RESUMEN

ABSTRACT Purpose: This prospective, randomized, unmasked, clinical trial aimed to report the visual outcomes of cataract surgery on both eyes versus cataract surgery on one eye in Brazilian patients. Methods: This study included patients with bilateral cataracts and binocular visual acuity worse than or equal to 0.3 logarithm of the minimum angle of resolution. The patients were randomly assigned to undergo surgery on one (Control Group) or both eyes (one eye at a time; Intervention Group). Postoperatively, self-reported visual function using Catquest-9SF (primary outcome measure), binocular visual acuity, stereopsis, and ocular dominance (secondary outcome measures) were compared. Results: A total of 151 patients (77 and 148 eyes in the Control and Intervention Groups, respectively) completed the follow-up. Patients who underwent surgery on both eyes exhibited significantly better self-reported visual function (p=0.036) and stereopsis (p=0.026) than those who underwent surgery on one eye. Binocular visual acuity and ocular dominance did not affect the group comparisons. Conclusions: Surgery on both eyes resulted in significantly better self-reported visual function and stereopsis than surgery on one eye.

RESUMEN

This article discusses the importance of diabetic retinopathy, an eye disease associated with diabetes, which is a systemic disease. The research question addresses the impact of diabetes on the retina of the eye through infection and clinical features. This approach is designed to improve the relationship between early diagnosis and treatment of disease, including laser surgery, corticosteroid injections, and vitrectomy. This article focuses on chronic diabetic retinopathy and eye examination recommendations in the United States and the United Kingdom. To prevent and manage diabetic retinopathy, it is recommended that diabetic patients have regular eye examinations. The results of this study include the importance of good glycaemic control, injections, photocoagulation, and vitrectomy as treatment options. Intravitreal long-acting steroids may also temporarily improve visual acuity by reducing macular oedema. However, long-term use of the drug may cause side effects and may lead to cataracts, steroid glaucoma, and endophthalmitis. It may cause reasons. A recent study of the disease in India shows that the incidence of high blood sugar and its effects on the eye is mainly diabetic retinopathy, including cataracts, neovascular glaucoma and even retinal detachment, which are very dangerous for eye health. Therefore, it is important to inform patients about this disease and perform timely screening because patients need to be informed carefully.

RESUMEN

AIM: To compare the short-term postoperative visual acuity, visual disturbance phenomena, and spectacle independence rate in patients who underwent monocular implantation with trifocal, multifocal, or extended range of vision intraocular lens(IOL).METHODS: A retrospective analysis was conducted on 67 cataract patients(67 eyes)who underwent phacoemulsification cataract extraction combined with IOL implantation from March 2019 to December 2022. A total of 35 cases(35 eyes)received Symfony extended range of vision IOL implantation, 21 cases(21 eyes)received AcrySof IQ ReSTOR +3D multifocal IOL, and 11 cases(11 eyes)received AcrySof IQ PanOptix trifocal IOL. The preoperative uncorrected distance visual acuity(UDVA), uncorrected intermediate visual acuity(UIVA), and uncorrected near visual acuity(UNVA)and 3 mo postoperatively were documented. Moreover, defocus curves, visual disturbance phenomena, and spectacle independence rates were recorded at 3 mo postoperatively.RESULTS: At 3 mo postoperatively, no statistically significant differences were observed in UDVA among the three groups(P>0.05). A comparison of UIVA showed superior results in the Symfony and PanOptix groups compared to the ReSTOR group(all P<0.01). The UNVA of both the ReSTOR and PanOptix groups outperformed the Symfony group(all P<0.01). The defocus curves indicated that in the intermediate vision range(-1.00 to -1.50 D), the Symfony group exhibited better performance than the ReSTOR group(P<0.05); while in the near vision range(-2.50 to -3.50 D), the ReSTOR group was superior to the Symfony group(P<0.05). The PanOptix group demonstrated superior visual acuity in the near vision range(-2.00 to -3.50 D)compared to the Symfony group(P<0.05)and was also superior in the intermediate vision range(-1.00 to -2.00 D)compared to the ReSTOR group(P<0.05). No significant differences were observed in the incidence of glare or halo and binocular interference phenomena among the three groups(P>0.05). The PanOptix and ReSTOR groups exhibited a higher spectacle independence rate compared to the Symfony group(P<0.0167).CONCLUSION: Compared to Symfony extended range of vision IOL and ReSTOR multifocal IOL, PanOptix trifocal IOL offers a balanced approach to distance, intermediate, and near visual acuity, without a high incidence of glare and halo, and with a higher spectacle independence rate. Caution is still advised when considering monocular implantation with presbyopia-correcting IOLs.

RESUMEN

AIM:To investigate the changes of retinal microcirculation after phacoemulsification and the influencing factors of visual acuity.METHODS: Retrospective analysis. A total of 264 cataract patients(264 eyes)who underwent phacoemulsification in our hospital from January 2022 to December 2022 were selected as the study objects. Patients were divided into < 0.3 group(66 eyes)and ≥0.3 group(198 eyes)according to the recovery of best corrected visual acuity(BCVA)at 3 mo after surgery. The changes of retinal microcirculation indexes were compared before and after treatment. Logistic regression and LASSO regression models were used to screen the influencing factors of postoperative BCVA. A nomogram prediction model of postoperative BCVA was constructed and verified. A restricted cubic spline Logistic regression model was established to analyze the dose-response relationship between end-diastolic velocity(EDV), peak systolic velocity(PSV)and the risk of BCVA recovery.RESULTS: At 3 mo postoperatively, EDV and PSV were significantly improved compared with those before treatment, and resistance index(RI)levels were significantly lower than those before treatment(all P<0.05). Preoperative EDV, PSV, aqueous humor cell grade, fundus lesion grade, advanced age and Emery grade were influencing factors for poor BCVA recovery after phacoemulsification in cataract patients(P<0.05). The AUC before and after validation of the nomogram model by Bootstrap method were 0.869(95%CI: 0.815-0.903)and 0.866(95%CI: 0.802-0.895), respectively. The sensitivity was 88.36% and 88.27%, and the specificity was 91.82% and 91.78%, respectively. Restricted cubic spline model analysis showed no nonlinear dose-response relationship between EDV and PSV levels and the risk of poor BCVA recovery in either male or female(P>0.05).CONCLUSION: After phacoemulsification, retinal microcirculation in cataract patients improved significantly. EDV, PSV, aqueous humor cell grade, fundus lesion grade, advanced age and Emery grade are all factors influencing poor BCVA recovery after cataract surgery.

RESUMEN

ObjectiveTo understand the visual environment sanitation in primary and secondary school classrooms in Minhang District, Shanghai, and to investigate the factors affecting the decline in students’ visual acuity. MethodsIn 2020, among all the primary and secondary schools in Minhang District, according to different types of classroom structures, levels, orientations, unilateral lighting, and bilateral lighting, a total of 20 231 students from 674 classrooms in primary and secondary schools were selected through stratified cluster sampling. Each monitored classroom was regarded as a research unit, and the naked eye vision of students was tested using a standard logarithmic vision chart during the 2020 and 2021 academic years. The change in average visual acuity between the two measurements was taken as the dependent variable, and the visual environment indices were considered as independent variables. The relationship between classroom visual environment indices and the decline in students’ naked eye vision was analyzed. ResultsThe qualification rates for per capita classroom area, window to floor area ratio, average illuminance of the blackboard, uniformity of blackboard illuminance, average illuminance of the desks, uniformity of desk illuminance, lighting coefficient of classroom, blackboard reflection ratio, back wall reflection ratio, and distance between lamps and desks were 79.53%, 88.58%, 46.74%, 70.33%,64.69%, 80.86%, 71.81%, 20.30%, 1.63%, and 97.53%, respectively. The average naked eye vision of primary and secondary school students in the 2020 academic year was 4.75±0.37 (right eye) and 4.76±0.37 (left eye), while in the 2021 academic year, it was 4.70±0.39 (right eye) and 4.71±0.38 (left eye). There was a significant decrease in the naked eye vision of primary and secondary school students in the 2021 academic year compared to that of 2020 (P<0.05). Univariate linear regression analysis showed a significant correlation between grade, average illuminance of the blackboard, average illuminance of the desks, uniformity of desk illuminance, lighting coefficient, and distance between lamps and desks, and the decline of students’ naked eye vision within one academic year (P<0.05). After adjusting for grade, multiple linear regression analysis revealed a significant correlation between the average illuminance of the blackboard, uniformity of desk illuminance, lighting coefficient, and distance between lamps and desks, and the decline of students’ naked eye vision within one academic year (P<0.05). Higher average illuminance of the blackboard, uniformity of desk illuminance, lighting coefficient, and distance between lamps and desks were protective factors for naked eye vision. ConclusionThe visual environment of primary and secondary school classrooms in Minhang District is a matter of concern. Average illuminance of the blackboard, uniformity of desk illuminance, lighting coefficient, and distance between lamps and desks are related to the degree of visual impairment in students over a year.

A total of 150 patients with painless gastroscopy from March 2021 to December 2022 in Shanxi Yuncheng Central Hospital were selected by convenience sampling in this study, they were assigned to control group, water group, and soda water group by random digits table method, each group contained 50 patients. All patients received oral administration of pronase + dimeticone + sodium bicarbonate solution. In addition, control group: prohibited from drinking 4 hours before examination; water group: drinking 200 ml of pure water 2 hours before examination; and soda water group: drinking 200 ml of soda water 2 hours before examination. The clarity score of gastric mucosa and the detection rate of small lesions were compared among the three groups. Results:There were 28 males and 22 females in the control group, aged (47.62 ± 13.83) years old. There were 30 males and 20 females in the water group, aged (44.68 ± 13.61) years old. There were 24 males and 26 females in the soda water group, aged (46.92 ± 12.79) years old. The difference of esophagus, gastric body, gastric antrum and total mucosal clarity scores among the three groups were statistically significant ( F values were 3.68-25.75, all P<0.05). Multiple comparison showed that the esophagus, gastric antrum and total mucosal clarity scores were (1.87 ± 0.58), (1.37 ± 0.34), (6.72 ± 0.92) points in the control group, which were higher than (1.47 ± 0.41), (1.18 ± 0.31), (5.97 ± 0.86) points in the water group, and (1.42 ± 0.41), (1.02 ± 0.22), (5.50 ± 0.79) points in the soda water group, the differences were statistically significant ( t values were 2.67-5.95, all P<0.05). The gastric antrum and total mucosal clarity scores in the water group were higher than in the soda water group, the differences were statistically significant ( t=7.11, 2.71, both P<0.05). The gastric body mucosal clarity score was (1.98 ± 0.74) points in the control group, which was higher than (1.64 ± 0.54) points in the soda water group, the difference was statistically significant ( t=2.66, P<0.05). The gastroscopy examination time and flushin times were (135.20 ± 21.60) s and (1.37 ± 0.43) times in the control group, while (115.52 ± 14.74) s, (0.90 ± 0.29) times and (107.48 ± 13.02) s, (0.62 ± 0.23) times in the water group and soda water group, the control group was higher than the water group and the soda water group, and the water group was also higher than the soda water group, the differences were statistically significant ( t values were 2.38-11.40, all P<0.05). However, there was no statistically significant difference in the detection rate of small lesions among the three groups (all P>0.05). Conclusions:Drinking soda water 2 hours before painless gastroscopy can significantly improve the clarity of patients′gastric mucosa, shorten the examination time and reduce flushing times, but it does not improve the detection rate of small lesions.

RESUMEN

BACKGROUND:The landing test can effectively observe the biomechanical indexes of the lower limbs of patients with chronic ankle instability, analyze the balance adjustment strategies that affect the stability of chronic ankle instability posture, and provide a valuable reference for chronic ankle instability patients to avoid re-injury and clinical rehabilitation after injury. OBJECTIVE:To analyze the effects of vision on the balance of chronic ankle instability and the changes in balance adjustment strategies by observing the kinematics and kinetics of chronic ankle instability patients during landing with and without vision. METHODS:Totally 31 patients with chronic ankle instability landed on a single leg from a 40 cm platform with their eyes open and closed. The dynamic and kinematic data during the landing process were collected using a three-dimensional motion capture system. The difference in landing between the healthy and the ill side legs under the two conditions was compared by paired t-test. RESULTS AND CONCLUSION:(1)Dynamic indexes:The relative value of the peak ground reaction force in the front, rear and vertical directions of the affected ankle joint when eyes were closed was greater than that when eyes were open, and the peak ground reaction force in the vertical direction appeared later than that when eyes were open(P<0.05).(2)Kinematics indexes:The flexion angle of the knee of the healthy and affected sides when touching the ground was greater under the condition of closed eyes than under the condition of open eyes(P<0.05).The maximum flexion angle and feedforward angle of the affected knee and ankle were greater than those of the affected side with eyes open(P<0.05).The time of landing stability was longer than that of eye opening(P<0.05).(3)It is indicated that under the condition of removing visual input, the peak of ground reaction force increases, the contact angle joint increases, and the time to stabilization of chronic ankle instability is prolonged during landing in patients with chronic ankle instability, which increases the probability of landing injury.Visual maintenance of the balance of chronic ankle instability patients cannot compensate for the vestibular sense and proprioception.

RESUMEN

Objective:To monitor intra-fractional set-up errors in tumor radiotherapy using a real-time intelligent capture system for precision displacement. Methods:A simulated radiotherapy environment was created in both the laboratory and the treatment room. A three-axis ( xyz) displacement platform (LD60-LM) and dial gauges were used as displacement measurement tools. Moreover, a real-time intelligent capture system for precision displacement was developed for displacement monitoring. With 23 patients treated with radiotherapy enrolled in this study, the above system was employed to monitor their intra-fractional set-up errors in fractionated radiotherapy. Descriptive analyses were conducted on the deviations between the data captured by cameras and the actual displacement, obtaining the mean values and standard deviation. Results:The monitoring calibration data from the laboratory revealed displacement differences of ≤ 0.5 mm within 20 mm and a maximum displacement difference of 1.47 mm for 50 mm. In contrast, the calibration result from the treatment room exhibited deviations of ± 0.2 mm on the y- z axes, as displayed by both the left and right cameras, and ± 0.31 mm on the x- z axes, as displayed by the middle camera. During 37 radiotherapy sessions in 23 patients, the monitoring result from the middle camera revealed five deviations exceeding the threshold of 5 mm, with the maximum deviation duration and displacement of 57.2 s and 9.24 mm, respectively. Conclusions:The real-time intelligent capture system for precision displacement based on machine vision can achieve real-time monitoring of set-up errors during tumor radiotherapy. Nevertheless, further improvements and service testing are necessary for this system.

RESUMEN

Abstract@#Prevention and control of myopia is the key to delaying its occurrence and progression. Understanding the causes of myopia and its related mechanisms is conducive to better prevention and control. The acceleration of urbanization and increased time in indoor environments have become significant factors for the lack of highfrequency visual information, which constitutes new etiological considerations. Therefore, the study focuses on the research progress on the association between highfrequency visual information and myopia in different scene environments, and further explores the etiology of insufficient highfrequency visual information in myopia from the perspectives of contrast sensitivity, ON-OFF visual pathways, and gene expression, so as to provide reference for carrying out targeted myopia prevention and control measures in different scenarios.

RESUMEN

AIM: To analyze the abnormal refractive status of infants and young children aged 6 to 48 months, and to provide basis for the correction of ametropia and the early prevention and treatment of amblyopia.METHODS: Infants and young children aged 6 to 48 months were examined for refraction by Spot vision screener for natural optometry. Clinical data of infants and young children with refractive abnormalities were collected, Ciliary muscle paralysis agent was used for retinoscopy and optometry, and the results were statistically analyzed.RESULTS: A total of 168 cases(336 eyes)with abnormal Spot refractive outcomes were collected, with a high proportion of hyperopia and astigmatism abnormalities, 38.4% and 28.6%, respectively, while the proportion of myopia was low(12.2%). There were 90 cases of anisometropia(≥1.00 D), among which 41 cases(45.6%)were astigmatic anisometropia, 33 cases(36.7%)were hyperopic anisometropia, and 16 cases(17.8%)were myopic anisometropia, accounting for the least proportion. A total of 109 infants and young children with Spot refractive abnormalities completed ciliary muscle paralysis retinal optometry. The analysis of the difference and correlation between Spot diopter and post ciliary muscle paralysis optometry results showed that the difference in astigmatism was 0.34±0.64 D(P<0.001), the difference in hyperopia was -2.10±1.27 D(P<0.001), and the difference in myopia was -0.43±0.91 D(P=0.023). Although there was a statistical difference between the two results, astigmatism, hyperopia, and myopia were highly positively correlated, respectively(r=0.694, 0.762, 0.909).CONCLUSION: The main refractive abnormalities in infants and young children aged 6 to 48 months are astigmatism, hyperopia, and anisometropia, with fewer abnormalities in myopia. For screening abnormalities, further ciliary muscle paralysis agent retinoscopy and optometry should be performed, and glasses correction should be given to effectively prevent refractive amblyopia in infants and young children.

RESUMEN

“The Kampo Future Vision Study Group responsible for health and medical care of people” was established in 2016 with the aim of discussions to resolve various problems related to Kampo, and issued six proposals as important items. Among these proposals, the emphasized themes that are directly linked to clinical practice are “Kampo as a supportive treatment for cancer” and “Kampo as a countermeasure against frailty in the elderly.” The Japan Society of Oriental Medicine decided on a policy to be involved in these themes and established the Proposal-Discussing Committee (later absorbed into the Policy Advisory Committee). At first, the committee asked experts to present the research status of Kampo medicine as supportive care for cancer and as countermeasures against frailty. The committee, then, decided to lead clinical research by itself on frailty treatment with Kampo medicine. In such clinical research, evaluation criteria for judging therapeutic effects are necessary. Therefore, the existing well-defined diagnostic criteria were combined to create a Kampo frailty score. This article explains the Kampo frailty score, citing the supporting literature.

RESUMEN

Congenital color vision deficiency (CVD) is a form of sensory diversity found in as many as one in 40 people, regardless of gender. A key characteristic of CVD is that it is not externally apparent, and those who require consideration often do not request it. Therefore, it is necessary to be aware that information and instructions conveyed through color coding and color names may not be accurately received by those affected. Addressing diverse color vision needs is not only an educational consideration for medical students with color blindness but is also essential for all medical personnel in their daily communication within the medical team and between physicians and patients. Moreover, for those aspiring to become physicians and protect people’s health, reflecting on the historical eugenic social response to CVD provides an opportunity to consider how to prevent genetic discrimination in a society that values diversity.

RESUMEN

As society advances and living standards improve, there is a growing emphasis on the impact of mental health on illness and the interaction between them. Diabetes mellitus is one of the most common underlying diseases, and it is often accompanied by depression and anxiety. There are also many complications of diabetes, such as diabetic retinopathy(DR). DR is the leading cause of vision loss in diabetes. Vision loss inevitably increases anxiety and/or depression, which in turn may directly or indirectly affect the treatment or progression of patients with DR. This article reviews how to determine the anxiety and depression status of patients with DR, related assessment tools and methods, and their interaction with the treatment of DR. The interaction of anxiety-depressive states with DR treatment was also discussed. This review aims to raise awareness of the mental health of patients with DR, enhance doctor-patient communication and build doctor-patient trust, thus enhancing treatment adherence and clinical efficacy for individuals with DR and helping them to improve the quality of life.
