RESUMEN

Aims Difficulties in diagnosis of phyllodes tumour are well known. Our aim was to observe whether the expressions of satb1, survivin, and ki-67 were helpful in differentiating fibroadenoma and phyllodes tumour. Settings and design Retrospective. Methods and material Paraffin embedded tissue samples from 60 female patients with phyllodes tumour and 60 female patients with fibroadenoma were studied. Expression of the gene products was studied and confirmed using immunohisto chemical and western blot analysis. Statistical analysis used The statistical analysis was performed using Fisher's exact test to find out the significant changes. Results Statistically significant difference was observed in Satb1, survivin expression between fibroadenoma and benign phyllodes cases. The difference between the Ki-67 expression in fibroadenoma and benign phyllodes cases was not statistically significant. Conclusions Our finding of strong stromal expression of satb1 and survivin in phyllodes as compared to fibroadenoma can be helpful for the development of additional diagnostic and prognostic indicators for otherwise difficult cases. Such immunochemical markers can be used to elucidate cellular basis of tumour behaviour. Validation of such immunochemical test in future will reduce diagnostic uncertainty in this rare tumour. In addition to that such parameters may serve as a therapeutic target that could increase effectiveness of chemotherapy or radiation therapy. A study with large number of samples along with clinical and follow-up data is required for confirmation.

RESUMEN

OBJECTIVE:To investigate the effect of X-ray repair cross complementing gene(XRCC1)Arg399Gln(G→A) polymorphism on efficacy of oxaliplatin+fluorouracil chemotherapy and survival time of advanced gastric cancer patients. METH-ODS:Totally 52 cases of advanced gastric cancer were selected from Hainan Provincial People's Hospital during Jan. 2013-Jan. 2015. They were given oxaliplatin+fluorouracil chemotherapy,for 3 courses(a treatment course lasted for 3 weeks). The genotypes of patients were detected by PCR-LDR. Disease control rate and progression free survival were compared among different geno-types. RESULTS:Among 52 cases of advanced gastric cancer,there were 28 cases of XRCC1 GG genotype(53.8%),21 cases of GA genotype(40.4%),3 cases of AA genotype(5.8%),frequencies of which were all in line with Hardy-Weinberg balance(P>0.05). Disease control rates of 52 cases were 76.9%,among which disease control rate(92.9%)of GG genotype was significantly higher than that of GA+AA genotype(58.3%),with statistical significance(P<0.05). The average progression free survival of 52 cases was(7.1+1.2)months,among which progression free survival of GG genotype [(8.6±0.8)months] was significantly longer than that of GG+GA genotype [(5.9 ± 0.7)months],with statistical significance (P<0.05). CONCLUSIONS:XRCC1 polymor-phism is correlated with efficacy of oxaliplatin+fluorouracil chemotherapy and progression free survival,and XRCC1 GG genotype is more sensitive to chemotherapy drugs. XRCC1 gene can be regarded as predictive indicator for therapeutic efficacy of chemothera-py and survival.

RESUMEN

Objective To explore the application and effects of the specialized management of telephone follow-up in patients with tumor intermission of chemotherapy. Methods A total of 400 patients with tumor intermission of chemotherapy were randomly divided into the observation group and the control group with 200 cases in each group. Combined with the current general telephone follow-up and through the status of the investigation, content and process of specialized management of telephone follow-up in patients with tumor intermission of chemotherapy were designed and applied to the observation group. The control group did not implement the intervention except for the ordinary telephone follow-up. The two groups of patients were evaluated by self-designed questionnaire, the effect of follow-up and the satisfaction degree of the patients were evaluated. Results In the observation group (5 cases were lost to follow-up), the mental state steady, reasonable diet, moderate activity, understand medical medicine, law and adopt measures to prevent infection accounted for 73.85% (144/195), 74.36% (145/195), 67.69% (132/195), 80.51% (157/195) and 72.31% (141/195), 77.95% (152/195);while in the control group (8 cases were lost to follow-up), the proportion was 53.13% (102/192), 51.04% (98/192), 43.75%(84/192), 55.73%(107/192), 52.08%(100/192), 53.13%(102/192),χ2 values ranged from 16.84 to 27.41, P < 0.01. The satisfaction degree with follow-up time, health guidance and problem solutions during follow-up process of the observation group accounted for 67.69% (132/195), 69.23% (135/195), 86.15% (168/195), the proportion of the control group was 45.83% (88/192), 46.88% (90/192), 47.92%(92/192),χ2 values were 20.49, 49.01, 77.65 (P<0.01). Conclusions The specialized management of telephone follow-up will help to promote the treatment of patients with tumor intermission of chemotherapy of home care norms, improve the effect of follow-up and satisfaction.

RESUMEN

Objective To investigate the efficacy and safety of HCPT combined with 5-fluorouracil(5-Fu) in treatment of bladder cancer .Methods Selected 92 patients with advanced bladder cancer in our hospi-tal from February 2011 to August 2015 , equally divided into observation group and control group of 46 patients based on the random number table ,the control group were received 5-Fu intravesical chemotherapy ,The obser-vation group were given the HCPT combined with 5-Fu chemotherapy ,the chemotherapy infusion were once a week,continuous perfusion were observed for 3 months.Results The response rates in the observation group was 95.7%and the control group was 76.1%,the observation group were significantly higher than the control group (P0.05),all adverse reactions after symptomatic treatment were improved .All patients were followed up for 1 year,The mortality and recurrence rates in the observation group were 8.7%and 4.3%re-spectively,while the control group were 30.4%and 17.4%,the mortality and recurrence rates of the observation group were significantly lower than the control group (P<0.05).Conclusion HCPT combined with 5 -Fu chemotherapy for bladder cancer has strong anti -cancer effect and without increase the incidence of adverse re-actions,it can promote the improvement of the long -term prognosis,so it is a safe and effective topical bladder chemotherapy drugs .

Objective To detect gene polymorphisms of ABCB1/MDR1 in breast cancer and to analyze the association between ABCB1 gene polymorphisms and curative effect of paclitaxel-based chemotherapy in breast cancer.Methods Genotyping of ABCB1exon12 (1236),exon21 (2677)and exon26 (3435)was determined by polymerase chain reaction (PCR)-high resolution melting (HRM)method in 146 cases of female stage IV breast cancer to explore the relationship between the efficacy of chemotherapy in breast cancer patients with paclitaxel chemotherapy.Results In 146 patients with stage IV breast cancer,C1236T CC genotype accounted for 15.86%, CT genotype 44.83%,TT genotype 44.83%,G2677T/A GG genotype 16.67%,GT genotype 45.83%,GA genotype 6.25%,TT genotype 28.47%,AT genotype 2.78%,C3435 CC genotype 22.22%,CT genotype 56.25%,and TT genotype 21.53%.ABCB1 gene polymorphisms did not significantly differ between patients of Hui and Han Nationalities (P>0 .0 5 ).Hardy-Weinberg genetic equilibrium testing showed that polymorphisms of C1236T,G2677T/A and C3435T had group representation (P>0.05).In 146 stage IV breast cancer patients who had received paclitaxel-based chemotherapy,CT/TT genotype of C3435T site had a better response rate (74.11%) (χ2 =12.556,P<0.05),better curative effect than CC genotype (OR=4.183,95% CI 1.838 -9.521,P<0.05),T allele carriers more efficient than C allele carriers with paclitaxel-based chemotherapy (χ2 =8 .5 0 7 ,P<0 .0 5 ). Conclusion Detection of ABCB1 C3435T gene polymorphisms has a significantly clinical value in predicting the efficacy of taxanes chemotherapy in breast cancer patients.

Glutamine is not only a special nutritional substance but a conditional essential amino acid,which is the most abundant amino acids in plasma,representing about 60% of free amino acids in the body,and it's normal plasma concentration is from 0.6 to 0.9 mmol/L.In recent years,with the increasing deepening study of the field of clinical nutrition,the concept of glutamine study has been gradually changed gradually changed it's concept from a simple nutritional support to immune nutrition therapy.Glutamine can not only improve the body's protein levels of nutrition,protect the gastrointestinal mucosa,but also reduce the incidence of infection and the inflammation in the body,improve immune function,reduce side effects of chemotherapy,and at the same time enhance tumor sensitivity to chemotherapy and promote gastrointestinal cancer patients' postoperative recovery,reduce surgical complications.This paper aims to review the application and research of glutamine in the gastrointestinal cancer patients after the operation.

Objective To evaluate the effect of community nurse training in peripherally inserted central catheter (PICC) on community nursing quality of tumour patients with indwelled PICC during the intermission of chemotherapy.Methods A prospective study was performed with a total of 220 patients with esophageal or lung cancer enrolled from September 2011 to March 2013 in the Department of Cardiothoracic Surgery,the First Affiliated Hospital of Chongqing Medical University.The 220 patients included 174 males (79.1%) and 46 females (20.9%),at the age of (61.1 ±9.0) years.With the start of community nurse training as the cut-off line,the patients were divided into pre-training group (n =119) and post-training group (n =101).PICC-related complications and patient satisfaction were compared between the 2 groups.Results The overall PICC-related complication rate was 20.9% (46/220) during community nursing,including,in the order of declining incidence,infection (7.2%,16/220),and venous thrombosis (4.5%,10/220),catheter occlusion (3.6%,8/220),catheter dropping (2.7%,6/220),and leaking/damage (2.7%,6/220).The total complication rate and incidence of infection in the post-training group were both significantly lower than those in the pre-training group (14.9% vs.26.1%,P =0.047 ; 3.0% vs.10.9%,P =0.035),and patient satisfaction (including satisfaction for answers to questions about PICC,feelings about PICC indwelling and PICC position,and overall satisfaction) was significantly higher than in the pre-training group (P =0.003,P =0.011,P =0.001).Conclusion Community nurse training programmes have positive effects on improving PICC-related nursing and patient satisfaction in cancer patients with PICC indwelling during intermissions of chemotherapy.

Objective:To analyze the nuclear proliferation in breast cancer tissue related antigen (Ki-67) and proliferating cell nucleus antigen ( PCNA ) expression changes and the relationship between breast cancer and its relationship to breast cancer chemotherapy sensitivity, provide theoretical basis for clinical effective chemotherapy of breast cancer.Methods: Subjects from our hospital in recent years,by clinical examination,84 cases of patients diagnosed with breast cancer,breast cancer tissue were measured with immunohistochemical method of Ki-67 and PCNA content, compared different Ki-67 and PCNA expression levels of patients undergoing chemotherapy curative effect difference.Results:Ki-67 positive cases for 52 cases,PCNA positive cases of 62 cases.Ki-67 positive rate and the patients with lymph node metastasis and tumor classification stage were positively correlated,the difference was sta-tistically significant,P0.05).The total effective rate of Ki-67+was significantly higher than that of Ki-67-(80.8%and 56.2%,P<0.05).Effective rate of PCNA-significantly higher than that of PCNA+(72.7% to 45.2%,P<0.05).Conclusion:Ki-67 clinical data and PCNA expression is closely related to breast cancer and chemotherapy sensitivity.It can be used as a prediction index of curative effect of chemotherapy.

Objective To explore the effects of the mindfulness mode intervention on the adverse reaction of the patients with breast cancer during the period of chemotherapy.Methods 75 patients with breast cancer during the period of chemotherapy were randomly assigned to the control group(38 cases) and the intervention group(37 cases).All patients received the routing nursing of chemotherapy.In addition the intervention group received the mindfulness intervention.The adverse reaction of chemotherapy questionnaire was used to investigate the level of their adverse reaction before the intervention and two months after the intervention.Results After the intervention,the scores of the adverse reaction in the intervention group was (32.19± 7.70)points,less than that of the control group,which was (36.97±10.03)points.There was statistically significant difference between two groups.Conclusions The mindfulness model intervention could reduce the adverse reaction of the patients with breast cancer.

OBJECTIVE:To observe the therapeutic effect of combined use of capreomycin in treating multiple-drug-resistant tuberculosis.METHODS:After 268 patients with multiple-drug-resistant tuberculosis had underwent intensive treatment with levofloxacin,pyrazinamide,pasiniazid,protioamide and capreomycin for 3 months,they underwent another 9 months of strengthened treatment with levofloxacin,pasiniazid,and rotioamide.RESULTS:The negative rate determined in sputum test for tuberculosis bacteria was up to 78.36%,and the closing rate of the cavity was up to 37.76%.CONCLUSION:The therapeutic effect for treatment of multiple-drug-resistant tuberculosis can be improved by carrying out the principle of whole range direct monitoring of chemotherapy and using capreomycin in combination with other drugs to increase the sensitivity to mycobacterium tuberculosis.

Objective To improve and innovate the dispensing equipment in oncology.Methods Plexiglass plate was used in the counter in order to give an isolate space for preparation of chemotherapy drugs.Through the ventilation hole,the air polluted by chemotherapy drugs was sent out of door.The operator put on special protective gloves before preparation of chemotherapy drugs.Results There was a decrease of dispensing errors and confusion,when the counter was used to separate the conventional dosage from chemotherapy dosage.The counter effectively prevented the skin and respiratory tract damage to the operator during the preparation of chemotherapy drugs.Conclusion It is practical,cheap,easily operated and produced,which is worth promoting.

BACKGROUND: The length of postoperative drug therapy remains controversial in pulmonary tuberculosis. We analyzed our experiences to determine the postoperative duration of chemotherapy after resection. METHODS: A retrospective review was performed in 66 of 95 patients that underwent pulmonary resection for pulmonary tuberculosis between January 1993 and December 1998. We compared the relapse rates according to the length of postoperative chemotherapy in each group, classified by the results of sputum AFB culture before the surgery, the number of resistant durgs, the number of prior treatment and the division of anti-TB drugs used postoperatively. RESULTS: Fifty three of 66(80.3%) were men and 13(19.7%) were women with a median age of 33.5 years(range, 16 to 63). The mean lengths of the pre-and post-operative chemotherapies were 4.9 months, and 12.9 months respectively. Five of 66 patients(7.6%) relapsed during the mean period of follow up(39.7 months). In the group less than three times of the prior treatment, there were two relapses(20%) in Ed-the highlight above-rephrase 10 patients that were medicated for 6 months or less, and one relapse in 43 patients(2.3%) that took medicine for more than 6 months(p=0.03). In the group using second-line drugs postoperatively, there was one relapse(25%) in four patients that were medicated for 12 months or less. No patient in a total of 17 that received medicine for more than 12 months relapsed(p=0.03). CONCLUSION: We recommend that patients with the prior treatment less than three times should be treated for more than 6 months after resection and patients using the second-line drugs postoperatively should be medicated for more than 12 months.

BACKGROUND: The majority of patients with locally advanced, unresectable, non-small cell lung cancer(NSCLC) were treated with conventional thoracic radiation therapy Throcic radiation therapy produces tumor regression in most patients but few cures and dismal 5-year survival rate. Several randomized studies have demonstrated that systemic chemotherapy controls micrometastasis and improve survival ratNes for patients who have locally advanced NSCI.C. Hut the optimal frequency of chemotherapy and sequence for chemotherapy and radiotherapy are yet to be determined. In this study, we analyzed response rate, median survival time, side effects and prognostic variables according to the frequency of chemotheray in locally advanced NSCLC patients. METHODS: We separated locally advanced, unresectable, NSCLC patients into two groups according to given number of chemotherapy cycles. Among 28 patients evaluated, eleven patients were classified to group A, receiving above 3 cycled chemotherapy and seventeen patients, classified to group B, receiving 3 cycled chemotherapy. In both groups, thoracic irradiation of 5940 cGy was given to all patients after chemotherapy. RESULTS: 1) Median survival time was 12.9 months for group A, 12.8 months for group B but there was no statistically significant difference(P>0.05), 2) Overall response rates were not significantly different between two groups(P>0.05). 3) Frequency rate of local failure and distant metastasis were not significantly different between two groups (P>0.05). 4) The grade and frequency of toxicities during treatment were not significantly different between two groups (P>0.05). 5) Clinical stage was the only major prognostic factor for overall survival (P<0.05). CONCLUSION: Median survival time, response rate, toxicities and frequency of local failure and distant metastasis were not significantly different between two groups. So, when we treat locally advanced, unresectable, NSCLC patients in sequential combined treatement, we should consider planned therapy(limiting chemotherapy cycles given), because planned therapy reduces many troubles of patients, that is, economic loss and time consuming, psychiatric anxiety etc, during treatment period. The optimal frequency of chemotherapy is remained to be validated in large scale study in the future in the setting of combined treatment.

RESUMEN

This study reported the primary application of FA-ELISA from 107-121kDa fration antibody of Schistosoma Japonicum for detection of the short-term antibody in patients with schistosomiasis. The result showed that this method provided high sensitivity (with positive rate of 93. 33% with 30 cases of schistosomiasis) and good specificity (no false postive in 60 health objects). When one group of schistosome patients were examined with FA -ELISA and with the routine SEA-ELISA before chemotherapy and at 8 months,1 year and 1. 5 year after treatment,it showed good property for evaluation of chemotherapy efficacy with FA -ELISA,which was much better than the routine method.
