RÉSUMÉ
La patología de piso pélvico es una entidad multifactorial con un conjunto de síntomas ginecológicos, urinarios, fecales y de sensibilidad pelviperineal. La Teoría Integral de la Continencia propone un tratamiento holístico con la reconstrucción de los ligamentos del piso pélvico. La presente técnica propuesta constituye una alternativa quirúrgica que permite dar respuesta de forma global a los síntomas que refieren las pacientes. Método: Reconstrucción de los ligamentos pubouretrales, cardinales y úterosacros con acortamiento de su longitud y colocación de cinta de malla de polipropileno que permita la formación de colágeno y mejore los resultados a largo plazo. Resultados: Se incluyeron 15 pacientes con prolapso de órganos pélvicos, incontinencia urinaria, vulvodinia, nocturia, alteración del vaciamiento vesical y nocturia. Se realizó seguimiento al 1, 3 y 6 años. Se obtuvo diferencia estadísticamente significativa al año en la incontinencia urinaria de esfuerzo, dolor pélvico, alteración del vaciamiento y nocturia y prolapso ( p = 0,33, 0,033, 0,002 y 0,001 respectivamente). En el seguimiento a 6 años se evaluó el 20 % de la muestra inicial, 2 pacientes con recidiva de la alteración del vaciamiento y la incontinencia urinaria, ninguna con recidiva de prolapso. Vulvodinia: Se incluyeron 2 pacientes las cuales no tuvieron el síntoma a los 3 años de seguimiento. Conclusiones: La técnica propuesta es una alternativa para el tratamiento de la patología del piso pélvico y requiere aumentar el tamaño de la muestra para aumentar el aprendizaje de la técnica y tener mayor evidencia estadística de sus resultados a corto y largo plazo(AU)
Pelvic floor pathology is a multifactorial entity with a set of gynecological, urinary, fecal and pelviperineal sensitivity symptoms. The Integral Theory of Continence proposes a holistic treatment with the reconstruction of the ligaments of the pelvic floor. This proposed technique constitutes a surgical alternative that allows a global response to the symptoms reported by the patients. Method: Reconstruction of the pubourethral, cardinal and uterosacral ligaments with shortening of their length and placement of polypropylene mesh tape that allows collagen formation and improves long-term results. Results: 15 patients with pelvic organ prolapse, urinary incontinence, vulvodynia, nocturia, impaired bladder emptying and nocturia were included. Follow-up was performed at 1, 3 and 6 years. A statistically significant difference was obtained at one year in stress urinary incontinence, pelvic pain, impaired voiding, and nocturia and prolapse (p = 0.33, 0.033, 0.002, and 0.001, respectively). At 6-year follow-up, 20% of the initial sample was evaluated, 2 patients with recurrence of impaired voiding and urinary incontinence, none with recurrence of prolapse. Vulvodynia: 2 patients were included who did not have the symptom at 3 years of follow-up. Conclusions: The proposed technique is an alternative for the treatment of pelvic floor pathology. A larger sample is necessary to improve the learning curve of this technique and achieve greater statistical evidence of its outcomes at short and long term(AU)

RESUMEN El leiomioma parauretral es una neoplasia benigna con pocos casos descritos en la literatura. El leiomioma abarca el 5% de los casos de las masas parauretrales, ocupando el cuarto lugar como una de las causas menos frecuentes, después del divertículo parauretral. Se presenta el caso de una paciente de 43 años con antecedente de infección urinaria a repetición, con una masa de 8 cm en pared vaginal anterior de cuatro años de evolución. La cistoscopia fue normal y la ecografía mostró una lesión sólida parauretral. Se realizó resección por colpotomía sin complicaciones, con resultado de patología de leiomioma. Control postquirúrgico y uroflujometría normal. Dentro de las posibilidades diagnósticas ante una masa parauretral, el leiomioma debe considerarse. La sintomatología es variable, siendo los síntomas urinarios los más frecuentes. La ecografía y la resonancia son herramientas fundamentales y el manejo se basa en la resección quirúrgica, generalmente vía vaginal. MÉD.UIS.2021;34(2): 77-82.
ABSTRACT Introduction: Paraurethral leiomyoma is a rare benign neoplasm, with few cases described in the literature. Most of the data on prevalence are based on case series, the leiomyoma covers 5% of the paraurethral masses, occupying the fourth place as one of the less frequent causes, after parurethral diverticulum. Case: 43 years old patient with antecedents of recurring urinary infections and a previous 8 cm mass on the vaginal walls with 4 years of clinical evolution course. Cystoscopy was normal and ultrasound with a solid paraurethral injury. Colpotomy resection was performed without complications, resulting in leiomyoma pathology. Post-surgical control and normal uroflowmetry. Discussion and conclusion: leiomyomas should be considered as a possible diagnosis in case of a periurethral mass presence. Symptomatology is variable. The urinary symptoms are the most frequent as in this case. Ultrasonography and resonance are essential tools treatment are based on surgical resection which is usually vaginally. The objective of this study was to evaluate the feasibility of posterior colpotomy for the surgical treatment of tubal ectopic pregnancy in hemodynamically stable women. We performed a retrospective analysis of medical records obtained over a period of 18 months. Twelve cases were identified, with the following characteristics: mean gestational age, 7.7 weeks; mean serum β-human chorionic gonadotropin level, 7,786 mIU/mL; and greater diameter of the mass, 15–69 mm. Treatment was successful in all cases. Salpingectomy was performed in 10 patients (83.3%) and salpingostomy, in 1 patient. The remaining patient only received peritoneal lavage, as the evidence of ectopic abortion with only a slightly dilated uterine tube was found during surgery. The mean surgical time was 42.5 minutes. In the analyzed cases, posterior colpotomy was found to be a feasible alternative method for the surgical treatment of tubal ectopic pregnancy in hemodynamically stable women.

ABSTRACT Objective: To describe the safety of colpotomy in terms of intra- and post-operative complications in women taken to laparoscopy for the removal of benign masses. Materials and methods: Case series of patients with benign adnexal or uterine masses taken to laparoscopic surgery and removal of the specimen through colpotomy. The patients signed the informed consent. Patients with obliterated posterior cul-de-sac, narrow vagina, absence or uterus, deep endometriosis, inflammatory pelvic disease, masses of less than 4 cm, were excluded. Consecutive sampling was used. Sociodemographic, clinical, qualitative and quantitative variables were studied by means of a descriptive analysis of the data. The research study was endorsed by the ethics committee of the institution. Results: During the study period, 12 procedures for the removal of benign pelvic masses using posterior colpotomy were performed. Mean age was 34.3 years (IQR: 29-39,5). Of these cases, 50% were nulliparous women. Mean surgical time was 127 minutes (IQR: 90-150). There were no conversions to laparotomy, and there were no intra- or postoperative complications. Conclusions: Posterior colpotomy is an option that can be considered in the removal of benign pelvic masses of more than 5 cm in size.
RESUMEN Objetivo: Describir la seguridad de la colpotomía en mujeres llevadas a laparoscopia por masas benignas en términos de complicaciones intra y posoperatorias. Materiales y métodos: Serie de casos que incluyó pacientes con masas anexiales o uterinas benignas, sometidas a laparoscopia operatoria con extracción de la pieza quirúrgica mediante colpotomía, previo consentimiento informado. Se excluyeron pacientes con fondo de saco posterior obliterado, vagina estrecha, ausencia de útero, endometriosis profunda, enfermedad inflamatoria pélvica, masas menores de 4 cm, muestreo consecutivo. Se estudiaron variables sociodemográficas, clínicas, cualitativas y cuantitativas mediante un análisis descriptivo. La investigación fue avalada por el comité de ética médica de la institución. Resultados: Durante el periodo de estudio se realizaron 12 extracciones de masas pélvicas benignas por colpotomía posterior. La mediana de la edad fue de 34,3 años (Rango Intercuartílico - RIC: 29-39,5). El 50 % de las pacientes eran nulíparas. La mediana de tiempo quirúrgico fue de 127 minutos (RIC: 90-150). No se realizó ninguna conversión a laparotomía. No se presentaron complicaciones intra ni posoperatorias. Conclusiones: La colpotomía posterior es una alternativa por considerar en la extracción de masas pélvicas benignas cuando tengan un tamaño mayor a 5 cm.

Objective To evaluate the results of sacrospinous colpopexy surgery associated with anterior colporrhaphy for the treatment of women with post-hysterectomy vaginal vault prolapse. Methods This prospective study included 20women with vault prolapse, PelvicOrgan Prolapse Quantification System (POP-Q) stage≥2, treated between January 2003 and February 2006, and evaluated in a follow-up review (more than one year later). Genital prolapse was evaluated qualitatively in stages and quantitatively in centimeters. Prolapse stage < 2 was considered to be the cure criterion. Statistical analysis was performed using the Wilcoxon test (paired samples) to compare the points and stages of prolapse before and after surgery. Results Evaluation of the vaginal vault after one year revealed that 95% of subjects were in stage zero and that 5% were in stage 1. For cystocele, 50% were in stage 1, 10% were in stage 0 (cured) and 40% were in stage 2. For rectocele, three women were in stage 1 (15%), one was in stage 2 (5%) and 16 had no further prolapse. The most frequent complication was pain in the right buttock, with remission of symptoms in all three cases three months after surgery. Conclusions In this retrospective study, the surgical correction of vault prolapse using a sacrospinous ligament fixation technique associatedwith anterior colporrhaphy proved effective in resolving genital prolapse. Despite the low complication rates, there was a high rate of cystocele, which may be caused by posterior vaginal shifting due to either the technique or an overvaluation by the POP-Q system.
Objetivo Avaliar os resultados da cirurgia de colpofixação sacroespinal associada a colporrafia anterior, para o tratamento de mulheres com prolapso de cúpula vaginal pós-histerectomia. Métodos Estudo prospectivo que incluiu 20 mulheres com prolapso de cúpula em estágio≥2 de acordo com o Sistema de Quantificação do Prolapso Genital (POP-Q) tratadas no período de janeiro de 2003 a fevereiro de 2006 e avaliadas emrevisão tardia (maior que um ano). O prolapso genital foi avaliado qualitativamente em estágios e, quantitativamente, emcentímetros. Como critério de cura, consideramos prolapso em estágio < 2. Para análise estatística, foi utilizado teste de Wilcoxon (amostras pareadas) para comparar os pontos e estágios dos prolapsos antes e depois da cirurgia. Resultados Na cúpula vaginal a avaliação após um ano mostrou 95% em estágio zero e 5% em estágio 1. Cistocele: 50% em estágio 1, 10% em estágio zero (curadas) e 40% emestágio 2. Retocele: trêsmulheres apresentavamestágio 1 (15%), uma emestágio 2 (5%) e dezesseis não apresentavam prolapso posterior. As complicações mais frequentes foramdor no glúteo direito com remissão do sintoma emtodos os três casos após três meses da cirurgia. Conclusões Neste estudo retrospectivo, a cirurgia de correção do prolapso de cúpula pela técnica de fixação no ligamento sacroespinal, associada à colporrafia anterior, se mostrou efetiva na resolução do prolapso genital com baixa taxa de complicações; porém com alta taxa de cistocele - o que pode ter sido decorrente do desvio vaginal posterior pela técnica, ou ainda uma supervalorização pelo sistema POP-Q.

INTRODUCCIÓN: La colposacropexia por vía abdominal abierta (CSPA) es el gold standard para el tratamiento del prolapso de la cúpula vaginal (PCV). OBJETIVO: Mostrar nuestra experiencia, en un Hospital de mediana complejidad, en los primeros siete casos sometidos a un CSPA, reportando las complicaciones intra y post operatorias. MÉTODO: Se realizó un estudio prospectivo descriptivo en nuestras siete primeras pacientes portadoras de un prolapso de cúpula vaginal estadio III o IV (POP-Q) sintomáticas, sin incontinencia urinaria y sometidas a una CSPA, con una malla mixta en el Servicio de Obstetricia y Ginecología del Hospital de Quilpué. RESULTADOS: Se reclutaron 7 pacientes. Ninguna de ellas presentaba complicaciones con riesgo vital peri-operatorio, en un período de seguimiento de 33 meses. En el 100% de las pacientes se logró cura objetiva definida como un POP-Q estadio 0 o I, y buena calidad de vida según el Cuestionario de Impacto del Piso Pélvico (PFIQ-7, versión validada en español). Durante el seguimiento, sólo una paciente presentó exposición asintomática de 5 mm de la malla, resuelta con su resección por vía vaginal. Ninguna paciente requirió de una cirugía por prolapso de órganos pélvicos después de la CSPA. CONCLUSIÓN: Los resultados obtenidos concuerdan con la literatura. La CSPA sigue siendo el gold standard para el tratamiento del PCV, no siendo superada por el momento, por ninguna otra técnica, incluso la colposacropexia laparoscópica. Estimamos que el uso de mallas parcialmente reabsorbibles pueden disminuir la exposición de mallas, sin embargo se requieren de más estudios.
INTRODUCTION: The abdominal sacral-colpopexy is currently considered the gold standard for the management of (CSPA) vaginal vault prolapse (PCV). OBJECTIVE: To report our surgical experience at a county hospital with our first seven abdominal sacral-colpopexy cases. Reporting intra and post-operative complications. METHODS: We conducted a prospective descriptive study involving our first seven cases of patients with symptomatic vaginal vault prolapse stage III or IV (POP-Q) without urinary incontinence. All patients were undergoing CSPA with a partially absorbable mesh in the Obstetrics and Gynecology Department at the Quilpué Hospital. RESULTS: Seven patients were recruited. None of them presented a life threatening complication during a mean follow up period of 33 months. A hundred percent of patients achieved objetive cure defined as POP-Q stage 0 or I and subjective cure defined as a significant improvement in a validated questionnaire (PFIQ-7 spanish version). During patients follow up, only one patient had an asymptomatic 5 mm mesh exposure, resolved with a vaginal resection. None of the patients required surgery for pelvic organ prolapse after the CSPA. CONCLUSION: These results are in agreement with the international literature. The CSPA continues to be the gold standard for the PCV treatment of vaginal vault prolapse and has not been surpassed by either vaginal technique or the laparoscopic sacral-colpopexy. We believe that the use of partially reabsorbable meshes can decrease the rate of mesh exposure, however further studies are required.

OBJETIVO: Relatar o uso da colpotomia para o tratamento de gestações ectópicas. MÉTODOS: Estudo transversal retrospectivo. Foram incluídos todos os casos internados com suspeita clínico-laboratorial de gestação ectópica que não preenchiam critérios para tratamento medicamentoso com Methotrexato no período de fevereiro de 2007 a agosto de 2008. Foram analisadas variáveis demográficas, história ginecológica e características associadas ao tratamento foram obtidas através de revisão de prontuário. RESULTADOS: Dezoito mulheres foram incluídas no estudo. A média de idade foi de 27±5,2 anos. Todos os casos apresentavam gestação ectópica rota e todas as pacientes foram submetidas à salpingectomia parcial. O tempo cirúrgico variou de 30 a 120 minutos (média de 64,5 minutos), sendo contabilizado desde o momento de entrada da paciente na sala cirúrgica até o horário da saída. Nenhuma paciente apresentou infecção pós-operatória. O tempo médio de internação foi de 40± 14,3 horas. As medicações utilizadas no período pós-operatório foram semelhantes em todos os casos, sendo baseadas em anti-inflamatório não esteroide, dipirona, paracetamol e meperidina, conforme a necessidade. A dieta foi reintroduzida 8 horas após o término da cirurgia. CONCLUSÕES: A utilização da colpotomia no tratamento da gestação ectópica apresentou bons resultados, com ausência de complicações importantes e tempo de internação curto. O instrumental cirúrgico básico para a realização desse procedimento é relativamente comum a todos os hospitais, e a técnica cirúrgica é reprodutível.
PURPOSE: To report the use of colpotomy for the treatment of ectopic pregnancies. METHODS: This was a retrospective cross-sectional study conducted on all women hospitalized with a clinical-laboratory suspicion of ectopic pregnancy who did not fulfill the criteria for drug treatment with methothrexate, during the period from February 2007 to August 2008. Demographic variables, gynecologic history and characteristics associated with treatment were obtained by reviewing the medical records. RESULTS: Eighteen women were included in the study. Mean age was 27±5.2 years. All patients presented ruptured ectopic pregnancy and all were submitted to partial salpingectomy. Surgical time ranged from 30 to 120 minutes (mean: 64.5 minutes) calculated from the moment when the patient entered the operating room to the moment when she left it. No patient presented postoperative infection. Mean time of hospitalization was 40±14.3 hours. The medications used during the postoperative period were similar in all cases, being based on nonsteroid anti-inflammatory drugs, dipyrone, paracetamol and meperidine, as needed. The diet was reintroduced 8 hours after the end of surgery. CONCLUSIONS: The use of colpotomy in the treatment of ectopic pregnancy showed good results, with the absence of important complications and a short hospitalization time. The basic surgical instruments needed for this procedure are relatively common to all hospitals, and the surgical technique is reproducible.

Antecedentes: La esterilización tubaria transvaginal, es una forma de abordar la esterilización quirúrgica, en la actualidad poco popularizada. Objetivo: Evaluar la colpotomía posterior como vía para la esterilización tubaria. Método: Análisis de 100 fichas clínicas de mujeres multíparas en estado no puerperal, a solicitud voluntaria de esterilización. Resultados: Hubo un 3 por ciento de complicaciones. Conversiones en el 4 por ciento y un tiempo operatorio promedio de 25,2 minutos. Conclusión: La colpotomía posterior es una buena alternativa quirúrgica para efectuar la esterilización tubaria, con baja morbilidad.
Background: Transvaginal tubal sterilization is a way for surgical sterilization, that currently is not widely used. Objective: To evaluate the posterior colpotomy as a pathway for tubal sterilization. Method: Analysis of 100 medical records of multiparous women non puerperal state, who request voluntary sterilization. Results: There were 3 percent of complications, 4 percent of conversions and a mean operative time of 25.2 minutes. Conclusion: The posterior colpotomy is a good surgical alternative to perform tubal sterilization with low morbidity and cost.

Esta comunicación muestra una paciente con hemorragia posparto tratada con ligadura vaginal de arterias uterinas, con cese del sangrado y recuperación de la paciente sin complicaciones.
Postpartum hemorrhage is one of the most serious obstetrics complications with significant maternal morbidity and mortality. Uterine arteries ligature is part of the chain of therapeutic procedures. This communication presents a patient with postpartum hemorrhage treated with vaginal uterine arteries ligature with cessation of bleeding after the intervention and recovery without complications.

Las complicaciones y secuelas de la EPI son penosamente graves para la enferma, la familia y la sociedad. Aproximadamente un 12 por ciento de mujeres se convierten en infértiles después de un simple episodio, casi un 25 por ciento después de dos episodios y casi un 50 por ciento después de tres episodios. Su diagnóstico y tratamiento tiene también un impresionante costo económico. Every year thousands of women has an event of acute pelvic inflammatory disease (PID) leading to a increasing risk of chronic discomforts in the lower hemi-abdomen, ectopic pregnancy, pains due to adhesions, tubal infertiliy, dyspareunia, the pyosalpinx, tubo-ovarian and more of then requiring surgical interventions. The complications and sequelae of PIE are distressing for patients, the family and the society. Approximately a 12 percent of women overcomes infertile after a single event, almost a 25 percent after two events and almost a 50 percent after three events. Its diagnosis and treatment had a high economic budget. La apendicectomía es la intervención quirúrgica abdominal, más comunmente practicada, actualmente la apendicectomía laparoscópica constituye la modalidad terapéutica ideal, en la búsqueda de técnicas quirúrgicas menos invasivas, se propone la realización de apendicectomía por orificios naturales, siendo la vía transvaginal el acceso para el desarrollo del procedimiento. En la presente investigación se expone y describe la técnica quirúrgica NOTES: apendicectomía hibrida transvaginal con instrumentos laparoscópicos convencionales. El acceso transvaginal parece ser factible y seguro con instrumental laparoscópico convencional, para la realización de apendicectomías en cuadros clínicos no complicados. Es indispensable el entrenamiento en cirugía laparoscópica convencional. La triangulación laparoscópica desaparece, lo que dificulta la maniobrabilidad. No es necesario el uso de trócares para acceder a la cavidad abdominal.
The apendicectomy is the most common surgical procedure practice. Laparoscopic apendicectomy it is the gold standard. Searching for less invasive procedure it is propose the transvaginal apendicectomy for selected cases. The investigation describes the technique of NOTES: hybrid transvaginal apendicectomy, using regular laparoscopics instruments. The transvaginal access it is feasible way to perform the apendicectomy. It is essential to be trained on advance laparoscopic technique. The triangulation trough transvaginal way is lost. It is not necessary to use throcars to access trough transvaginal way.

Las vías de abordaje para la realización de la colecistectomía han cambiado dramáticamente en los últimos 20 años, desde la cirugía abierta hasta la cirugía de invasividad mínima y ahora desde 2007 se comienza a realizar cirugía a través de orificios naturales. Con la ausencia de instrumentos ideales, puede ser posible crear accesos seguros a través del abordaje trans-vaginal con el uso de instrumentos laparoscópicos convencionales en pacientes bien seleccionados. Describir una nueva técnica quirúrgica para la realización de colecistectomía transvaginal video asistida con instrumentos laparoscópicos convencionales. Descripción de la técnica: con la paciente bajo anestesia general, colocada en posición ginecológica, accedemos a la cavidad abdominal a través de la cicatriz umbilical. Insertando un trocar de 10mm bajo visión directa, a través de este se vigilará la entrada a través de la colpotomía, a través de la vagina se introducirá una pinza de grasper de 5mm y la óptica de 30°. Con el grasper transvaginal se sujetara la vesícula y a través del portal umbilical el cirujano realizará la disección de los elementos de triangulo de Calot, extrayendo la pieza por vía vaginal. El acceso trans-vaginal es factible y seguro con material laparoscópico convencional, para la realización de colecistectomía. Es indispensable el entrenamiento en laparoscopia convencional. El cirujano debe adaptarse a los ángulos de visualización ofrecidos.
The access to perform a cholecystectomy has changed dramatically in the last 20 years, from open surgery to minimal invasive surgery such as laparoscopy and now since 2007, starting toperform through natural orifices. In the absence of ideals instruments, it could be possible to create safes access through trans-vaginal way with regular laparoscopic instruments in well selected patients. Describe a new surgical technique to perform a trans-vaginal cholecystectomy video assisted, with common laparoscoic instruments. Technical description: with the patient under general anesthesia, placed her in gynecological position, we get access to the abdominal cavity through umbilicus, inserting a 10mm trocar, them under direct vision we perform the trans-vaginal access inserting one grasping forceps and 30º scope. The vaginal grasping forceps will hold the gallbladder between the fundus and the infundibulus, exposing the Calot triangle. Through the umbilicus trocar the surgeon dissect de cystic duct and artery, them it will be separated from its bed. The gallbladder is taking out side from the abdomen through vaginal way. The trans-vaginal access is a feasibly way to perform cholecystectomy with common laparoscopic instruments. It is indispensable to dominate basic laparoscopic techniques, and before start practicing this procedure, it has to perform under animals models.

Describir la técnica quirúrgica en la colecistectomía transvaginal sin cicatriz visible con el uso de instrumental laparoscópico convencional, en el Hospital Dr. Domingo Luciani. Caracas. Se presenta caso de paciente femenina de 52 años de edad, con litiasis vesicular sintomática a quien se le practica colecistectomía transvaginal. Se realizó colecistectomía transvaginal sin complicaciones intraoperatorias. No se administraron analgésicos orales ni parenterales, el alta hospitalaria se dio a las 12 horas del procedimiento. Evolución satisfactoria de la paciente con un seguimiento de 30 días. La cirugía asistida por minilaparoscopia puede considerarse intermedia entre el NOTES, y la cirugía laparoscópica. El siguiente caso demuestra la factibilidad y seguridad del procedimiento realizado por cirujanos generales con entrenamiento laparoscópico y con el uso de instrumental convencional.
Describe the surgical technique in the transvaginal cholecystectomy without a scar using conventional laparoscopic instruments, in the Dr. Domingo Luciani Hospital. Caracas. Presents female patient 52 years old, with sintomatic gallstones who is practicing transvaginal cholecystectomy. Was performed transvaginal cholecystectomy without intraoperative complications. No analgesics were administered oral or parenteral, was discharged at 12 hours of the procedure. There was a satisfactory progress of the patient with a follow upof 30 days. Minilaparoscopy assisted surgery can be considered intermediate between NOTES, and laparoscopic surgery. The following case demonstrates the feasibility and safety of the procedure performed by general surgeons with laparoscopic training and the use of conventional instruments.

Objetivo: revisar las consideraciones técnicas, indicaciones, riesgos y beneficios de este tipo de abordaje en masas pélvicas gigantes. Se presenta un caso en el que se realizó el abordaje laparoscópico de una masa quística ovárica de gran tamaño y su extracción a través de colpotomía posterior. La cirugía laparoscópica de los quistes gigantes de ovario surge como una alternativa a considerar en el manejo de estas pacientes.
Objective: reviewing technical considerations, indications, risks and benefits of a laparoscopic approach to giant ovarian cysts. A case is presented where a giant ovarian cyst was extracted by posterior colpotomy. Laparoscopic surgery on giant pelvic mass represents an alternative for managing such patients.

<p><b>OBJECTIVE</b>To investigate the types, clinical features and therapeutic approaches of congenital anomalies of the vagina.</p><p><b>METHODS</b>The clinical data of 81 patients with congenital anomalies of the vagina were analyzed retrospectively.</p><p><b>RESULTS</b>There were 5 types in these 81 patients, and 16 (19.7%) patients showed absence of the vagina, 15(18.5%) had vaginal obstruction, 10 (12.3%) had transverse vaginal septum,14(17.2%) had longitudinal vaginal septae,18(22.2%) had septum obliquus, and 8 (9.8%) had imperforate hymen. Forty-eight (59.2%) patients presented with primary amenorrhea, and 22(27.1%) complained of irregular pelvic pain. Fifteen of the patients with absent vagina underwent amnion artificial vaginoplasty, and the others were treated with incising and removing the septum, all having good clinical outcomes.</p><p><b>CONCLUSION</b>Amnion artificial vaginoplasty is a good option for treatment of absent vagina.</p>

OBJECTIVE: The aim of this study was to evaluate the clinical effectiveness and safety of vaginal myomectomy via colpotomy with analyzing clinical course and postoperative complication. METHODS: A retrospective study was performed in 59 patients, who underwent vaginal myomectomy between November 1997 and July 2006 in department of gynecology, Chonnam National University Hospital. Operating time, hemoglobin change, postoperative pain, perioperative complication, and days of hospital stay were recorded. RESULTS: Fifty-nine patients were evaluated, mean age of patients was 41.54+/-5.8 years. Mean diameter of the uterine myoma was 6.79+/-1.75 cm. Mean operating time was 66.69+/-34.49 minute. Mean hemoglobin loss was 2.67+/-1.26 g/dL, and mean days of hospital stay was 4.91+/-1.25 days. One patient had febrile morbidity. Only one patient had suffered from retroperitoneal bleeding enough to get on operation. CONCLUSION: Vaginal myomectomy seems to be a feasible and safe surgical procedure. Further randomised studies are needed to compare this procedure with laparotomy and laparoscopy, and vaginal myomectomy is expected to be more effective and generalized surgical procedure.

OBJECTIVE: Recently, laparoscopic operations are widely used for the resection of uterine myomas and the method for extraction of resected tissue leiomyoma is chiefly used with the morcellator or minilaparotomy. We investigated the feasiblity of colpotomy for the extraction of myoma tissue in the laparoscopic myomectomy. METHODS: A retrospective study was performed from May 2004 to July 2007 after a review of the medical records of 75 patients who have undergone myomectomy utilizing colpotomy. The patients profiles characteristics included myoma size, operation time, amount of blood loss, and complications in the Hanyang University Guri Hospital. RESULTS: Seventy-five patients were involved. The mean age of the patients was 40.5 (27-53) years, multiparity was found in 55 (73%) patients, the mean weight of the myomas was 204.3 (30-890) gm, the mean operation time was 2.47 (1.1-5.8) hours, the mean decrement of hemoglobin was 2.98 (0.2-5.9) gm/dL, the number of patients that needed transfusion was 8 (10.6%) and the number of the cases experiencing complications was one (0.01%). CONCLUSIONS: We concluded that colpotomy for the extraction of myoma tissue in laparoscopic myomectomy is a safe procedure in aspects of risk and cosmetic outcome and on an individual basis, can be used as an alternative to the usage of morcellator or minilaparotomy.

Objetivo: Evaluar la curación de la incontinencia urinaria de estrés y las disfunciones miccionales postoperatorias observadas mediante una modificacion en la técnica de inserción del sling transobdurtriz. Material y métodos: Estudio prospectivo exploratorio con asignación alternada, comparando la técnica clásica con la comparada. La modificación consistió en realizar una incisión vaginal en U invertida en lugar de clásica vertical. Las pacientes fueron evaluadas clínicamente con interrogatorio examen urogenital, ecografía transvaginal y RMN dinámica. Resultados: Se incluyeron 17 pacientes. Ambos fueron de características similares. Edad promedio 59,5 años. Seguimiento entre 3 y 15 meses. Los resultados de curación y posicionamiento del sling fueron similares. La tasa de disfunciones de la micción fue significativamente menor en el grupo de cirugía modificad (12 por ciento vs 72 por ciento p<0.0001). Conclusiones: La modificación propuesta impresiona como una alternativa para disminuir las funciones postoperatorias, constituyéndose en un interesante punto de partida para futuras investigaciones...
