RESUMEN

Background Previous studies show that aluminum exposure could increase the expression of miRNA-134-3p, which is involved in the mechanism of aluminum induced learning and memory impairment. However, it has not been investigated whether the expression level of miRNA-134-3p in the peripheral blood of occupational aluminum exposed workers is related to the blood aluminum concentration yet. Objective To evaluate a potential correlation between aluminum concentration in peripheral blood and miR-134-3p expression in occupational aluminum exposed workers. Methods A total of 184 male aluminum workers in the electrolytic aluminum workshop, aluminum oxide workshop, and thermal power workshop of an aluminum plant in Shanxi were selected by cluster sampling. They were divided into four groups (Q1-Q4) according to the quartiles of blood aluminum concentration, with 46 workers in each group. The basic information of workers was collected by questionnaire survey, and the cognitive function of workers was evaluated by Montreal Cognitive Assessment (MoCA). The plasma of workers was collected, and the relative expression level of miR-134-3p in plasma was detected by real-time quantitative polymerase chain reaction (RT-PCR). The plasma aluminum concentration was detected by inductively coupled plasma mass spectrometry (ICP-MS). The associations among workers' peripheral blood aluminum concentration, plasma miR-134-3p expression level, and total MoCA score were evaluated by generalized linear models. Results The workers' medians (P25, P75) of blood aluminum concentration, plasma relative expression level of miR-134-3p, and MoCA score were 39.31 (25.30, 57.41) μg·L-1, 2.93 (2.29, 3.74), and 22.0 (20.0, 26.0), respectively. The results of the generalized linear model showed that after adjusting for age, body mass index, smoking, and alcohol consumption, compared with the Q1 group, blood aluminum in the Q2, Q3, or Q4 group had an impact on related plasma miR-134-3p expression level and total MoCA score (P<0.05). With increasing blood aluminum concentration, the expression level of miR-134-3p in workers' plasma gradually increased, showing a positive correlation (b>0, Ptrend<0.001), while the total score of MoCA gradually decreased, showing a negative correlation (b<0, Ptrend<0.001). As the expression level of miR-134-3p in plasma increased, the total score of MoCA gradually decreased, showing a negative correlation (b<0, Ptrend<0.001). There was a linear relationship between peripheral blood aluminum concentration and plasma relative expression level of miR-134-3p of the workers in the middle school and below group and the high school group (Ptrend<0.05), b (95%CI)=1.796 (1.248, 2.344) and 1.192 (0.874, 1.510), and no correlation was found in the workers in the college and above group (Ptrend>0.05). Conclusion Occupational aluminum exposure can lead to an increase in the expression level of miR-134-3p in plasma of workers, which may be related to a decrease in cognitive function of workers.

RESUMEN

ObjectiveTo explore the association between air pollutants and hospital outpatient visits in a district of Shanghai. MethodsDaily meteorological data, environmental data, data of outpatient visits to two secondary hospitals and two tertiary hospitals in this district from January 1, 2015 to December 31, 2019 were collected. A Poisson regression generalized linear model was used to analyze the exposure-response relationship between the air pollutants and hospital outpatient visits in this area. ResultsDuring the study period, the total number of outpatient visits in the included hospitals was 17 802 634, with an average daily total of (9 750±4 191) outpatient visits, and an average daily of (761±341) respiratory outpatient visits. In the lag effect of single pollutant model, when the concentration of air pollutant increased by 10 μg·m-3, PM2.5, SO2, NO2 had the maximum lag effect on the number of outpatient visits in the department of internal medicine for respiratory diseases on lag day 4, day 5 and day 7, respectively. And the RR values and 95%CI were 1.002 0(1.001 3‒1.002 6), 1.0154(1.012 3‒1.018 5), and 1.006 1(1.005 3‒1.006 9), respectively. ConclusionThere is a exposure-response relationship between air pollutants and the number of outpatient visits in each department of the hospitals, and different pollutants have different degrees of lag effects.

RESUMEN

Objective:To analyze the staff salary status and the influencing factors in traditional Chinese medicine(TCM) hospitals implementing the salary system reform in Sichuan province in 2020, for reference in optimizing the salary system reform of such hospitals. Methods:Cluster sampling and institutional survey were used to collect the salary information of 26 TCM hospitals in 21 cities(prefectures)of Sichuan province implementing the salary system reform in 2020. Such information was then subject to descriptive analysis, while the influencing factors of salary were subject to one-way analysis of variance and generalized linear model multifactor analysis.Results:15 428 staff from 26 TCM hospitals were included as the research objects. In 2020, personnel expenditure accounted for 40.23% of the total expenditure, and 24.34% of which came from financial subsidy in 26 TCM hospitals. The average annual payable income per person was(149 312±74 288)yuan, 67.82% of which being performance pay. Analysis of variance showed that there were significant differences among the salary levels of staff in different economic regions, hospital grades, hospital levels, gender, educational background, position, seniority, performance pay ratio, employment in the government system and other natures, senior and other professional titles, doctors and other positions( P<0.05), and the differences were still statistically significant after adjustment by generalized linear model( P<0.05). Conclusions:The reform of the salary system of Sichuan TCM Hospitals has basically achieved equal pay for equal work, and the income of low-level personnel has been improved. However, the salary level was not very motivated and the salary structure was not guaranteed. It is necessary to strengthen financial precision subsidies, increase the proportion of personnel expenditure, so as to support the increase of the absolute value of salary in non-core economic areas, improve the salary structure, reasonably widen the salary gap among different educational backgrounds and positions, further optimize internal distribution, and ensure the sustainable development of Chinese medicine talents.

RESUMEN

OBJECTIVE: To explore the relationship between occupational aluminum exposure and fasting blood glucose level in workers. METHODS: A cluster sampling method was used to select 178 occupational aluminum-exposed workers as the exposure group, and 178 workers without occupational aluminum exposure as the control group in a large aluminum factory in Shanxi Province. Glucose oxidase method was used to measure the fasting blood glucose level, and inductively coupled plasma mass spectrometry was used to measure the plasma aluminum level in these workers. A generalized linear model was used to analyze the correlation between plasma aluminum exposure level and fasting blood glucose in these workers. RESULTS: The blood aluminum level of workers in the exposure group was higher than that of the control group [median: 39.58 vs 16.67 μg/L, P<0.01]. The fasting blood glucose level of workers in the exposure group was higher than that of the control group [(5.33±0.79) vs(5.15±0.41) mmol/L, P<0.01]. The results of the generalized linear model analysis showed that the blood aluminum level of workers was positively correlated with their fasting blood glucose level after adjusting for age, body mass index, smoking, drinking, exercise, family history of diabetes, and incidence of diabetes(P<0.05). There was a dose-response relationship between the blood aluminum level and fasting blood glucose level of workers in the groups of junior high school and below and high school(all P_(trend)<0.01). There was no correlation found between blood aluminum level and fasting blood glucose level in the group of college and above(P_(trend)>0.05). There was a dose-response relationship between the blood aluminum level and the fasting blood glucose level in the workers in the non-exercise group(P_(trend)<0.01). There was no correlation found between the blood aluminum level and the fasting blood glucose level in the exercise group(P_(trend)>0.05). CONCLUSION: The blood aluminum level of workers exposed to occupational aluminum is positively correlated with their fasting blood glucose level. Higher education level or exercise can moderately reduce the effect of blood aluminum level on fasting blood glucose.

RESUMEN

ABSTRACT: Coffee crops play an important role in Brazilian agriculture, with a high level of social and economic participation resulting from the jobs created in the supply chain and from the income obtained by producers and the revenue generated for the country from coffee bean export. In coffee plant growth, leaves have a determinant role in higher production; therefore, the leaf count per plant provides relevant information to producers for adequate crop management, such as foliar fertilizer applications. To describe count data, the Poisson model is the most commonly employed model; when count data show overdispersion, the negative binomial model has been determined to be more adequate. The objective of this study was to compare the fitness of the Poisson and negative binomial models to data on the leaf count per plant in coffee seedlings. Data were collected from an experiment with a randomized block design with 30 treatments and three replicates and four plants per plot. Data from only one treatment, in which the number of leaves was counted over time, were employed. The first count was conducted on 8 April 2016, and the other counts were performed 18, 32, 47, 62, 76, 95, 116, 133, and 153 days after the first evaluation, for a total of ten measurements. The fitness of the models was assessed based on deviance values and simulated envelopes for residuals. Results of fitness assessment indicated that the Poisson model was inadequate for describing the data due to overdispersion. The negative binomial model adequately fitted the observations and was indicated to describe the number of leaves of coffee plants. Based on the negative binomial model, the expected relative increase in the number of leaves was 0.9768% per day.

This paper presents estimates of the discard excess catch in the industrial purse seine fisheries of the North-Center stock of the Peruvian anchoveta (Engraulis ringens). Discard excess catch is defined as the portion of the catch that is thrown into the sea when has captured more than the hold capacity can store. The analysis is based on the "On-board observer of the purse seine vessels program" led by the Instituto del Mar del Peru (IMARPE), and in the period 2005 - 2014, 5837 trips were sampled, corresponding to 1.6% of the total trips. The methodologies used in the discard excess catch estimation were: Generalized Linear Model (GLM) and Delta Model. The estimations showed difference in magnitudes but similar trends, although the delta model appears to be a better alternative procedure for estimating the discard excess catch. The Delta Model estimated the purse seine fisheries discarded a range from 2954 to 199 164 tons, an average estimated 121 312 tons, which represent 2.6% of the landings in the period 2005 - 2014. Spatial variables (e.g. distance from the shore, latitude, longitude), physical (SST), and school behavior (e.g. fish aggregation) should be included to improve the analysis and a better understanding of the discard excess catch behavior.

RESUMEN

BACKGROUND: This study purposed to analyze and understand how spatial accessibility of patients influenced the number of outpatient visits for the internal medicine of a hospital. METHODS: A hospital with 100 beds in Seoul, South Korea provided data from 2013 January 1 to 2013 June 30. Euclidean distance and road ares were used to represent the spatial accessibility. Patient level data and dong level data were collected and used in spatial analysis. Dong level data was converted into grid level (500×500 m) for the multivariate analysis. Hot-spot analysis and generalized linear model were applied to the data collected. RESULTS: Hot-spots of outpatient visits were found around the study hospital, and cold-spots were not found. Number of outpatient visits was varied by the distance between patient resident and hospitals, and about 80% of total outpatient visits was occurred in within the 5 km from study hospital, and 50% was occurred in within 1.6 km. Spatial accessibility had significant influences on the outpatient visits. CONCLUSION: Findings provide evidences that spatial accessibility had influences on the patients' behaviors in utilizing the outpatient care of internal medicine in a hospital. Results can provide useful information to health policy makers as well as hospital managers for their decision making.

RESUMEN

Background: A key objective of universal health coverage is to address inequities in the financial implications of health care. This paper examines the level and trend in out-of-pocket spending (OOPS) on health, and the consequent burden on Nepalese households. Methods: Using data from the Nepal Living Standard Survey for 1995–1996 and 2010–2011, the paper looks at the inequity of this burden and its changes over time; across ecological zones or belts, development regions, places of residence, or consumption expenditure quintiles; and according to the gender of the head of the household. Results: The average per capita OOPS on health in Nepal increased sevenfold in nominal terms between 1995–1996 and 2010–2011. The share of OOPS in household consumption expenditure also increased during the same period, primarily as a result of higher health spending by poorer households. Thirteen per cent of all households were found to incur catastrophic health expenses in 2010– 2011. This proportion of households incurring such expenditure rose between the two time periods most sharply in the Terai belt, eastern region and poorest quintile. Conclusion: The health-financing system in Nepal has become regressive over the years, as the share of the bottom two quintiles in the total number of households facing catastrophic burden increased by 14% between the two periods.

RESUMEN

OBJECTIVES: To examine whether the nursing practice environment at the hospital-level affects the job satisfaction and turnover intention of hospital nurses. METHODS: Among the 11 731 nurses who participated in the Korea Health and Medical Workers' Union's educational program, 5654 responded to our survey. Data from 3096 nurses working in 185 general inpatient wards at 60 hospitals were analyzed using multilevel logistic regression modeling. RESULTS: Having a standardized nursing process (odds ratio [OR], 4.21; p<0.001), adequate nurse staffing (OR, 4.21; p<0.01), and good doctor-nurse relationship (OR, 4.15; p<0.01), which are hospital-level variables based on the Korean General Inpatients Unit Nursing Work Index (KGU-NWI), were significantly related to nurses' job satisfaction. However, no hospital-level variable from the KGU-NWI was significantly related to nurses' turnover intention. CONCLUSIONS: Favorable nursing practice environments are associated with job satisfaction among nurses. In particular, having a standardized nursing process, adequate nurse staffing, and good doctor-nurse relationship were found to positively influence nurses' job satisfaction. However, the nursing practice environment was not related to nurses' turnover intention.

RESUMEN

Nowadays, an important and interesting alternative in the control of tick-infestation in cattle is to select resistant animals, and identify the respective quantitative trait loci (QTLs) and DNA markers, for posterior use in breeding programs. The number of ticks/animal is characterized as a discrete-counting trait, which could potentially follow Poisson distribution. However, in the case of an excess of zeros, due to the occurrence of several noninfected animals, zero-inflated Poisson and generalized zero-inflated distribution (GZIP) may provide a better description of the data. Thus, the objective here was to compare through simulation, Poisson and ZIP models (simple and generalized) with classical approaches, for QTL mapping with counting phenotypes under different scenarios, and to apply these approaches to a QTL study of tick resistance in an F2 cattle (Gyr x Holstein) population. It was concluded that, when working with zero-inflated data, it is recommendable to use the generalized and simple ZIP model for analysis. On the other hand, when working with data with zeros, but not zero-inflated, the Poisson model or a data-transformation-approach, such as square-root or Box-Cox transformation, are applicable.

Leptospirosis is an infectious disease that causes losses of productivity in cattle due to reduction of milk production, reproductive disorders and calf mortality, while also increasing costs associated with veterinary care, vaccines and diagnostic tests. The objective of this study was to analyze serological findings for bovine leptospirosis in the State of Rio Grande do Sul, Brazil, in order to detect temporal trends in the frequency of seropositive animals and to generate hypothesis about the fluctuation in seroprevalence along the period from 1996 to 2006. Simple linear regression and generalized linear models were used to analyze trends and verify factors influencing the frequency of seropositive animals, respectively. During 11 years, 18,806 serum samples were analyzed, in which 9,385 were positive (49.9%). The time-series analysis by serovars revealed a statistically significant increasing trend in the frequency of the following serovars: Australis, Autumnalis, Bratislava, Copenhageni, Grippotyphosa, Pyrogenes and Tarassovi (p < 0.01). Conversely, a statistically significant decreasing trend was observed for the serovars Hardjo and Wolffi (p < 0.01). There was a significant association between rain precipitation (mm) and the number of samples seropositive for leptospirosis (p = 0.01, OR = 1,003).

The goal of this work was to evaluate the effects of the pesticides abamectin, carbaryl, fenitrothion, methidathion, sulphur and trichlorfon on eggs of two Chrysoperla externa (Hagen) populations, as well as on the subsequent developmental stages and reproduction capacity of this green lacewing. Pesticide exposure and distilled water (control) were accomplished using a Potter's tower. None of the evaluated pesticides affected the egg viability of the C. externa populations studied, but abamectin and carbaryl caused significant mortality of other developmental stages of the predator. Sex ratio and reproduction capacity of females originated from treated eggs were unaffected. However, egg viability of C. externa from Bento Gonçalves, RS, Brazil, was reduced by sulphur. Considering to the total effect (E) of each pesticide, trichlorfon, fenitrothion, methidathion and abamectin were harmless to both C. externa populations (Bento Gonçalves and Vacaria), when applied on eggs, whereas carbaryl was slightly harmful. On the other hand, the fungicide sulphur was slightly harmful to C. externa from Bento Gonçalves, but harmless to those from Vacaria.

RESUMEN

This article investigates the hypothesis that gender inequality in income in Brazil is influenced by the social class context. Class order and occupational segregation act as relevant factors in gender inequality. The effect of social class on the gender wage gap, although less pronounced as compared to interactions between class and race, plays an important "moderating" role on gender disparity, particularly by accentuating the gap in the case of capitalists, managers, and holders of professional qualifications (as assets) and attenuating them in the case of ordinary workers. The study identifies sharp inequality in gender treatment that appears to supersede the inequality in access to valuable contexts and resources, representing a direct effect of gender.

In this paper, we provide an overview of statistical models for haplotype-based association studies, and summarize their features based on the design matrix. We classify the design matrix into the two types: direct and indirect. For these two kinds of matrices, we present and compare characteristics using a simple hypothetical example, and a real data set. The motivation behind this study was to provide practitioners with an improved understanding, to facilitate the informed selection of the appropriate haplotype-based model and to improve the interpretability of the models.
