of emerging snail habitats (Z = -2.20 to 1.71, all P values > 0.05), and a scattered distribution was found in local regions. Standard deviation ellipse analysis showed relatively stable distributions of the actual areas of snail habitats in Anhui Province from 2011 to 2020, which was consistent with the flow direction of the Yangtze River, and the focus of the distribution of areas of emerging snail habitats shifted from the lower reaches to upper reaches of Anhui section of the Yangtze River. Space-time scanning analysis identified two high-value clusters in the distribution of actual areas of snail habitats in lower and middle reaches of Anhui section of the Yangtze River from 2011 to 2020, and two high-value clusters in the distribution of areas of emerging snail habitats were identified in mountainous and hilly regions.@*CONCLUSIONS@#There were spatial clusters in the distribution of O. hupensis snails in Anhui Province from 2011 to 2020, which appeared a tendency of aggregation towards the south side and upper reaches of the Yangtze River; however, the spread of O. hupensis snails could not be neglected in mountainous and hilly regions. Monitoring of emerging snail habitats should be reinforced in mountainous and hilly regions and along the Yangtze River basin.

RESUMEN

Currently, the national schistosomiasis control program of China is moving from transmission interruption to elimination, and there are multiple challenges during the stage moving towards the progression of schistosomiasis elimination, including a high difficulty in shrinking snail-infested areas, unstable achievements for infectious source control, imperfect surveillance system and a reduction in schistosomiasis control and administration. Based on the core suggestions proposed in the 2022 WHO guideline on control and elimination of human schistosomiasis, recommendations on schistosomiasis surveillance system building, development of novel diagnostics, adjustment of the schistosomiasis control strategy and maintaining and improvements of the schistosomiasis control capability are proposed for the national schistosomiasis control program of China in the new era according to the actual status of schistosomiasis control in China. Formulation of the national schistosomiasis control strategy and goal from One Health perspective, verification of transmission interruption and elimination of schistosomiasis, precision implementation of schistosomiasis control interventions with adaptations to local circumstances, development and application of highly sensitive and specific diagnostics are recommended for elimination of schistosomiasis in China. In addition, the implementation of the 2022 WHO guideline on control and elimination of human schistosomiasis may guide the elimination of schistosomiasis in China.

RESUMEN

Preventive chemotherapy is one of the pivotal interventions for the control and elimination of schistosomiasis, which is effective to reduce the morbidity and prevalence of schistosomiasis. In order to promote the United Nations' sustainable development goals and the targets set for schistosomiasis control in the Ending the neglect to attain the Sustainable Development Goals: a road map for neglected tropical diseases 2021-2030, WHO released the guideline on control and elimination of human schistosomiasis in 2022, with major evidence-based updates of the current preventive chemotherapy strategy for schistosomiasis. In China where great success has been achieved in schistosomiasis control, the preventive chemotherapy strategy for schistosomiasis has been updated several times during the past seven decades. This article reviews the evolution of the WHO guidelines on preventive chemotherapy and Chinese national preventive chemotherapy schemes, compares the current Chinese national preventive chemotherapy scheme and the recommendations for preventive chemotherapy proposed in the 2022 WHO guideline on control and elimination of human schistosomiasis, and proposes recommendations for preventive chemotherapy during the future implementation of the 2022 WHO guideline, so as to provide insights into schistosomiasis control among public health professionals engaging in healthcare foreign aid.

RESUMEN

ObjectiveTo explore the mechanism of antidepressant effect of lily polysaccharide (LLP)and astragalus polysaccharide(APS). MethodSixty KM mice were randomly divided into blank group, model group, fluoxetine hydrochloride (8 mg·kg-1)group, LLP (0.2 g·kg-1)group, APS (0.2 g·kg-1)group and polysaccharide combination (LLP+APS,0.1 g·kg-1+0.1 g·kg-1)group, with 10 mice in each group. Except the blank group, the other groups were given chronic unpredictable mild stress (CUMS) induced mouse depression model. On the 29th day of modeling,fluoxetine hydrochloride group was given corresponding dose of fluoxetine hydrochloride, and polysaccharide groups were given corresponding drug. The depressive behavior of mice was evaluated by behavioral indexes such as body mass change, open field test. The morphological changes of hippocampal CA1 neurons were observed by Nissl staining. The contents of 5-hydroxytryptamine (5-HT), adrenocorticotropic hormone (ACTH), and corticosterone (CORT), in brain tissue and plasma were measured by enzyme-linked immunosorbent assay (ELISA). Western blot was used to detect the expression levels of related proteins in adenylate cyclase/cyclic adenylate phosphate/protein kinase A (AC/cAMP/PKA) signal pathway. ResultCompared with the blank group, mice in the model group gained weight slowly, the total distance, central distance and sugar water preference rate decreased significantly (P<0.01), the depressive behavior was significant, the hippocampal neurons were seriously damaged, the content of 5-HT decreased (P<0.01), the contents of ACTH and CORT increased significantly (P<0.01), adenylate cyclase 6(ADCY6), PKA and cAMP response element binding protein-1 (CREB-1) and brain-derived neurotrophic factor (BDNF) protein expression decreased significantly (P<0.01). Compared with the model group, depressive behavior of mice in LLP group, APS group and LLP+APS group was significantly improved (P<0.01). The antidepressant effect of LLP+APS was better than that of LLP and APS. Each administration group could alleviate the damage of hippocampal neurons in varying degrees, significantly increase the content of 5-HT in brain tissue (P<0.01), and reduce the levels of ACTH and CORT in plasma (P<0.05). The protein levels of ADCY6, PKA, CREB-1 and BDNF were significantly increased (P<0.05). ConclusionThe antidepressant effect of LLP+APS is significantly enhanced and has a synergistic effect. The mechanism may be closely related to affecting the content of neurotransmitters, inhibiting HPA axis activity and activating AC/cAMP/PKA signal transduction pathway.

OBJECTIVE@#To study the effects of total ginsenosides (TG) extract from Panax ginseng on neural stem cell (NSC) proliferation and differentiation and their underlying mechanisms.@*METHODS@#The migration of NSCs after treatment with various concentrations of TG extract (50, 100, or 200 µ g/mL) were monitored. The proliferation of NSCs was examined by a combination of cell counting kit-8 and neurosphere assays. NSC differentiation mediated by TG extract was evaluated by Western blotting and immunofluorescence staining to monitor the expression of nestin and microtubule associated protein 2 (MAP2). The GSK-3β/β-catenin pathway in TG-treated NSCs was examined by Western blot assay. The NSCs with constitutively active GSK-3β mutant were made by adenovirus-mediated gene transfection, then the proliferation and differentiation of NSCs mediated by TG were further verified.@*RESULTS@#TG treatment significantly enhanced NSC migration (P<0.01 or P<0.05) and increased the proliferation of NSCs (P<0.01 or P<0.05). TG mediation also significantly upregulated MAP2 expression but downregulated nestin expression (P<0.01 or P<0.05). TG extract also significantly induced GSK-3β phosphorylation at Ser9, leading to GSK-3β inactivation and, consequently, the activation of the GSK-3β/β-catenin pathway (P<0.01 or P<0.05). In addition, constitutive activation of GSK-3β in NSCs by the transfection of GSK-3β S9A mutant was found to significantly suppress TG-mediated NSC proliferation and differentiation (P<0.01 or P<0.05).@*CONCLUSION@#TG promoted NSC proliferation and neuronal differentiation by inactivating GSK-3β.

OBJECTIVE@#To investigate the pharmacodynamic material basis, mechanism of actions and targeted diseases of Salicornia europaea L. (SE) based on the network pharmacology method, and to verify the antidepressant-like effect of the SE extract by pharmacological experiments.@*METHODS@#Retrieval tools including Chinese medicine (CM), PubMed, PharmMapper, MAS 3.0 and Cytoscape were used to search the components of SE, predict its targets and related therapeutic diseases, and construct the "Component-Target-Pathway" network of SE for central nervous system (CNS) diseases. Further, protein-protein interaction (PPI) network, Kyoto Encyclopedia of Genes and Genomes (KEGG) and Gene Ontology (GO) function annotation of depression-related targets were analyzed to predict the antidepressant mechanism of SE. Chronic unpredictable mild stress (CUMS) model was used to construct a mouse model with depression-like symptoms. And the animals were randomly divided into 6 groups (n=10) including the normal group (nonstressed mice administered with distilled water), the CUMS group (CUMS mice administered with distilled water), the venlafaxine group (CUMS mice administered with venlafaxine 9.38 mg/kg), SE high-, medium-, and low-dose groups (CUMS mice administered with SE 1.8, 1.35 and 0.9 g/kg, respectively). Then some relevant indicators were determined for experimental verification by the forced swim test (FST), the tail suspension test (TST) and open-field test (OFT). Dopamine (DA) concentration in hippocampus and cerebral cortex, IL-2 and corticosterone (CORT) levels in blood, and nuclear factor E2 related factor 2 (Nrf2), kelch-like epichlorohydrin related protein 1 (Keap1), NAD(P) H dehydrogenase [quinone] 1 (NQO1) and heme oxygenase-1 (HO-1) levels in mice were measured by enzyme linked immunosorbent assay (ELISA) and Western blot respectively to explore the possible mechanisms.@*RESULTS@#The "target-disease" network diagram predicted by network pharmacology, showed that the potential target of SE involves a variety of CNS diseases, among which depression accounts for the majority. The experimental results showed that SE (1.8, 1.35 g/kg) significantly decreased the immobility period, compared with the CUMS group in FST and TST in mice after 3-week treatment, while SE exhibited no significant effect on exploratory behavior in OFT in mice. Compared with CUMS group, the SE group (0.9 g/kg) showed significant differences (P<0.05) in DA levels in the hippocampus and cerebral cortex. In addition, compared with CUMS control group, SE (1.8 g/kg) group showed a significant effect on decreasing the activities of CORT (P<0.05), and serum IL-2 level with no statistical significance. Finally, Western blot results showed that compared with the model group, Nrf2, Keap1, NQO1 and HO-1 protein expressions in SE group (1.8 g/kg) were up-regulated (all P<0.01).@*CONCLUSION@#The SE extract may have an antidepressant effect, which appeared to regulate Nrf2-ARE pathway and increased levels of DA and CORT in the hippocampus and cortex.

Objective:To study the protective effect of Xiayuxue Tang on adenine-induced renal fibrosis model in rats and its impact on Wnt/<italic>β</italic>-catenin and transforming growth factor <italic>β</italic><sub>1</sub>(TGF-<italic>β</italic><sub>1</sub>)/Smad signal pathway. Method:A total of 50 SPF-grade male SD rats were randomly divided into 5 groups: the normal group, the model group, the losartan group (9 mg·kg<sup>-1</sup>) and low and high dose (2.43,4.86 g·kg<sup>-1</sup>) of Xiayuxue Tang groups. The rat model of renal fibrosis was established by ig administration adenine (250 mg·kg<sup>-1</sup>) for 24 consecutive days. The rats were then given the corresponding drugs for 30 consecutive days. The levels of serum creatinine(SCr) and blood urea nitrogen (BUN) were measured by enzyme-linked immunosorbent assay(ELISA). The histopathological changes of renal tissues in rats were observed by hematoxylin and eosin (HE) staining. The collagen deposition in rat renal tissue was observed by Masson staining; the protein expression levels of Wnt5a, Wnt5b, <italic>β</italic>-catenin, TGF-<italic>β</italic><sub>1</sub>, Smad4, Smad7 in renal tissue were detected respectively by immunohistochemistry(IHC) and Western blot. Result:Compared with the normal group, the results of each experimental group showed that SCr and BUN levels significantly increased in the model group (<italic>P</italic><0.01). SCr and BUN levels decreased significantly after the intervention with the Xiayuxue Tang (<italic>P</italic><0.01). Compared with the normal group, HE and Masson staining results showed that rats in the model group had severe renal interstitial damage and massive deposition of renal interstitial collagen. The renal interstitial tubule injury was relieved after the intervention with the Xiayuxue Tang, and the renal interstitial collagen deposition decreased. The results of IHC and Western blot showed that compared with the normal group, the expressions of Wnt5a, <italic>β</italic>-catenin, TGF-<italic>β</italic><sub>1</sub> protein in the kidney of rats up-regulated (<italic>P</italic><0.01), while the expressions of Wnt5b and Smad7 protein down-regulated (<italic>P</italic><0.01). After the intervention with Xiayuxue Tang, the expressions of Wnt5a, <italic>β</italic>-catenin, TGF-<italic>β</italic><sub>1</sub> protein down-regulated (<italic>P</italic><0.01), while the expressions of Wnt5b and Smad7 protein up-regulated(<italic>P</italic><0.01). There was no significant difference between the low-dose and high-dose groups with Xiayuxue Tang. Conclusion:Xiayuxue Tang has the protective effect on RIF rats induced by adenine, and its mechanism is related to the inhibition of Wnt/<italic>β</italic>-catenin and TGF-<italic>β</italic><sub>1</sub>/Smad signal pathway.

Malignant tumors are important diseases that threaten human health. Although targeting and immunotherapy have been gradually applied in the treatment of malignant tumors in recent years, which can alleviate the suffering of patients to a certain extent, there are still problems of large adverse reactions and easy drug resistance. At present, chemotherapy is still the main method for the treatment of malignant tumors. While killing tumor cells, chemotherapy also damages normal cells, which often leads to drug toxicity, such as bone marrow suppression, gastrointestinal adverse reactions, liver and kidney function damage, and oral mucosal reactions. Although modern medicines have a certain effect on the toxicity of chemotherapy drugs, there are still limitations. Traditional Chinese medicine(TCM) has a long history of treating malignant tumors, and considers that chemotherapy is a drug with toxin invading the body, exacerbating the "deficiency", "toxin" and "stasis" of the body, causing damage to Qi, blood and organs, especially in spleen, stomach, liver and kidney, and leading to bone marrow suppression, liver and kidney function injury and other adverse reactions. Studies have confirmed that the use of TCM treatment has a better clinical efficacy. Therefore, new therapies shall be explored based on the basic theory of traditional Chinese medicine. As the core part of the theoretical system of TCM, Viscera-state doctrine is closely related to looking, listening, questioning and feeling the pulse of TCM, and has constantly developed and improved. It has important significance in guiding diagnosis and treatment of diseases. This study is guided by viscera-state doctrine of TCM and based on the etiology and pathogenesis of TCM. It starts with the clinical manifestations of common drug toxicity of chemotherapy drugs, such as bone marrow suppression, gastrointestinal adverse reactions, liver and kidney function damage, oral mucosal reactions, which are external signs of the toxicity of chemotherapeutic drugs, and associates pathological manifestations of viscera to physiological functions. From elephants and Tibetans, it systematically summarizes the viscera characteristics of various common chemotherapeutic drugs and provides new ideas and methods for clinical use of TCM in treating the toxicity of chemotherapeutic drugs, so as to promote the application of viscera-state doctrine in diagnosis and treatment of diseases.

RESUMEN

Objective:To observe the effect of Shengyutang on the levels of monoamine neurotransmitters in the hippocampus, and explore its possible mechanism on improving the learning and memory abilities of sleep deprivation (SD) mice. Method:The 50 mice were divided into normal group, model group, estazolam group, Shengyutang low and high dose groups, with 10 mice in each group. A multi-platform water environment was used to prepare SD mouse models. The low and high-dose Shengyutang groups received intragastric administration of 12.5, 25 g·kg<sup>-1</sup>, respectively. The mice in the model group were intragastrically administered with the same dose of normal saline daily for 8 weeks. Morris water maze experiment was used to observe the behavioral changes of SD mice in the evasion latency period, the number of crossing platforms, and the stay time in the target quadrant of each group. HE staining was used to observe the pathomorphological changes of the hippocampal tissue of each group. The expression levels of eight monoamine neurotransmitters including serotonin (5-HT),dopandne (DA),epinephrine (EP),norepinephrine (NE),5-hydroxyindole acetic acid(5-HIAA), high vanillic acid (HVA), levodopa(<italic>L</italic>-DOPA),and 3,4-dihydroxyphenylacetic acid(DOPAC)were detected by high performance liquid chromatography, and the expression levels of c-Fos protein in hippocampus were detected by immunohistochemistry. Result:Compared with the normal group, the SD mice in the model group were in a poorer general state and severe fatigue was observed. Compared with the model group, SD mice in each dose group of Shengyutang got improved in eating, activity, sleep, hair color, and response to external stimuli. Compared with the normal group, the body weight of SD mice in the model group was significantly reduced (<italic>P</italic><0.05), but the body weight in the Shengyutang high-dose group increased the most as compared with the model group (<italic>P</italic><0.05). Compared with the normal group, the hippocampal cells in the model group were disorderly arranged, incomplete in shape, increased in gap and decreased in number. Compared with the model group, the number of neurons in the hippocampus of SD mice in each dose group of Shengyutang increased. Compared with the normal group, the escape latency time of SD mice in the model group was significantly prolonged, the times of crossing platform and the residence time in the target quadrant significantly decreased (<italic>P</italic><0.01). Compared with the model group, the times of crossing platform and the residence time in the target quadrant of mice in each dose group of Shengyutang significantly increased (<italic>P</italic><0.05, <italic>P</italic><0.01). Compared with the normal group, the levels of 5-HT, 5-HIAA, <italic>L</italic>-DOPA, DOPAC, EP, NE, HVA and DA in the model group significantly decreased (<italic>P</italic><0.05,<italic> P</italic><0.01); but these levels in each dose group of Shengyutang were higher than those in model group (<italic>P</italic><0.05). Compared with the normal group, the average MD value of c-Fos protein in the hippocampus of the model group significantly increased (<italic>P</italic><0.01), and the expression levels of c-Fos protein in the hippocampus of Shengyutang groups were significantly lower than those in model group (<italic>P</italic><0.01). Conclusion:Shengyutang can improve the learning and memory abilities of SD rats, and its mechanism may be related to the decrease of monoamine neurotransmitter and c-Fos protein expression.

Diosgenin is widely distributed in many plants, such as Polygonatum sibiricum, Paris polyphylla, Dioscorea oppositifolia, Trigonella foenum-graecum, Costus speciosus, Tacca chantrieri, which has good anti-tumor activity and preferable effects on preventing atherosclerosis, protecting the heart, treating diabetes, etc. This review combed through the anti-tumor mechanisms of diosgenin encompassing lung, breast, gallbladder, liver, oral cavity, stomach, bladder, bone marrow, etc. Besides, it was discovered that diosgenin mainly exerts its effect by inhibiting tumor cell migration, suppressing tumor cell proliferation and growth, and inducing cell apoptosis. However, problems like low yield and bioavailability frequently exist in natural diosgenin. This review introduced methods such as structural modification, dosage form optimization and combination medication to improve the yield and anti-tumor activity of diosgenin. Via the summary of this paper, it is expected to provide theoretical basis for the rational exploitation and utilization of diosgenin.

Surveillance is an important part of the national schistosomiasis control program of China, which play a critical role in facilitating the progress towards elimination of schistosomiasis in China. The coverage of schistosomiasis surveillance has shifted from the high-risk regions at the initial stage of the national schistosomiasis control program of China to the current all endemic counties (cities, districts), and the surveillance contents have extended from single surveillance of endemic status to comprehensive surveillance of endemic status, epidemic factors and transmission risks. With the continuous progress towards schistosomiasis elimination, the national schistosomiasis control program has shifted from “extensive control” to surveillance and early warning-based “precision control” in China, which proposes much higher requirements for schistosomiasis surveillance. Currently, the surveillance of schistosomiasis has been covered in all schistosomiasis-endemic counties (cities, districts) of China, and the development of rapid and early identification of Schistosoma japonicum cercariae and S. japonicum-infected Oncomelania hupensis snails will greatly improve the sensitivity of and the duration of responses to schistosomiasis surveillance, which will provide powerful tools for elimination of schistosomiasis in China.

Objective::To discuss the efficacy of compound Huangteng mixture for acute radiation enteritis (ARE) and to investigate its regulatory effect on serum inflammatory factors. Method::One hundred and forty patients were randomly divided into control group (70 cases) and observation group (70 cases) by random number table. Patients in control group got precise radiotherapy. And take montmorillorillonite powder orally based on the treatment in control group, patients in observation group additionally received compound Huangteng mixture, 1 dose/day. The treatment was continued to the second week after the ending of radiotherapy in both groups. Time of appearance of ARE (1 and 2 levels), irradiation dose and incidence of severe ARE (3 and 4 levels) were recorded. Endoscope examination was conducted at the second week after the ending of radiotherapy. ARE symptom scores and KPS scores of quality of life were graded. Levels of serum interleukin-1 (IL-1), IL-4, IL-6, tumor necrosis factor-α (TNF-α) and C-reactive protein (CRP) were detected both before and after treatment. Result::Time of appearance of ARE (1 and 2 levels) in observation group was shorter than those in control group (P<0.01), and irradiation dose was more than that in control group (P<0.01). At the fourth and fifth week of radiotherapy and at the second week after the ending of radiotherapy, incidence rate of ARE (levels 2 and above) was 42.86%(30/70), 50.00%(35/70) and 54.29%(38/70), all lower than 61.43%(43/70), 68.57%(48/70) and 74.29%(52/70) in control group (χ2=4.837, P<0.05, χ2=5.001, P<0.05, χ2=6.097, P<0.05). After radiotherapy, incidence of ARE was 62.86%(44/70) in observation group, lower than 78.57%(55/70) in control group (χ2=4.173, P<0.05), and the incidence of severeARE was 13.64%(6/44) in observation group, lower than 32.73%(18/55) in control group (χ2=4.851, P<0.05). Scores of endoscope and ARE symptoms in observation group were lower than those in control group, while score of KPS was higher than that in control group (P<0.01). After radiotherapy, levels of IL-1, IL-6, TNF-α and CRP in observation group were lower than those in control group (P<0.01), while level of IL-4 was higher than that in control group (P<0.01). Conclusion::Compound Huangteng mixture combined with radiotherapy can reduce the incidence of ARE, postpone the occurrence of ARE, relieve the severity of ARE, regulate the expression of inflammatory factors, alleviate clinical symptoms, and improve the quality of life in patients withradiotherapy, thus it is conducive to the sequential development of radiotherapy.

Objective::To observe the effect of Bimin decoction(BMD) on nuclear factor-kappa B(NF-κB) signaling pathway and aquaporin 5(AQP5) expression in allergic rhinitis (AR) rats with lung and spleen Qi deficiency syndrome(LSQDS), in order to study the mechanism in treating AR. Method::Fifty-six SD rats were randomly divided into seven groups: control group, AR group, LSQDS AR group, BMD low dose two weeks group and four weeks group, BMD high dose two weeks group and four weeks group. The control group did not intervened, the AR group established the AR disease model with ovalbumin (OVA) as the allergen, the other five groups established the LSDQS model with smoke and senna gavage, and also established the AR disease model with OVA sensitization at the same time as the AR group. After the model was established successfully, four BMD intervention groups were separately given low dose BMD (11.3 g·kg-1) for 2 weeks and 4 weeks, and high dose BMD (22.6 g·kg-1) for 2 weeks and 4 weeks. To observe the general situation of the rats, hematoxylin eosin (HE) staining was used to observe the pathological changes of the nasal mucosa, immunohistochemistry and Western blot were used to detect the expression levels of NF-κB and AQP5, real-time fluorescent quantitative polymerase chain reaction technique (Real-time PCR)was used to detect mRNA levels of interleukin-1β (IL-1β), tumor necrosis factor-α (TNF-α), interleukin-6 (IL-6). Result::The typical AR symptoms were found in AR rats, the AR symptoms and lung and spleen Qi deficiency symptoms were found in AR rats with LSQDS at the same time, and the AR symptoms and lung and spleen Qi deficiency symptoms were significantly improved after the intervention of BMD. Compared with the control group, the typical histopathological changes of nasal mucosa were found in AR group and LSQDS AR group, with a higher behavioral score (P<0.05), and the expression of NF-κB and AQP5 protein increased (P<0.05), the expression of IL-1β, TNF-α, IL-6 and AQP5 mRNA increased (P<0.05). Compared with AR group, the pathological changes of nasal mucosa in LSQDS AR group were more serious, and the expression of NF-κB protein in nucleus increased (P<0.05), the expression of TNF-α and AQP5 mRNA increased (P<0.05). Compared with LSQDS AR group, the pathological changes of nasal mucosa in the groups which interfered by BMD were improved, and the expression of NF-κB and AQP5 protein decreased (P<0.05), the expression of IL-1β, TNF-α, IL-6 and AQP5 mRNA decreased (P<0.05). Compared with BMD low-dose two-week group, the expression of NF-κB protein in nucleus decreased (P<0.05) in BMD high dose four week group. Conclusion::Compared with AR group, the AR condition of the rats with LSQDS is more serious under the same allergen stimulation, BMD can treat AR and reduce the over secretion of glands, which may be related to inhibit the expression of AQP5 by inhibiting the NF-κB signaling pathway.

Objective:To analyze the general situation and traditional Chinese medicine (TCM) syndrome characteristics in patients with coronavirus disease-2019(COVID-19) in Anhui province, and to provide a basis for clinical diagnosis and treatment. Method:The 144 cases of COVID-19 patients in Anhui province were collected from designated hospitals by means of multi-center cross-sectional epidemiological survey. The TCM syndrome information collection table of patients with novel coronavirus pneumonia was created to collect the general data, etiology and pathogenesis, pulse and signs, syndrome type and frequency statistics of the patients, and then summarize and analyze the main symptoms and syndrome distribution characteristics of patients with novel coronavirus pneumonia. Result:The mean age of 144 patients with COVID-19 infection was (43.54±15.91) years old, and the number of males was larger than the number of females (9/7). The age distribution and gender difference of the patients were statistically significant (P<0.05). 36.11%(52/144) of the patients had a history of travel/residence in Wuhan/Hubei province, and 63.89% (92/144) of the patients had a history of close contact with the confirmed patients. The main clinical manifestations included fever, cough, diarrhea, fatigue and poor tolerance. Light red tongue and red tongue were dominant in tongue quality, with mainly greasy coating, slippery pulse, rapid pulse and soft pulse were the main types of pulse. The main types of syndrome differentiation were the common type (76.38%, 110/144), most of which were demonstrated as dampness obstructing the lung and spleen (56.25%, 81/144). There was no significant difference in gender composition and age distribution between two groups. Conclusion:The pathogenesis of COVID-19 patients in Anhui province is closely related to the lung and spleen, and the dampness caused by pathogen of the epidemic virus is the main pathological factor of disease, which is in line with characteristics of dampness epidemic in TCM, also can see concurrently "heat, poison, stasis" for a characteristic.

Objective::To observe the clinical efficacy of dialectical therapy of Bufeitang combined with Shengesan and Fujiu application on chronic obstructive pulmonary disease (COPD) and lung-kidney Qi deficiency syndrome, and its effect on inflammatory damage and airway remodeling. Method::One hundred and thirty-four patients were randomly divided into control group (66 cases) and observation group (68 cases) by random number table. Patients in control group got spiriva by powder inhaler, 1 grain/time, 1 time/day, and salmeterol xinafoate and fluticasone propionate powder for inhalation for spray as appropriate, 1 suction/time, 1-2 times/days, for a continued 12 months. In addition to the therapy of control group, patients in observation group were also given Fujiu application at two-tailed acupoints of Feiyu, Piyu and Shenyu for the first day of the every San Fu and San Jiu, and dialectical therapy of Bufeitang combined with Shengesan were given at the first day of San Fu and San Jiu for 2 months. The course of treatment was 12 months. Before and after treatment, FEV1% of self-assessment questionnaire of patients with COPD (CAT), 6-min walking distance, St George's respiratory questionnaire (SGRQ), severity of dyspnea (mMRC) and index of BODE were assessed. And levels of tumor necrosis factor-alpha (TNF-alpha), interleukin-6 (IL-6), matrix metalloproteinase-9 (MMP-9) and matrix metalloproteinase inhibitor-1 (TIMP-1) were detected. Result::After treatment, scores of CAT, the total score of SGRQ, scoring of each dimension and index of BODE in observation group were all lower than that in control group (P<0.01), while FEV1% was higher than that in control group (P<0.01). And 6-min walking distance was more than that in control group (P<0.01), and the numbers of acute exacerbations were less than that in control group (P<0.01). The severity of dyspnea was lighter than that in control group (Z=2.047, P<0.05). And levels of MMP-9, TNF-α, IL-6 and ratio of MMP-9/TIMP-1 were lower than those in control group (P<0.01), whereas the level of TIMP-1 was higher than that in control group (P<0.01). Conclusion::Dialectical therapy of Bufei decoction combined with Shenge powder and Fujiu application can alleviate the current symptoms of dyspnea, improve exercise tolerance, quality of life and pulmonary function, reduce the number of acute exacerbations, relieve inflammation damage and airway remodeling. The comprehensive clinical efficacy is better than that of conventional western medicine.

Outbreak of COVID-19 is ongoing all over the world. Spine trauma is one of the most common types of trauma and will probably be encountered during the fight against COVID-19 and resumption of work and production. Patients with unstable spine fractures or continuous deterioration of neurological function require emergency surgery. The COVID-19 epidemic has brought tremendous challenges to the diagnosis and treatment of such patients. To coordinate the diagnosis and treatment of infectious disease prevention and spine trauma so as to formulate a rigorous diagnosis and treatment plan and to reduce the disability and mortality of the disease, multidisciplinary collaboration is needed. This expert consensus is formulated in order to (1) prevent and control the epidemic, (2) diagnose and treat patients with spine trauma reasonably, and (3) reduce the risk of cross-infection between patients and medical personnel during the treatment.
