RÉSUMÉ

Traditional Chinese medicine(TCM)plays a unique role in preventive treatment of disease,treatment of chronic disease and recovery.Integrating modern science and technology and methods to carry out integrated and innovative research will help to bring the value of traditional Chinese medicine into full play.In recent years,with the rapid development of digital therapeutics such as artificial intelligence and wearable devices,digital therapeutics have emerged.Under the guidance of TCM theory,TCM integrates digital therapeutics technology methods,personalized treatment of patients through advanced program-driven software or combined with hardware devices,and improves patient compliance,gives full play to the advantages of TCM.This paper uses CiteSpace to visualize the research hotspots of digital therapeutics,combined with the literature and the website data of the Food and Drug Administration and the National Medical Products Administration,to sort out the application of digital therapeutics in the prevention,treatment and recovery of diseases,analyze the key technologies and core theories in the current application of digital therapeutics.And taking TCM pentatotherapy,kinesiatrics and aromatherapy as examples,this paper discusses the application forms of digital therapeutics under the guidance of TCM theory,and provides a preliminary paradigm for TCM digital therapeutics.

Objective:To explore the effect of Internet + technical liaison service mode on the postoperative continuing nursing of elderly patients with osteoporotic hip fractures.Methods:A total of 92 elderly patients with osteoporotic hip fractures who were admitted into in Orthopaedics Department of the First Affiliated Hospital of Soochow University from July 2018 to December 2019 were selected, and randomly divided into intervention group and control group by random number table method, with 46 cases in each group. The control group was given routine follow-up outside hospital, while the intervention group was given the continuing nursing led by the orthopedic specialist nurses based on the Internet + Technology home orthopedic care platform. All patients were followed up to 6 months after operation, and the differences of Harris Hip Score (Harris), Numeric Rating Scales (NRS), Barthel Index, Short Form 36-item Health Survey (SF-36) between the two groups were compared.Results:The Harris scores were 49.74 ± 4.28, 76.59 ± 4.33 and 90.78 ± 2.61 in the intervention group, and 46.17 ± 3.85, 74.26 ± 4.24 and 88.65 ± 2.17 in the control group in 1, 3 and 6 months after operation, respectively. The differences were statistically significant ( t=-4.20, -2.60, -4.26, all P<0.05). The NRS scores in the intervention group were 1.33 ± 0.47, 0.83 ± 0.38 and 0.76±0.43 in 1, 3 and 6 months after operation respectively, while the NRS scores in the control group were 1.61 ± 0.54, 0.96 ± 0.42 and 0.84 ± 0.38, respectively. The difference in pain scores between the two groups was statistically significant only at 1 month after operation ( t=2.68, P<0.05). The Barthel Index in the intervention group were 61.74 ± 8.90, 93.80 ± 5.29 and 98.26 ± 2.83 in 1, 3 and 6 months after operation respectively, while those of the control group were 60.33 ± 5.81, 91.09 ± 7.52 and 97.83 ± 3.10, respectively. The difference in Barthel Index between the two groups was statistically significant only in 3 months after operation ( t=-2.00, P<0.05). The scores of SF-36 in the intervention group were 93.73 ± 3.89, 100.26 ± 3.77 and 107.50 ± 3.56 at 1, 3 and 6 months after operation respectively, while those of the control group were 92.67±3.42, 97.71±2.67 and 103.68±2.83, respectively. The difference in SF-36 scores between the two groups was statistically significant only at 3 and 6 months after operation ( t=-3.74, -5.71, P<0.05). Conclusions:The continuing nursing based on multi-disciplinary team cooperation in hospital + Internet + Technology home orthopaedic nursing platform led by orthopaedic specialist nurses can improve joint function, relieve pain and improve quality of life of elderly patients with osteoporotic hip fractures after operation.

Objective To observe the clinical effect of isosorbide mononitrate combined with metoprolol in the treatment of coronary heart failure.Methods From January 2015 to May 2017,100 patients with coronary heart failure treated in the Second People's Hospital of Yuyao were randomly divided into two groups according to the digital table,with 50 cases in each group.The conventional group was treated with conventional therapy.The observation group was treated with isosorbide mononitrate combined with metoprolol on the basis of conventional treatment for 30 days.The clinical efficacy,cardiac function index and plasma BNP level were compared between the two groups.Results The total effective rate of the observation group was 96.0% (48/50),which was higher than 78.0% (39/50) in the conventional group(x2 =7.161,P < 0.01).After treatment,the cardiac drainage index(CI) [(3.14 ± 0.53) L · min-1 · (m2)-1],cardiac output blood volume (CO) [(5.62 ± 0.95) L/min],each stroke volume (SV) [(79.24 ± 2.56) mL],left ventricular ejection fraction (LVEF) [(61.62 ± 4.95) %] in the observation group were better than those in the conventional group[(2.41 ±0.45) L · min-1 · (m2)-1,(4.79 ± 0.23) L/min,(72.30 ± 2.78)mL,(53.79 ±4.23)%,t =7.043,5.696,12.318,8.066,all P <0.05].The BNP level of the observation group was (172.17 ± 10.36) pg/mL,which was lower than (215.47 ± 12.48) pg/mL of the conventional group (t =17.908,P < 0.05).Conclusion The treatment of coronary heart failure with isosorbide mononitrate and metoprolol can improve the clinical effect and improve cardiac function.

Objective To discuss the relationship between the injury patterns of medial patellofemoral ligament(MPFL) and injury degrees of vastus medialis obliquus(VMO) after acute patellar dislocation(PD). Methods Knee joint MRI was performed in 79 patients with acute PD. Images were acquired and evaluated using standardised protocols. Injury patterns of MPFL were grouped by severity (partial tear and complete tear)and location (isolated patellar-side tear (PAT), isolated femoral-side tear (FEM) and combined tear (COM) ) for analysis of the prevalence of VMO injury. The VMO elevation was calculated on sagittal and coronal planes. Results The prevalence rates of MPFL tear and VMO lesion were 96.2%(76/79)and 54.4%(43/79)after acute PD.The prevalence rate of VMO lesion was 40.6%(13/32)and 68.2%(30/44)in the partial and complete MPFL tear subgroups,respectively.Complete MPFL tear subgroup showed higher prevalence rate of VMO lesion when compared with partial tear subgroup(χ2=5.727, P=0.017). The prevalence rates of VMO lesion in the PAT, FEM and COM subgroups were 31.8% (7/22), 67.7% (21/31) and 70% (14/20), respectively. There were statistically significant differences among them(χ2=8.549,P=0.014).About the mean VMO elevation,there were statistically significant differences between the complete and partial MPFL tear subgroups, the FEM and PAT subgroups, and the COM and PAT subgroups (P=0.00). Conclusions Compared with partial MPFL tear, complete tear predisposes to VMO lesion and has a higher elevation of the torn VMO after acute PD.The femoral-sided and combined MPFL tears predispose to VMO lesion and have higher elevations of the torn VMO.

Objective To investigate the clinical efficacy of amoxicillin combined with pectin in the treatment of chronic atrophic gastritis. Methods According to the different treatment methods will be January 2015 to March 2017 patients with chronic atrophic gastritis in our hospital 80 cases groups:control group with clarithromycin+amoxicillin+bismuth pectin bismuth pectin treatment,observation group treatment;detailed records of two groups before and after treatment(pathological atrophy,intestinal metaplasia,dysplasia,chronic the inflammatory activity and the integral changes),the levels of inflammatory factors(IL-2/IL-6/CRP)incidence level changes and adverse reaction,the obtained related data for comparative analysis. Results Amoxicillin and bismuth pectin(observation group)in treatment of chronic atrophic gastritis is better than clarithromycin+bismuth pectin(control group)treatment,patients with pathological score decreased than the control group,the levels of inflammatory factors is better than the control group,the incidence rate of adverse reaction was lower than the control group,the difference was statistically significant(P<0.05). Conclusion Chronic atrophic gastritis patients choose amoxicillin and bismuth pectin treatment effect significantly,can greatly reduce the pathological integral,improve clinical symptoms,and can reduce the level of inflammatory factors in patients with a low incidence of adverse reactions,it is worthy of widely used to treat patients with chronic.

Objective To evaluate the change situation of cognitive function in the patients with chronic renal failure (CRF) complicating obstructive sleep apnea syndrome(OSAS) .Methods Sixty‐two inpatients with non‐dialysis CRF in our hospital from September 2013 to January 2015 were selected and divided into the group O (complicating OSAS ,31 cases) and group N(without complicating OSAS ,31 cases) ,and contemporaneous 33 individuals undergoing the physical examination were taken as the control group(group C) .The cognitive function was evaluated by the Montreal cognitive assessment (MoCA) .The apnea hypopnea index (AHI) and lowest pulse oxygen saturation(LSpO2 ) at night were measured with a portable sleep monitor .The comparative analysis was performed .Results In the group O ,there was 14 cases of patients(45 .16% ) whose MoCA total score was less than 26 points , while there was only 3 cases(9 .68% ) in the group N ,the difference was statistically significant (P<0 .05) .The MoCA total score , scores of visuospatial and executive function ,attention ,delayed recall and orientation in the group O and N were lower than those in the group C ,while the MoCA total score ,attention and delayed recall in he group O were lower than those in the group N ,the score of language was lower than that in the group N and C ,the differences were statistically significant (P< 0 .05) .The MoCA total score was negatively correlated with serum creatinine and AHI (r= -0 .374 ,r= -0 .408 ,P<0 .05) ,and positively correlated with the GFR level and night LSpO2 (r=0 .382 ,r=0 .519 ,P<0 .05) .Conclusion The patients with CRF complicating OSAS increase the risk of cognitive impairment .

<p><b>OBJECTIVE</b>To compare the clinical efficacy difference in dysantonomia between transcutaneous electrical stimulation at Renying(ST 9) combined with stellate ganglion block(SGB) and simple SGB.</p><p><b>METHODS</b>Sixty patients in accord with the diagnostic criteria of dysantonomia were randomly divided into two groups,30 cases in each group. In the observation group,transcutaneous electrical stimulation at Renying(ST 9) combined with SGB were adopted; in the control group,simple SGB was applied. In the two groups, treatment was used three times a week,and nine treatments were considered as one course. There was an interval of one week between courses,and two courses were treated. Total seven weeks were required. Scores were evaluated according to subjective symptoms before treatment,one month and three months after treatment in the two groups.</p><p><b>RESULTS</b>The scores of subjective symptoms were not statistically different before treatment in the two groups(P>0. 05). The scores of subjective symptoms one month and three months after treatment were all lower than those before treatment(all P< 0. 01), and subjective symptoms scores in the observation group were lower than those in the control group(both P<0. 01).</p><p><b>CONCLUSION</b>Transcutaneous electrical stimulation at Renying(ST 9) combined with SGB could obviously enhance the clinical effects for dysantonomia, and the control and improvement for clinical symptoms are apparently superior to simple SGB.</p>

Objective To explore the risk factors of schistosomiasis cirrhosis esophageal varices hemorrhage. Methods A total of 113 cases of schistosomiasis cirrhosis esophageal varices hemorrhage and 128 schistosomiasis cirrhosis esophageal varices patients without hemorrhage were selected and their relevant factors were analyzed statistically. Results There were significant differences between the two groups in the prothrombin time,portal vein diameter,degree of esophageal varices,and varicose vein tumor-like lesions(P<0.01). Conclusion The risk factors of schistosomiasis cirrhosis esophageal varices hemorrhage are the prothrombin time,portal vein diameter,degree of esophageal varices,and varicose vein tumor-like lesions.

Objective To investigate the changes in tumor necrosis factor-α(TNF-α) and plasminogen activator inhibitor(PAI) in the presence of obstructive sleep apnea syndrome(OSAS) in patients with type 2 diabetes mellitus,and to investigate if that 2 inflammatory factors is associated with hypoxemia during sleep.Methods Fiftyfour patients with type 2 diabetes mellitus hospitalized for glycemic control were subgrouped into 27 patients with OSAS (group O) and 27 patients without OSAS (group N).26 people without diabetes and OSAS were selected from a health check-up program to serve as control group (group C).Serum TNF-α and PAI levels were measured by commercial ELISA kits.Apnea hypopnea index (AHI),the lowest pulse oxygen saturation (LSpO2) at night were detected by portable sleep monitor.The relativity regarding TNF-α,PAI,homeostasis model assessment for insulin resistance (HOMA-IR),fasting blood glucose(FBG),AHI,LSpO2,and age was discussed.TNF-α and PAI were regard as dependent variables,and FBG,HOMA-IR,AHI,LSpO2,and age as independent variables in multivariate stepwise linear analysis.Results The levels of HOMA-IR,TNF-α,PAI in group O [2.69 ± 1.53,(1.97 ±0.13) ng/ml,and (2.27 ± 0.85) ng/ml] were higher than the those in other 2 groups,and those in group N [1.70 ± 0.88,(1.09 ±0.29) ng/ml,and (1.59 ± 0.13) ng/ml] were higher than group C [1.15 ± 0.67,(0.73 ± 0.19) ng/ml,and (0.89 ±0.25) ng/ml].The levels of FBG in the 2 diabetic groups were higher than those in group C.The level of AHI in group O was higher than that in group N and group C,while LSpO2 in group O was lower than that in group N and group C.Spearman univariate analysis showed that TNF-α and PAl both were associated positively with HOMA-IR,FBG,and AHI,while negatively with LSpO2.Multivariate stepwise linear analysis denoted that TNF-α was independently associated with AHI,FBG,while PAI was independently associated with AHI and HOMA-IR.Conclusions Patients with diabetes and OSAS manifest raised level of chronic inflammatory activity,and therefore,OSAS might be implicated in the pathogenesis of chronic complication of patients with type 2 diabetes.

ObjectiveTo investigate the effect of isoflurane preconditioning on glutamate-induced apoptosis in rat neuronal PC12 cells.MethodsThe PC12 cells were cultured for 5 d with nerve growth factor in vitro.The cells were seeded into 6-cm-diameter culture dishes (3 ml/dish) or 6-well plates (2 ml/well) with the density of 5 × 104/ml and randomly divided into 4 groups (n =18 each): normal control group (group C); glutamate group (group G) ;glutamate + isoflurane group (group GI) and glutamate + isoflurane + xestospongin C (an antngon of inositol trisphosphate receptors) group (group GIX).The neuronal PC12 cells were exposed to glutamate 500 μmol/L in groups G,GI and GIX.The neuronal PC12 cells were exposed to 1.2% isoflurane for 2 h in groups GI and GIX.Xestospongin C was added to the culture medium immediately before isoflurane preconditioning.Glutamate was added to the culture medium at 10 min after isoflurane preconditioning in groups GI and GIX.The cells were collected from six dishes or wells in each group after being incubated with glutamate for 20 min.The apoptosis and mitochondiral membrane potential (MMP) were assessed by flow cytometry.Intracellular Ca2+ concentration ([ Ca2+ ] i)was detected by confocal fluorescence microscopy.ResultsCompared with group C,the apoptotic rate and [Ca2+ ]i were significantly increased and MMP was decreased in groups G and GIX ( P < 0.01 ),but there was no significant difference in the variables mentioned above in group GI (P > 0.05).Compared with group G,the apoptotic rate and [ Ca2 + ]i were significantly decreased and MMP was increased in groups GI and GIX ( P < 0.05 or 0.01).Compared with group GI,the apoptotic rate and [Ca2+ ]i were significantly increased and MMP was decreased in group GIX ( P < 0.01 ).ConclusionIsollurane preconditioning can inhibit apoptosis in rat neuronal PC12 cells by activating inositol trisphosphate receptors,inhibiting Ca2+ release from the endoplasmic reticulum and increasing MMP.

The abnormality of serine/threonine kinase Aurora-A is seen in many types of cancers. Although in physiological context it has been shown to play a vital role in cellular mitosis, how this oncogene contributes to tumorigenesis remains unclear. Here we demonstrate that Aurora-A overexpression enhances both the expression level and transcriptional activity of c-Myc. The inhibition of c-Myc expression by RNA interference significantly impaired the oncogenic potential of Aurora-A, resulting in attenuated cellular proliferation and transformation rates as well as fewer centrosomal aberrations. Furthermore, downregulation of c-Myc effectively overcame Aurora-A-induced resistance to cisplatin in esophageal cancer cells. Taken together, our results suggest an important role for c-Myc in mediating the oncogenic activity of Aurora-A, which may in turn allow for future targeting of c-Myc as a potential therapeutic strategy for tumors with Aurora-A overexpression.

Objective To investigate the effects of different concentrations of isoflurane on viability in rat primary cortical neurons. Methods Primary cortical neurons were isolated from neonatal Wistar rats (less than 24 h after birth) and exposed to 0.6 %, 1.2 % and 2.4 % isoflurane for 1, 2, 4, 8, 12 or 24 h, cell viability was evaluated by MTT reduction and LDH release assays. Changes in intracellular [Ca2+]i were detected by real-time confocal microscopy after the neurons were exposed to different concentrations of isoflurane. Results Exposure to 0.6% isoflurane for 24 h significantly increased viability of the primary rat cortical neurons as shown by decrease in LDH release and increase in MTT and elevation of peak [Ca2+]i. Exposure to 1.2% isoflurane for 12 or 24 h and to 2.4% isoflurane for 8, 12 or 24 h significantly reduced viability of cortical neurons associated with high and fast elevation of peak [Ca2 +] i. Conclusion Exposure to 0.6 % isoflurane increases viability of the primary rat cortical neurons. Exposure to 1.2% isoflurane and to 2.4% isoflurane decreases cell viability and the mechanism may be related to the changes in calcium concentrations in the neurons.

Objective To evaluate the applicability of Mini-Nutritional Assessment(MNA),Short-Form Mini-Nutritional Assessment(MNA-SF)in the malnutrition screening of patients with Alzheimer's disease(AD).Methods MNA and MNA-SF were performed in 267 AD patients.The correlation between the screening results were compared.Results According to MNA,the rates of malnutrition,potential malnutrition,and normal nutritional status were 59.7%,20.9%,and 19.4%,respectively.The sensitivity was 73.4%and the specificity was 60.1%.According to MNA-SF,the rates of malnutrition and normal nutritional status were 70.4%and 29.6%.The sensitivity was 50.6%and the specificity was 92.4%.The screening results of these two tools were significantly correlated(r=0.924,P<0.01).Conclusions MNA and MNA-SF are reliable tools for evaluating the nutritional status of AD patients.While MNA-SF is more convenient and feasible,the combination of these two tools may be helpful.

This article began from the characteristic and research method of the syndrome in Traditional Chinese Medicine(TCM)and proteomics,analyzed the superiority and the feasibility of introduction of proteomics into the study of syndrome essence.The author thought that proteome and syndrome in TCM were amazing similar in the aspects of integrity,developments,space and complexity,there was maybe some kind of inner link and the corresponding relations between them.Therefore, the proteomics had the special superiority when it was compared with the general microscopic parameter in study of syndrome essence,also even more accord with the characteristics of syndrome in itself.This article also generalized the research achievement of syndromic proteomics in TCM in recent years from the aspects of specification,thought and method,the relativity of concrete syndrome type and proteome.This article demonstrated the magnificent prospect of proteomics in the study of the syndrome essence of the TCM.

Objective To investigate the effect of dexamethasone with subdose ketamine on restraining the pulling-dragging reflex during appendectomy. Methods 114 ASA grade Ⅰ~Ⅱpatients of appendectomy undergoing T 11 ~L 1 epidural anesthesia were randomly divided three groups,each group containing 38 cases. Group Ⅰ recieved the dexamethasone 10mg and ketamine 0 5mg/kg by venous injection before opening abdominal cavity, groupⅡ received pethidine 50mg and droperidol 2 5mg before begining operation, and group Ⅲ without any drugs served as control. The pulling-dragging reflex of appendix, the sedative state at pulling-dragging appendix and the change of pulse oxygen saturation (SpO 2) or mean arterial pressure (MAP) were recorded and analyzed. ObjectTo study comparatively the transdermal absorption of baicalin per five kinds of animal skin. Methods Improved Franz-call was used for the transdermal study, and the transdermal speed constants of baicalin per five kinds of animal skin were compared. Results The transdermal speed constants of baicalin per skin of mice, hairless mice, rabbits, rats, human are (67.31?23.66), (78.35?3.29), (117.35?29.48), (86.25?7.55), and (84.43?11.27) ?g/(cm2?h), respectively. Conclusion The transdermal absorption of baicalin per skin of hairless mice is more similar to that of human skin.

Methods: The uniform design was used for the selection of transdermal enhancers for the baicalin from various proportion of azone, propylene glycol and oleic acid. Results: The transdermal constant of baicalin is maxim when the proportion of is 6%,10%, and 0%, prespectively. Conclusion: The effects of compound transdermal enhancers is better than that of single one.

OBJECTIVE:To study the kinetics of percutaneous permeability of baicalin in nude mice.METHODS:The transdermal constants of baicalin via nude mouse’s skin were determined,and the effects of stratum corneum on the kinetics studied.RESULTS:The kinetic equation of baicalin via nude mouse’s skin was Q=76.07T 1/2 -32.11,while the kinetic equa?tion of baicalin via nude mouse’s skin without stratum corneum was Q=134.25T 1/2 +29.07,and there was baicalin exuded from skin after detachment of patch.CONCLUSION:The kinetics of the patches is accord with the characteristics of frame?work type;the main barrier for baicalin permeance is stratum corneum and the skin without stratum corneum could store drugs.
