RESUMEN Introducción: lepra (enfermedad de Hansen) es una patología infecciosa desatendida que afecta piel y nervios periféricos, desencadenando neuropatías y complicaciones como deformidades y discapacidades. Objetivos: determinar características epidemiológicas y clínicas de pacientes con lepra en el Centro de Especialidades Dermatológicas del Ministerio de Salud Pública y Bienestar Social de Paraguay, de enero 2021 a diciembre 2022. Metodología: estudio descriptivo, observacional, retrospectivo, de corte transversal. La gestión y análisis de datos se realizó en planilla electrónica (Excel del programa Microsoft Office 365 versión 2405), luego se realizó estadística descriptiva. Resultados: se diagnosticaron 123 casos de lepra, 45 (36,58 %) eran mujeres y 78 (63,42 %) varones; las edades estaban entre 17 y 86 años, el rango más afectado fue de 41 a 50 años. Eran lepra multibacilar 112 casos (91,06%). El signo más frecuente fue la placa infiltrada, presentada en 88 casos (71,54%). 58 pacientes (47,15%) presentaron reacciones lepromatosas, siendo, 31 casos de tipo 1 (53,45 %). 34 casos (27,64%) presentaron discapacidad grado 1 y 10 casos (8,13%) discapacidad grado 2. Conclusión: el perfil del paciente con lepra fue del sexo masculino entre 41 a 50 años, siendo la mayoría casos multibacilares. Es fundamentales el diagnóstico precoz y el tratamiento adecuado para reducir la carga de la enfermedad y las discapacidades.
ABSTRACT Introduction: Leprosy (Hansen's disease) is a neglected infectious disease that affects skin and peripheral nerves, triggering neuropathies and complications such as deformities and disabilities. Objective: determine epidemiological and clinical characteristics of patients with Leprosy in the Dermatological Specialties Center of the Ministry of Public Health and Social Welfare of Paraguay, from January 2021 to December 2022. Methodology: descriptive, observational, retrospective, cross-sectional study. Data management and analysis was carried out in an electronic spreadsheet (Excel from Microsoft Office 365 version 2405), then descriptive statistics were performed. Results: 123 cases of leprosy were diagnosed, 45 (36.58 %) were women and 78 (63.42 %) were men; the ages were between 17 and 86 years, the most affected range was 41 to 50 years. 112 cases (91.06 %) were multibacillary leprosy. The most frequent sign was infiltrated plaque, present in 88 cases (71.54 %). 58 patients (47.15 %) presented lepromatous reactions, 31 cases of type 1 (53.45 %). 34 cases (27.64 %) presented grade 1 disability and 10 cases (8.13 %) grade 2 disability. Conclusion: The profile of the patient with leprosy was male between 41 and 50 years, the majority being multibacillary cases. Early diagnosis and adequate treatment are essential to reduce the burden of the disease and disabilities.

ABSTRACT Objectives: to identify and synthesize, from the literature, the impacts of physical disability caused by leprosy on the quality of life of individuals receiving care within the Health Care Network. Methods: this is a scoping review conducted following the JBI recommendations. The databases used in the search included the Medical Literature Analysis and Retrieval System Online, Cochrane Library, Web of Science, Lilacs, Cumulative Index to Nursing and Allied Health Literature, Scopus, Embase, Leprosy Information Services, and Google Scholar. Results: 1690 documents were identified, of which 36 were included in the review. Physical disability caused by leprosy affects the quality of life in the areas of daily activities, socioeconomic aspects, psychological well-being, pain, and overall well-being. Conclusions: we identified the impairments caused by physical disability in the quality of life, highlighting the need for prevention, promotion, and rehabilitation actions, such as screening, case management, and health education.

Métodos: se trata de una revisión de alcance, realizada de acuerdo con las recomendaciones del JBI. Las bases de datos utilizadas en la búsqueda fueron el Sistema de Análisis y Recuperación de Literatura Médica en Línea, la Biblioteca Cochrane, Web of Science, Lilacs, el Índice Cumulativo de Literatura en Enfermería y Ciencias Afines, Scopus, Embase, Leprosy Information Services y Google Scholar. Resultados: se identificaron 1690 documentos, de los cuales 36 fueron parte de la revisión. La discapacidad física causada por la lepra afecta la calidad de vida en términos de actividades de la vida diaria, socioeconómicas, psicológicas, dolor y bienestar general. Conclusiones: se identificaron los compromisos causados por la discapacidad física en la calidad de vida, indicando la necesidad de acciones de prevención, promoción y rehabilitación, como el seguimiento, el acompañamiento de los casos y la educación en salud.

BACKGROUND The impact of nutrient availability on the survival of Mycobacterium leprae and the development of leprosy remains largely unknown. Iron is essential for the survival and replication of pathogens, while vitamin D has been involved with pathogen elimination and immunoregulation. OBJECTIVES We evaluated the influence of dietary iron and vitamin D supplementation and restriction on the inflammatory response of mouse immune cells in vitro. METHODS After 30 days of standard or modified diets, peritoneal cells and splenocytes were stimulated with the alive microorganisms and sonicated antigens of M. leprae, respectively. The production of inflammatory cytokines, reactive oxygen species, and cell proliferation were evaluated. FINDINGS In peritoneal cells, vitamin D supplementation and iron restriction reduced the production of IL-6 and TNF in response to M. leprae, while splenocytes presented a reduction in TNF production under the same conditions. Lower levels of IFN-γ and TNF were observed in both iron-supplemented and iron-deficient splenocytes. Besides, iron supplementation also reduced the production of IL-6 and IL-10. No changes in the production of reactive oxygen species or in cell proliferation were observed related to different diets. MAIN CONCLUSIONS Taken together, these data point to an interference of the status of these nutrients on the interaction between the host and M. leprae, with the potential to interfere with the progression of leprosy. Our results highlight the impact of nutritional aspects on this neglected disease, which is significantly associated with unfavourable social conditions.

Objetivo: Avaliar a quantidade anual de baciloscopias para hanseníase realizadas no Sistema Único de Saúde (SUS) nos últimos dez anos. Métodos: Foi realizado um estudo ecológico, longitudinal, retrospectivo e quantitativo, utilizando dados do Sistema de InformaçõesAmbulatoriais (SIA/SUS). A quantidade de baciloscopias para hanseníase a cada 100.000 residentes foi estimada para o Brasil e suas cinco macrorregiões, avaliada entre 2013 e 2022 com nível de significância (a) de 5%. Resultados:Mais de 1,3 milhões de baciloscopias foram notificadas nos últimos dez anos no SUS. A mediana da incidência anual foi de 67 baciloscopias para hanseníase por 100.000 residentes, sendo a máxima observada em 2013 (82) e a mínima em 2022 (46). As incidências anuais nas macrorregiões Norte, Centro-oeste e Nordeste foram significativamente superiores à estimativa nacional, enquanto no Sul e Sudeste foram inferiores (p<0,05). A tendência temporal foi considerada decrescente para a estimativa nacional (p= 0,002), com variação percentualanual de -5,6% (IC95% = -3,8%; -8,2%). Entretanto, após desconsiderar os anos da pandemia de COVID-19 (2020-2022), a tendência tornou-se estacionária (p= 0,181). Além disso, a incidência durante o período pré-pandêmico foi significativamente maior quandocomparada ao terceiro ano após o advento da pandemia em todas as macrorregiões do Brasil (p<0,05). Conclusão:Foi possível concluir que o SUS realizou um número expressivo de baciloscopias para hanseníase nos últimos dez anos, mas existem disparidades macrorregionais no Brasil, bem como um impacto significativo da pandemia de COVID-19
Objective:To evaluate the annual number of skin smear microscopies for leprosy performed in the Unified Health System (SUS) in the last decade. Methods:An ecological, longitudinal, retrospective, and quantitative study was conducted using data from the Ambulatory Information System (SIA/SUS). The number of skin smear microscopies for leprosy per 100,000 residents was estimated for Brazil and its five macroregions, between 2013 and 2022, with a significance level (a) of 5%.Results:More than 1.3 million skin smear microscopies were reported in the last decade in the SUS. The median annual incidence was 67 skin smear microscopies for leprosy per 100,000 residents, with the maximum observed in 2013 (82) and the minimum in 2022 (46). Annual incidences in the North, Central-West, and Northeast macro-regions were significantly higher than the national estimate, whereas in the South and Southeast, they were lower (p <0.05). The temporal trend was considered decreasing for the national estimate (p = 0.002), with an annual percentage variation of -5.6% (95%CI = -3.8%; -8.2%). However, after disregarding the years of the COVID-19 pandemic (2020-2022), the trend became stationary (p = 0.181). Furthermore, the incidence during the pre-pandemic period was significantly higher compared with the third year after the advent of the pandemic in all macro-regions of Brazil (p<0.05).Conclusion:It was possible to conclude that the SUS performed a significant number of skin smear microscopies for leprosy in the last ten years, but there are macro-regional disparities in Brazil and a significant impact of the COVID-19 pandemic

Introducción: La lepra o enfermedad de Hansen es una enfermedad infectocontagiosa, crónica, causada por el bacilo Mycobacterium leprae. Es causante de neuropatía periférica, y afecta a la piel, nervios periféricos, vías aéreas superiores y ojos. El objetivo fue evaluar las características clínicas y sociodemográficas a los pacientes con Lepra que acudieron Hospital Menonita Km 81 de Itacurubi de la Cordillera en los años 2018-2022. Material y Método: Estudio observacional descriptivo. retrospectivo, de corte transversal en pacientes con Lepra del Hospital Menonita Km 81 en la ciudad de Itacurubi de la Cordillera del Departamento de Cordillera del año 2018 al 2022. Los datos se obtuvieron de la revisión de las fichas epidemiológicas de pacientes con diagnóstico de lepra en el período abarcado en un periodo de 5 años, 2018 al 2022. Resultados: Fueron reportados 286 casos de lepra en pacientes provenientes de los 18 departamentos del Paraguay. El año con más casos detectados fue el 2019 con 79 casos, siendo el departamento de Alto Paraná con mayor incidencia con 17 casos diagnosticados, seguido del año 2018 con 76 casos. La ocupación más frecuente de los pacientes fue la agricultura. El sexo más frecuente fue el masculino 203 (71%) casos con lepra. El rango de edad de mayor frecuencia fue entre 49- 63 años de edad (29,56%). El motivo de consulta más frecuente fue el de Mácula en 139 (49%). Conclusión: Se requiere aumentar la calidad de la atención primaria y el compromiso del personal de salud para el control y el diagnóstico oportuno de la Lepra en nuestro país.

It causes peripheral neuropathy and affects the skin, peripheral nerves, upper airways, and eyes. The objective was to evaluate the clinical and sociodemographic characteristics to patients with Leprosy who attended Mennonite Hospital Km 81 of Itacurubí de la Cordillera in the years 2018-2022 Material and Method: descriptive observational study, retrospective of cross-sectional in patients with Leprosy of the Mennonite Hospital Km 81 in the city of Itacurubi de la Cordillera of the Department of Cordillera from the year 2018 to 2022. Data were obtained from the review of epidemiological records of patients with a diagnosis of leprosy in the period covered in a period of 5 years, 2018-2022. Results: Of the 18 departments in Paraguay, 286 cases of leprosy were reported. The year with the most cases detected was 2019, with 79 cases, with the department of Alto Paraná having the highest incidence, with 17 cases diagnosed, followed by 2018 with 76 cases. Agriculture was the most common occupation. The most frequent sex was male (203 [71%] patients) with leprosy. The most frequent age range was 49-63 years (29.56%), and the most frequent reason for consultation was macular in 139 patients (49%). Conclusion: It is necessary to increase the quality of primary care and commitment of health personnel in the control and timely diagnosis of leprosy in our country.

Hansen's disease is highly infectious and contagious. This disease in some individuals manifests with subtle symptoms such as isolated lesions like nodules over the ear, polyarthritis etc. These manifestations should be considered for the differential diagnosis of leprosy. We present this case with multiple cutaneous nodules over the helix and lobule of the right ear. A histopathological examination confirmed the diagnosis. Early detection and treatment of this disease prevent further complications.

Objetivo: Avaliar a qualidade de vida de pacientes durante e após o tratamento de hanseníase. Método: Estudo descritivo, exploratório e quantitativo. Realizado através da escala de qualidade de vida WHOQOL-bref. CAAE: 58389422.0.0000.5587. Resultados: Participaram 33 (100%) pacientes que tiveram diagnóstico de Hanseníase, a amostra foi tabulada separando entre dois grupos, 13 (100%) em tratamento ativo e 20 (100%) com tratamento finalizado. Para os pacientes ainda em tratamento: no domínio físico 31% referem boa qualidade de vida e 23% referem ruim, já no meio ambiente, psicológico e relações sociais 08% referem boa qualidade de vida e 23% ruim, respectivamente. Para os participantes com tratamento finalizado, o domínio físico, meio ambiente, psicológico e relações sociais obtiveram 40% com boa qualidade de vida e 05% ruim. Conclusão: os pacientes que finalizaram tratamento apresentam melhor qualidade de vida do que os pacientes em tratamento de hanseníase, demonstrando a importância de realizar o tratamento corretamente.(AU)
Objective: To evaluate the quality of life of patients during and after leprosy treatment. Method: Descriptive, exploratory and quantitative study. It was carried out using the WHOQOL-bref quality of life scale. CAAE: 58389422.0.0000.5587. Results: 33 (100%) patients diagnosed with leprosy took part in the study. The sample was divided into two groups: 13 (100%) in active treatment and 20 (100%) with treatment completed. For patients still undergoing treatment: in the physical domain 31% reported a good quality of life and 23% a poor one, while in the environmental, psychological and social relations domains 08% reported a good quality of life and 23% a poor one, respectively. For participants who had completed treatment, 40% reported good quality of life in the physical, environmental, psychological and social relations domains and 05% reported poor quality of life. Conclusion: Patients who have completed treatment have a better quality of life than patients undergoing leprosy treatment, demonstrating the importance of carrying out treatment correctly.(AU)

La muestra se tabuló en dos grupos: 13 (100%) en tratamiento activo y 20 (100%) con tratamiento finalizado. En el caso de los participantes que habían completado el tratamiento, el 40% informó de una buena calidad de vida en los dominios físico, ambiental, psicológico y de relaciones sociales y el 05% informó de una mala calidad de vida. Conclusión: Los pacientes que han completado el tratamiento tienen una mejor calidad de vida que los pacientes en tratamiento contra la lepra, lo que demuestra la importancia de llevar a cabo el tratamiento correctamente.(AU)

Se describe el caso clínico de un paciente infectado por el virus de la inmunodeficiencia humana en fase sintomática precoz, quien fue asistido en el Servicio de Dermatología del Hospital Clínico Quirúrgico Docente Dr. León Cuervo Rubio en Pinar del Río por presentar lesiones eritematosas infiltradas de tres meses de evolución en el miembro inferior derecho, con alteración de la sensibilidad térmica y dolorosa. Se le indicó baciloscopia y estudio histopatológico que confirmaron el diagnóstico de sospecha de lepra dimorfa tuberculoide, por lo que se comenzó a aplicar poliquimioterapia combinada con terapia antirretroviral, lo cual favoreció la evolución del paciente.
The case report of a patient infected by the human immunodeficiency virus in early symptomatic phase is described, who was seen at the Dermatology Service of the Dr. León Cuervo Rubio Teaching Clinical Surgical Hospital of Pinar del Río for presenting infiltrated erythematous lesions of three months of evolution in the right lower limb, with altered pain and thermal sensation. A basiloscopic and histopathologic study was indicated, which confirmed the presumptive diagnosis of borderline tuberculoid leprosy, and therefore polychemotherapy combined with antiretroviral therapy was started, which favored the patient's evolution.

La lepra o enfermedad de Hansen es una infección crónica causada por Mycobacterium leprae (M. leprae) o Mycobacterium lepromatosis (M. lepromatosis), es una enfermedad infecciosa curable que es endémica en más de 140 países de todo el mundo. La Organización Mundial de la Salud lo declaró eliminado como un problema mundial de salud pública en el año 2000, sin embargo, en 2017 se informaron aproximadamente 200 000 casos nuevos en todo el mundo. A pesar de la terapia farmacológica combinada disponible, continúa como un importante problema de salud pública, que conlleva un fuerte estigma. En Cuba, según el Anuario Estadístico de Salud de 2020, se reportaron durante ese año 116 casos, las provincias más afectadas fueron Santiago de Cuba con 25 nuevos casos y Guantánamo con 17 nuevos casos. La presentación de caso se trata de una joven de 20 años, en edad reproductiva, con manifestaciones generales de astenia, pérdida del cabello, adenopatías, poliartritis, lesiones cutáneas y fiebre, que hizo sospechar en una enfermedad del colágeno. Leprosy or Hansen's disease is a chronic infection caused by mycobacterium leprae (M. leprae) or mycobacterium lepromatosis (M. lepromatosis). It is a curable infectious disease, endemic in more than 140 countries around the world. Despite being declared eradicated as a global public health problem by the World Health Organization in 2000, approximately 200,000 new cases were reported worldwide in 2017. Despite available combination drug therapy, it remains a major public health problem, carrying strong stigma. In Cuba, according to 2020 health statistical yearbook, 116 cases were reported during that year. The most affected provinces were Santiago de Cuba (25 new cases) and Guantánamo (17 new cases). The case to be reported concerns a 20-year-old young woman of reproductive age, with general manifestations of asthenia, hair loss, adenopathies, polyarthritis, skin lesions, and fever; she suspected was a collagen disease. Objective: to evaluate leprosy bacilloscopy exam notifications in the Brazilian Unified Health System (SUS) from April 2018 to March 2022. Methods: an ecological study was carried out using open-access data from the SUS. Results: the temporal tendency of leprosy bacilloscopy was considered significantly decrease over time (p <0.05), as well as there was a significant decrease in the first and second pandemic years when compared to the control interval in all Brazilian regions (p <0.05). Conclusion: leprosy bacilloscopy exams remain negatively affected by the COVID-19 pandemic in the SUS.
Objetivo: avaliar o número de notificações de baciloscopia para hanseníase no Sistema Único de Saúde brasileiro (SUS) de abril de 2018 até março de 2022. Método: foi realizado um estudo ecológico com dados de acesso aberto do SUS. Resultados: a tendência temporal da baciloscopia da hanseníase foi considerada significativamente decrescente ao longo do tempo (p <0.05), bem como houve uma redução significativa no primeiro e segundo ano de pandemia quando comparado ao intervalo-controle em todas as regiões brasileiras (p <0.05). Conclusão: os exames de baciloscopia da hanseníase permanecem afetados negativamente pela pandemia de COVID-19 no SUS.

ABSTRACT Background: Brazil has the second largest number of leprosy cases worldwide, and the state of São Paulo has been considered non-endemic since 2006. Methods: We analyzed 16 variable number tandem repeats loci and three single nucleotide polymorphisms loci of Mycobacterium leprae (M. leprae) in 125 clinical isolates from patients in different municipalities in the state. Results: The clustering pattern of M. leprae indicated that the transmission of leprosy persisted in the state and included scenarios of intra-extra-familial transmission in areas with low endemicity. Conclusions: A significantly active circulation of M. leprae was observed. Therefore, surveillance and control measures must be implemented.

SUMMARY OBJECTIVE: Leprosy is a disabling infectious disease caused by Mycobacterium leprae. This study aimed to investigate the prevalence of leprosy among household contacts of leprosy patients. METHODS: This study is a serological survey in household contacts of leprosy patients who had been treated or were undergoing treatment in the city of Presidente Prudente, São Paulo, Brazil, from 2006-2016, using clinical examination and screening for anti- Phenolic glycolipid-I antibodies with Mycobacterium leprae-flow serology. RESULTS: A total of 263 index cases of leprosy were identified during the study period. Of these, 53 were approached, and among their household contacts, 108 were examined. The ML-flow test was positive in 2 (1.85%) individuals, but clinical examination revealed no signs or symptoms of leprosy in them. Therefore, they were considered to have a subclinical infection. Leprosy was not confirmed in any household contacts. In this study, a lower percentage of household contacts, when compared to that in the literature, had a positive Mycobacterium leprae-flow test result. CONCLUSION: The use of Mycobacterium leprae-flow should be encouraged during the follow-up of at-risk populations, such as the household contacts of leprosy patients.

Leprosy is a chronic disease with clinical presentations according to the immunologic spectrum. Lepromatous form is the most advanced, with the highest transmissibility and risk of causing disabilities. Lucio’s phenomenon is a rare manifestation among lepromatous patients with a rapid and severe evolution and high mortality. It is difficult to differentiate from ulcerative/necrotic erythema nodosum leprosum and has no consensus on how it should be treated. This article is a qualitative review of the literature after the introduction of multidrug therapy, aiming to bring consensus related to the clinical, laboratory and histopathological diagnostic criteria of the disease and its management

Abstract Objective: To analyze the prevalence of Mycobacterium leprae detection and the associated factors among social contacts in the school environment of multibacillary cases living in a hyperendemic municipality of the state of Mato Grosso. Methods: Cross-sectional study with 236 social contacts of multibacillary leprosy from public schools and residents in Cuiabá (Mato Grosso) in 2018. The sources of information were interviews and nasal swab tests for molecular analysis by polymerase chain reaction - PCR. For the prevalence ratio estimates, crude and adjusted analyses were performed using robust Poisson regression and their respective confidence intervals (95% CI). The ArcGIS 9.1 software was used for the geographic distribution analyses. Results: The prevalence of detection of M. leprae in social contacts was 14%. A total of 63.6% of the schools surveyed had 5.1% to 50% of the social contacts of leprosy with positive PCR. The analysis of the geographic distribution in the neighborhoods showed a high prevalence of infection, being higher than 50% in some localities. The highest proportion of positive results occurred in the northern region of the city and from a precarious socioeconomic class. Abstract This study's objective was to analyze the spatial distribution pattern of new leprosy cases under 15 years and their contacts. A cross-sectional descriptive study covering sociodemographic characteristics and spatial analysis was carried out. The participants were from the city of Sobral, Ceará and the study was conducted between August 2014 and September 2015. Contacts were identified by the persons responsible for the children. Seropositivity was determined with the NDO-LID antigen, and positive cases were plotted on Voronoi polygons. Nine new cases of leprosy under 15 years of age have been found. The average number of people living with the cases was higher than the number of people living with non-household contacts. All household contacts were aware of other leprosy cases and had a higher rate of seropositive tests than non-household contacts. The index cases lived in the poorest regions of the municipality and hyper-endemic areas. Spatial analysis revealed a cluster of subclinical infection within a radius of 102 meters, suggesting that non-household transmission is related to proximity with seropositive individuals. In conclusion, the search for new leprosy cases cannot be restricted to household contacts.
Resumo O objetivo deste estudo foi analisar o padrão de distribuição espacial de novos casos de hanseníase em menores de 15 anos e seus contatos. Estudo transversal, descritivo, abrangendo características sociodemográficas e análise espacial. Os participantes eram de Sobral, Ceará e o estudo foi realizado entre agosto de 2014 e setembro de 2015. Os contatos foram identificados pelos responsáveis pelas crianças. A soropositividade foi determinada com o antígeno NDO-LID e os casos positivos foram plotados em polígonos de Voronoi. Nove novos casos de hanseníase em menores de 15 anos foram encontrados. O número médio de pessoas que conviviam com os casos foi superior ao número de pessoas que conviviam com contatos não domiciliares. Todos os contatos domiciliares sabiam de outros casos de hanseníase e apresentaram maior taxa de testes soropositivos do que os contatos não domiciliares. Os casos índice residiam nas regiões mais pobres do município e em áreas hiperendêmicas. A análise espacial revelou um agrupamento de infecção subclínica em um raio de 102 metros, sugerindo que a transmissão não domiciliar está relacionada à proximidade com indivíduos soropositivos. Concluindo, a busca por novos casos de hanseníase não pode se restringir aos contatos domiciliares.

Objetivo: identificar a prevalência de pacientes notificados com hanseníase em São Luís, Maranhão, durante 2010-2020. Método: estudo descritivo, exploratório, retrospectivo, transversal, quantitativo, realizado no Banco de Dados público do DATASUS cuja coleta ocorreu em agosto de 2021. A amostra foi composta por 9.387 pacientes notificados com Hanseníase. Resultados: observa-se prevalência amostral em maio de 2012, sexo masculino, faixa etária entre 30-39 anos, Ensino Médio completo, pardos, casos multibacilares, avaliação de incapacidade física e de cura no estágio Grau 0, apresentaram mais que cinco lesões, baciloscopia negativa, episódios sem reação,sendo abordados como casos novos cuja forma prevalente foi a dimorfa, tendo como esquema terapêutico mais prescrito a poliquimioterapia durante 12 meses e prevalência de cura. Conclusão: houve uma prevalência dos casos de hanseníase em homens jovens com bom grau de instrução escolar. Apesar da alta prevalência de casos novos, o tratamento foi efetivo de modo a levá-los à cura.(AU)
Objective: to identify the prevalence of patients notified with leprosy in São Luís, Maranhão, during 2010-2020. Method: descriptive, exploratory, retrospective, cross-sectional, quantitative study, conducted in the public database of the DATASUS whose collection occurred in August 2021. The sample was composed of 9,387 patients notified with Leprosy. Results: sample prevalence was observed in May 2012, male gender, age range 30-39 years, complete High School, brown, multibacillary cases, evaluation of physical disability and cure in Grade 0 stage, presented more than five lesions, negative bacilloscopy, episodes without reaction, being addressed as new cases whose prevalent form was the dimorphic, having as the most prescribed therapeutic scheme the polychemotherapy for 12 months and prevalence of cure. Abstract Patients with lepromatous or borderline leprosy may present two types of vasculonecrotic reactions: Lucio's phenomenon (LP) and necrotic erythema nodosum leprosum (nENL). These are serious conditions, which mostly lead to life-threatening infectious and thrombotic complications. The authors report the case of a patient with leprosy recurrence associated with an atypical type II reaction with LP characteristics on histopathology.

Introducción: La meta mundial para la eliminación de la lepra, es su detección temprana, lo que requiere un abordaje integral, saber los diversos factores que pueden favorecer su adquisición, para así desarrollar estrategias que contribuyan a su erradicación. Objetivo: Caracterizar los factores clínico-epidemiológicos en la incidencia de lepra del municipio Camagüey. Métodos: Se realizó un estudio observacional, descriptivo, transversal, retrospectivo cuyo universo de trabajo estuvo constituido por todos los pacientes con diagnóstico de lepra en el municipio Camagüey, durante el período de estudio. Las variables estudiadas fueron: formas clínicas según clasificación de Madrid, edad, sexo, primeros síntomas y signos, localización de las lesiones, baciloscopia, modo de detección, fuente de infección y situación del enfermo en relación al foco. La información obtenida fue procesada mediante el paquete estadístico SPSS v21. Los métodos empleados fueron estadística descriptiva de distribución de frecuencias absolutas y relativas. Los resultados del estudio se expusieron en tablas y gráficos. Resultados: La lepra dimorfa fue la de mayor incidencia, prevaleció el grupo de edad entre los 60 y más años con predominio del sexo masculino, más de la mitad de los enfermos presentaron manchas anestésicas como primer síntoma y baciloscopia codificación cero. El modo de detección más relevante fue espontáneo, la fuente de infección la ignorada y en la situación del enfermo en relación al foco prevaleció el caso índice. Conclusiones: Los resultados del estudio evidencian que se hace necesario incrementar la capacitación del personal de la salud y profundizar en las labores de búsqueda de casos.
Introduction: The global goal for the elimination of leprosy is its early detection, which requires a comprehensive approach, knowing the various factors that can favor its acquisition, to develop strategies that contribute to its eradication. Objective: To characterize the clinical-epidemiological factors in the incidence of leprosy in the Camagüey municipality. Methods: A retrospective cross-sectional descriptive observational study was carried out, whose work universe consisted of all the patients with a diagnosis of leprosy in the Camagüey municipality, during the studied period. The variables studied were: clinical forms according to Madrid classification, age, sex, first symptoms and signs, location of the lesions, bacilloscopy, mode of detection, source of infection, and situation of the patient about the focus. The information obtained was processed using the statistical package SPSS v21. The methods used were descriptive statistics of the distribution of absolute and relative frequencies. The results of the study were presented in tables and graphs. Results: Dimorphic leprosy was the one with the highest incidence, the age group between 60 and older prevailed with a predominance of males, more than half of the patients presented anesthetic spots as the first symptom, and smear coding was zero. The most relevant detection mode was spontaneous, the source of infection was ignored, and the index case prevailed in the patient's situation in relation to the outbreak. Conclusions: The results of this study show that it is necessary to increase the training of health personnel and deepen the search for cases.

Introducción: La conexión entre la lepra y los factores sociales, tienen implicación tanto para el paciente, como para la operatividad de los programas de control de esta enfermedad. Objetivo: Caracterizar factores antropológicos socio-demográficos en la incidencia de lepra en el municipio Camagüey. Métodos: Se realizó un estudio observacional, descriptivo, transversal retrospectivo. El universo lo conformaron los pacientes con diagnósticos de lepra incluidos en el periodo que abarcó el estudio y pertenecientes al municipio Camagüey. Las variables estudiadas fueron: formas clínicas según clasificación sanitaria, edad, sexo, área de salud, procedencia geográfica (rural o urbana), ingreso económico familiar e índice de hacinamiento. La información obtenida fue procesada mediante el paquete estadístico SPSS v21. Los métodos empleados fueron estadística descriptiva de distribución de frecuencias absolutas y relativas. Los resultados del estudio se expusieron en tablas y gráficos. Resultados: La lepra multibacilar fue la de mayor incidencia, predominó el grupo de edad entre los 20 y 59 años, el sexo masculino fue el más afectado, el mayor por ciento de casos se observó en las áreas de salud Norte y Previsora, más de las cuatro quintas parte de los pacientes pertenecían a la zona urbana, la mayoría de ellos tenían un ingreso económico familiar malo y predominó el índice sin hacinamiento. Conclusiones: Se hace necesario profundizar en el estudio de los factores antropológicos sociales que pudieran favorecer la adquisición de esta enfermedad.
Introduction: The connection between leprosy and social factors has implications both for the patient and for the operation of the control programs for this disease. Objective: To characterize socio-demographic anthropological factors in the incidence of leprosy in the Camagüey municipality. Methods: A retrospective cross-sectional descriptive observational study was conducted. The universe was made up of patients diagnosed with leprosy included in the period covered by the study and belonging to the municipality of Camagüey. The variables studied were: clinical forms according to health classification, age, sex, health area, geographical origin (rural or urban), family economic income and overcrowding index. The information obtained was processed using the statistical package SPSS v21. The methods used were descriptive statistics of absolute and relative frequency distribution. The results of the study were presented in tables and graphs. Results: Multibacillary leprosy was the one with the highest incidence, the age group between 20 and 59 years old prevailed, with a predominance of males, the highest percentage of cases was observed in the North and Previsory health areas, more than four fifths parts of the patients belonged to urban areas, most of them had a poor family income and the index without overcrowding prevailed. Conclusions: It is necessary to deepen the study of the social anthropological factors that could favor the acquisition of this disease.

RESUMEN Objetivo. Describir el comportamiento epidemiológico de la lepra en varios países latinoamericanos durante 2011-2020, tomando como base los indicadores de la Organización Mundial de la Salud (OMS). Método. Estudio transversal, descriptivo y cuantitativo con datos oficiales sobre incidencia y prevalencia en la población en general, niños, forma clínica y casos de discapacidad de grado 2 registrados por la OMS entre 2011 y 2020. Se seleccionaron los ocho países latinoamericanos con más casos y se realizaron análisis de estadística descriptiva simple y comparativa entre las variables. Resultados. Entre 2011-2020 se reportaron 301 312 casos de lepra en los países seleccionados: Argentina, Brasil, Colombia, Cuba, México, Paraguay, República Dominicana y Venezuela. Brasil fue el único con una prevalencia mayor a 1 por 10 000 habitantes y representó 93,77% de casos. En Brasil y República Dominicana se observó un aumento de la prevalencia durante 2011-2019, mientras que en los demás países esta tendió a disminuir. La enfermedad fue más frecuente en hombres, y los casos multibacilares superaron significativamente a los paucibacilares. En Brasil se encontraron las mayores incidencias de casos de lepra infantil y discapacidad de grado 2 durante el período evaluado. Conclusión. A pesar de que en América Latina la lepra solo es considerada un problema de salud pública en Brasil, la mayoría de los países de la región continúa reportando casos cada año, lo que revela una falta de atención médica adecuada. El estudio confirmó la importancia de la vigilancia activa, el diagnóstico temprano y la planificación de acciones contra la enfermedad en todos los países evaluados, con el propósito de disminuir o detener su transmisión.
ABSTRACT Objective. To describe the epidemiological behavior of leprosy in several Latin American countries during 2011-2020, based on World Health Organization (WHO) indicators. Methods. Cross-sectional, descriptive and quantitative study with official data on incidence and prevalence in the general population, children, clinical form and cases with grade 2 disability from WHO records between 2011 and 2020. The eight countries in Latin America that reported most cases were selected and analyses were carried out using simple descriptive and comparative statistics between different variables. Results. During the study period, 301 312 cases of leprosy were reported in the selected countries: Argentina, Brazil, Colombia, Cuba, Dominican Republic, Mexico, Paraguay, and Venezuela. Brazil is the only country in the region with a prevalence greater than 1 per 10 000, representing 93.77% of all cases. Brazil and the Dominican Republic showed an increase in prevalence during 2011-2019, while in other countries the trend was decreasing. The disease is more frequent in men, and multibacillary cases significantly exceed paucibacillary ones. Brazil showed the highest incidences of cases of childhood leprosy and grade 2 disability during the evaluated period. Conclusion. In Latin America, leprosy is only considered a public health problem in Brazil; however, most countries in the region continue to report cases annually, revealing a lack of adequate medical care. This study confirmed the importance of active surveillance, early diagnosis and planning of actions against the disease in all the countries evaluated with the aim of reducing its transmission.

Se selecionaram os oito países da América Latina que relataram a maioria dos casos e as análises foram avaliadas por meio de estatísticas descritivas e comparativas simples entre as variáveis. Resultados. No período, foram notificados 301 312 casos de hanseníase nos países selecionados: Argentina, Brasil, Colômbia, Cuba, México, Paraguai, República Dominicana e Venezuela. O Brasil é o único país da região com prevalência maior que 1 por 10 000 habitantes, representando 93,77% do total de casos. O Brasil e a República Dominicana mostraram um aumento na variação da prevalência durante 2011-2019, enquanto nos demais a tendência foi decrescente. A doença é mais frequente em homens e os casos multibacilares superam significativamente os paucibacilares. O Brasil apresentou as maiores incidências de hanseníase infantil e incapacidade de grau 2 durante o período avaliado. Conclusão. Na América Latina, a hanseníase só é considerada um problema de saúde pública no Brasil; no entanto, a maioria dos países da região continua notificando casos anualmente, revelando falta de assistência médica adequada. O presente estudo confirmou a importância da vigilância ativa, do diagnóstico precoce e do planejamento de ações contra a doença em todos os países avaliados, com o objetivo de reduzir e interromper a sua transmissão.
