RESUMEN

The tiered medical treatment system stands as a cornerstone in the deepening reforms of China′s medical and health sectors, playing a crucial role in building a healthy China. Exploring the harmonious coexistence mechanism of multiple entities on the supply and demand sides of the tiered diagnosis and treatment system, and promoting the formation of a scientific and reasonable tiered diagnosis and treatment order, has become an urgent public proposition that needs to be answered and has significant social impact. Addressing the challenges in China′s tiered medical treatment system, particularly its need for a more systematic, comprehensive, and collaborative approach, this study is informed by a thorough literature review. Based on the harmonious management and symbiosis theories, the authors proposed a theoretical concept and future research path for the formation of a harmonious symbiotic mechanism in the tiered diagnosis and treatment system, in order to pave the way for exploring the harmonious symbiotic mechanism in the tiered diagnosis and treatment system.

Sedentary behavior is ubiquitous in modern lifestyles and defined as any waking behavior with an energy expenditure of ≤ 1.5 metabolic equivalents while sitting, reclining, or lying. Epidemiological evidence suggests that high volumes of sedentary behavior are independently associated with an elevated risk of cardiometabolic disease and all-cause mortality. By contrast, a growing body of experimental evidence showing the potential benefits for cardiometabolic risks of reducing and breaking up sedentary time. Therefore, recent physical activity guidelines indicate the importance of reducing and regularly interrupting prolonged sitting. In this narrative review, we summarize the findings from experimental studies that investigated the acute impacts of prolonged, uninterrupted sitting and interrupting sitting on several cardiometabolic risk factors, including vascular function, blood pressure, and glucose metabolism. Here, we highlight experimental evidence from controlled laboratory trials that may lead to a better understanding of biological plausibility, the causal structure of relationships, and potential mechanistic insight on linking sedentary behavior with adverse cardiometabolic outcomes. Our literature review collectively suggests that in addition to increasing moderate- to vigorous-intensity physical activity, reducing sedentary time may contribute to cardiometabolic health. However, the recent evidence remains limited and inconclusive, thus future studies are needed to develop a deeper causal and mechanistic understanding of the biological pathways through which prolonged sitting can adversely influence cardiometabolic health outcomes.

In the Qing dynasty Cheng Zhongling based on the Cheng's Bixie-Fenqing formula of Yang Tan, a doctor in the Southern Song dynasty, retained the original Sichuan Bixie and Acorus tatarinowii, and add Phellodendron chinense, and Semen plantaginis to form Cheng's Bixie-Fenqing formula. It had the function of get rid of dampness and heat, and is a classic prescription for stranguria, like the treatment of white turbidity. The author retrieved the related literature of Cheng's Bixie-Fenqing formula in recent years, analyzed the composition and clinical application of Cheng's Bixie-Fenqing formula, and summarized the advantages and disadvantages of the existing researches, providing reference for the further development of Cheng's Bixie-Fenqing formula.

A Psicoterapia Pais-Bebê (PPB) é uma intervenção utilizada no tratamento das relações iniciais pais-bebê. O presente estudo teve como objetivo apresentar uma proposta de avaliação do processo psicoterápico em PPB, com destaque para os mecanismos de mudança ao longo da psicoterapia. Foram investigadas as intervenções da terapeuta (1) para obter informações, (2) para informar o paciente, (3) de apoio, (4) de encenação e (5) de interpretação transgeracional; e, os insights (1) cognitivo, (2) afetivo, e (3) pragmático, associados às intervenções. Cada uma destas categorias foram ilustradas através das verbalizações de dois casos de atendimentos pai-mãe-bebê, em que as mães apresentavam depressão pós-parto. Evidências sugerem que a proposta para análise de psicoterapia pais-bebê apresentada neste estudo pode contribuir para analisar o processo terapêutico.


Parent-Infant Psychotherapy (PIP) represents an intervention used in the treatment of initial parent-infant relationships. Thus, the present study aimed to present a proposal for the evaluation of the psychotherapeutic process in PIP, with emphasis on the mechanisms of changing throughout psychotherapy. It was investigated the therapist's interventions (1) to collect information, (2) to inform the patient, (3) to give the support, (4) of staging and (5) transgenerational interpretation; e, the insights (1) cognitive, (2) affective, and (3) pragmatic associated with interventions. Each of these categories was illustrated through verbalizations of two father-mother-infant consultations, in which the mothers presented postpartum depression. Evidences suggest that the proposal for analysis of parent-infant psychotherapy presented in this study may contribute to analyze the therapeutic process.


La Psicoterapia Padres-Bebé (PPB) constituye una intervención utilizada en el tratamiento de las relaciones iniciales entre padres y bebé. De esta forma, el objetivo del presente estudio fue presentar una propuesta de evaluación del proceso psicoterápico en PPB, destacando los mecanismos de cambio a lo largo de la psicoterapia. Se investigaron las intervenciones del terapeuta (1) para la obtención de información, (2) para informar al paciente, (3) como soporte, (4) como puesta en escena y (5) para la interpretación transgeneracional; (1) cognitivo, (2) afectivo, y (3) pragmático, asociados a las intervenciones. Cada una de estas categorías fue ilustrada a través de las verbalizaciones de dos atendimientos padre-madre-bebé, en los que las madres manifestaban depresión posparto. Las evidencias sugieren que la propuesta para análisis de la psicoterapia padres-bebé presentada en este estudio, puede colaborar para analizar el proceso terapéutico.

RESUMEN

To elaborate the the main thoughts and treatment experience of dampness in Medical Origin and Insights, written by Zhang Yuansu. Dampness-resolving drugs and prescriptions were comprehensively and carefully summarized and analyzed. Strengthening spleen and stomach was viewed as guideline, and Rhizoma Atractylodis Macrocephalae as well as its brilliant combinations, was taken as preferred drug to resolve dampness, with the help of brilliant combinations. Emphasis was laid on the treatment according to patterns, positions and characters of diseases. Medical Origin and Insights, essence collector of Zhang’s thoughts, has enriched and developed basic traditional Chinese medicine theories and practices. Thus, it showed guiding significance on dampness treatments.

PURPOSE: This study is a descriptive research in which the pathological syndromes of the schizophrenic patients in the community mental health centers, the degrees of the insights, and the correlations between them were examined. METHOD: The subjects included 114 SPR pt. people who were enrolled in community mental health centers located in D City. The research was conducted during the period from Aug. 1st, 2002 to Jul. 30th, 2004. The instruments were PANSS developed by Kay(1987) and SUMD developed by Amador(1993). RESULTS: Among the pathological syndromes, positives ones were 12.64+/-3.80 on the average, negative ones were 32.00+/- 5.36, and general pathological syndromes were 30.50+/-6.24. In the evaluation of the insights, the mean score was 11.81+/-5.09. There were some significant correlations between positive syndromes and general pathological ones (r=.572, p=.000), and negative syndromes and general pathological ones(r=.262, p=.029), while there was no significant correlation between the insights and the sub-measures of general pathological syndromes. CONCLUSION: Therefore, the schizophrenic patients taking advantage of the community mental health centers have more negative syndromes than those in hospitalized, and rehabilitation programs are needed to help them continuously. And the further study of the correlations between the pathological syndromes and the degrees of the insights are required, and still, it should be analyzed what effects the insight acquirement brings about to the improvements of the pathological syndromes after the application of the insight-oriented programs.

The higher medical education systemin China has experienced six significant curricular program changes within the last century. And these changes seem to run in a circle:from the pluralistic to the singleness,and then back to the pluralistic.The process repeated itself.These changes in history could provide some valuable insights into the future reform of the higher medical education system in China.
