RESUMEN

Objective:Bibliometric analysis of SCI papers in Shanghai tertiary obstetrics and gynecology specialized hospitals from 2017 to 2021 was conducted to provide reference for further improving management measures and optimizing the quality of SCI papers in specialized hospitals.Methods:The Web of Science database was used to retrieve SCI papers published by tertiary obstetrics and gynecology specialized hospitals in Shanghai from 2017 to 2021. Based on bibliometric methods VOSviwer 1.6.17 and Scimago Graphica 1.0.23 was adopted to analyze the number, journals, domestic and international cooperation, and hot research areas.Results:2 721 papers were published by Shanghai Tertiary A obstetrics and Gynecology hospitals from 2017 to 2021. These papers published in 765 journals. The number of papers published and publication journals increased year by year. The number of papers and journals with an impact factor of 3~5 is the largest, accounting for 37.96% and 35.95% of the total number of papers and journals respectively. However, papers and journals with more than 10 points accounted for only 7.98% and 10.20% respectively. Scientific Reports, a comprehensive journal, published the largest number of papers. International cooperation is concentrated in the United States. Domestic cooperation is concentrated in universities and research institutions in Jiangsu, Zhejiang and Shanghai. The hot research areas focused on Oncology, Obstetrics & Gynecology, Genetics, Reproductive Biology and other basic medical fields.Conclusions:The number of papers published by Shanghai's tertiary obstetrics and gynecology specialized hospitals has increased year by year, but there are fewer highly influential journals. Domestic and international scientific research cooperation is limited to several institutions. The achievements in the field of basic research are far more than those in clinical research.

RESUMEN

This article summarized the overview of diagnosis related groups (DRGs), the necessity of comprehensively popularizing and applying DRGs in specialized hospitals and general hospitals, the different methods and effects of nursing human resource allocation based on DRGs in specialized hospitals and general hospitals at home and abroad, and analyzed the different challenges and opportunities faced by DRGs in the implementation of human resource allocation in two types of hospitals. According to the types and characteristics of hospitals, this paper put forward some corresponding suggestions and prospects for the future, such as intelligent human resource prediction system and the construction of information sharing platform, so as to provide reference for the comprehensive promotion of DRGs in different types of hospitals in China.

RESUMEN

Objective:To summarize the experiences in the construction of Clinical Research Unite (CRU) at a hospital level, provide reference for CRU construction in the Chinese hospitals.Methods:The CRU construction should take clinical research project management as working priority, strengthen clinical research team building, improve process management, as well as improve project performance and output.Results:During the CRU construction period, the number and level of clinical research projects have been improved in various disciplines. The sample database and disease specific data have been accumulated. The CRU mode helps the high-quality development of clinical researches in hospital.Conclusions:The way our hospital take in the construction of CRU was a good example, and it may truly improve the quality and capacity of clinical researches.

ObjectiveThe clinical research team is the key to improving clinical research ability. The purpose of this study is to identify the main problems with the development of clinical research teams in specialized hospitals, and to explore countermeasures. MethodsOn the basis of literature review and consultation with experts, a self-made questionnaire was distributed online to survey 300 relevant staff engaged in clinical research in 10 tertiary specialized hospitals in Shanghai. The severity of existing problems was analyzed from four aspects: clinical research personnel planning, recruitment and allocation; training and development; evaluation and performance management; career management. Then in-depth personal interviews were conducted with the main heads of clinical research departments in some hospitals. ResultsFour problems have relatively high severity scores, with 3.65±1.0, 3.43±1.0, 3.53±1.0, and 3.44±1.0 points, respectively. They are: “The development of the clinical research team is incomplete, and there is a lack of stable multidisciplinary and multi-level talent teams”, “A systematic and continuous clinical research training system has not been established”, “An objective and fair evaluation index system has not yet been established”, and “There are obstacles in the career development and promotion path of clinical research professionals”. ConclusionTo further enhance the clinical research capabilities of hospitals, we urgently need to establish multidisciplinary and multi-level clinical research teams, strengthen the training of clinical researchers on their clinical research capabilities, establish a scientific evaluation index system and incentive methods, and explore career development that meets the characteristics of clinical researchers.

Objective:To better understand the satisfaction with medical worker′s scientific research, raise the management and service levels of scientific research management department, and stimulate hospital workers devote to scientific research.Methods:This paper analyzes the main factors that affect the scientific researchers′ job satisfaction in a tertiary hospital in Shanghai based on questionnaire survey. SPSS 26.0 was used to test the reliability and validity of the scale. Two independent sample t-test, one-way ANOVA and LSD-T test are used to explore the factors that affect the scientific research enthusiasm of staffs in different positions.Results:The incentive measures of scientific research in hospital can be summarized into three different aspects: material/spiritual incentive mechanism, scientific research management system and personnel training system, the average scores were: X-≈3.97, X-≈ 4.09, X-≈ 3.84, according to which the scientific research management system has the highest satisfaction.Conclusions:The overall satisfaction of staff in the hospital needs to be improved. According to the characteristics of different staff, tailored research management measurements should be developed to mobilize the enthusiasm of hospital staff in scientific research, enhance their satisfaction, and further improve the scientific research capacity of the hospital.

RESUMEN

Objective: To construct an information system for preview and triage during novel coronavirus pneumonia (NCP) epidemic time in stomatological hospitals and put it to clinical use. Methods: To sort out and formulate the preview and triage process of stomatological hospitals during the epidemic, we constructed B/S development platform with Jeesite open source framework, sort outed and developed relevant input and query functions. ActiveX widget was used to connect the hardware part with the ID card reader through the browser. At the initial stage of the system put online, the parallel mode of manual registration and system registration was used to verify the actual effect of the system. Results: After the system went online, the triage data of 35 patients were analyzed. The average triage time was shortened from (90.82±31.85) seconds to (38.97±12.14) seconds, with an efficiency increase of about 125%. Results: The system's performance can meet the actual business requirements, reduce the risk of cross infection, improve the preview and triage efficiency, and improve patients' medical experience. Meanwhile it ensures the accuracy and safety of the information record of people seeking medical service.

BACKGROUND: Less attention is given to cancer treatment and control in Ethiopia. METHODS: To investigate the challenges and opportunities facing cancer treatment services in Ethiopia. A purposive sampling technique was applied to recruit the study subjects from Black Lion Specialized Hospital Oncology Department. A semi-structured interview guide was used to investigate challenges and opportunities in oncology service in Ethiopia. Data was transcribed and coded by two independent coders and analyzed thematically in reference to the objectives. RESULTS: Fifteen professionals from four different disciplines were interviewed on opportunities and challenges facing cancer treatment in Ethiopia. Out of these respondents 3 were senior clinical oncologists while 4 individuals were senior oncology residents. The rest were 2 medical Physicists, 2 radiotherapy technologists and 4 oncology nurses. Majority (80%) of the respondents were males. We demonstrate that the challenges of cancer treatment service in Ethiopia emanate from the patients themselves, the administrating body, the professionals, and the technology limitations. In general, the result of this study was grouped under the following five themes: Customer-related challenges, provider-related challenges, facility-related challenges, technology-related challenges and the opportunities. CONCLUSIONS: Several assignments are waiting for the policy makers, the professionals, the communities and other concerned bodies to combat the alarmingly growing burden of cancer in Ethiopia. Escalating the awareness of the general population about cancer, expanding well-developed diagnostic and treatment centers, and producing well-trained competent oncology professionals are the forefront challenges in combating cancer in Ethiopia.

Objective:To analyze the operational efficiency of public specialized hospitals and private specialized hospitals in Guangdong.Methods:Using the Malmquist index model of DEA to analyze operational efficiency of public specialized hospitals and private specialized hospitals in Guangdong from 2010 to 2015.Results:From 2010 to 2015,the overall operation efficiency of specialized hospitals were in low condition;the operational efficiency of public specialized hospitals was better than private specialized hospitals.Conclusion:Medical and health reform policy should attach great importance to the construction of specialized subject hospitals.Private specialized hospital was the cornucopia of hospital development which needed accurate and professional.

OBJECTIVE:To understand the situation of essential medicines and promote their rational use in our hospital. METHODS:The prescription variety and cost percentages of essential medicines in outpatient department in the two halves years of 2013 in our hospital(eye and ENT specialized hospitals)were compared;the medicines with forefront prescription kind percent-ages in eye and ENT departments were sorted and rankings in 4 quarters were compared. RESULTS:After the approval of the Na-tional Essential Medicine List(2012 edition),compared with the first half year of 2013,prescription variety and cost percentages of essential medicines in outpatient department in the second half year were significantly increased (P<0.01). Prescription variety and cost percentages of essential medicines in eye department in the second half year were significantly increased(P<0.01),in the top 20 prescription variety,totally 2 kinds were contained by the list 2012 edition and 3 were contained by the supplementary list of Shanghai(2012 edition). Compared with the first half year of 2013,prescription variety and cost percentages of essential medi-cines in ENT department in the second half year were significantly decreased(P<0.01),the cost percentage was significantly in-creased(P<0.01),in the top 20 prescription variety,totally 2 kinds were contained by the list 2012 edition and 3 were contained by the supplementary list of Shanghai (2012 edition). CONCLUSIONS:The prescription variety and cost percentages of essential medicines in our hospital show increase trend. Nevertheless,the non-essential medicines are still dominant and further improvement of essential medicines is still needed. Medicine variety and dosage forms of eye in ENT department are insufficient concluded in Es-sential Medicine List. Further adjustment of Essential Medicine List still needs more consideration for the medication characteristics in specialized hospital.

RESUMEN

OBJECTIVE To know the situation and problems of anti-infective agents used in clinics in our hospital. METHODS Antibacterial use of 1771 cases discharged from hospital in April 2005 was retrospectively analyzed. RESULTS The use rate of anti-infective agents was 94.81%,in which the drug combination therapy was the majority and the rate was 55.63%,and the rate of the prophylaxis of infections was 63.61%;of the first 10 drugs the use rate of second line and better drugs was 36.21%;the use rate of anti-infective agents used for Ⅰ incision for more than 4 days was 90.30%;the use rate of anti-infective agents used in obstetrics was 98.17%. CONCLUSIONS We should enhance the supervising of the use of anti-infective drug and mush adopt management measure.

RESUMEN

The significance for specialized hospital's resident physicians participating in the standardized clinical resident training in general hospital is expounded and its existing prob-lems are pointed out.It is suggested that the administration division should give full play to subjective initiative,coordinate problem solving and improve training quality,and meanwhile speciality hospitals should make an active effort in the application for national subspeciality physician training base.
