RESUMEN

This article reports a patient who suffered from Wolffian adnexal tumor.We also briefly elucidate the pathogenesis,clinicopathological features,diagnosis,differentiation,and treatment of Wolffian adnexal tumor,with an attempt to increase the awareness of the disease and reduce misdiagnosis.

<p><b>OBJECTIVE</b>To compare the diagnostic values of CD64 infection index, C-reactive protein (CRP), procalcitonin (PCT) and the percentage of neutrophils (NEU%) in leukemia patients complicated with bacterial infection.</p><p><b>METHODS</b>Sixty cases of leukemia complicated with bacterial infection (combination with infection group), 60 cases of leukemia without bacterial infection (non-combination with infection group) and 60 cases of healthy persons (control group) were selected in our hospital. CD64 infection index, CRP, PCT and NEU% were detected in the 3 group.</p><p><b>RESULTS</b>CD64 infection index, CRP, PCT and NEU% in combination with infection group were significantly higher than those in the non-combination with infection group and also significantly higher than those in the control group (all P<0.05). The CD64 infection index, CRP, PCT and NEU% in the non-combination with infection group were significantly higher than those in the control group (all P<0.05). The sensitivities of CD64, CRP, PCT and NEU% in the diagnosis of the leukemia with bacterial infection were 71.06%, 84.86%, 66.93% and 59.25% respectively, and the specificities of CD64, CRP, PCT and NEU% were 91.46%, 75.94%, 88.79% and 85.36% respectively.</p><p><b>CONCLUSION</b>Compared with CRP, PCT and NEU%, CD64 infection index for diagnosis of leukemia complicated with bacterial infection has higher specificity, which is helpful for the early diagnosis of the disease.</p>

Objective Few studies are reported on the factors influencing the selection of the surgical access for adenocarci-noma of the esophagogastric junction (AEG). This study aimed to investigate the factors influencing the selection of the surgical access for CT type-ⅡAEG. Methods We retrospectively analyzed 118 cases of CT type-ⅡAEG surgically treated in the Second Affiliated Hospital of Zhengzhou University and Henan Tumor Hospital from January 2015 to January 2017,92 via the abdominal access and the other 26 via the thoracoabdominal access. We performed univariate and multivariate logistic regression analyses on the factors that might influence the surgical approaches,such as gender,body mass index (BMI),degrees of the epigastric angle,pathological classifica-tion,and clinical stages. Results Logistic regression analysis showed that the degree of the epigastric angle was an independent fac-tor influencing the selection of the access of AEG (OR=0.250,95% CI: 0.098-0.639) (P<0.05). Conclusion In the surgical treatment of CT type-ⅡAEG,the abdominal access is preferable for patients with the epigastric angle> 90 degrees and the thoracoab-dominal access is recommended for those with the epigastric angle≤90 degrees.

AIM To study the antithrombotic effects and mechanism of ethyl acetate fraction of Curcuma kwangsiensis S.G.Lee et C.F.Liang (CKEAF).METHODS To observe the antithrombotic effects of CKEAF on tail thrombotic mouse models and arteriovenous bypass thrombotic rat models induced by carrageenan,and in vivo thrombosis rat models induced by FeCl3.The serum levels of ET-1,6-keto-PGF1α,TXB2 in rats were measured by enzyme linked immunosorbent assay (ELISA),and the level of NO was determined by nitrate reductase method.Rat models of acute blood stasis were further established for hemorheological study.RESULTS CKEAF significantly decreased the number and length of blackened thrombotic tail of carrageenan-treated mice.Rat models shared significant wet weight loss in arteriovenous bypass thrombosis and in vivo thrombosis,increased levels of NO,6-keto-PGF1α,decreased levels of ET-1,TXB2,and decreased whole blood and plasma viscosity (rats of acute blood stasis model).CONCLUSION Significant antithrombotic effects by CKEAF may contribute to elevated levels of NO,6-keto-PGF1α,and decreased levels of ET-1,TXB2,and lowered whole blood and plasma viscosity.

Based on the analysis of telemedicine development status and internal and external environment of Henan Province,the effect factors of telemedicine development in Henan Province are analyzed through SWOT,and then DANP method,in which decision experiment analysis method and layer of the network analysis method is combined,is used to identify the key factors.The results show that the opportunities(O) is one of the most important dimensions.Medical resources reorganization (S1),the development of the technology (O1),government support (O2),national health consciousness (O3),social awareness of telemedicine (T1) and industrial policy (S3) are the six key factors affecting the development of telemedicine in Henan Province.Finally,some suggestions are put forward according to the research results.

Objective To explore and establish the performance evaluation system for telemedicine in China.Methods By the method of literature and brainstorming,building evaluation model and the initial indicators,using Delphi method and Analytic Hierarchy Process to determine the index and its weight.Results Based on the PATH model,a telemedicine performance evaluation framework was established with five primary indicators,11 secondary indicators and 32 tertiary indicators.And the five primary indicators include patient-centered,medical effective and efficiency,staff-oriented,safety and response management.Conclusion The PATH model is suitable for the establishment of telemedicine performance evaluation framework.The patient-centered indicator has the highest weight,and the cost of telemedicine services should be controlled,and the patient satisfaction should be improved from all angles.

RESUMEN

Arsenic trioxide (ATO) is used as a chemotherapeutic agent for the treatment of acute promyelocytic leukemia. However, increasing drug resistance is reducing its efficacy. Therefore, a better understanding of ATO resistance mechanism is required. In this study, we established an ATO-resistant human epidermoid carcinoma cell line, KB/ATO, from its parental KB-3-1 cells. In addition to ATO, KB/ATO cells also exhibited cross-resistance to other anticancer drugs such as cisplatin, antimony potassium tartrate, and 6-mercaptopurine. The arsenic accumulation in KB/ATO cells was significantly lower than that in KB-3-1 cells. Further analysis indicated that neither application of P-glycoprotein inhibitor, breast cancer resistant protein (BCRP) inhibitor, or multidrug resistance protein 1 (MRP1) inhibitor could eliminate ATO resistance. We found that the expression level of ABCB6 was increased in KB/ATO cells. In conclusion, ABCB6 could be an important factor for ATO resistance in KB/ATO cells. The ABCB6 level may serve as a predictive biomarker for the effectiveness of ATO therapy.

Protein phosphatases play critical roles in regulating cell division, cell apoptosis, and cell cycle in eukaryotic cells, participating in numerous signal transduction processes and exerting a large amount of significant biological functions. A variety of protein phosphatases are identified to maintian the phosphorylation level of key proteins with a moderate level in the antiviral innate immune response to virus infectioa In this paper, we reviewed the roles of serine/threonine protein phosphatases, tyrosine protein phosphatases, and lipid phosphatases in regulating antiviral innate immune responses.

RESUMEN

Gap junctions play a critical role in electrical synchronization and exchange of small molecules between neighboring cells; connexins are a family of structurally related transmembrane proteins that assemble to form vertebrate gap junctions. Hyperglycemia changes the structure gap junction proteins and their expression, resulting in obstruction of neural regeneration, vascular function and wound healing, and also promoting vascular atherosclerosis. These pathogenic factors would cause diabetic foot ulcers. This article reviews the involvement of connexins in pathogenesis of diabetic foot.

Multidrug resistance (MDR), defined as the resistance of cancer cells to compounds with diverse structures and mechanisms of actions, significantly limits the efficacy of antitumor drugs. A major mechanism that mediates MDR in cancer is the overexpression of adenosine triphosphate (ATP)-binding cassette transporters. These transporters bind to their respective substrates and catalyze their efflux from cancer cells, thereby lowering the intracellular concentrations of the substrates and thus attenuating or even abolishing their efficacy. In addition, cancer cells can become resistant to drugs via mechanisms that attenuate apoptosis and cell cycle arrest such as alterations in the p53, check point kinase, nuclear factor kappa B, and the p38 mitogen-activated protein kinase pathway. In this review, we discuss the mechanisms by which β-elemene, a compound extracted from Rhizoma zedoariae that has clinical antitumor efficacy, overcomes drug resistance in cancer.

It has been shown that Insulin-like growth factor-1 [IGF-1] may be related with bone mineral density [BMD] or osteoporosis. But there are few evidences on the role of genetic variation of IGF-1 on the BMD or osteoporosis. We observed the relationship between polymorphisms of IGF-1[rs35767, rs2288377 and rs5742612] with osteoporosis and BMD in the postmenopausal female population in our study. A total of 216 postmenopausal women with a primary diagnosis of osteoporosis and 220 normal healthy women were included in the study. Genomic DNA of IGF-1 rs35767, rs2288377 and rs5742612 was extracted from the whole blood using QIAamp blood DNA mini kits [QIAGEN, Hilden, Germany] according to the methods recommended by the manufacturer. We found that T allele of rs35767 had higher increased risk of osteoporosis [OR=1.34, 95%CI=1.0- 1.81]. Those carrying T allele of rs35767 had a significant lower BMD at L1-L4 vertebrae, femoral neck, total hip and trochanter when compared with those carrying C allele [P < 0.05]. In addition, the BMD of L1-L4 vertebrae, femoral neck, total hip and trochanter decreased by 2.09%, 3.74%, 3.52% and 2.54% in women carrying T alleles compared with those carrying C alleles. Our study suggests that polymorphism in IGF-I rs35767 was significantly associated with BMD and osteoporosis in postmenopausal female population, and polymorphism of rs35767 could be a marker for lower BMD and risk of osteoporosis

To compare the curative effects of two surgical treatment methods for open angle glaucoma and cataract patients. ●METHODS: Totally 69 patients with primary open angle glaucoma and cataract were divided into two groups:group A and group B. Thirty- four patients (34 eyes) in group A underwent small incision non -phacoemulsification cataract surgery combined with trabeculectomy, just combined operations, 35 patients (35 eyes) in group B underwent small incision non -phacoemulsification cataract surgery beyond 6mo after trabeculectomy, just two stage operations. Postoperative intraocular pressure at 1wk and 3mo, postoperative corrected visual acuity and filtering bleb at 3mo were observed, and intraoperative and postoperative complications were compared. All the patients were followed up for 3-6mo. ● RESULTS: All the visual acuity were increased compared with that before surgery in two groups, the difference was not statistically significant. All the intraocular pressure was controlled in two groups, the difference was not statistically significant, and the difference about the filtering bleb was neither statistically significant ( P > 0. 05 ). There were no serious intraoperative and postoperative complications in two groups. ● CONCLUSlON: The two stage operations and the combined operations both can control theintraocular pressure and improved visual acuity of open angle glaucoma and cataract. The two stage operations may prevent a second operation for post trabeculectomy cataract, allowing earlier visual rehabilitation.

<p><b>BACKGROUND</b>Data on the epidemiology of hypertension in Chinese non-dialysis chronic kidney disease (CKD) patients are limited. The aim of the present study was to investigate the prevalence, awareness, treatment, and control of hypertension in the non-dialysis CKD patients through a nationwide, multicenter study in China.</p><p><b>METHODS</b>The survey was performed in 61 tertiary hospitals in 31 provinces, municipalities, and autonomous regions in China (except Hong Kong, Macao, and Taiwan). Trained physicians collected demographic and clinical data and measured blood pressure (BP) using a standardized protocol. Hypertension was defined as systolic BP ≥ 140 mmHg and/or diastolic BP ≥ 90 mmHg, and/or use of antihypertensive medications. BP < 140/90 mmHg and < 130/80 mmHg were used as the 2 thresholds of hypertension control. In multivariate logistic regression with adjustment for sex and age, we analyzed the association between CKD stages and uncontrolled hypertension in non-dialysis CKD patients.</p><p><b>RESULTS</b>The analysis included 8927 non-dialysis CKD patients. The prevalence, awareness, and treatment of hypertension in non-dialysis CKD patients were 67.3%, 85.8%, and 81.0%, respectively. Of hypertensive CKD patients, 33.1% and 14.1% had controlled BP to < 140/90 mmHg and < 130/80 mmHg, respectively. With successive CKD stages, the prevalence of hypertension in non-dialysis CKD patients increased, but the control of hypertension decreased (P < 0.001). When the threshold of BP < 130/80 mmHg was considered, the risk of uncontrolled hypertension in CKD 2, 3a, 3b, 4, and 5 stages increased 1.3, 1.4, 1.4, 2.5, and 4.0 times compared with CKD 1 stage, respectively (P < 0.05). Using the threshold of < 140/90 mmHg, the risk of uncontrolled hypertension increased in advanced stages (P < 0.05).</p><p><b>CONCLUSIONS</b>The prevalence of hypertension Chinese non-dialysis CKD patients was high, and the hypertension control was suboptimal. With successive CKD stages, the risk of uncontrolled hypertension increased.</p>

<p><b>BACKGROUND</b>Transurethral resection of prostate (TURP) has been widely used as a golden standard therapy of benign prostatic hyperplasia for over 40 years. However, not all patients achieved favorable outcome postoperatively. Since the level of bladder outlet obstruction and the dysfunction of detrusor (overactive and underactive) were both found to affect surgical efficacy, urodynamics was recommended as routine preoperative examination in selecting proper surgical candidates by International Continence Society in spite of its invasiveness and high cost. The aim of this research was to compare the predictive value between ultrasonography and urodynamics for TURP efficacy and determine if preoperative urodynamic test could be replaced by ultrasonography.</p><p><b>METHODS</b>Two hundred and seventy-one patients took part in the retrospective analysis. All the subjects had preoperative evaluation of symptoms, life quality, and combined examination of ultrasonography and urodynamics. Surgical efficacy was measured according to the recovery of international prostate symptom score, quality of life score, and maximal flow rate 6 months after TURP. Fisher's linear discriminant analysis was applied to establish the predictive models of surgical efficacy by choosing parameters from ultrasonography or urodynamics as independent factors. Receiver's operating characteristic curve was then plotted to compare the values between the models.</p><p><b>RESULTS</b>Sensitivity, specificity, positive and negative predictive value of models consisting of parameters from both ultrasonography and urodynamics were favorable. Corresponding models of ultrasonography and urodynamics were found to have non-significant difference in area under curve (P > 0.05).</p><p><b>CONCLUSIONS</b>Preoperative ultrasonography has as strong value as urodynamics does in predicting surgical outcome of patients undergone TURP and might take the place of urodynamics in selecting surgical candidates. Further prospective analysis with larger popularity and longer period of follow up should be launched to verify the result of this research.</p>

OBJECTIVE@#To determine the distribution and influence of lysosomal neuraminidase (Neul), protective protein/cathepsin A (PPCA) and beta-galactosidase (beta-gal) in the inner ear of the mouse, and to observe their auditory alterations in enzyme deficiency.@*METHODS@#Six wild type (2 months postnatal) (Neu1+/+, PPCA+/+ and beta-gal+/+) mice were used, and Neu1, PPCA and beta-gal homozygous (Neu1-/-, PPCA-/- and beta-gal-/-) mice at the same age used as control in this experiment. The auditory thresholds were examined through the auditory brainstem responses (ABR) to click, which tone pips were 8, 16, and 32 kHz. The mice were intracardically perfused with 4% paraformaldehyde. The bulla were further fixed in 4% paraformaldehyde, processed and sectioned with paraffin embedded method. Immunohistochemistry was used to determine the cellular localizations of Neu1, PP-CA, and beta gal in the inner ear.@*RESULTS@#There was a similar distributive pattern of Neu1, PPCA and betagal in the inner ear. Neu1 intense staining was observed in the cochlear spiral ganglion cells, spiral limbus, spiral ligament, vestibular ganglion cells, cristae, maculae hair cells, and weak staining in inner hair cells, outer hair cells, supplying cells of the organ of Corti and stria vascularis. The intense staining of PPCA and beta-gal were observed in the spiral ganglion and vestibular ganglion cells, and weak staining in the spiral limbus, spiral ligament, stria vascularis and organ of Corti. The inner ear exhibited no staining when Neul, PPCA and beta-gal were deficient, respectively. A positive staining of PPCA and beta-gal was presented in Neu1-/- mice, and as well as Neu1 and PPCA in beta-gal-/- mice. However, the staining of Neu1 was not presented, and only very weak staining of beta-gal in PPCA-/- mice. The auditory thresholds of Neul, PPCA, and beta-gal mice were elevated for 60-69 dB, 40-48 dB, and 7-10 dB above those of wildtype littermates, respectively.@*CONCLUSION@#Neu1 PPCA and beta-gal are distributed in the inner ear of mouse, and the three enzymes also form a lysosomal multi-enzyme complex in the inner ear. The respective enzyme deficiencies can induce the hearing the loss of different levels.

Objective To investigate the effects of radix salviae militiorrhizae (RSM) on acute lung injury induced by"two hits"and to study its probable mechanism.Method Thirty Wister rats were randomly divided into three groups:namely normal control group,model group and RSM treatment group.The model was created by"two-hits"in which 0.2 ml/kg oleic acid was injected into tail vein first,and then 2 mg/kg lipopolysaccharide was administered four hours later.After model rats sacrificed,the pathological changes of lung were observed,and lung wet/dry weight ratio,protein content,and the ratio of neutrophiles in brochoalveolar lavage fluid (BALF) were calculated.In addition,the expression of Fas,FasL protein and apoptosis were evaluated by immunohistochemical studies and TUNEL technique.Results The acute lung injury rat model was successfully induced by"two hits".The gross and micrographic injury of lung was milder in RSM treatment rats than in model rats.The W/D ratio,protein contents and the ratio of neutrophiles in BALF were also markedly reduced in comparison with model rats,while the expression of Fas and Fasl,and the apoptosis index in model rats were significantly increased compared with other two groups.Furthermore,it showed a positive correlation between the expression of Fas,FasL,and the number of cell with apoptosis.Conclusions RSM shows a protective effect on ALT rats caused by"two hits"likely reaulted from inhibiting the expressions of Fas and Fasl,which are associated with the cell apoptosis of lung tissue.

<p><b>OBJECTIVE</b>To study the extent and incidence of sensorineural hearing loss (SNHL) after radiotherapy (RT).</p><p><b>METHODS</b>Twenty-eight patients with diagnosed nasopharyngeal carcinoma (NPC) were selected. The pure-tone audiography, auditory brain stem evoked response (ABR), impedance audiometry and evoked otoacoustic emissions (EOAE) recordings were performed before RT, 1 month, 1, 2 and 5 years after RT.</p><p><b>RESULTS</b>At 1 month after RT, there were 7.1 and 25.7 dB increased mean bone conduction (BC) thresholds at speech (0.5 - 4.0 kHz) and at high frequency (8.0 kHz), and their BC thresholds were statistically significant increase than those before RT, respectively (P < 0.001). At 1 year after RT, there were 17.6 and 28.1 dB increased respectively, and their thresholds were statistically significant increase than those at pre-irradiation (P < 0.001). There were also significant increases in thresholds than those at 1 month of post-irradiation (P <0.001 or P < 0.05). At 2 years after RT, 21 and 27.4 dB were increased at respective those two frequencies, and there was a statistically difference only at speech frequencies when compared with those at 1 year after RT (P < 0.05). At 5 years after RT, 26.7 and 35.8 dB were increased at these two frequencies, and there were significant increases in threshold than those before, 1 month, 1 and 2 years after RT, respectively (P < 0.001). From 1 month to 5 years after RT, 37. 5% to 94. 7% of ears had a BC hearing threshold of at least 15 dB losses at speech frequency, whereas the percentage at high frequency was 85.4 to 97.4%. Up to 63.2% and 73.7% of ears had 30 dB SNHL at least at speech and high frequency, respectively. Furthermore, the degree of mean threshold loss was greater at high frequency than at speech frequency. The mean value of wave I, III and V latency, and I -V interpeak latency intervals of ABR had no significant difference between at 1 month after RT and before RT (P > 0.05). The wave I , III and V latency, and I - V interpeak latency intervals at 1 year and 2 years were significantly prolonged when compared with those before and 1 month after RT (P < 0.05), but there were no significant difference between 1 year and 2 years after RT (P > 0.05). The wave I, III and V latency, and I -V interpeak latency intervals at 5 years after RT were also significantly longer than those before RT (P < 0.001). There were significant difference in wave I , III and V latency (P < 0.05), and no significant difference in wave I - V interpeak latency intervals (P > 0. 05) between 5 years after RT and 1 year or 2 years after RT. Seven of 10 ears at 1 year after RT and 4 of 7 ears at 5 years after RT had normal EOAE, but they all had abnormal ABR response.</p><p><b>CONCLUSIONS</b>SNHL in NPC patients start soon after completion of RT, especially more commonly in high frequency. The incidence and the extent of hearing loss are increased with time of follow-up. The hearing impairment could occur in the cochlea and/or the retrocochlear auditory pathway, which show that the sensitivity of radiation damage may be different in different patient and anatomic site of auditory system.</p>

<p><b>OBJECTIVE</b>To observe the alterations of the auditory function and morphology of the ear in the mouse sialidosis models which has been generated by targeted deletion of lysosomal neuraminidase gene (Neul) and closely resembled the phenotypes in corresponding human conditions, and to explore pathophysiological mechanisms of hearing impairment.</p><p><b>METHODS</b>Neul homozygous (Neul -/-) mice at 3 weeks, 2 and 4 months of age, and their wildtype littermates (Neu1 +/+) were examined for auditory thresholds through auditory brainstem responses (ABR) to click, 8, 16, and 32 kHz stimuli. Morphological analyses in ears were performed by series temporal bone section and light microscopy.</p><p><b>RESULTS</b>Neul -/- mice at 3 weeks of age showed an elevated ABR threshold, 50-55 dB above those of Neul +/+ mice. Up to 2 and 4 months of age, their thresholds were further elevated for 60-68 dB. There were distinct pathological changes of middle and inner ear of 3 weeks of age in Neul -/- mice, especially at 2-4 months of age there were significant cerumen occlusion in the external auditory canal and severe otitis media. Vacuolation associated with lysosomal storage was observed within ossicles and cochlear bone cells, stria vascularis cells, spiral ganglion neurons and macrophages, spiral limbus, spiral prominence, Reissner's membrane cells, and the mesothelial cells of the perilymphatic scala and basilar membrane, but not within the organ of Corti. Vestibular ganglion neurons, hair cells and supporting cells in cristae and maculae also showed vacuolation.</p><p><b>CONCLUSIONS</b>The deficiency of lysosomal neuraminidase may result in a serious hearing loss and morphological alterations of ear. The external auditory canal obstruction, otitis media and ossicle changes may cause conductive hearing loss, and the defects in lysosomal storage of neurons, stria vascularis, spiral limbus, Reissner's membrane and basilar membrane cells may contribute to sensorineural deafness.</p>

RESUMEN

AIM: To explore the significance of platelet activation, fibrinolytic activity and the changes of vasoactive mediators in acute myocardial infarction in rabbits and the intervention of amiodarone and metoprolol. METHODS: Fifty New Zealand white rabbits were randomly assigned to five groups, ten for each. Group Ⅰ: sham group, group Ⅱ: acute myocardial infarction(AMI) group, group Ⅲ: AMI and lidocaine group, group Ⅳ: AMI and amiodarone group, group Ⅴ: AMI and metoprolol group.The middle point of left ventricular coronary artery was ligated (groupⅡ,Ⅲ, Ⅳ and Ⅴ ) or a sham ligation(group Ⅰ). Four hours later, blood was collected for measuring plasma concentration of TXB 2, 6-Keto-PGF 1?, ET, NO, plasma activity of t-Pa and PAI.After that, the heart was taken out to evaluate the infarction size(IS). RESULTS: Plasma concentration of TXB 2, ET, NO and plasma activity of PAI were significantly higher in groupⅡ,Ⅲ, Ⅳ and Ⅴ than those in group Ⅰ(P0.05).Compared to group Ⅱ, plasma concentration of ET, NO and PAI activity were significantly decresed (P
