RESUMEN

The present study analyzes the psychometric properties of the Inventory of Family Integration (IFI) by means of a dyadic analysis based on the responses of fathers and mothers in families from the city of Arequipa, southern Peru. For this purpose, 264 married couples living with their families were non-probabilistically selected and administered the 52-item IFI. An analysis of dimensionality and reliability was carried out, and a factorial invariance analysis was applied under a dyadic approach and, finally, a comparative analysis using Cohen's d was performed. The results reported moderate correlations between the values of fathers and mothers in each family holon: personal, spousal, parental, sibling and familial. Likewise, the dimensionality fit of each holon in fathers and mothers was adequate with acceptable magnitudes, and the reliability indices calculated with Cronbach's alpha and McDonald's Omega tests were high. The five-factor internal structure and internal consistency in both fathers and mothers were corroborated.


El presente estudio analiza las propiedades psicométricas del Inventario de Integración Familiar (IIF) mediante un análisis diádico en función de las respuestas de padres y madres de conforman una familia procedente de la ciudad de Arequipa, al sur de Perú. Para ello, se seleccionó de manera no probabilística a 264 parejas casadas que viven con sus familias, a quienes se les aplicó el IIF que consta de 52 ítems. Se realizó un análisis de la dimensionalidad y la confiabilidad, y se aplicó un análisis de invarianza factorial bajo un enfoque diádico y finalmente un análisis comparativo mediante la d de Cohen. Los resultados reportaron correlaciones moderadas entre los valores de los padres y madres en cada holón familiar: personal, conyugal, parental, fraternal y familiar. Asimismo, el ajuste de dimensionalidad de cada holón en padres y en madres fue adecuado con magnitudes aceptables, y los índices de confiabilidad calculados con las pruebas alfa de Cronbach y Omega de McDonald fueron elevados. Se corrobora la estructura interna de cinco factores y la consistencia interna tanto en padres como en madres.

Objetivo: traçar parâmetros para estruturar conceitos da abordagem One Health através dos pensamentos de Alfred North Whitehead, Arthur George Tansley, Amartya Sen e Norberto Bobbio. Metodologia: tratou-se de pesquisa original, com abordagem dedutiva e viés hermenêutico, baseada nos pensamentos selecionados e na orientação de Saúde Única. Resultados: One Health estrutura-se na afirmativa holística e integrada que a saúde humana, animal e ambiental estão interligadas. Sob a perspectiva de Whitehead, a abordagem One Health pode ser considerada um processo dinâmico e relacional, onde humanos, animais e meio ambiente interagem constantemente, interconectando-se por relações e processos, formando um todo. Pela perspectiva de Tansley, a ideia de One Health pode alinhar-se ao conceito de ecossistema, não podendo a saúde ser analisada isoladamente em indivíduos, mas, necessariamente, pelas interações complexas entre seres humanos, animais e o ambiente. Sob o prisma de desenvolvimento (direitos e liberdades), proposto por Sen, a abordagem One Health pode ser considerada um meio para alcançá-lo, através da interrelação de mecanismos, sistemas e instituições focados na promoção da saúde e do bem-estar. Na visão de Bobbio, direitos fundamentais, democracia e a paz, são formas éticas e primordiais para assegurar direitos, especialmente um novo direito da natureza (humanos, animal e ambiente) na busca conjunta de garantias para a convivência pacífica. Conclusão: a abordagem One Health não é apenas uma estratégia prática, mas também uma visão renovada da antiga percepção que reconhecia a interconexão de todas as formas de vida.


Objective: draw parameters to structure concepts of the One Health approach through the thoughts of Alfred North Whitehead, Arthur George Tansley, Amartya Sen, and Norberto Bobbio. Methodology: this was original research, with a deductive approach, hermeneutic bias based on the selected thoughts and the One Health. Results: One Health is structured on the holistic and integrated assertion that human, animal, and environmental health are interconnected. From Whitehead's perspective, the One Health approach can be considered a dynamic and relational process, where humans, animals, and the environment constantly interact, interconnecting through relationships and processes, forming a whole. From Tansley's perspective, the idea of One Health can align with the ecosystem concept, where health cannot be analyzed in isolation in individuals, but necessarily through the complex interactions between humans, animals, and the environment. From Sen's development prism (rights and freedoms), the One Health approach can be seen to achieve it, through the interrelation of mechanisms, systems, and institutions focused on promoting health and well-being. In Bobbio's view, fundamental rights, democracy, and peace are ethical and primary ways to ensure rights, especially a right of nature (humans, animals, and the environment) in the joint pursuit of guarantees for peaceful coexistence. Conclusion: the One Health approach is not just a practical strategy, but also a renewed vision of the old perception that recognized the interconnection of all forms of life.


Objetivo: establecer parámetros para estructurar conceptos del enfoque One Health a través de los pensamientos de Alfred North Whitehead, Arthur George Tansley, Amartya Sen y Norberto Bobbio. Metodología: se trató de una investigación original, con un enfoque deductivo, sesgo hermenéutico basado en los pensamientos seleccionados y el Salud Única. Resultados: One Health se estructura en la afirmación holística e integrada de que la salud humana, animal y ambiental están interconectadas. Desde la perspectiva de Whitehead, el enfoque One Health puede considerarse un proceso dinámico y relacional, donde humanos, animales y el medio ambiente interactúan constantemente, interconectándose a través de relaciones y procesos, formando un todo. Desde la perspectiva de Tansley, la idea de One Health puede alinearse con el concepto de ecosistema, donde la salud no puede analizarse aisladamente en individuos, sino necesariamente a través de interacciones complejas entre seres humanos, animales y el ambiente. Desde el prisma del desarrollo (derechos y libertades) propuesto por Sen, el enfoque One Health puede considerarse un medio para alcanzarlo, a través de la interrelación de mecanismos, sistemas e instituciones enfocados en la promoción de la salud y el bienestar. Desde la visión de Bobbio, los derechos fundamentales, la democracia y la paz son formas éticas y primordiales para asegurar derechos, especialmente un derecho de la naturaleza (humanos, animales y ambiente) en la búsqueda conjunta de garantías para la convivencia pacífica. Conclusión: el enfoque One Health no es solo una estrategia práctica, sino también una visión renovada de la antigua percepción que reconocía la interconexión de todas las formas de vida.

INTRODUCCIÓN: El virus del papiloma humano de alto riesgo (VPH-AR) es responsable del cáncer de cuello uterino y sus lesiones preneoplásicas. Los genotipos VPH16 y VPH18 son los más frecuentes en este cáncer. La integración del VPH-AR en el genoma de la célula hospedera es crucial en la carcinogénesis cervical, pero la etapa en que ocurre en la población chilena es incierta. OBJETIVO: Evaluar la integración de VPH16 y VPH18 en lesiones pre-neoplásicas de cuello uterino. MÉTODOS: Se analizaron 108 muestras de raspados cervicales. El VPH se genotipificó mediante reacción de polimerasa en cadena (RPC) e hibridación no radiactiva. La integración de VPH16 y VPH18 se determinó por presencia del gen E2 mediante RPC. RESULTADOS: VPH16 y VPH18 se detectaron en 36,1% y 12,0% de las muestras, respectivamente. El VPH16 se integró en 23,1% de los casos de VPH16, mientras que VPH18 se integró en 100% de las muestras positivas para este genotipo. CONCLUSIONES: La integración VPH-AR es un evento temprano en la carcinogénesis cervical que ocurre en casi la mitad de las lesiones pre-neoplásicas y es más frecuente en VPH18 que en VPH16. La evaluación de la integración VPH-AR puede ser una herramienta útil para detectar el virus en la población chilena.


BACKGROUND: High-risk Human Papillomaviruses (HR-HPVs) are the etiological agents of cervical cancer and its preneoplastic lesions. HPV16 and 18 are the most frequent HR-HPV genotypes detected in cervical cancer. HR-HPV genome integration into the host cell is an important event in the carcinogenic process. However, it remains uncertain which stage of cervical carcinogenesis HPV16 and 18 integration occurs in the Chilean population. AIM: The goal of this study was to evaluate HPV16 and HPV18 integration in preneoplastic lesions of the cervix. METHODS: DNA was extracted from 108 cervical scrape samples with preneoplastic lesions. HPV was genotyped using PCR and non-radioactive hybridization. The integration status of HPV16 and HPV 18 was determined by evaluating the E2 gene presence through PCR. RESULTS: HPV16 and HPV18 tested positive in 36.1% and 12.0% of samples, respectively. HPV16 was found integrated in 23.1% of HPV 16 cases, while HPV 18 in 100% of samples positive for this viral genotype. CONCLUSIONS: HR-HPV integration is an early event in cervical carcinogenesis, occurring in nearly half of preneoplastic lesions and being more frequent in HPV18 than in HPV16. The evaluation of HR-HPV integration can be utilized as a complementary tool for detecting HPV in the Chilean population.

Objective:To investigate the current status of the teaching methods used in the integrated basic and clinical courses and the approaches for improving such teaching methods, and to improve the teaching quality of the integrated basic and clinical courses for "4+4" Medical Doctor Program in Peking Union Medical College.Methods:Students in the Class 2020 of "4+4" Medical Doctor Program who completed the integrated courses from September 2020 to August 2021 were enrolled as subjects. Questionnaire surveys and interviews were performed for the subjects in terms of class hours, integration effect, and self-learning methods. R4.0.0 software was used to perform a statistical analysis of data.Results:The mean weekly class hours of existing modules were (27.59±2.61) hours per week, which was higher than the expectation of students. There was a negative linear correlation between weekly class hours and mean satisfaction score of each module ( r=-0.71, P=0.022). The integration of basic and clinical medical courses had a mean satisfaction score of 2.652. The median preview time for case-based learning/problem-based learning was 30 minutes. Conclusions:In the reform of the integration of basic and clinical medical courses, it is crucial to select and apply proper teaching methods. In order to further improve teaching effect, it is advised to reduce the class hours of theoretical lectures and carefully design and prepare learning materials, and the teaching contents should be organized according to the internal logic of knowledge and the association between clinical knowledge and the knowledge learned during the stage of basic medicine.

RESUMEN

Nursing skill competitions are the wind vane for clinical nursing development, and the teaching concepts, proposition forms, and evaluation systems demonstrated by competitions have a strong reference value for nursing teaching. This article introduces the knowledge selection and integration from nursing-related courses according to the problems and objectives to be solved by drawing on the STEM education concept in comprehensive nursing practical training in the context of integration of competition and education. Realistic problem scenarios can be created with clinical cases as the carrier or main line, and then the Tanner clinical judgment model is used to guide students to conduct a comprehensive analysis of cases, thereby helping students to develop clinical thinking ability and comprehensive practical skills, so as to bridge the gap between the contents learned at school and nursing work in clinical practice and help nursing students adapt to clinical practice as soon as possible.

Objective:The sensory processing measure-preschool scale(SPM-P)was transformed into Chinese,and its re-liability and validity were tested in preschool children. Method:According to Brislin's translation model,the SPM-P source scale was translated into Chinese and the Chinese version of SPM-P scale was formed.From September 2021 to December 2021,395 preschool children were investigated by cluster stratified random sampling method to test the reliability and validity of the scale. Result:The Chinese version of the SPM-P scale includes 8 dimensions of social participation,vision,hear-ing,touch,body awareness,balance and movement,planning and conception,and overall sensory system,with a total of 75 items.Cronbach alpha coefficient for the overall scale was 0.899,the split half reliability coefficient was 0.700;the test-retest reliability coefficient was 0.899.The item content validity(I-CVI)value was 0.920,the average content validity(S-CVI/Ave)value was 0.984,and the content validity was good;the results of confirmatory factor analysis showed that the construct validity was good;The results of confirma-tory factor analysis showed that the construct validity was good(x2/df=2.41,CFI=0.992,TLI=0.960,RMSEA=0.060,SRMR=0.046);The Chinese version of SPM-P scale was negatively correlated with the corresponding evaluation dimensions of the screening questionnaire for children's sensory integration disorder,and the correla-tion coefficient was between-0.585 and-0.399,with good criterion validity. Conclusion:The Chinese version of SPM-P scale has good reliability and validity and can be used for stan-dardized evaluation of sensory integration ability of preschool children in China.

The development of the integration of sports and health care(hereinafter referred to as"integration of sports and health care")is a key initiative to promote the health of residents.Literature,logical analysis and other research methods are applied to study the connotation,value and dilemma of the integration of sports and health care,and propose strategies.Body and health integration has demonstrated practical value in implementing the strategy of Healthy China,improving the health of the whole population,and optimizing the allocation of health resources.However,there are also practical dilemmas such as synergistic barriers in the institutional mechanism,insufficient knowledge of the population's health,a relative lack of professionals,and difficulties in promoting the results on the ground.Therefore,the following optimization paths are proposed:strengthening coordination and management,and improving the top-level design;strengthening health education to improve residents'health awareness;optimizing talent training programs and building a talent training mechanism;strengthening the application of results to guide and improve the application and promotion platform.

The development of the"integration of physical education,medical care,and elderly care"model is an important part of building a higher quality elderly care service system during the 14th Five Year Plan period.Based on literature review and logical analysis,it analyzes the driving forces and difficulties of the development of the"integration of sports,medicine,and nutrition"model during the 14th Five Year Plan period,and proposes optimization paths.It is believed that the core driving forces for the development of the"integration of physical education,medical care,and elderly care"model are the aging population and increasing demand for health,the continuous improvement of medical and health services,and the gradual increase in national policy support.The development of the"integration of physical education,medical care,and nursing"model faces practical difficulties such as incomplete laws and regulations,shortage of professional talents,low social awareness,imbalanced resource allocation,and unsmooth industrial collaboration.Based on this,combined with the national health development demand during the 14th Five Year Plan period,it is proposed to strengthen the construction of laws and regulations and formulate unified standards and norms;innovate talent training channels and establish a diversified training system;increase publicity and education efforts to raise public awareness and understanding;integrate and optimize resource allocation to improve resource utilization efficiency;build an industrial integration complex to promote industrial synergy and linkage.

Objective:To evaluate the effect of urban-rural integrated medical insurance on rural households'catastrophic health expenditure(CHE),thereby proposing targeted optimization strategies for the integration.Methods:Based on the five tracking data of the China Household Tracking Survey(CFPS)from 2010 to 2018,Process Specification Model-Dynamic Integrity Dimension(PSM-DID)was used to empirically test the impact of urban-rural integrated medical insurance on rural households'catastrophic health expenditures.Results:The urban-rural integrated medical insurance system significantly reduces the incidence of CHE in ru-ral households.Mechanism testing indicates that health levels,human capital expenditures,and household asset accumulation are important channels of action.Conclusion:It is suggested to continuously promote the urban-rural integrated medical insurance sys-tem,formulate comprehensive policies for medical insurance according to local conditions,and incorporate catastrophic health indi-cators into the detection and warning indicator system for rural residents returning to poverty.

This paper analyzes the main obstacles in the process of promoting the integration of medicine and prevention in China,including the mindset of prioritizing medical care over prevention,fragmented system architecture,conflicting direction of funding and incentives,and inherent contradictions in the division of labor and cooperation in service provision.Furthermore,this article attempts to present the current status and constraints of medical and health integration in China from the perspectives of management mechanisms and service mechanisms,highlighting the mismatch between these two aspects as the root cause.It proposes that the reform logic of current medical and health integration practices should prioritize the service mechanism.This involves clarifying the service interface of medicine and prevention integration and establishing patient-centered service pathways for integration.In the establishment of the service mechanism,a balance should be struck between efficiency and quality,while also integrating based on division of labor.Subsequently,adjustments to the management mechanism should be made in accordance with the medicine and prevention integration service mechanism.In the reform of the management mechanism,various stakeholders'interests should be coordinated and the costs of reform should be controlled.

Objective Based on social cognitive theory,to explore the main factors influencing the behavioral inten-tion of medical staff in merged medical community and the differences among county-township-village three levels medical staff.Methods The stratified random sampling was used to investigate the county-township-village three levels medical staff in merged county medical community,and multiple linear regression method and seemingly unrelated model(SUEST)were used to quantitatively analyze the influencing factors and differences among coun-ty-township-village three level medical staff's behavioral intention.Results The environmental factors including or-ganizational support(0.098),team integration(0.227),incentive mechanism(0.126)and individual factors including cognitive degree(0.118)and participation ability(0.053)can impact the behavioral intention of medical staff,and team integration has the greatest impact on the behavioral intention of medical staff at all levels.The incentive mecha-nism has more influence on the behavioral intention of medical staff at township(0.190)and county(0.165)level than village(0.083)level.Participation ability had more influence on the of village(0.187)level than township(0.129)and county(0.126)level.Cognition had a moderate effect,and organizational support had a smaller effect.Conclusion There are both similarities and differences in the influencing factors of medical staff's behavioral inten-tion,and targeted reforms should be carried out according to different characteristics.

Objective To explore the implementation effects and application value of the Clinical Rehabilitation Integration Plan based on the graded management of rehabilitation treatment programs in a 3A general hospital.Methods The details of the programme were firstly formulated according to the needs of clinical rehabilitation and the implementation plan was formed,and then the integrated clinical rehabilitation work was carried out for 1 year accordingly,and finally the effects before and after the implementation of the programme were compared and analysed by selecting the indicators of departmental operation and patient satisfaction.Results The rehabilitation programme was classified into 4 levels according to the degree of technical difficulty and medical risk,and its connotation and management requirements were defined in detail.The implementation of the programme included organisational structure,training and assessment,authorisation management and quality control.The programme was carried out in 8 clinical departments in the hospital,and the overall willingness of the clinical departments to develop early rehabilitation was improved since 2022.Conclusion This program can improve the operational efficiency of clinical departments and patient prognosis.

Undergraduate medical education is a crucial period for the growth of medical students, and how to do a good job in undergraduate teaching under the background of "new medicine" is currently a research hotspot. The clinical teaching stage is an important period for medical students to fully understand clinical disciplines and cultivate their understanding of specialties. Therefore, we should explore new teaching methods and means to adapt to the needs of the new era. In the context of "new medicine", the medical-engineering fusion diagnosis and treatment technology has become an important trend in the clinical diagnosis and treatment of oncology. In order to adapt to this change, clinical teaching and teaching management in oncology also need new exploration and research. Taking the clinical teaching of oncology as an example, this article discusses how to cultivate medical students' thinking of medical-engineering fusion.

Objective To systematically evaluate qualitative studies on the experience of transition from adolescent to adult medical care for patients with congenital heart disease(CHD),and to provide a reference for exploring CHD transition management options and developing intervention strategies.Methods A computerized search of PubMed,Embase,Web of Science,Cochrane Library,EBSCO,CINAHL,China Knowledge Network,Wanfang database,Vipshop database,and China Biomedical Literature Database for qualitative studies on the transition experience of CHD patients from adolescence to adult medical care was conducted for the period from the establishment of the database to April 2023.The quality of the literature was evaluated using the Joanna Briggs Institute(JBI)Australian Centre for Evidence-Based Health Care Quality Assessment Criteria for Qualitative Research(2016),and the results were integrated using meta-integration methods.Results A total of 9 studies were included,and 49 research results were extracted,and 11 categories were summarized.The final synthesis included 4 integrated results:①Complex attitudes towards healthcare transition,with both attachment and expectation:attachment to paediatric healthcare providers,expectation of transition to adult healthcare providers.(2)Facing multiple healthcare transition challenges:lack of adequate preparation for healthcare transition,parents withdrawing from the role of disease manager,large differences in services between paediatric and adult healthcare providers.③Expect to receive multiple supports:expect to receive comprehensive health education from healthcare personnel,expect healthcare institutions to set up healthcare transition counselling clinics and achieve handover of illness,expect to receive companionship and support from parents,expect to receive understanding and help from peers.④ Per-ceived benefits of medical transition:increased ability to manage illness,role change and personal growth.Conclusion Adolescents with CHD have a complex experience of transitioning to adult healthcare,and healthcare professionals should be attentive to their feelings,encourage them to deal with challenges positively,and provide adequate information and joint parental and peer support to facilitate a smooth transition to adult healthcare for adolescents.

The medical-research-industry integrated development is an effective way to promote the development of the research and development chain, industrial chain and market chain in the medical and health field, and helps to solve the technical problems of medical field. Based on the development of the eye health industry, the Optometry Hospital Affiliated to Wenzhou Medical University, combined with the scientific and technological innovation research of the hospital, jointly built the China Eye Valley with the government and universities, and built a platform for achievement transformation. The authors summarized the joint construction path of the Affiliated Optometry Hospital of Wenzhou Medical University, Wenzhou Medical University and China Eye Valley from the aspects of organizational structure, management functions, platform construction, policy formulation and resource gathering, as well as the achievements made. In view of the existing problems such as insufficient policy support, insufficient talent reserve and limited sharing of platform resources in the current development, the following suggestions were proposed: continue to increase government support and give more policy " concessions" ; Optimize the management organization of the hospital in the development of medical-research-industry integration, and set up a specialized cooperative management department; Continuously strengthen the reserve of disciplinary talents and cultivate and introduce interdisciplinary and composite talents; Establish a unified information resource sharing management platform; Clarify the benefits distribution of medical personnel, medical institutions, research and development institutions, and enterprises in the achievement transformation.

RESUMEN

Objective:To analyze the correlation between the grouping and weighting of two sets of disease combination systems, namely diagnosis-related groups(DRG) and diagnosis-intervention packet(DIP), and to establish a multidimensional analysis and evaluation mode by applying DRG, DIP, and clinical pathway to guide the standardized diagnosis and treatment and management of disease types.Methods:DRG grouping and DIP simulation full enrollment were applied to patients discharged from a tertiary Grade A general hospital in 2019. The correlation analysis between DRG, DIP, and clinical pathway inclusion(entry), correlation analysis between relative weight of DRG group and DIP standard score, and correlation analysis between clinical pathway entry and cost structure of the two disease groups were conducted by using chi-square test, Pearson correlation analysis, t-test, structural change value, degree of structural change, and incremental contribution rate. Results:Among the 130 395 patients, 41 460 cases entered the clinical pathway, 127 535 cases were enrolled in DRG, and 104 227 cases were enrolled in DIP. There was a correlation between the enrollment of DRG, DIP, and clinical pathway( P<0.05), and there was also a correlation between the relative weight of DRG groups and the enrollment of clinical pathway. The relative weight of the DRG disease group was positively correlated with the DIP standard score( r2=0.761 7, P<0.001). There was a significant difference in hospitalization costs between patients with and without clinical pathway access for some diseases( P<0.05), and different cost categories had different impacts on the total costs. Conclusions:The weight assignment and value orientation of DRG and DIP disease types are consistent, and the multi-dimensional fusion evaluation mode for DRG-DIP-clinical pathway is feasible. The correlation analysis of DRG, DIP, and clinical pathways can serve as the basis for disease classification and cost structure evaluation, which could help to carry out hospital′s refined management and optimize disease structure.
