RESUMEN

Over the past decade, dramatic progress has been made in dental research areas involving laser therapy. The photobiomodulatory effect of laser light regulates the behavior of periodontal tissues and promotes damaged tissues to heal faster. Additionally, photobiomodulation therapy (PBMT), a non-invasive treatment, when applied in orthodontics, contributes to alleviating pain and reducing inflammation induced by orthodontic forces, along with improving tissue healing processes. Moreover, PBMT is attracting more attention as a possible approach to prevent the incidence of orthodontically induced inflammatory root resorption (OIIRR) during orthodontic treatment (OT) due to its capacity to modulate inflammatory, apoptotic, and anti-antioxidant responses. However, a systematic review revealed that PBMT has only a moderate grade of evidence-based effectiveness during orthodontic tooth movement (OTM) in relation to OIIRR, casting doubt on its beneficial effects. In PBMT-assisted orthodontics, delivering sufficient energy to the tooth root to achieve optimal stimulation is challenging due to the exponential attenuation of light penetration in periodontal tissues. The penetration of light to the root surface is another crucial unknown factor. Both the penetration depth and distribution of light in periodontal tissues are unknown. Thus, advanced approaches specific to orthodontic application of PBMT need to be established to overcome these limitations. This review explores possibilities for improving the application and effectiveness of PBMT during OTM. The aim was to investigate the current evidence related to the underlying mechanisms of action of PBMT on various periodontal tissues and cells, with a special focus on immunomodulatory effects during OTM.

Dental crowding, which is a frequent malocclusion, and dental aesthetic requirements are a common cause for requesting orthodontic treatment. The time that the treatment requires and the inconvenience that it could cause worries the patient and makes orthodontists strive to optimize time and prevent adverse effects. Multidisciplinary treatments would allow a better aesthetic, functional and post-treatment stability response. The dental alignment and completion time in patients treated with low-intensity laser could improve both gingival indices and response to pain. Additionally, the bone cortical of orthodontic patients treated with laser could be less affected compared to those of untreated patients. A case of low-intensity laser photobiostimulation applied to a patient in the alignment phase is presented, which is part of a prospective randomized study carried out at FOUBA and was approved by the institution's Ethics Committee. The patient accepted and signed the informed consent. After the alignment phase, the effectiveness of low-level laser therapy actually called photobiomodulation in upper incisors in the alignment phase is evaluated to accelerate tooth movement; the gingival response; the pain; the vestibular alveolar cortical and the aesthetics of the profile (AU)

AIMS: To perform a literature review on the treatment of postherpetic neuralgia with low-intensity laser irradiation. METHODS: The review was conducted in November and December 2015 by searching the terms "herpes zoster", "neuralgia", "low-level laser therapy", and "low-intensity laser therapy" in the LILACS, SciELO, and Medline databases. Full articles published in Portuguese, English, German, or Spanish between 2008 and 2015 were included. Incomplete articles, those that did not directly address the topic of interest, and those appearing in the same or in different databases were excluded. In the latter case, only one article was included. RESULTS: Thirty-two studies were selected using the search terms. After the application of the eligibility criteria, 15 studies were read; two of them were excluded for being available in more than one database and one was eliminated for not adding relevant information. Therefore, 11 studies were included in the review: five of them conducted in the United States, two in Brazil, two in Germany, and two in Italy. There were four clinical trials, two systematic reviews and five case reports. The 11 studies found positive results after laser application, especially reduction of pain, with only one article mentioning little reduction of neuralgia. In the three studies that reported the irradiation parameters, these have not been uniform, with a medium wavelength of 740.6 nm, a medium power of 28 mW and a medium time of irradiation per point of 53.4 seconds. There was great variation in the number of sessions, with an average of about 12 sessions on alternate days. CONSLUSIONS: The low-intensity laser therapy provided positive results to patients with postherpetic neuralgia, however there is no consensus about the parameters to be used. In view of the different protocols, it is important to perform new studies in order to standardize the lowintensity laser therapy parameters applied to postherpetic neuralgia.

Lung emphysema is a pulmonary inflammatory process, with extrapulmonary repercussions. Twenty-four rats were selected and divided into three groups with eight animals each: control (GC: received only saline solution); group DPOC (GD: with instillation of papaya proteinase 2 mg/kg); group DPOC + laser (GDL: with instillation of papaya proteinase + 15 days of laser therapy). Data analysis comprised: CKMB, LBA, Histology (morphometric analysis) and thickness of the cardiac chambers (morphometric analysis), respectively. The number of inflammatory cells in LBA of GDL decreased (p < 0.05) when compared with GC and GD; there was an increase in CKMB levels in group GD (p < 0.05); significant decrease in GDL (p = 0.0009) when compared to GD, left ventricle hypertrophy (p < 0.05) in group GD and GDL when compared to control. Photobiomodulation decreased the release of inflammatory cells by the release of CKMB in the blood and decreased CKMB levels and reduced myocardial infarction risks.

One of the greatest challenges when rehabilitating fully edentulous patients, is to manufacture a denture to be placed on an alveolar ridge with advanced bone resorption. This requires performing pre-prosthetic surgery and prepare groundwork bone and surrounding soft tissues to receive a tissue-borne prosthesis, as well as providing suitable retention and support. This will elicit lesser trauma and thus allow prompt tissue healing. The case here presented is that of a 56 year old female, with non-contributory medical history. The patient had been wearing dentures for the last 14 years; her dentures were ill-adapted. Intra-oral clinical examination revealed collapsed and re-absorbed ridges. Due to a lack of retention in the lower ridges, a laser surgery was performed to deepen the vestibule. 18 days after surgery tissues were completely healed, the vestibule had recovered 6 mm depth in the anterior section and 4 mm in the posterior section. Rehabilitation was completed with the manufacture of full bi-maxillary dentures. It was thus concluded that laser vestibuloplasty confers depth to the vestibule and offers the advantages of laser incision, which decreases hemorrhage, provides better operating field and reduces post-operative inflammation and pain.

RESUMEN

INTRODUCTION: Low-intensity laser (LILT) and LED therapy are indicated in repairing surgical incisions, which occur in cardiac surgery. One major concern in this case is the presence of metallic wire used for sternal sutures, its temperature may rise while using a laser or LED. This study aimed to analyze the tensiometric properties of the skin tissue with longitudinal incision and metallic wire implant used for sternal suture irradiated with LILT and LED. METHODS: Twenty-five subjects (Rattus norvegicus) were submitted to thoracic incision with implantation of metallic wire, and subsequently divided into five groups: GI (control group); GII (laser group 6J/cm²); GIII (laser group 10J/cm²); GIV (LED group 6 J/cm²); and GV (LED 10J/cm²). Photobiomodulation was performed on a daily basis for eight consecutive days and the local temperature was measured before and after phototherapy. After euthanasia the tissues were subjected to tensiometric and histological analysis. RESULTS: All irradiated groups showed increase in break strength (p = 0.009), and decrease in tissue deformation strength compared with the control group (p = 0.03). CONCLUSION: Both LILT and LED can promote wound healing at the tested doses with increased tissue resistance, although its elasticity is reduced. The imprecision of the assessing instruments precluded an accurate measurement of a possible local tissue heating post irradiation and future studies are need to elucidate this effect.

BACKGROUND AND OBJECTIVES: Pain is a typical symptom during early orthodontic treatment. This study aimed at reviewing the literature on the use of low intensity laser to relieve pain during orthodontic treatment. CONTENTS: Titles, summaries and articles were searched in the following databases: Pubmed/Medline, Cochrane Library, LILACS and Scielo. Three researchers have independently searched using defined inclusion and exclusion criteria. Eight clinical trials were included and six have observed significant pain relief after therapeutic laser. CONCLUSION: There are scientific evidences that low intensity laser decreases pain symptoms during dental movements after the placement of orthodontic elastics and after orthodontic adjustments. Its use by dentists is a feasible alternative for inducing less adverse effects as compared to anti-inflammatory analgesics, being indicated for allergic patients, children and patients with systemic impairment. However, there is the need for further scientific investigations using well-defined protocols.

OBJECTIVE: To evaluate the efficiency of infrared diode laser on pain reduction during canine initial retraction. METHODS: Twelve patients in need of canine retraction were selected. The canines were retracted with closed NiTi coil springs activated to 150g per side. One canine of each patient was randomly selected for laser irradiation immediately after activation and after 3 and 7 days. The contralateral canines were taken as control group and were submitted only to simulation of laser application. Diode laser (ArGaAl) was employed at wavelength of 780nm, power of 20mW and energy density in the target tissue of 5J/cm², for 10 seconds per point, delivering an energy of 0.2J per point and total energy (TE) of 2J. The analgesic effect was evaluated with aid of a visual analogue scale (VAS), on which the patients indicated 0 to 10 according to the pain experienced at 12, 24, 48 and 72 hours after coil spring activation and laser application. The procedure was repeated after one month, upon reactivation of canine retraction. RESULTS AND CONCLUSIONS: There was no statistically significant difference between irradiated side (LG) and control side (CG). Thus, utilization of infrared diode laser (780nm) according to the present protocol was not statistically effective to reduce pain sensitivity caused by orthodontic movement.

RESUMEN

Objective To quantify the effect of 650 nm semiconductor laser irradiation on cutaneous wound healing in rabbits. Methods Model wounds were induced in 21 adult male rabbits. They were then divided randomly into a control group, a 5 mW irradiation group and a 10 mW irradiation group. Biometry and light microscopy were used to investigate the effect of low intensity laser therapy (LILT) on cutaneous wound healing. Results Statistically significant differences were observed among the groups in terms of wound shrinkage. The differences between the irradiated and control groups were significant at the 5th, 9th and 13th day. However, any differences between the S mW and 10 mW groups were not significant. Conclusions LILT at 650 nm at either 5 mW or 10 mW and 1 J/cm2 accelerates cutaneous wound healing in rabbits. But irradiation at 10 mW is no more effective than at 5 mW.

OBJECTIVE: The aim of this study was to compare the effect of the low level laser therapy in the organization of the collagen fibers with 3 J/cm² of dose in two wavelengths (670nm and 904nm) and in the combination of both on the tissue repair of the calcaneus tendon. METHODS: We used 37 female Wistar rats divided in 5 groups which had been submitted to the total tenotomy. Group GA (n= 8) was submitted to the laser irradiation in lambda= 904nm, GaAS diode, pulsated emission, peak power 15W and dose of 3J/cm², group GB (n= 8) was submitted to laser irradiation lambda= 670nm AlGaInP diode, continuous emission, power 30 mW and dose 3J/cm², group GAB (n= 8) was submitted to association of both of lasers and group GCL (n= 7) which the calcaneum tendon was submitted to placebo treatment. The group GP (n= 6) was a standard group without lesion and treatment. The animals received 12 sessions of lasertherapy and submitted to euthanasia on 14th day after surgery. The tendons were submitted to histological procedure and were analyzed to evaluate the optic retardation (in nm) for the birefringence measurement. RESULTS: The statistic analysis showed that the irradiated groups have higher values of optic retardation statistically significant than placebo group (p= 0,00001). It indicates that all the treated groups show better organization of collagen fibers in the longitudinal axis of the tendon, evidencing better quality in the tissue repair. The group GAB was that shown best results (p= 0,00001) when compared to another groups.

RESUMEN

Objective To observe the effect on the patients of diabetes mellitus and cerebral infarction in terms of level of blood rheology and endothelin after using both intravenous low intensity laser irradiation (ILILI) and traditional Chinese medicine Tangnaoluotong in the treatment. Method Ninety patients of diabetes mellitus and cerebral infarction, having the symptom of deficiency in both vital energy and yin with blood stasis, were divided into three treatment groups at random:ILILI group (A), TCM treatment group (B), and a group with compound treatment of ILILI and TCM (C). The level of blood rheology and endothelin were observed after 1 month’s treatment. Result Compared with group A and group B, group C could significantly reduce the level of fibrinogen and endothelin, improve the deformed ability of RBC. Conclusion ILILI with Tangnaoluotong can significantly alter the blood rheology and endothelin on the patients of diabetes mellitus and cerebral infarction, which result in a better curative effect.

RESUMEN

Objective To observe the effects of low-intensity(650 nm,5 mW)laser irradiation on the pathological changes associated with aortic atherosclerosis using a rabbit model. Methods Thirty-six male Zelanian rabbits were randomly divided into 4 equal groups:a control group,a laser irradiation group,a simvastatin treatment group and a laser plus simvastatin group, and were treated accordingly. After being fed the basic diet for a week, all the animals were fed a high fat diet during the experiment.Blood samples were taken for lipid assay at the 60th day. The animals were sacrificed at the 61st day and their aortas removed for gross and microscopic examination.Any pathological changes were graded as mild, intermediate or severe according to the indicators of atherosclerosis observed. Results There were no significant differences in blood fat levels among the four groups before the experi- ment, but significant differences in serum triglyceride prevailed after the treatments.Low-density lipoprotein(LDL)- C level in the control group was significantly higher than those in the laser irradiation, simvastatin treatment and laser plus simvastatin groups. There was also a very significant difference in high-density lipoprotein(HDL)-C levels among the laser irradiation group, the laser plus simvastatin group and the control group. The pathological changes observed were correlated with blood fat levels.Mild atherosclerosis was found in the treated groups, but severe or in- termediate atherosclerosis was more prevalent in the control group. Conclusion Low-intensity laser irradiation a- lone or combined with simvastatin can significantly decrease blood lipid levels and the severity of pathological changes associated with aortic atherosclerosis in this animal model.
