RESUMEN

OBJECTIVE@#To compare the therapeutic efficacy of governor vessel moxibustion combined with fluoxetine hydrochloride capsule, simple fluoxetine hydrochloride capsule and placebo moxibustion combined with fluoxetine hydrochloride capsule for mild to moderate depression with kidney-yang deficiency.@*METHODS@#A total of 126 patients with mild to moderate depression with kidney-yang deficiency were randomized into a governor vessel moxibustion group (42 cases, 2 cases dropped off), a western medication group (42 cases, 1 case dropped off) and a placebo moxibustion group (42 cases, 1 case dropped off). The western medication group was given fluoxetine hydrochloride capsule orally, 20 mg a time, once a day. On the basis of the treatment in the western medication group, governor vessel moxibustion was applied from Dazhui (GV 14) to Yaoshu (GV 2) in the governor vessel moxibustion group, once a week; placebo moxibustion was applied in the placebo moxibustion group, once a week. Treatment of 8 weeks was required in the 3 groups. Before and after treatment, the scores of Hamilton depression scale-17 (HAMD-17), Asberg's rating scale for side effects (SERS) and TCM clinical symptom were compared, and the clinical efficacy was evaluated.@*RESULTS@#After treatment, the scores of HAMD-17, SERS and TCM clinical symptom were decreased compared before treatment in the 3 groups (P<0.05), the decrease ranges of above scores in the governor vessel moxibustion group were larger than those in the western medication group and the placebo moxibustion group (P<0.05). The total effective rate was 92.5% (37/40) in the governor vessel moxibustion group, which was higher than 75.6% (31/41) in the western medication group and 80.5% (33/41) in the placebo moxibustion group (P<0.05).@*CONCLUSION@#Governor vessel moxibustion combined with fluoxetine hydrochloride capsule can improve the degree of depression and relieve the clinical symptoms in mild to moderate depression patients with kidney-yang deficiency, the efficacy is superior to simple fluoxetine hydrochloride capsule, and can reduce the fluoxetine hydrochloride capsule-induced adverse effect to a certain extent.

RESUMEN

Background: Bipolar disorder is one of the common mental disorders(CMD) in India. It leads to significant cognitive deficit even during the period of euthymia. So, it is important to identify the various cognitive deficits in euthymic bipolar disorder patients in Indian population. Methods: The study was conducted over a period of 2 years in AIIMS Jodhpur, Rajasthan. 30 cases and 30 controls were selected. Cases were selected by administering Hamilton dep ression rating scale (HAM - D) and Young mania rating scale (YMRS) to the bipolar disorder patient, recruited from Psychiatry OPD, to select the patients who were in euthymia. Cognitive domains such as attention, memory, fluency, language and Visuospatial we re tested in both the groups and compared. Kruskal - Wallis test was employed to compare the 2 sample groups for different variables. In this study we wanted to observe the decline in cognitive function in healthy vs euthymic bipolar disorder patients and to know the various domains of cognition that are affected. Results: This study shows a presence of cognitive decline in euthymic bipolar disorder patients as compared to controls. A significant difference was found in all the cognitive domains namely atte ntion, memory, fluency, language and visuospatial between cases and controls. Conclusion: This study shows a positive correlation between cognitive decline and bipolar patients during the period of euthymia. This suggests that even though the bipolar disor der patient is in remission, there is still a residual effect on cognition during euthymic state which can eventually affect the day to day life and can hamper the quality of life.

RESUMEN

Objective:To analyze the imaging changes of patients with monophasic depression based on the changes in CT parameters of each cerebral lobe. Methods:A total of 100 patients with monophasic depression who received treatment in Department of Radiology, Shaoxing 7 th People's Hospital between June 2019 and June 2021 were included in the observation group. An additional 100 healthy individuals who concurrently received physical examination were included in the control group. CT scan of the brain was performed in all participants. CT value of each cerebral lobe was compared between the two groups. Results:CT values of the left frontal lobe and left temporal lobe in the observation group were (40.88 ± 2.01) HU and (40.21 ± 3.01) HU, respectively, which were significantly lower than those in the control group ( tleft frontal lobe = 2.94, P = 0.004; tleft temporal lobe = 3.29, P = 0.001). CT values of the right frontal lobe and right temporal lobe in the observation group were (40.09 ± 2.52) HU and (44.93 ± 3.03) HU, respectively. There were no significant differences in CT values of the right frontal lobe and right temporal lobe between the two groups ( tright frontal lobe = 1.89, P = 0.060; tright temporal lobe = 1.89, P = 0.060). There were no significant differences in CT values of the left and right occipital lobes and the left and right parietal lobes between the two groups (all P > 0.05). One-way analysis of variance revealed that the CT values of the left and right frontal lobes were negatively correlated with age (-0.441, -0.211) and course of disease (-0.445, -0.191) (both P < 0.05); the CT values of the left and right frontal lobes were not significantly correlated with 24-item Hamilton Depression Rating Scale score and the number of attacks (all P > 0.05). The CT values of the left and right temporal lobes (-0.207, -0.213), the left and right occipital lobes (-0.185, -0.195) and the left and right parietal lobes (-0.185, -0.180) were negatively correlated with age (all P < 0.05). The CT values of the left and right temporal lobes, the left and right occipital lobes, and the left and right parietal lobes were not significantly correlated with course of disease, 24-item Hamilton Depression Rating Scale score, and the number of attacks (all P > 0.05). Conclusion:The left and right frontal lobe and the left temporal lobe are atrophic in patients with monophasic depression. The degree of atrophy is closely related to the course of disease. Therefore, these findings can be used as a reference for imaging diagnosis of degenerative diseases of the central nervous system.

RESUMEN

Background: Polycystic ovary syndrome (PCOS) is one of the most common endocrinopathy among women of reproductive age. Physical symptoms in PCOS cause depression and decrease the Quality of Life (QoL). The objective was to study the prevalence of depression in women suffering from PCOS and to assess its correlation with QoL.Methods: This was a cross sectional study conducted among one hundred patients diagnosed with PCOS. Women 18-40 years of age diagnosed with PCOS were included in the study. Patients with known psychiatric illness were excluded. Depression severity was assessed with Hamilton depression (HAM-D) scale. The quality of life (QoL) was assessed with Polycystic ovary syndrome questionnaire (PCOSQ). The data was analyzed using SPSS 20.0 for Windows. Results: The mean age and BMI was 25.64±3.25 years and 26.78±2.72 kg/m2 respectively. The mean depression score was 12.46±6.18. QoL domains showed that the lowest score was in the menstrual problem with a mean of 12.48±4.44 and the highest was in the emotional domain with a mean of 28.07±11.17. The negative correlations were between age and Hamilton score (p < 0.05). The Hamilton score was negatively correlated with the emotional score (p <0.01) and weight score was positively correlated with infertility score (p <0.01).Conclusions: PCOS was clearly associated with depression and reduced QoL. This should warrant health professionals to consider routine screening for depression and assess the impact of symptoms on their QoL to improve patient outcomes.

RESUMEN

Objective To compare the cognitive functions between patients with treatment-resistant depression and first-episode depression. Methods A total of 80 major depressive disorder patients admitted in our hospital from March 2015 to December 2016 were included in this study. The patients were divided into treatment-resistant depression group (n=40) and first-episode depression group (n=40). Another 40 healthy individuals were used as the control group. The basic data of patients were collected, and Hamilton depression scale (HAMD)-17 was used to assess the severity of the disease. The performing functions were assessed by trail marking test and stroop color-word test. The attention functions were assessed by digital span test. Memory functions were assessed by Hopkins verbal learning test revise (HVLT-R). After treatment for 6 months, cognitive functions were assessed again in first-episode depression group. Results Compared with control group, the scores of trail marking test (TMT) increased, while the scores of digital span test, stroop color-word test, stroop color-colorword test, HVLT-R reduced in treatment-resistant depression group and first-episode depression group ( P<0.05). The scores of TMT, HAMD-17 were lower in first-episode depression group than those of treatment-resistant depression group (P < 0.05). There were no significant differences in other indexes between groups. After six months treatment, the trail marking test score and HAMD-17 reduced, but digital span test, stroop color-word test increased in first-episode depression group (P<0.05). Conclusion The cognitive function damages severely in patients with treatment-resistant depression and first-episode depression, but there is no obvious difference in severity between two groups of patients.

RESUMEN

Objective By observing the clinical efficacy of acupuncture plusBu Shen Jie Yu Qing Xin decoction (herbal decoction for tonifying kidney, anti-depression and clearing heart fire) in treating perimenopausal depression, to discuss about the effect of acupuncture plus medication on Hamilton Depression Scale (HAMD), Self-rating Depression Scale (SDS) and hypothalamic-pituitary-ovarian (H-P-O) axis in perimenopausal depression patients.Method By following a completely randomized design, sixty perimenopausal depression patients were divided into a Chinese medication group (Bu Shen Jie Yu Qing Xin decoction) with 30 cases, and an acupuncture-medication group (acupuncture plusBu Shen Jie Yu Qing Xin decoction) with 30 cases, to receive 12-week treatment in total. The HAMD and SDS were estimated prior to the treatment and in the 4th, 8th and 12th weeks; the serum follicle stimulating hormone (FSH), luteinizing hormone (LH), estradiol (E2), testosterone (T), prolactin (PRL), and progesterone (P) contents were detected before and after the treatment.Result Acupuncture plus medication and Chinese medication alone were both effective in treating perimenopausal depression, and the therapeutic efficacy of acupuncture plus medication was superior to that of Chinese medication alone (P0.05). After 12-week treatment, the contents of FSH and LH declined significantly in both groups (P<0.01, P<0.05), and the contents of E2 and T increased significantly in the acupuncture-medication group (P<0.01,P<0.05). The decrease of FSH and LH, and the increase of E2 were more significant in the acupuncture-medication group (P<0.05,P<0.01).Conclusion Acupuncture plus medication can improve the symptoms of perimenopausal depression, and it takes a faster action than Chinese medication alone. Acupuncture plus medication can mitigate 7 items in HAMD, while Chinese medication can improve anxiety, insight, sleep and despair. Acupuncture plus medication can produce a more significant efficacy in improving anxiety, insight, sleep, diurnal-nocturnal, obstruction, and body weight compared to Chinese medication. For perimenopausal depression, acupuncture plus medication can correct the endocrine disorder and improve depression state by down-regulating the levels of FSH and LH, and up-regulating the contents of E2 and T to improve the function of H-P-O axis.

RESUMEN

Objective To observe the therapeutic efficacy of paired needling in treating insomnia with depression after cerebral stroke. Method Eighty patients with insomnia plus depression after cerebral stroke were randomized into a treatment group and a control group, 40 cases in each group. The treatment group was intervened by paired needling at Xinshu (BL15), Juque (CV14), Ganshu (BL18), and Qimen (LR14), once a day, 20 min each session;the control group was by oral administration of Paroxetine hydrochloride tablet 20 mg, 1 tablet each time prior to sleep at night. Two treatment courses were observed in the two groups (successive 2-week treatment as a course). The Pittsburgh Sleep Quality Index (PSQI) was adopted to observe the two groups before treatment, after 2-week treatment, and after 4-week treatment, and the clinical efficacy was also evaluated. Result Respectively after 2-week and 4-week treatment, the PSQI scores and Hamilton Depression Scale (HAMD) scores dropped significantly in the two groups compared to that before the intervention (P<0.05);the improvements of the PSQI and HAMD scores in the treatment group were more significant than that in the control group;the scores were markedly improved in the 2nd week in the treatment group and were significantly different from that in the control group (P<0.05);the significant differences were maintained in the following 2 weeks;the total effective rate was respectively 90.0%(36/40) in the treatment group and 15.0%(6/40) in the control group, and the therapeutic efficacy of the treatment group was markedly superior to that of the control group (P<0.05). Conclusion The results show that paired needling is safe and effective in treating insomnia with depression after cerebral stroke, and its therapeutic efficacy is superior to that of Paroxetine hydrochloride tablets.

RESUMEN

Objective To observe the clinical efficacy of electroacupuncture at different frequencies in treating post-stroke depression. Methods A hundred patients with post-stroke depression were randomized into treatment group 1, treatment group 2, treatment group 3, and a control group, 25 in each group. The treatment groups were intervened by electroacupuncture, while 2 Hz was adopted in treatment group 1, 50 Hz in treatment group 2, and 100 Hz in treatment group 3. The control group was intervened by oral administration of Sertraline Hydrochloride tablets. The Function Independent Measure (FIM) and Hamilton Depression (HAMD) scale were adopted for observation before and after intervention, and the clinical efficacies were compared. Results The FIM and HAMD scores were significantly changed after intervention in the four groups (P<0.05). After treatment, the FIM and HAMD scores in treatment group 1 were significantly different from that in treatment group 2 and 3 (P<0.05). The FIM score in treatment group 1 was significantly different from that in the control group (P<0.05). The total effective rate was 96.0%in treatment group 1, versus 92.0%in treatment group 2, 82.0%in treatment group 3, and 84.0%in the control group. The total effective rate in treatment group 1 was significantly different from that in the control group (P<0.05). Conclusions Electroacupuncture at 2 Hz, 50 Hz, and 100 Hz all can improve the activities of daily living and depression degree in patients with post-stroke depression, while 2 Hz ranks the top in comparing the therapeutic efficacy.

RESUMEN

Objective To observe the clinical efficacy and reverse reactions of acupuncture combined with paroxetine hydrochloride in the treatment of mild or moderate depression. Methods The patients with mild or moderate depression (n=73) were randomly divided into control group (treated with paroxetine hydrochloride,n=33) and observation group (treated with acupuncture and paroxetine hydrochloride,n=40). The therapeutic course lasted for 6 weeks. The total score changes of Hamilton Depression Scale (HAMD) were observed before treatment and treated for 1, 2, 4 and 6 weeks. Rating Scale for Side Effects (SERS) was evaluated before treatment and treated for 2, 4, 6 weeks. Results The total effective rate of clinical efficacy in the observation group was 78.95% (30/38) and the control group was 68.75% (22/32), without significant difference between the two groups (P>0.05). The scores of HAMD decreased 4, 6 weeks after the treatment in the control group and 2, 4, 6 week after the treatment in the observation group compared with those in the same group before the treatment (P<0.05,P<0.01). There was a significant difference in HAMD scores between two groups after the treatment for 4 weeks (P<0.05). The scores of SERS showed a significant difference 4, 6 weeks after the treatment between the two groups (P<0.05).Conclusion Acupuncture can improve the curative effect of paroxetine hydrochloride and decrease its side effects in the treatment of depression.
