The aim of this work was to study the difference in interferon gamma (IFN-gamma) production by T lymphocytes after early secretory antigen target 6 (ESAT-6) or purified protein derivate (PPD) stimulation in whole blood culture supernatants from children with suspected tuberculosis (TB) disease (n = 21), latent TB infection (n = 16) and negative controls (NC) (n = 22) from an endemic area in Brazil. The concentration of IFN-gamma (pg/ml) was measured by enzyme linked immunosorbent assay and the differences in the IFN-gamma levels for each group were compared and evaluated using an unpaired Student's t-test; p values < 0.05 were considered significant. Measurement of IFN-gamma levels after ESAT-6 stimulation raised the possibility of early diagnosis in the latent TB group (p = 0.0030). Nevertheless, the same group showed similar responses to the NC group (p > 0.05) after PPD stimulation. The IFN-gamma assay using ESAT-6 as an antigenic stimulus has the potential to be used as a tool for the immunodiagnosis of early TB in children.

RESUMO

A anemia falciforme caracteriza-se como quadro hemolítico hereditário que evolui cronicamente causando danos físicos e emocionais às pessoas acometidas. Até o presente momento não se dispõe de tratamento curativo, a não ser o transplante de medula óssea, que ainda tem sido realizado de maneira experimental. A triagem neonatal de hemoglobinopatias, principalmente da anemia falciforme, tem sido essencial ao diagnóstico precoce e à instituição de medidas preventivas e promotoras de saúde. No entanto, o Ministério da Saúde do Brasil recomenda o exame dos pais a partir da identificação de heterozigotos, mas não faz alusão quanto à ampliação da triagem para outros familiares. Uma família que possua uma criança afetada com estas síndromes passa a ter um marcador para um grupo genético de risco. Neste caso, a triagem ampliada para os familiares mais próximos (avós, pais, irmãos, tios e primos) poderá identificar muitos portadores ou casais em risco, antes do casamento e procriação, além de servir de base a programas de assessoramento genético e de controle epidemiológico das hemoglobinopatias, uma herança genética bastante freqüente em nossa população.


Sickle cell anemia is a hereditary condition that evolves to a chronic illness, causing physical and emotional disorders to those involved. As yet there is no cure except for bone marrow transplantation which is still in the experimental stage. Neonatal screening for hemoglobin disorders, particularly sickle cell anemia, has been crucial for ensuring early diagnosis and the application of preventive and health-promoting measures. The Brazilian Health Ministry recommends testing parents thereby identifying heterozygotes, but does not propose extending this screening to other family members. A family that has a child affected by one of these syndromes is a marker for an at-risk group. In this case extending screning to close relatives (grandparents, siblings, aunts and uncles, and cousins) may identify individuals affected by the disease or couples at risk before marriage and reproduction and serve as the basis for programs providing genetic evaluation and epidemiological control of hemoglobin diseases that are relatively common in the Brazilian population.

This report describes the development of a SYBR Green I based real time polymerase chain reaction (PCR) protocol for detection on the ABI Prism 7000 instrument. Primers targeting the gene encoding the SSU rRNA were designed to amplify with high specificity DNA from Schistosoma mansoni, in a real time quantitative PCR system. The limit of detection of parasite DNA for the system was 10 fg of purified genomic DNA, that means less than the equivalent to one parasite cell (genome ~580 fg DNA). The efficiency was 0.99 and the correlation coefficient (R²) was 0.97. When different copy numbers of the target amplicon were used as standards, the assay could detect at least 10 copies of the specific target. The primers used were designed to amplify a 106 bp DNA fragment (Tm 83°C). The assay was highly specific for S. mansoni, and did not recognize DNA from closely related non-schistosome trematodes. The real time PCR allowed for accurate quantification of S. mansoni DNA and no time-consuming post-PCR detection of amplification products by gel electrophoresis was required. The assay is potentially able to quantify S. mansoni DNA (and indirectly parasite burden) in a number of samples, such as snail tissue, serum and feces from patients, and cercaria infested water. Thus, these PCR protocols have potential to be used as tools for monitoring of schistosome transmission and quantitative diagnosis of human infection.

RESUMO

The detection of specific DNA sequences by polymerase chain reaction (PCR) has proved extremely valuable for the analysis of genetic disorders and the diagnosis of a variety of infectious disease pathogens. However, the application to the detection of Schistosoma mansoni is rare, despite a recommendation of the World Health Organization that a major focus of research on schistosomiasis should be on the development and evaluation of new strategies and tools for control of the disease. In this context, a few studies were published for the detection of the parasite in snails, monitoring of cercariae in water bodies, and diagnosis of human infection. The present minireview describes sensitive and specific PCR based systems to detect S. mansoni, indicating possible applications in the detection of snail infection, monitoring of transmission sites, and diagnosis of human infection.

Schistosoma mansoni infected C57Bl/6 inducible nitric oxide synthase (iNOS)-deficient and non-deficient malnourished mice, both fed a balanced controlled diet were studied. Interleukins, IL-4 and IL-10 responses to soluble egg antigens (SEA) 90 days after infection, were determined. Our results suggest that in iNOS deficient, malnourished mice, 90 days after of infection, nitric oxide has a downregulating effect on IL-4 and IL-10 production. We are currently investigating the biological significance of these findings.

RESUMO

Evaluation of hepatic fibrosis is usually performed by histopathological examination of biopsies. However, this is an invasive and potentially dangerous procedure. Several studies have proposed serum biological markers of hepatic fibrosis. This communication evaluates the use of serum cytokines as markers of hepatic fibrosis in hepatitis C, schistosomiasis, and co-infection.

RESUMO

Specific IgG and IgM responses to soluble egg antigen (SEA) and keyhole limpet haemocyanin (KLH) were measured by ELISA in patients with acute and chronic schistosomiasis. The tests based upon IgM and IgG antibodies responses to KLH presented the best diagnostic discrimination, and can be used in conjunction with clinical and epidemiological data to the differential diagnosis of acute schistosomiasis.

RESUMO

In the present communication we analyzed the levels of IgG1, IgG2, IgG3, IgG4 and IgE isotypes to soluble egg antigen of Schistosoma mansoni by ELISA in individuals from an endemic area for schistosomiasis in Northeast Brazil. The analysis was performed before and after treatment to evaluate the age-dependent pattern, and to identify differences in the reactivities to antigens. Our results suggest that schistosomiasis treatment would not interfere with this sort of immune response

Sm8 is a major tegumental antigen of Schistosoma mansoni. The partial cDNA was isolated and analyzed. Sequence analysis revealed transmembrane compatible hydrophobic domains and a putative leucine zipper pattern. The mRNA and the protein are predominantly expressed in adult worms

The zinc finger motifs (Cys2His2) are found in several proteins playing a role in the regulation of transcripton. SmZF1, a Schistosoma mansoni gene encoding a zinc finger protein was initially isolated from an adult worm cDNA library, as a partial cDNA. The full sequence of the gene was obtained by subcloning and sequencing cDNA and genomic fragments. The collated gene sequence is 2181 nt and the complete cDNA sequence is 705 bp containing the full open reading frame of the gene. Analysis of the genome sequence revealed the presence of three introns interrupting the coding region. The open reading frame theoretically encodes a protein of 164 amino acids, with a calculated molecular mass of 18,667Da. The predicted protein contains three zinc finger motifs, usually present in transcription regulatory proteins. PCR amplification with specific primers for the gene allowed for the detection of the target in egg, cercariae, schistosomulum and adult worm cDNA libraries indicating the expression of the mRNA in these life cycle stages of S. mansoni. This pattern of expression suggests the gene plays a role in vital functions of different life cycle stages of the parasite. Future research will be directed to elucidate the functional role of SmZF1

Os polipeptídeos de 46 e 58kDa foram reconhecidos em diferentes cepas de T. cruzi (Y, WSL e Colombiana) pelo soro de todos os pacientes chagásicos estudados. Estes polipeptídeos foram isolados da cepa Y e usados em ELISA. A sensibilidade e especificidade foram 97,6 por cento [CI 95 por cento: 86-100 por cento] e 100 por cento [CI 95 por cento: 89,3-100 por cento], respectivamente quando Tc 46 foi usada. Quando Tc 58 foi utilizada a sensibilidade e especificidade foram de 100 por cento [CI 95 por cento: 89,6-100 por cento] e 90,2 por cento [CI 95 por cento: 75,9-96,8 por cento], respectivamente

Sm15 and Sm13 are recognized by antibodies from mice protectely vaccinated with tegumental membranes, suggesting a potencial role in protective immunity. In order to raise antibodies for immunochemical investigations, the genes for these antigens were expressed in pGEX and pMAL vectors so that comparisons could be made among different expression systems and different genes. The fusion proteins corresponding to several parts of the gene for the precursor of Sm15 failed in producing antibodies recognizing the parasite counterpart. On the other hand, antibodies raised against Sm13 MBP-fusion proteins recognized the 13 kDa tegumental protein. Thus the peculiarities of the gene of interest are important and the choice of the expression system must sometimes be decided on an impirical basis.

Two contiguous villages in Tracunhaem country (State of Pernambuco) endemic for schistosomiasis, were studied: Itapinassu (138 inhabitants) and Sao Joaquim (91 inhabitants). Agriculture predominates in the former region while ceramics is the main activitiy in the latter. Although no statistical difference was found regarding prevalence, severe infection (<400 epg) predominated in Itapinassu, probably related to the kind of occupation. No association was found between parasite burden and severity of disease, in spite of the high infection rates for Schistosoma mansoni in both communities (aprox. 60 per cent). Typical epidemiological features of schistosomiasis such as age-realted prevalences and intensities of infection (high in children, low in adults) were also mutual characteristics. Nutritional status determined through anthropometric evaluation was carried out by measuring specific anthropometric indicators. A deficit of energy intake, as well as vitamin A and riboflavin deficiencies were detected. The prevalence of moderate or severe undernutrition in patients under 18 years old was 21.9 per cent in Itapinassu and 24.1 per cent in Sao Joaquim. In this group as association was found between prevalence of schistosomiasis and chronic undernutrition. Similarly, for patients over 18 years old the prevalence of undernutrition was higher than 20 per cent. However, in this case no association between nutritional status and either prevalence of schistosomiasis or parasite burden could be detected. The two communities had not been treated for eight years.

Neste trabalho foram avaliados os efeitos do Corynebacterium parvum na protecao do hospedeiro, reacao tecidual e quimiotaxia "in vivo" em camundongos infectados pelo S. mansoni. O C. parvum foi dado intraperitonealmente usando uma dose de 0,7 mg, duas vezes por semana (durante 04 semanas), 30 dias antes (tratamento profilatico) e 30 dias apos a infeccao (tratamento curativo). A protecao do hospedeiro foi avaliada atraves da contagem de vermes adultos obtidos atraves da perfusao hepatica de camundongos infectados e esse numero foi bem menor no grupo profilatico comparado ao grupo controle (p=0,018), obtendo-se 44 por cento de protecao. A resposta quimiotatica "in vivo", nos grupos curativo e profilatico, foi maior do que no grupo infectado/nao tratado (p=0,009 e p=0,003, respectivamente). As reacoes teciduais foram descritas em todos os grupos, embora nao tenham ocorrido diferencas marcantes entre eles. As possiveis implicacoes biologicas e a relevancia dos achados para a resposta defensiva do hospedeiro e controle da esquistossomose sao discutidas neste trabalho

Indicadores do estado nutricional de proteínas foram estudados em camundongos albinos suíços recém-desmamados e infectados com S. mansoni, aos quais foi administrada a Dieta Básica Regional (DBR) do Nordeste do Brasil. Cada camundongo foi infectado com 80 cercárias, por via percutânea. O experimento teve a duraçäo de 63 dias. Os parâmetros investigados foram: evoluçäo ponderal, consumo alimentar, ingestäo protéica, ganho ponderal, Coeficiente de Eficiência Protéica (PER) e Coeficiente de Eficiencia Proetéica Líquida (NPR). Os camundongos alimentados com a DBR revelaram acentuada perda de peso, menor consumo alimentar e protéico, maior lentidäo no ganho em peso relacionado com a fase de crescimento e taxas menos elevadas de utilizaçäo da proteína dietética, quando comparados com os controles (alimentados com dieta balanceada, à base de 22,60 por cento de caseína). As diferenças encontradas entre camundongos infectados e näo infectados, näo foram consistentes. Os resultados sugerem que os efeitos provocados pela má-nutriçäo induzida pelo consumo da DBR säo muito mais perniciosos para a saúde e estado nutricional do hospedeiro, do que aqueles resultantes da infecçäo pelo S. mansoni, na fase inicial da doença

For many years the epidemiological significance of immunity in human schistosomiasis has been the subject of inconclusive debate. Recently, the results of studies from Brazil and Kenya, on Shistosoma mansoni and from Zimbabwe and The Gambia on S. haematobium have confirmed the importance of protective immunity. In communities in endemic areas the development of immunity to infection only occurs after many years of exposure. In part this due to the slow development of antibodies wich are protective but also to the earlier development of antibody isotypes which lack protective capacity and wich are capable of interfering with the functioning of protective antibodies. Protective antibodies appear to be of the IgE class but some IgG subclasses may be also be important. Initially, blocking antibodies were thought to be predominantly IgM and IgG2 but IgG4 also seems to posses blocking activity. The early production of blocking antibodies and late production of protective antibodies may be indicative of cytokine induced immunoglobulin class swiching caused by the sequential involvment of different lymphokines
