RESUMEN

Background:Repeat hospitalization is a process for a patient who needs to return to hospital as a result of inability to control and take care of their disease. The objective of study is to gather deeper understanding about the meaning experience of repeat hospitalization process among diabetes mellitus type 2 patients. The study is qualitative research within phenomenology method by using deep interview instruments. In this study, the research seeks the experience of diabetes mellitus type 2 patients who are being treated with repeat hospitalization process. Methods:The study applies descriptive phenomenology approach. By using the method, the subject could be explored, analyzed, and explained directly about their experience of repeat hospitalization. The size of sample, in descriptive study, is essential in order to reveal the saturation data. The participants collecting procedure is using the snowball sampling technique.Results:The age of participants are ranging from 40-79 years old. The level of education among participants are varies, from elementary schools to senior high schools. The occupation type are housewife, farmers or merchants.The participants’ income is ranging between 750.000-2.000.000 rupiah/month.Conclusions:The result reveals four findings among 10 participants: 1) the characteristics of participants 2) the lack of knowledge from diabetes mellitus(DM)management, 3) quality of service, 4) hope of recovery.

Abstract Objective: To show the cytotoxicity of Porphyromonas gingivalis lipopolysaccharide (LPS) on human umbilical cord mesenchymal stem cells (HUCMSCs) to better understand the characteristics for its application in regenerative procedures under periodontopathogen LPS influence. Material and Methods: Ultrapure Porphyromonas gingivalis LPS was used in this study. This research used a frozen stock HUCMSCs, previously confirmed by flow cytometry. The biological characteristics, such as cell morphology, proliferation, and protein expression, were screened. To check the cytotoxicity, HUCMSCs were cultured and divided into two groups, the control group and LPS group with various concentrations from 25 to 0.39 µg/mL. MTT assay was done and the cells were observed and counted. The significance level was set at 5%. Results: The percentage of living HUCMSCs on LPS group were not significantly different among concentrations (p>0.05) from 25 to 0.39 µg/mL, even though there were slight mean decrease between groups, but they were not significant. The duration of 24 hours of exposure of LPS does not significantly lower HUCMSCs viability. Conclusion: LPS does not affect the viability of HUCMSCs. The lower the concentration of LPS, the higher the viability of HUCMSCs.

RESUMEN

A novel of polymer combination promotes an increase of the ability for controlling the drug release. The objectiveof this research was to characterize the inter-polymer complexes (IPCs) of Eudragit (Eud) types (Eud RS, Eud L, orEud E) and Kollidon SR (KSR), and elucidate their effects on the drug release kinetics and mechanism. Differentpreparation techniques were proposed using spray drying and ultrasonic-assisted anti-solvent techniques. The thermalactivity, e.g., glass transition temperature (Tg) and Fourier transform infrared spectroscopy were used to characterizethe molecular interaction of these IPCs. Theophylline (THP) was selected as a drug model. The effect on the drugrelease kinetics and mechanism was the main concern of this study. Depending on the results, the hydrogen bondingformation between polymers was observed by a shifting of OH and carbonyl group vibrations. In addition, the van derWaals interaction was identified by an alteration in the vibrational band around the 1,000–1,500 cm−1. Meanwhile, thechange of physicochemical characteristic was identified by the Tg of IPCs. Eud E-KSR and Eud E-Eud L IPC wereunable to control the THP release. Meanwhile, Eud L-KSR IPC and Eud RS-KSR IPC were success to control theTHP release, but it was pH dependent and independent, respectively. This study concluded that the IPCs allowed theTHP release in a controlled manner based on the IPC characteristics and their interactions. Either positive or negativeinteractions on the drug release were observed due to native characteristics of polymers.

Abstract Objective: To investigate the regeneration of rat's salivary gland diabetic defect after intraglandular transplantation of Human Dental Pulp Stem Cells (HDPSCs) on acinar cell vacuolization and Interleukin-10 (IL-10). Material and Methods: HDPSCs isolated from the dental pulp of first premolars #34. HDPSCs from the 3rd passage was characterized by immunocytochemistry of CD73, CD90, CD105 and CD45. Twenty-four male Wistar rats, 3-month-old, 250-300 grams induced with Streptozotocin 30 mg/kg body weight to create diabetes mellitus (DM) divided into 4 groups (n=6); positive control group on Day-7; positive control group on Day-14; treatment group Day-7 (DM+5.105HDPSCs); treatment group on Day-14. On Day-7 and Day-14, rats were sacrificed. Histopathological examination performed to analyze acinar cells vacuolization while Enzyme-linked Immunoabsorbent Assay to measure IL-10 serum level. Data obtained were analyzed statistically using multiple comparisons Bonferroni test, Kruskal Wallis, Shapiro-Wilk and Levene's test result Results: The highest acinar cell vacuolization found in control group Day 14 (0.239 ± 0.132), meanwhile the lowest acinar cell vacuolization found in treatment group Day 7 (0.019 ± 0.035) with significant difference (p=0.003). The highest IL-10 serum level found in treatment group Day 14 (175.583 ± 120.075) with significant difference (p=0.001) Conclusion: Transplantation of HDPSC was able to regenerate submandibular salivary gland defects in diabetic rats by decreasing acinar cell vacuolization and slightly increase IL-10 serum level.

The evaluation of cell growth was determined using methylene blue assay.</p><p><b>RESULTS</b>Methanol extract from the leaves showed significant anticancer activity against MCF-7 cell lines with the IC50 value of 7.14 µg/ml while methanol and chloroform extract from the flowers exhibited a moderate activity towards MCF-7 cell line with the IC50 value of 33.63 µg/mL and 45.76 µg/mL respectively after 72 h of treatment.</p><p><b>CONCLUSIONS</b>The extracts from leaves and flowers of M. malabathricum showed promising anticancer activity toward human breast cancer cell lines with the lowest IC50 at 7.14 µg/mL while the extracts from stems showed less growth inhibition activity.</p>

Background: Distal radius morphometry (radial inclination, palmar tilt, radial height, and ulnar variance) is an important parameter in the evaluation and treatment of distal radius fractures in which anatomical alignment must be corrected. Currently, treatment of distal radius fractures in Indonesia is still based on morphometry of western population or from the contralateral side. The aim of this study is to determine distal radius morphometry of Indonesian population and to compare between right and left side, male and female gender. Methods: Distal radius morphometry was measured from 400 plain X-ray of right and left wrist AP and lateral projection. Samples were taken consecutively in Moh. Ridwan Meuraksa Army Hospital, Jakarta, from June to September 2010. Radial inclination, palmar tilt, radial height, and ulnar variance was measured. Data were recorded using tables and grouped between male and female, right and left side, statistical analysis was performed using Mann-Whitney test. Results: From 400 plain X-ray evaluated, there were 300 males and 100 females with the mean age of 25.5 years old (18-48). The mean of radial inclination was 23.99 ± 3.75°, palmar tilt 13.76 ± 4.36°, radial height 11.31 ± 1.66 mm, and ulnar variance -0.45 ± 2.03 mm. There were statistically significant differences between right and left side of radial inclination, palmar tilt, radial height, and ulnar variance. There was also statistically significant difference between male and female. Conclusion: Distal radius morphometry in Indonesian population may provide valuable data for the treatment of distal radius fractures. The use of contralateral side as reference should be reconsidered.

RESUMEN

Background: This study aims to determine the association between neuroinflammation and oxidative stress with prognosis of brain injury patients and the association between neurosurgical procedure with neuroinflammation and oxidative stress condititons. Methods: The study design is a prospective observation of 40 brain injury patients who underwent surgery. IL-6, uric acid, MDA, NR2A antibodies and GSH serum level of pre- and 1 day post-operation on brain injury patients were measured, and their association with GCS, GOS and neurosurgical procedures were analyzed. Results: The post-operative IL-6 serum level showed a downward trend compared to pre-operative value (mean decrease: -190.61 pg/mL). The post-operative IL-6 level was significantly associated with GCS 7 days post-operation (p = 0.006), with OR 24. The post-operative IL-6 serum level was significantly associated with GOS 3 months post-trauma (p = 0.016) with OR 11.6. The post-operative uric acid serum level showed a downward trend compared to pre-operative value (mean decrease: -0.26 mg/dL). There was a significant difference between the mean value of post-operative uric acid serum level in patients with 7 days post-trauma with GCS ≤ 8 (mean: 4.16 mg/dL) and GCS > 8 (mean: 2.71 mg/dL), (p = 0.042). The post-operative MDA serum level showed a downward trend compared to pre-operative value (mean decrease: -0.08 nmol/mL). There is no significant association between MDA serum level, GCS and GOS and no significant association of NR2A antibody and GSH serum level with GCS, GOS and neurosurgical procedure. From the multivariate analysis, the most important neuroinflammatory variable associated with GCS and GOS is IL-6. Conclusion: Neuroinflammation and oxidative stress may have prognostic values in brain-injured patients, in particular IL-6. Neurosurgical procedures may decrease the neuroinflammation process

RESUMEN

Child abuse is a pervasive social and medical problem that remains a major cause of disability and death among children. The annual incidence of abuse is estimated to be 15 to 42 cases per 1,000 children and appears to be increasing. Fractures are the second most common presentation of physical abuse after skin lesions, and approximately one third of abused children will eventually be seen by an orthopedic surgeon. We report a 7-month-old boy who was suspected to be abused. Our diagnosis was based on findings of multiple fractures, delay in seeking medical treatment and discrepancy between the history of illness and the clinical findings. He sustained multiple fractures in variety of healing, namely fractures on left supracondylar humeri, left radius and ulna, right radius and ulna, both femora, right tibia, and left tibia and fibula. Radiological examination was an important modality in revealing the possibility of abuse on this child. He had received medical treatment, protection, consultation team for the parents and an underway police investigation.
