RESUMO

Introduction: attention deficit hyperactivity disorder is one of the most common psychiatric disorders in children. The present study aimed to evaluate the effectiveness of stress management program using cognitive behavior approach on mental health of mothers of children with attention deficit hyperactivity disorder


Method: in this interventional study, 52 mothers of the children with attention deficit hyperactivity disorder were randomly allocated into two intervention and control groups. The general health questionnaire was used to measure mental health. In addition, depression - anxiety - stress scale [DASS-21] was used to measure stress. The two instruments were completed by mothers at baseline and immediately one month after the intervention. Cognitive-behavioral stress management program was consisted of eight sessions, each held for 90-minute for 8 weeks. This study was conducted in specialty and subspecialty clinic of Imam Reza and Hafez Hospital in Shiraz in 2012


Results: data were analyzed with SPSS version 16, repeated measures oneway ANOVA., and independent- samples t-test. At the baseline, no significant difference was found among the three groups regarding the means of stress, anxiety, depression, and mental health. However, there was a significant difference immediately after the intervention. In the intervention group, mean score of stress was significantly lower [12.29 +/- 4.32] than the control group [14.77 +/- 3.69] [P=0.001]. Also, mean score of mental health in the intervention group [20.50 +/- 10.56] was significantly lower than the control group [33.70 +/- 12.60] [P=0.009]. One month after the intervention, mean difference of stress and mental health scores was still significant in the intervention group [P=0.001 and P=0.004]. Moreover, mean difference of anxiety and depression for intervention group was significant [P=0.005 and P=0.008]


Conclusion: stress management program had a statistically significant influence on the mental health of mothers of the children with attention deficit hyperactivity disorder

RESUMO

Statement of Problem: Systematic reviews of the literature show that the dental erosion is associated with the gastroesophageal reflux disease [GERD]. The prevalence of the problem may not be exclusively similar in different countries


Purpose: The purpose of this study was to investigate the association of gastro-oesophageal reflux disease [GERD] with dental erosion in a sample of Iranian population regarding the standing difference in the Iranian oral hygiene and diet


Materials and Method: 140 patients with the average age of 30 to 50 years old comprised the study group. The participants were already eligible for the endoscopy examination, diagnosed by their gastroenterologist. All patients completed a detailed questionnaire regarding the medical and dental situations. After completing the questionnaire and before endoscopy, dental examination was performed by two blinded dentists. The endoscopy was then performed by a gastroenterologist and the patients were divided into three groups of healthy, suspected to GERD, and positive GERD. Data were collected and analysed by Chi- Square test. The cross tabulation test was performed to compare the qualitative variants and discover the correlations. The statistical significance was adopted as p < 0.05


Results: The prevalence of dental erosion in GERD patients [22.6%] was found to be higher than the suspected [5.3%] and the healthy [7%] individuals


Conclusion: This study declared the GERD patients are at higher risk of developing dental erosion compared to the healthy individuals in a sample of Iranian population

Statement of Problem: The flexure of the fiber- reinforced composites [FRC] which can be generally used instead of fixed metal- framework prostheses have been more advocated due to the enormous demands for the conservative and esthetic restoration. The flexure of the fiber should be well-fitted to its covering composite. No study has been reported the comparison of the combination of glass and polyethylene fiber with particulate filled composite and fiber reinforced composite yet


Purpose: This study compared the flexural strength of two types of fibers combined with three types of composites


Materials and Method: Sixty-six specimens were prepared in a split mold [25x2x2 mm]. The specimens were divided into six groups according to the type of resin and the fiber [N = 11]: group 1: Z250 composite + Polyethylene fiber; group 2: Build It composite + Polyethylene fiber; group 3: Nulite F composite+ Polyethylene fiber; group 4: glass fiber + Z250 composite; group 5: glass fiber + Build-It composite and group 6: glass fiber + Nulite F. The mean flexural strengths [MPa] values were determined in a 3-point bending test at a crosshead speed of 1 mm/min by a universal testing machine [Zwick/Roell Z020, Germany]. The results were statistically analyzed, using one and two- way ANOVA and LSD post-hoc tests [p< 0.05]


Results: The highest flexural strength was registered for glass fiber in combination with Z250 composite [500 MPa] and the lowest for polyethylene fiber in combination with Build-It composite [188 MPa]. One-way ANOVA test revealed that there was no statistically significant difference between polyethylene fiber combinations [p= 0.62] but there was a significant difference between glass fiber combinations [p= 0.0001]. Two way ANOVA revealed that the fiber type had a significant effect on flexural strength [p= 0.0001]


Conclusion: The choice of fiber and composite type was shown to have a significant positive influence on the flexural properties of the fiber-reinforced composite. Glass fiber has a significant influence on the flexural properties of directly- made specimens

Development of new composite resins is one of the most important technological advances in the field of cosmetic dentistry. The aim of this study was to investigate the effectiveness of different light curing units and composite thicknesses on the shear bond strength composite to dentin. In this experimental in-vitro study, the test groups were composed of 80 dentinal samples, prepared on occlusal surfaces of 80 maxillary premolar teeth and routinely divided into 8 groups of 10. Two cylindrical molds [2×2 and 2×5 mm] were filled in bulk using P60 and Ceram X composites and light cured with LED and QTH light curing units .After curing and 3 months aging in 37 degree water, shear bond strength of all samples were obtained using a universal testing machine with 1mm/min crosshead speed. One- and three-way ANOVA were used for statistical analysis using SPSS software. The highest mean of shear bond strength [31.75 MPa] was in group [P60, QTH, 2] and the lowest [15.34 MPa] was recorded in [Ceram X, LED, 5]. Type and thickness of composites had significant effects on the value of shear bond strength and no significant differences were obtained between two light curing units. The shear bond strengths of P60 and CeramX composites tested were much lower when cured in a 5-mm than in a 2-mm increment. The two types of light curing units tested did not show any significant differences in the amount of shear bond strengths

Introduction: Deficiencies of vitamins, minerals and some trace elements have been observed in hemodialysis patients, for which inadequate nutrient intake is considered to be one of the most important reasons. Correcting the micronutrient status of these patients will improve their quality of life. This study was performed to evaluate vitamin and mineral intakes and some related factors in hemodialysis patients referred to Imam Khomeini hospital in Tehran


Materials and Methods: This cross-sectional study was performed on 114 hemodialysis patients. To evaluate the patients' diet, 24 hour recall for 3 days [two usual days and one holiday] was applied. Food Processor II [FP II] was utilized to calculate the amount of micronutrient intakes


Results: Low micronutrient intakes were highly prevalent in the hemodialysis patients studied [36-100%]. There was a significant negative relationship between age and thiamine, vitamin E, iron and zinc [P<0.05]. Means for thiamine, riboflavin, niacin, pyridoxine, folate, iron, magnesium and zinc intakes were significantly higher in men than in women [P<0.05]. Moreover, mean vitamin E intake in the group with dialysis durations less than 3 years was higher than in the group with dialysis durations of over 3 years [P=0.009]. Mean magnesium and folate intakes were higher in the group without comorbidity than the group with comorbidity[P<0.05]


Conclusion: According to the present study, low micronutrient intake had a very high prevalence in hemodialysis patients

RESUMO

This case report presents a coincidence of trisomy 18 and balanced Robertsonian translocation [13;14]. Aneuploidy was suspected based on anomalies detected in ultrasound scan and confirmed with karyotype. In a 31 years-old healthy woman with a history of one miscarriage, second trimester ultrasound scan reported IUGR [<3rd percentile] with normal amniotic fluid, bilateral choroid plexus cysts, suspicious agenesis of corpus callosum and clenched hands. Amniocentesis was performed and karyotype was 46xx,der[13;14] [q10;q10],+18. Maternal karyotype was 45xx,der[13;14][q10;q10]. Pregnancy was continued due to legal limitation for termination after 20 weeks gestation. Delivery was done at 36 weeks gestation. A female newborn was borned and a physical feature was hypotonia, small mouth, prominent occiput, low-set and posteriorly rotated ears, clenched hands with overlapping fingers and rocker bottom feet. Ultrasound scan and echocardiography detected agenesis of corpus callosum and VSD, ASD, PDA and cardiomegaly. These features are typical of trisomy 18. Balanced Robertsonian translocation usually has no phenotypic expression. Genetic counseling and prenatal diagnosis for future pregnancy was recommended

Contamination of milk and dairy products to aflatoxin M1 is a risk for human and it can be a public heath concern. The aim of this study was to evaluate the presence of aflatoxin M1 in raw milk samples produced in Ardebil City [Iran] by ELISA [Enzyme Linked Immuno Sorbent Assay] technique. 122 samples of raw milk were collected from milk collecting centers and dairy plants in the region and aflatoxin M1 contamination was detected in all of milk samples. The mean concentration of aflatoxin M1 was 40.01ng/L and 14.75 percent of the samples had higher levels than the maximum recommended limits by ISIRI, European Community and Codex Alimentarius. With a view of the fact that milk is used by all the age groups including infants and children in the city of Ardebil, it is necessary to apply an ideal recommended limit to minimize the health hazard from aflatoxin M1 contamination in milk. Application of Good Agricultural Practices and Good Veterinary Practices by agriculture and also the Hazard Analysis and Critical Control Points [HACCP] system as a draft code of practice for preharvest and postharvest control of dairy cow's feed and in milk and dairy products processing is effective

Chronic obstructive pulmonary disease [COPD] is currently the fourth leading cause of death in the United States. As there is systemic as well as local inflammation in COPD patients and evaluating the stage of the disease is not possible by spirometery alone, we evaluated High-Sensitivity C-reactive Protein [HS-CRP] in a group of COPD patients as an available and cost effective auxiliary marker in determining COPD stages. In a cross-sectional study in 160 COPD patients who were admitted for exacerbations in Razi Hospital in Rasht, Data on patients' demographic characteristics, pulmonary function test [PFT] and laboratory results consist of arterial blood gases and HSCRP levels were analyzed. A significant positive correlation was seen between serum HSCRP level and stages of the disease [as GOLD criteria]. There was a significant relationship between HSCRP level and patients' sex, BMI, and smoking history in a way that men and smokers showed higher and patients with normal BMI showed lower HSCRP levels. The patients with higher PCO2 also showed a higher level of serum HSCRP. This survey supports the role of HSCRP as a simple auxiliary marker in staging and determining the prognosis of COPD for early management

Statins are the most common type of cholesterol- lowering drugs which have anti-inflammatory properties that may be beneficial in the treatment of inflammatory diseases such as asthma. Regarding to examine new medications for asthma management due to the side effects of existing routine asthma treatments, statins are one of drugs which have been suggested recently. Effect of atorvastating on lung function and airway inflammation. In this triple blind clinical trial study sixty seven patients with moderate to severe asthma were entered. They were divided to two groups randomly. Case group were treatment by oral atorvastatin 40 mg daily and control group were treatment by placebo on lung function. Patients were visited and their lung volumes [FEV1, FVC, FEV1/ FVC, FEF25-75%] and inflammatory biomarkers [ESR, Hs-CRP] and asthma control questionnaire score were measured every 4 weeks during the course. Fifty patients completed the study. Data was analyzed by chi[2], t-test and Mann-Whitney test. P value <0.04 was significant. There were no significant differences in lung volumes and inflammatory biomarkers between atorvastatin and placebo groups. We observed significant differences in the trend of lung volumes include FEV1, FEV1/FVC and FEF 25-75% in each of the atorvastatin and placebo groups during the course of study [p<0.001] but there were no significant difference between them. There were significant differences in the trend of asthma control according to asthma control questionnaires in each of atorvastatin and placebo groups during the course [p<0.001]. According to this study, atorvastatin does not show any significant anti-inflammatory activity in patients with moderate to severe asthma therefore it could not be beneficial for the short term treatment of asthma. But it seems that regular visit and patient education could lead to better asthma control

RESUMO

Cystic echinococcosis is a public health problem in endemic areas. Recurrent pneumonia is a rare complication of ruptured hydatid cysts of the lung. We report a 26-year-old lady with a recurrent right lower lobe [RLL] pneumonia. Her CXR and chest CT scan revealed involvement of RLL. In her history, it was revealed that she had undergone a surgery for liver hydatid cyst two years ago. During fiberoptic bronchoscopy [FOB] for unresolved pneumonia, we extracted the whitish-yellow bright gelatinous membrane obstructing the orifice of the posterior basal segment of RLL. A follow-up CT scan done six months later revealed clearance of RLL opacity and pleural effusion. Although treatment of pulmonary hydatid cyst by FOB is an exceptional event, our exceptional case illustrates a complete treatment without any further need for a thoracic surgical procedure which could have many complications and even morbidity

Metabolic syndrome [Mets] is a common phenotype, which is affected on 32% of Iranian population. It is associated with an increased risk for cardiovascular disease. Low HDL-C is the most frequent phenotype in this disease. In this study in order to find some chromosomal region in related to the HDL-C in families with metabolic syndrome, twelve microsatellite markers were investigated in 91 families and done the Haseman-Elston Regression methods [oHE, sHE, rHE, W2, W3, tHE]. Finally, the results of these methods are compared. For analysis of the data, these softwares were used: power. HE [inR], Excel, PowerMarker, SPSS, S.A.G.E. In results of 4 methods, from these 7 methods, genetic linkage of HDL-C is significant with D11S1998 marker [p<0.05]. Other studies also show this result. These results can help me in future studies in Iranian population. Results show in which theoretical power of these methods is better, the empirical significance become less and this is an evidence for accepting significant linkage between D11S1998 marker and location of HDL-C gene

Analgesics and anesthetic pain relief in labor often have unwanted sedative effects in the mothers and their newborns. It seems that using non-pharmacologic methods, such as acupressure, affect on labor pain and delivery time. The purpose of this study was to evaluate the effects of SP6 acupressure on labor pain and delivery time in nulliparas women in Iranshahr hospital in 2008-2009. A randomized clinical trial study was performed on 60 eligible nulliparas women who were at the beginning of active phase of labor [4 cm dilatation of cervix]. The women were randomly assigned to one of the two groups: SP6 acupressure or SP6 touch. The participants, received acupressure or SP6 acupoint, 3 Kun above the tip of the inner malleous just posterior to the border of tibia above the ankle, for 20 minute during contractions on two feet. Labor pain was measured four times with visual-analogue scale [VAS]: before and after the intervention in 4,6, 8 and 10 cm cervical dilation. Length of delivery time was calculated in two stages: from 4 cm cervical dilation to full cervical dilation and full cervical dilatation to the delivery. The data were analyzed using SPSS software and descriptive statistical, paired t-test, chi square and independent t-test. There was a significant reduction in the severity of pain in 4, 6 and 8 cervical dilatation in case group compared to control group [P<0.001]. The length of delivery time in active phase was less in case group compared to control group P<0.001, but there was hot difference in second place of delivery. SP6 acupressure may be applied in a non-invasive and easy to use fashion for pain control in the labor

Nowadays, MTA has numerous clinical applications because of desirable properties. Many teeth which are treated by MTA need final composite restorations. In this situation, the bonding between composite and MTA has the main role in retention, durability and seal of the restoration. Evaluation of the bond strength between MTA and composite resin and its comparison with Resin Modified Glass Ionmoer [RMGI]. Sixty specimens were prepared for three experimental groups of white MTSA, gray MTA, and glass ionomer Fuji II LC. Ten specimens in each group were etched with 37% phosphoric acid for evaluation of the etching effect while ten speicments were not etched. All specimens were then bonded to the composite and shear bond strength test was performed, using Instron testing machine. In addition, the fractured surfaces of the samples were checked with stereomicroscope [x10]. Data were analyzed by ANOVA and Tukey HSD test. The highest shear bond strength value was seen in unetched glass ionomer subgroup and there were significant differences in shear bond strength of the composite to unetched glass ionomer in comparison with the other groups [p<0.001]. There were no significant differences in shear bond strength of the composite to etched and unetched white MTA, etched and unetched gray MTA, and etched glass ionomer [p > 0.05]. Shear bond strengthe between MTA and the composite is comparable with etched glass ionomer. The etching of the RMGI surface causes as significant decrease of bond strength to the composite but it has no effect on the bond strength of MTA to Composite

Improvement of the quality of life [Qol] in addition to controlling symptoms of the disease in children and adolescents with chronic illness such as diabetes mellitus is important. Problems such as diabetes are not only recognized by the physician and nurses but also by the patients. The impact of the diabetes and its complications on Qol is still unknown. The number of studies on Qol of diabetic patients is limited. The aim of this study was to carry out a comparative assessment of the views of children and adolescents with type I diabetes on the Qol. In this historical cohort study the views of the children and adolescents with type I diabetes, and the views of their parents were assessed. A questionnaire on pediatric quality of life including children's and parents' views was used to collect data in four sections of physical [8 items], emotional [5items], social [5 items], and school performance [5items]. Data were analyzed by inferential statistics, independent t-test, Mann-Whitney test and spearman correlation. The results of this study showed no significant difference between the mean scores of Qol of diabetic children and control group, but there was a significant difference between two groups in physical aspect of Qol in diabetic children and control subjects. Parents' views showed that parents of children with diabetes were more worried than those of controls. There was a significant difference between scores of total quality of life in its physical, emotional and social aspects between parents of the diabetic children and parents of the control group [p=0.001]. This study showed that views of the children and adolescents with type I diabetes were different from those of control group in regard to physical aspect. Also the views of parents of children and adolescents with type I diabetes were different from those of control group concerning quality of life in particular, emotional and physical aspects. Therefore we recommend in addition to providing medical care, programs for improvement of the level of mental health of these patients and their families be designed

Adverse surface interactions between one-bottle adhesives and self-cured composites may occur similarly with light-cured composites during the use of different modes of light activation. The study examined the effect of different light activation modes of light-cured composite on the shear bond strength of 2 one-bottle adhesives with different acidity to the dentin. The experimental study was performed on 110 extracted sound molars. Then, the teeth were randomly divided into 10 equal groups [n=11]. One-Step plus [OS] and Prime and Board NT [P and B NT] were [SSI]. Soft- Start II [SSII], Pulse-Delay I [PDI], Pulse- Delay II [PDII]. Then, shear bond strength [SBS] test was performed, using an Instron at 1mm/min. The SBS was recorded in MPa. Statistical analysis included two way ANOVA with a significance level of 0.05. The highest SBS [MPa] was shown in OS conventional group [19.62] and the lowest in P and B NT, PDII [5.43]. In each group of 5 curing mode, mean SBS in P and B NT was significantly lower than that in OS as follows: In conventional; P and B NT [17.27] vs OS [19.82] in SSI; P and B NT [10.89] vs OS [13.09] in SSII; P and B NT [14.48] vs. OS [18.79] in PDI; P and B NT [5.93] vs OS [11.97] and in PDII; P and B NT [11.82] vs OS [16], [p<0.001]. Recent studies have indicated the role of apoptosis and angiotensin in the pathogenesis of bleomycin induced-pulmonary fibrosis. Losartan, an angiotensin type 1 receptor [AT1R] antagonist, has ameliorated apoptosis and fibrosis from bleomycin. In this study, alterations in the expression of apoptosis-regulatory genes [bcl-2 and bax] were investigated in different cells of lung tissue of mice treated with bleomycin in the presence of losartan. Losartan [10 mg/kg, i.p.] was given to mice two days before administration of bleomycin [3 U/kg] and throughout the test period. After two weeks, lung tissues of mice were evaluated for fibrosis by biochemical measurement of collagen deposition and semiquantitative analysis of pathological changes of the lung. The expression of bcl-2 and bax was assessed by immunohistochemical assay using biotin-streptavidin staining method on paraffin-embedded lung tissues. Pre-treatment with losartan significantly [P < 0.05] reduced the increase in lung collagen content and also inhibited the histological changes induced by bleomycin. Immunohistochemical studies showed that losartan significantly [P < 0.05] reduced the bax/bcl-2 expression ratio in the alveolar epithelial cells, lymphocytes, macrophages and interstitial myofibroblasts. Losartan also inhibited the bcl-2 upregulation which was educed by bleomycin in neutrophils. By reduction of bax/bcl-2 ratio as a determinant of susceptibility of a cell to apoptosis, losartan exerted protective effects on the alveolar epithelial cells that may be important in the amelioration of pulmonary fibrosis. These results may help to better understanding of the role of angiotensin II and apoptosis in pulmonary fibrosis

This study was performed to evaluate the normative data and psychometric properties and the internal consistency of the Farsi [Persian] version of the Strength and Difficulties Questionnaire [SDQ] self-report form, as a screening tool in a community-based sample of 12 to 17 year-old adolescents of urban Tehran. In this investigation, 1105 adolescents [12 to 17 years old], selected from 250 clusters from all the 22 municipality areas of Tehran, responded to 25 questions of the Farsi version SDQ self-report form. The frequency of each symptom domains according to Goodman's cutoff points and 90th percentile and the mean score in each subscale were determined. The 90th percentile cutoff points were somewhat different from those of the previous reports. Using Goodman's cutoff points, the prevalence of symptom domains was relatively high. For example, 13.7 percent of the adolescents studied had total scores equal to 20 or more. There were significant correlations between different subscales and their constituting questions. Self-report form of SDQ is a valuable tool in the screening of adolescent psychopathologies. Frequency of majority of the symptom domains seems to be higher in the adolescents in Tehran urban areas

Pulmonary complication of IBD includes airway inflammation involving small and large airways, pulmonary paranchymal disease and serositis. The aim of this study was to determine the prevalence of Pulmonary Function Test [PFT] abnormality in ulcerative colitis [UC] patients. During spring and summer of 2006, PFT [spirometry and body box plethysmography] of 50 UC patients were compared with 50 healthy persons matched for age and sex [control]. Data collection form including demographic specification and UC condition were filled. Mean age of patients was 37.2 years [SD=14.5]. Active UC was seen in 24% of patients while 18% of patients suffered from severe UC. PFT results included 42% air trapping [only increase in residual volume/total lung capacity], 20% small airway obstructive pattern [only decrease in maximal expiratory flow at 25-75% of vital capacity], 12% restrictive ventilation defect, 2% obstructive airway, 2% hyperinflation and 6% upper airway obstructive pattern. There was a significant relationship between small airway obstructive pattern and duration of UC and no relationship was noticed between other pulmonary disorders and severity, activity, duration of UC. According to high prevalence of air trapping, small airway disease may be the prominent feature of lung involvement in UC patients. Therefore a meticulous work up for respiratory diseases is necessary in UC patients

Resin composites are one of the most popular tooth colored restorative materials. Their enamel and dentin bonding ability are based on many factors, including elasticity and filler concentration. This study was undertaken to evaluate the effect of volumetric filler percentage and modulus of elasticity of six composites by measuring shear bond strength to dentin of the bovine teeth as experimental bonding substrate. Eighty bovine incisors were prepared and divided into eight groups. Tetric Ceram, Tetric Flow, Compo glass F, Helimolar RO, Definite, and Degufill Mineral were applied respectively. A fifth generation of dentin bonding system [Single Bond [3M, USA]] was used for all the groups. The volumetric filler percentage of each composite was obtained from their specification data. The modulus of elasticity was determined, using the formula presented by Bream et al [1986]. Then, in last two groups Heliomolar RO and Definite were bonded using their own bonding systems, Syntac Multicomponent, and Definite Multibond. The shear bond strength was measured according to the ISO/TR 11405. Pearson's correlation test was used to compare the result with groups 4 and 5. The results indicated that the relationship between both the filler percentage and the modulus of elasticity with the shear strength was 16% [P < 0.005]. There was a significant difference between groups 4 and 7, but no significant difference was observed between groups 5 and 8. Using composites with a higher filler content and modulus of elasticity resulted in higher shear bond strength in dentin bonding system. Our results showed that the corresponding dentin bonding system of composites had better results
