RESUMEN

Gag reflex is a complex physiologic reflex which can leadto a compromised dental treatment and can prove a barrierfor optimal patient care and management. This gag reflexis protective in nature, but can present a bewildering,annoying and frustrating problem during various dentalprocedures. The aim of this article is to review normal gagreflex, neurophysiology of gag reflex, factors associated withetiology of gagging and role of different methods to managegagging during routine dental procedures.

RESUMEN

Objective To evaluate the point plaster therapy with ginger powder combined with ondansetron hydrochloride in preventing nausea and vomiting usually occurred after platinum-based interventional chemotherapy in patients with primary or metastatic liver cancer,and to compared its effectiveness with that by using ondansetron hydrochloride only. Method Sixty-two patients with primary or metastatic liver cancer,who were scheduled to receive platinum-based interventional chemotherapy,were randomly and equally divided into two groups with 31 cases in each group. The patients in the study group (n = 31) were given point plaster therapy,i.e. externally applying ginger powder (20 g) to the point of Shenque,for four days together with arterial infusion of ondansetron hydrochloride (8 mg) during interventional procedure,while the patients in the control group (n = 31) were given point plaster therapy with placebo (potato powder) together with arterial infusion of ondansetron hydrochloride (8 mg) during interventional procedure. The questionnaire of INVR (index form for evaluating nausea and vomiting) was used to assess the effectiveness,and the results were compared between two groups. Results The incidence of nausea and vomiting in study group was significantly lower than that in control group at all observed points of time during the period of 0 -72 hours after the treatment (P 0.05). After the treatment the scores of nausea,vomiting and retching in the study group were 0.45,0.25 and 0.19 respectively,while these in the control group were 2.77,0.87 and 0.97 respectively,the differences between two groups were statistically significant (P

RESUMEN

Nausea, vomiting and retching are universal symptoms that affect individuals' state of health and self-care activities of individuals. Accurate measurements of individual symptoms are required to gather more definitive data, and enhance understanding, planning, and implementation of self-care actions. Recently the Rhodes Index of Nausea, Vomiting, and Retching(INVR), a new format of the INV-2(the Rhodes Index of Nausea and Vomiting), was developed to measure the symptoms of nausea, vomiting and retching in an English speaking population. To determine the reliability and validity of the INVR, and the possibility of using the instrument in Korea, the Korean translation of the INVR and the INV-2 were administered to a convenient sample of 105 patients at two University Hospitals in Kwangju, Korea. The Cronbach's alpha to estimate the internal consistency of reliability for INVR was 0.844. Equivalent measures of reliability were conducted to determine the percentage of agreement and the Spearman rank correlation coefficients for responses on the two instruments. The percent agreement was 83% and the correlation coefficient was 0.906 over all. A significant differences between the INVR scores of the patients with and without nausea, vomiting, or retching were seen, which indicated a construct validity. The INVR was found to be more user friendly for the patient and the healthcare providers. As a result, it is suggested that the INVR can provide a scientific base for measuring the symptoms of nausea, vomiting, and retching for nurses to improve patients' care and quality of life.
