RESUMEN

Purpose@#The purpose of this study is to clearly classify the roles of Physician assistants and to identify obstacle factors and necessary factors for the operation of the system when they are performing the roles of Physician assistants. @*Methods@#This is a descriptive research study to analyze the roles of Physician assistants and to understand the expected roles of Physician assistants according to each medical personnel (doctor, nurse, Physician assistants). @*Results@#The obstacles to work are the lack of legal protection, and unclear work guidelines, and the matter necessary for the operation of the system is the legal protection. There were statistically significant differences in health collection ability, physical examination, drug response monitoring, and reporting in terms of the differences in role expectations considered by medical professionals regarding the roles of Physician assistants. @*Conclusion@#Physician assistants need to develop various educational programs and materials, cultivate the ability to perform Physician assistants, and develop an advisory role through effective problem-solving between departments and increase the quality and satisfaction of Nursing care.

Purpose@#The purpose of this study was to evaluate the prevalence of frailty among an older adult population living in rural communities and to determine if frailty is associated with nutritional status after adjusting for sarcopenia and depression. @*Materials and Methods@#This research used baseline data from the Namgaram-2 study. Frailty was evaluated using the Kaigo-Yobo checklist in an older Korean population. The nutritional statuses of older people were measured using the Korean version of the mini nutritional assessment (MNA). The recent criteria of the Asian Working Group for Sarcopenia were applied for diagnosis of sarcopenia, and depression was assessed using the Geriatric Depression Scale-Short Form. @*Results@#The prevalence of frailty was 18.8% (male: 9.6%; female: 23.4%) and was significantly higher in individuals in their 80s [male, 35.3% (p<0.001); female, 42.3% (p<0.001)], those of poor economic status [male, 18.2% (p=0.012); female, 34.9% (p<0.001)], those with hypertension [female, 27.7% (p=0.008)], those with sarcopenia [male, 34.1% (p<0.001); female, 37.2% (p<0.001)], those with depression [male, 46.4% (p<0.001); female, 51.7% (p<0.001)], and those at high risk of malnutrition [male, 44.4% (p<0.001); female, 51.7% (p<0.001)]. After adjusting for demographic variables, including hypertension, diabetes, sarcopenia and depression, frailty was significantly associated with nutritional status [male: odds ratio (OR)=6.73, 95% confidence interval (CI), 1.84–24.65; female: OR=4.83, 95% CI, 2.88–8.11]. @*Conclusion@#For older adults, MNA is a suitable tool of use in assessing both nutritional status and frailty. Moreover, the nutritional status of older adults appears to be associated with frailty, even after corrections for physical and psychological function.

PURPOSE: This study aimed to evaluate the validity of the Western Ontario and McMaster Universities Arthritis Index Short Form (WOMAC-SF) for the assessment of musculoskeletal disorders. We evaluated whether WOMAC-SF correlated with the World Health Organization Disability Assessment Schedule 12 (WHODAS-12) and Kaigo-Yobo questionnaires for assessing health-outcomes in Korea.MATERIALS AND METHODS: This cross-sectional study used data from the Namgaram-2 cohort. WOMAC, WOMAC-SF, WHODAS-12, and Kaigo-Yobo questionnaires were administered to patients with musculoskeletal disorders, including radiology-confirmed knee osteoarthritis (RKOA), sarcopenia, and osteoporosis. The relationships among WOMAC-SF, WHODAS-12, and Kaigo-Yobo scores were analyzed by stepwise multiple regression analysis.RESULTS: WOMAC-SF was associated with the WOMAC questionnaire. The results of confirmatory factor analysis for the hypothesized model with two latent factors, pain and function, provided satisfactory fit indices. WOMAC-SF pain and function were associated with RKOA. Kaigo-Yobo was associated with WOMAC-SF pain (B=0.140, p=0.001) and WOMAC-SF function (B=0.042, p=0.004). WHODAS-12 was associated with WOMAC-SF pain (B=0.679, p=0.003) and WOMAC-SF function (B=0.804, p<0.001).CONCLUSION: WOMAC-SF was validated for the evaluation of low extremity musculoskeletal disorders and health-related quality of life in a community-based population. Furthermore, we confirmed that WOMAC-SF were reflective of disability and frailty, which affect health outcomes.

PURPOSE: The purpose of this study was to evaluate the effects of essential oil on oxidative stress, immunity, and skin condition in atopic dermatitis (AD) induced mice. METHODS: This study was a 3x3 factorial design. Factors were oil type (Lavender, Thyme, and 2:1 mixture of lavender and thyme oil [blending oil]) and treatment period (0 day, 7 days, and 21 days). The samples were 45 mice with AD and randomly assigned to nine groups of five mice per group. The dependent variables such as superoxide radical, IgE, degranulated mast cells, and epidermal thickness were measured. Data were collected from February to April in 2014. Descriptive statistics, One-way ANOVA, Two-way ANOVA, and Tukey's HSD test were performed using the SPSS WIN 20.0 program. RESULTS: Dependent variables were not statistically significantly different by the three oil types (p >.05). Essential oils such as lavender, thyme, and blending oil were all effective in reducing AD symptoms and especially 2:1 blending oil were most effective. There were statistically significant differences by the three treatment periods in all dependent variables (p <.001). There were statistically significant interactions between oil types and treatment periods in all dependent variables (p <.01). For decreasing superoxide radical, degranulated mast cells, and epidermal thickness, 2:1 mixed oil should be applied for at least 21 days. Otherwise to reduce IgE, 2:1 mixed oil should be used for at least 7 days. CONCLUSION: These findings provide bases for developing effective interventions for AD patients to manage their AD symptoms.

PURPOSE: Most patients attempting suicide present to the emergency department (ED). Active post-event intervention is helpful in preventing additional self harm. However, the current management system and decision-making processes of individual doctors are highly variable. This survey aimed to describe the ED management system and to compare the decisions of doctors in emergency medicine and psychiatry for patients for attempting suicide. METHODS: Questionnaires were sent by e-mail to directors of both ED and psychiatry departments in all Korean teaching hospitals. The questionnaires surveyed the current management system for suicidal patients in the ED, the medical director's opinion about the management of suicidal patients, and decisions about disposition of the three clinical scenarios. RESULTS: Sixty-four out of 180 surveys(36%) were returned. The average number of suicidal patients visiting the ED was 13.6 per month and the number referred to a psychiatrist was 11.3 per month. Half of EDs(15/30) had established criteria for patient disposition, but only 7% of ED and 82% of psychiatry departments had practice guidelines for doctors. Only 4% of ED performed any active post-event intervention other than psychiatry out-patient visits. Decisions of emergency physicians and psychiatrists about referring a patient to the psychiatrist in the three clinical scenarios were in part statistically different. CONCLUSION: This study suggests that appropriate facilities and standard guidelines for suicidal patients are needed. Each hospital should develop a common management protocol with collaboration between emergency medicine and psychiatry departments.

RESUMEN

PURPOSE: This study was conducted to evaluate the effects of stroke patient care education on the knowledge and practice of caregivers of stroke patients. METHOD: Data was collected from December 15, 2004 to March 30, 2005. The research design was a non-equivalent control group non-synchronized design. The subjects were forty primary caregivers of stroke patients who were hospitalized in a neurology unit of a university hospital. Forty caregivers, twenty in the experimental group and twenty in the control group were assigned. The experimental group participated 2 times in an education class given by the researcher. Data analysis included -test, and t-test using the SPSS program. RESULT: Knowledge(t=5..87, p=o.oo) and practice(t=5.53, p=0.00) of the experimental group were significantly different from the control group. CONCLUSION: The stroke patient care education developed in this studyshows a significant promotion of knowledge and practice of caregivers. Thus this program can be recommanded as an intervention model for stroke patients and caregivers.

RESUMEN

OBJECTIVE: To obtain information on the growth inhibition effect of arsenic compounds and gene expression profiles using cDNA microarray technique in SiHa cell lines. METHODS: We cultured 103 SiHa cell in 96 well plate and we investigated growth inhibition effects using MTT assay and also we performed gene expression profile experiment using 384 cDNA chip in SiHa cell after exposure of arsenics (As2O3, As4O6 - 1 (micro)M) for 48 hrs. RESULTS: Arsenics (As2O3, As4O6) inhibit the growth of SiHa cells (As2O3: 0.5, 1, 2, 3, 4, 5 (micro)M - 9.2, 56, 89, 93, 96, 96%, As4O6: 0.5, 1, 2, 3, 4, 5 (micro)M- 54, 84, 84, 85, 85, 87%) in 4 days culture. As2O3 and As4O6 induced apoptosis in SiHa cells. After exposure of As2O3, 47 genes were changed more than 2 times (eg, thymidylate synthetase, cyclin B1, CDC 20). In case of As4O6, 78 genes were changed more than 2 times (eg, CDC 20, cyclin B1, primase, proliferating cell nuclear antigen). CONCLUSION: we observed arsenic compound (As2O3, As4O6) inhibit the growth of SiHa cell. In gene expression profiling experiment, 78 genes was changed the expression level 2 times more than that of reference RNA after treatment of As4O6 and 47 genes after treatment of As2O3. Through these result, we thought more study need in functional genomics after arsenic treated cervical cancer cells.

RESUMEN

The main purpose of this study is to identify nursing needs of parents who have hospitalized child. Research design is a descriptive survey. The subjects for the study were 79 parents who have hospitalized child at a pediatric ward of one of the general hospital attached to a university in J city. The data was collected by researchers using a structured questionnaire. The questionnaire was Liken type 5 point scale, composed of 5 categories with 61 items. The data was analyzed by SPSS/PC. The results of the study were as follows : 1) Mean score of nursing needs of subjects were 3.93 at 2nd day of admission and 3.99 at 7th day of ad mission. 2) Among the categories, the highest nursing need at the 2nd day and 7th day of admission was 'direct nursing', in desending order, 'facilities and environment', 'education and counseling', 'nursing assessment'. The lowest nursing need was 'reference'. 3) Differences between the nursing needs of subjects at 2nd day and 7th day were as follows ; (1) By categories, there was a significant difference only in the 'reference' category(t=2.74, P=.008). (2) By items, there were significant differences in items of 'to check necessary materials(t=2.31, P=.024)', 'to understand family function and family relationship(t=2.12. P=.041), 'to set up study room(t=2.22, P=.030)', and 'to mediate parent's meeting group(t=3.89, P=.000)'. The above result indicated that nursing needs of parents with hospitalized child were above average, especially very high in items about disease process, and items directly associated with treatment and nursing care. So, nurses have to focus on information about the patient's state of disease, treatment, test and procedure, and in efficiently giving direct nursing care to implement more effective care for the hospitalized children and their parents. And some future research is needed to identify the difference of degree of nursing needs of parents with hospitalized child according to admission duration using a different sample and a longer sampling interval.

RESUMEN

The purpose of this study was to find educational needs of toddler's mother in child-caring experience. The subject of the study was toddler's mother who visited one health care center in J city for immunization and had a child or children between twelve and thirty six months old. The subjects were 37 mothers. The instrument used for this study was a semi-structured questionnaire developed by researcher and based on Objective Content Test of Garretson(1967). The Collected data were analized by two researchers using the method of content Analysis. The results of this study are summarized as follows ; 1) Of the large categories, the need of the highest frequency was 'prevention and management of disease and injury', that of the lowest 'growth and development'. 2) In 'physical care' category, 'care of sleeping' was the highest. In 'nutrition and eating-habitual management' category, 'weaning management' was the highest. In 'prevention and management of disease and injury' category, 'disease management' was the highest. In 'discipline and teaching' category, 'discipline' was the highest. 3) In comparison with preceding references, the educational needs of child-caring which had not been shown in preceding references but had been shown in this study were hair-caring, kind of weaning diet, method of cure and nursing, management of deformity, and so on. In conclusion, educational needs of toddler's mother in child-caring experience were matter-of-fact and inclusive. Needs in management of special situation such as disease were higher than usual parenting procedure or method. In addition, there were higher needs in questions happened through child-caring experience or confirmation of procedures practicing now. Then, it is suggested that parent education program should be developed on the basis of educational needs found in this study to be more effective in preparing mothers's child-caring.

This study was to find whether the educational program contributed to the increase of knowledge, confidence, and accuracy of behavior in newborn care of the primiparas. The educational program consists of individual lectures, demonstrations, discussion, and practice of newborn care. Also two telephone counseling with the subjects after they are discharged from hospital. This study is a quasi-experimental design using non-equivalent control group pretest-posttest design. Data collection was done from July 21 to Oct 4 in 1997. the subjects were selected from 2 general hospitals and 1 university hospital in C city. Subjects were 44 primiparas(control group 22, experimental group 22). they were tested on knowledge, confidence, and accuracy of behavior in newborn care. A pretest was done 2-3 years after vaginal delivery(5-6 days after c-sec delivery). A posttest was done 21-28 day(vaginal delivary, c-sec delivary) after delivery. The instruments used for this study were knowledge scale about newborn care developed by the researcher, Pharis' confidence scale modified by the researcher and accuracy of behavior scale developed by the researcher. Primiparas' knowledge and confidence was tested by questionnaire and Primiparas' accuracy of behavior was tested by structured observational method. Analysis of data was done by using of x2-test, t-test, paired t-test. The results of this study are summarized as follows; 1) Knowledge of the experimental group was significant higher than the control group(t=-4.94, P=.000). 2) Confidence of the experimental group was significant higher than the control group(t=-.262, P=.012). 3) Accuracy of behavior of the experimental group was significant higher than the control group(t=-.969, P=.000). In conclusion, the newborn care education along with intensive telephone counseling shows a significant promotion of newborn care in primiparas. Thus this program can be recommended as an intervention model for the newborn and primiparas.
