RESUMEN

Paciente femenina que presentó durante sus dos embarazos eritema nodoso, úlceras nasales y artritis. Debido a la persistencia del eritema nodoso después del segundo parto, se practicó determinación de ANA y anti-DNA ­ IF, resultando este último positivo, por lo que se plantea el diagnóstico de lupus eritematoso y es referida al Centro Nacional de Enfermedades Reumáticas, (CNER) donde se le practicó biopsia de piel que resultó compatible con lepra: borderline o dimorfa (BB-BL) en estado reaccional tipo II (eritema nodoso leproso: ENL). Se discuten algunos de aspectos de la lepra haciendo especial énfasis en síntomas que presentó la paciente y la exacerbación de los mismos durante los embarazos, posiblemente en relación con algunos cambios inmunológicos adaptativos que son generados para mantener la tolerancia materno fetal(AU)


Female young patient with erythema nodosum, nasal ulcers and arthritis during her two pregnancies. Due to the persistence of the erythema nodosum after the second delivery, ANA and anti-DNA - IF determination were done, the latter being positive, and she was diagnosed with lupus erythematosus and was referred to the National Center for Rheumatic Diseases, where skin biopsy showed to be compatible with leprosy: borderline or dimorphic (BB-BL) in type II reactional state (leprous erythema nodosum: LEN). Some aspects of leprosy are discussed with special emphasis on symptoms that the patient presented and their exacerbation during pregnancies, possibly in relation to some adaptive immunological changes that was generated to maintain maternal fetal tolerance(AU)

RESUMEN

Dasatinib, a second generation multi-target tyrosine kinase inhibitor (TKI) is active against many imatinib-resistant BCR-ABL mutant forms, Src, and c-Kit tyrosine kinases. While skin hypopigmentation is a well recognized adverse effect of first generation TKIs; it has rarely been reported with dasatinib. We report a rare case of diffuse cutaneous hypopigmentation and bilateral supercilliary madarosis induced by dasatinib. A 51 year-old Indian male with no co-morbidities and with history of chronic myelogenous leukaemia with complex variant of Philadelphia translocation and E 225 V mutation in P loop domain of bcr-abl transcript who was initiated on imatinib followed by dasatinib as a part of treatment. After 5 months of treatment with dasatinib, he developed supercilliary madarosis bilaterally. Cutaneous side effects may adversely affect patient’s quality of life and, therefore, require prompt attention to prevent long-term complications or suboptimal outcomes due to poor compliance.

RESUMEN

BACKGROUND: Madarosis or loss of eyebrows is one of the complications in leprosy, which takes expressions out of the face and is considered as a stigma of leprosy in society. So the patients with leprosy are usually eager to have the situation corrected through various methods such as tatoo or surgery including scalp flap or hair transplantation. OBJECTIVE: To describe the use of single hair transplantation for the correction of madarosis in patients with leprosy. METHODS: An elliptical donor strip was removed with a No.10 blade from the occiput. It was sectioned into small slices and subsequently into single hair micrografts. Then these micrografts were inserted into preformed slits of eyebrows using hair implantation device. RESULTS: Excellent cosmetic results and high patient satisfaction were seen in almost all patients. Reoperations were done in 3 patients due to partial sparsity or unnatural shape of the eyebrows. There were no complications except for pruritic papules which developed from the transplanted areas in 2 patients with eyebrow tatoos. CONCLUSIONS: Single hair transplantation proved to be useful in the reconstruction of eyebrows in patients with leprosy and provided superior cosmesis to other grafts despite a technically difficult and time-consuming procedure.
