RESUMEN

Irritable bowel syndrome (IBS) is a functional gastrointestinal disease, but it often causes extreme gastrointestinal discomfort and prolonged illness, which seriously affects the quality of life of patients. The global incidence rate is increasing year by year. Clinically, western medicine mainly uses oral antispasmodics, secretagogues, and antidepressants, but there are many disadvantages such as adverse reactions and poor long-term efficacy. Therefore, finding an efficient and safe treatment method is an urgent problem to be solved. A large number of studies have shown that traditional Chinese medicine has definite curative and long-lasting effects on the treatment of IBS, which has become a hot research direction in recent years. By searching Chinese and foreign literature, it is found that electroacupuncture, moxibustion, Chinese medicine monomers, and compound decoctions are the main methods in the mechanism research of traditional Chinese medicine in the treatment of IBS-related pathways, and their signaling pathways involve nuclear transcription factor kappa B (NF-κB), transient receptor potential vanillin subfamily 1 (TRPV1), 5-hydroxytryptamine (5-HT), mitogen-activated protein kinase (MAPK), and so on. Traditional Chinese medicine can repair intestinal inflammation, reduce visceral sensitivity, enhance intestinal mucosal barrier, and regulate intestinal motility by regulating this series of signaling pathways, thereby playing an important role in the treatment of IBS with multi-level, multi-link and multi-target characteristics. Based on the cell signaling pathways, this paper reviewed the research progress on the mechanism of traditional Chinese medicine in the treatment of IBS, hoping to provide theoretical support and diagnosis and treatment ideas for the clinical treatment of IBS with traditional Chinese medicine.

RESUMEN

Objective To develop a reasonable plan of monitoring personal internal exposure dose. Methods This paper introduced the methods of monitoring the individual dose and direct measurement of three representative radionuclides. Results The maximum monitoring periods were determined according to the radionuclide retention characteristics and the reporting standards and requirements, which were m(1)/m(T/2) ≤ 3 and m(T/2)/m(T) ≤ 3. The lower detection limit of the instrument was derived from the monitoring periods and the annual radionuclide intake limit, which should be lower than the derived method detection limit of the corresponding radionuclide. Then the measuring duration of the instrument that meets the corresponding conditions was derived from the derived method detection limit of the instrument and the maximum monitoring period. Conclusion Our results provide a reference for the formulation of a plan of monitoring personal internal exposure dose.

RESUMEN

BACKGROUND@#Epidermal growth factor receptor (EGFR) is the gene with the highest mutation rate in non-small cell lung cancer (NSCLC) patients, and the accurate evaluation of its mutational status can facilitate patients receiving targeted drug therapy and thereby prolong patients' survival. The gene testing platform has adequacy requirements for the specimen quality in order to obtain accurate examination results. It has been reported that the number and proportion of tumor cells in samples will affect the detection rate of EGFR gene mutation. The present study aims to analyze the relationship between the quality of small biopsy specimens of NSCLC and the mutation rate of EGFR gene with amplification refractory mutation system (ARSM) test.@*METHODS@#After collecting the clinical characteristics of 299 cases small biopsy of lung adenocarcinoma, DNA concentration of the specimens and the mutational status of EGFR gene, the number and proportion of tumor cells in HE stained sections evaluated using light microscopy, the relationship between specimen quality and the mutation rate of EGFR gene were analyzed.@*RESULTS@#The mutation rates of EGFR for the groups with tumor cell number ≤500 and >500 were 40.7% (11/27) and 43.8% (119/272) respectively, without significant difference (P=0.764). The mutation rates for the groups with DNA concentration ≤20.4 ng/μL and >20.4 ng/μL were 42.7% (64/150) and 44.3% (66/149) respectively, without significant difference (P=0.776). The mutation rates for the groups with tumor cells proportion ≤30% and >30% were 29.4% (20/68) and 47.6% (110/231) respectively, demonstrating significant difference (P=0.008). Multivariate Logistic analysis showed that male, thyroid transcription factor-1 (TTF-1) negative, smoking history and tumor cell proportion less than 30% were main factors that contributes to the low detection rate of EGFR gene mutation.@*CONCLUSIONS@#After meeting the minimum requirements for detection, the EGFR mutation rate is affected by the proportion of tumor cells in the sample. Therefore, it is necessary to re-evaluate the tumor cell proportion in the last section after the genetic test section. For samples with lower tumor cell proportion, enriching tumor cells through microdissection and other methods is recommended for a more accurate detection result. For specimens that cannot be enriched with tumor cells, circulating tumor DNA (ctDNA) test can be performed as a supplement. If the result is still negative, another biopsy should be considered to obtain enough tumor specimens for molecular testing.

RESUMEN

Objective:To evaluate the status quo of humanistic care of clinical nursing staff, and analyze its influencing factors to provide theoretical basis for improving the humanistic care ability of clinical nursing. Methods:The self-compiled questionnaire on the knowledge, attitude and practice of humanistic care of clinical nursing staff was adopted in June to July 2019 to select 2 136 nurses from five first-class tertiary general hospitals in Shandong province by random sampling method for the status survey. Results:The total score of humanistic care knowledge and practice of clinical nurses was 259.54±33.55, and the standard score was 82.39±10.65. Single factor analysis showed that the total score of humanistic care knowledge, credit and practice was statistically significant for clinical nurses with different gender, age, fertility status, relationship with parents, employment reasons, initial education background, department, working years, professional title, position, employment mode and night shift frequency ( t value was -2.604, F values were 2.667-23.692, all P < 0.01). Pearson correlation showed that in the pairwise correlation analysis of knowledge, belief/attitude and behavior dimension, there was positive correlation and the correlation coefficient ( r values were 0.638, 0.431, 0.560, all P < 0.01). Multiple stepwise regression analysis showed that: the influencing factors of knowledge, attitude and practice total score of clinical nursing staff on humanistic care were the relationship between the parents, employment way, night shift frequency, fertility status, and the employment reason, the initial degree, department, position ( β values were -0.132-0.126, all P < 0.05). Conclusions:The current situation of humanistic care knowledge and practice of clinical nursing staff is in the middle level. Relationship with parents, position, night shift frequency, fertility status, employment reasons, initial education background, department, and employment mode are the factors that affect the current situation of humanistic care, knowledge, attitude and practice of clinical nurses. Therefore, medical institutions should adopt targeted and focused training methods and intervention measures to improve the level of nursing staff′s knowledge, faith and practice from the aspects of different influencing factors of clinical nursing staff′s humanistic care.

RESUMEN

Background: Helicobacter pylori lipopolysaccharide (Hp-LPS) plays an important role in gastric carcinogenesis. Aims: To investigate the regulatory effect of Hp-LPS on MDM2 gene expression and its impact on malignant transformation of gastric epithelial cells. Methods: After stimulated with Hp-LPS 1 μg/mL or Escherichia coli (E. coli)-LPS 1 μg/mL for 24 hours, expression of MDM2 in human gastric epithelial cell line (GES-1) and gastric cancer cell line (HGC-27 and MKN45) was detected by Western blotting. Plasmid containing MDM2 promoter luciferase reporter gene was constructed and transfected into HGC-27 cells, and the relative luciferase activity was measured after treated with Hp-LPS for 24 hours. RNA interference plasmid targeting to MDM2 was constructed and transfected into GES-1, HGC-27, and MKN45 cells, then changes in cell apoptosis and invasiveness were analyzed by flow cytometry and Transwell invasion assay. Growth inhibition of MKN45 and HGC-27 xenograft tumors in nude mice was observed after receiving caudal vein injection of siRNA-hMDMA2-3 for silencing MDM2 expression. Results: Hp-LPS enhanced the transcriptional activity of MDM2 promoter and up-regulated MDM2 protein expression in gastric epithelial and gastric cancer cells. In GES-1, HGC-27, and MKN45 cells transfected with siRNA-hMDMA2-3, the cell apoptosis was increased and the cell invasive capacity was reduced as compared with those transfected with negative control siRNA (P<0.05). Furthermore, the tumor inhibition rate of nude mice treated with siRNA-hMDMA2-3 was increased significantly as compared with those treated with negative control siRNA (P<0.05). Conclusions: Hp-LPS can induce malignant transformation of gastric epithelial cells by up-regulating MDM2 expression. Inhibiting MDM2 expression might suppress the malignant transformation and tumor growth by inducing cell apoptosis and reducing cell invasiveness.

RESUMEN

Chronic kidney disease (CKD) is now recognized as a worldwide public health challenge, and the incidence rate and hospitalization rate have significantly increased in recent years. Without prompt diagnoses and effective treatment in the early renal function damage of CKD, the symptoms will continue to worsen and eventually develop into end-stage renal disease. Functional imaging techniques such as single photon emission computed tomography (SPECT), contrast-enhanced ultrasound (CEUS), computerized tomography perfusion (CTP), and magnetic resonance perfusion weighted imaging (MR-PWI) could be used to quantitatively analyze renal perfusion and renal filtration function. Their diagnostic values are increasingly evident and have become the research hotspot in evaluating renal function. The aim of this review is to briefly evaluate the research and application advances in the early renal function damage assessment of CKD, so as to raise the efficiency of clinical applications.

RESUMEN

In recent years, according to the etiology and pathology researches of slow transit constipation (STC) STC is considered as a kind of "enteric neuropathy", indicating it is a kind of disease caused by abnormity of the enteric nervous system (ENS). Through reviewing the mechanism of acupuncture to regulate STC, it is found out that there is a close relationship between acupuncture regulating STC and ENS. Through various channels including ganglion cells, nerve plexus, neurotransmitter and TRPV1 (the primary sensory neurons receptor of the ENS) of the ENS, acupuncture is likely to make comprehensive adjustment on STC.

There were 46 cases in each group. The treatment of acupuncture together with warm acupuncture was given for three month. The observation was made on the fasting plasma glucose (FPG), fasting leptin (FLP), fasting insulin (FINS), insulin sensitivity index (ISI), insulin resistance index (Homa-IR), insulin secre-tion index (Homa-β), total cholesterol (TC), triglyceride (TG), low density lipoprotein cholesterol (LDL-C), high den-sity lipoprotein cholesterol (HDL-C), body mass, body mass index (BMI) and body fat percentage (F%) before and af-ter treatment. The results showed that the total effective rate of group 1 was 69.57%, while the total effective rate of group 2 was 84.78%. There was no significant statistical difference between two groups. Compared with pretreatment, there were significant decrease on FLP, FPG, FINS, Homa-IR, TC, TG, LDL-C, body mass, BMI, and F% of both groups after treatment (P < 0.01). There was obvious increase on ISI, Homa-β, and HDL-C (P < 0.01). After treat-ment, the FPG, Homa-β and BMI of group 2 were better than group 1 (P< 0.05). After treatment, the level of LDC-C and HDC-L of both groups reached normal level. And the FPG of group 2 also reached normal level. It was con-cluded that acupuncture together with warm acupuncture can significantly adjust the level of fat-pancreatic islet shaft of T2DM patients of dual deficiency of yin and yang syndrome, reduce the leptin resistance and insulin resistance, enhance the sensibility of insulin and the function of pancreatic β cell, and correct the endocrine disorder, gly-cometabolism disorder and lipometabolism disorder. Acupuncture together with warm acupuncture did much better in lowering blood sugar and improving the function of pancreatic β cells of T2DM patients whose priority of yang defi-ciency in the type of dual deficiency of yin and yang than those whose priority of yin deficiency in the type of dual deficiency of yin and yang.

RESUMEN

Objective To establish homogeneous immunoassay for detecting serum cardiac troponin I (cTnI) by using light induced chemiluminescent immunoassay (LiCA). Methods Polyclonal antibodies of cTnI were coated on the receptor particles, monoclonal antibodies of cTnI were biotinylated, and the donor particles were coated with streptavidin, all of which were composed of LiCA reagents. The optimal test conditions and analytical performance of the detection method were studied. Results The method was rapid, sensitive, and detection time was 17.5 min.The analytical sensitivity was 0.045 ng/mL and the functional sensitivity was 0.053 ng/mL.The recovery rate was 104.96%-108.21%;The within-run and the between-run coefficients of variation were 3.88%-5.53%and 7.60%-8.75%, respectively. The interference rates for the endogenous substances were less than 10%. The reference value of cTnI was less than 1.05 ng/mL;Results of cTnI LiCA correlated well with direct chemiluminescence detection (r2 =0.979). Conclusions This approach can be used for the quantitative detection of serum cTnI, and it is homogeneous and is free of clean separation. It provides a convenient, highly sensitive detection platform for clinical practice.
