Résumé

Implantable cardioverter defibrillators (ICD) are recommended in heart failure with reduced ejection fraction (HFrEF) patients to reduce arrhythmic deaths. This study aimed to identify risk factors associated with mortality within one-year following the ICD. The data from our hospital's electronic database system was extracted for patients who were implanted ICD secondary to HFrEF between 2009 and 2019. Overall, 1107 patients were included in the present analysis. Mortality rate at one-year following the device implantation was 4.7%. In multivariate analysis; age, atrial fibrillation, New York Heart Association classification >2, blood urea nitrogen, pro-brain natriuretic peptide and albumin independently predicted one year mortality

Résumé

Implantable cardioverter defibrillator (ICD) is the most effective measure to prevent sudden cardiac death. However, ICD patients frequently have problems such as decline of exercise tolerance and quality of life, anxiety and depression after operation, which require cardiac rehabilitation (CR). A considerable amount of studies have proved that CR is an effective secondary prevention measure for patients with cardiovascular disease. Its safety and effectiveness have been supported by evidence-based medicine research, which can prevent the recurrence of cardiovascular events, reduce the readmisson rate, and promote healthy behavior and active lifestyle formation of patients, so as to improve the quality of life of patients. This review mainly summarized the necessity, safety, exercise program and economic benefits of CR in ICD patients.

Résumé

Objective:To adopt defibrillation analyzer to detect performance of delivered energy of cardiac defibrillator and to reveal its relevant changing regular, and carry out scientific verification so as to take the detection data to play a role in the maintenance work of equipment.Methods: According to National Metrological technical specification "JJF 1149-2014 Calibration Specification for Cardiac Defibrillators", the FLUKE IMPULSE 7000DP Defibrillator Analyzer was applied to obtain data, and then a comprehensive analysis method with graphs and table was used to analyze these test data.Results: Through analyzed the detection data of delivered energy to grasp the whole performance situation of using equipment and to enhance the cognition of technique and application level for Defibrillator Analyzer.Conclusion: Through analyzes data to find out the changing regular of delivered energy and the difference phenomenon of detection data distribution, and to elaborate the reason of phenomenon or influence factor. Through detects performance to screen out the poorest equipment of delivered energy so as to avoid medical risk that comes from hidden danger of equipment performance in the process of clinical therapy.

Abstract: The transvenous implantable cardiac defibrillator (T-ICD) is currently considered the standard of care for prevention of sudden cardiac death in patients with structural cardiac disease or channelopathies. However, the use of these devices is associated with a significant increase of short and long-term complications, mostly related to intravascular leads. The subcutaneous implantable cardiac defibrillator (S-ICD) is a novel alternative for high-risk patients susceptible to intravascular lead complications, with a similar efficacy as T-ICD. Multiple ongoing clinical trials involving the S-ICD are expected to provide additional information about safety, use and benefits in the clinical setting.


Resumen: El desfibrilador cardiaco implantable transvenoso (DCI-T) se considera actualmente el tratamiento estándar para la prevención de la muerte súbita cardiaca en pacientes con enfermedad cardiaca estructural o canalopatías. Sin embargo, el uso de estos dispositivos se asocia con un aumento significativo de complicaciones a corto y largo plazo, principalmente relacionadas con derivaciones intravasculares. El desfibrilador cardiaco implantable subcutáneo (DCI-S) es una alternativa novedosa para pacientes de alto riesgo susceptibles a complicaciones intravasculares, con una eficacia similar al DCI-T. Se espera que varios ensayos clínicos en curso que involucran al DCI-S brinden información adicional sobre seguridad, uso y beneficios en el entorno clínico.

Abstract Introduction: Cardiomyocytes are more sensitive to stimulatory electrical fields when the latter are applied longitudinally to the cell major axis. In the whole heart, cells have different spatial orientations, which may limit the effectiveness of conventional electrical defibrillation (i.e., shock delivery in a single direction). This article describes the constructive aspects of a portable system for rapidly-switching, multidirectional stimulus delivery, composed of an electrical defibrillator and multielectrode-bearing paddles for direct cardiac defibrillation. Methods: The defibrillator delivers monophasic, truncated monoexponential waveforms with energy up to 7.3 J. Upon selection of the defibrillation modality (unidirectional or multidirectional), shock delivery is triggered through 1 or 3 outputs. In the latter case, triggering is sequentially switched to the outputs, without interval or temporal overlap. Each paddle contains 3 electrodes that define shock pathways spaced by 60°. The system was tested in vivo for reversal of experimentally-induced ventricular fibrillation in healthy swine, using 30- and 20-ms long shocks (N= 4 in each group). Results: The defibrillator delivers identical stimulus waveforms through all outputs in both stimulation modalities. In all animals, successful defibrillation required lower shock energy when 20 ms-long stimuli were applied in 3 directions, compared to a single direction. However, performance was poorer with multidirectional defibrillation for 30 ms-long shocks. Conclusion: The delivery of identical shock waveforms allowed confirmation that multidirectional defibrillation can promote restoration of sinus rhythm with lower shock energy, which may reduce myocardial electrical damage during defibrillation. Nevertheless, increase in shock duration greatly impairs the effectiveness of this defibrillation modality.

To master the performances of the cardiac defibrillator-monitor and enhance its reliability and effi-ciency through quality control. ESA612 electrical safety analyzer and Phase3 defibrillator analyzer were used to test the performances of 56 cardiac defibrillator-monitors, involving in basic items detection, electrical safety detection and performance detection. The qualified items included charging times, internal discharge, ECG monitoring and etc, while the unqualified ones included appearance, electrical safety as well as delay time under synchronous mode. The quality control can improve the safety of the cardiac defibrillator-monitor.

Objective:To use defibrillators calibration detection method, some problems could be find out and analyzed. Moreover, the quality safety of defibrillators calibration can be guaranteed. Methods:According to WSB 64-2003 verification regulation for cardiac defibrillators and cardiac defibrillator-Monitors, and the practical experience of effective detection methods, defibrillation analyzer verification can be used to detect the performance parameters of cardiac defibrillators. Results:In accordance with the effective test method, through regular check, not only can cardiac defibrillators risk be eliminated but also quality safety of clinical use and the life safety of patients will be safeguarded. Conclusion:The quality control can prevent medical accidents that caused by facility loses, at the same time, if we are pay attention to the daily maintenance, the defibrillator can be played a role in emergency time.
