ABSTRACT Purpose: To analyze the histology and distribution of abdominal testicular vessels in human fetuses Patients and Methods: We studied 19 fetuses (34 testes) ranging in age from 12 to 19 weeks post-conception. The fetuses were evaluated regarding crown-rump length (CRL), total length (TL) and body weight immediately before dissection. Each testis was dissected and embedded in paraffin, from which 5 µm thick sections were obtained and stained with Masson's trichrome and Anti-CD31 antibody to quantify the vessels. The stereological analysis was carried out with the Image Pro and Image J programs, using a grid to determine volumetric densities (Vv). Means were statistically compared using the unpaired T-test (p<0.05). Results: The fetuses presented mean weight of 222.5g, mean CRL of 15.3 cm and mean TL of 23.2 cm. All testes were in the abdominal position. The mean percentage of vessels (Vv) in the upper portion of the testis was 7.6% (4.6 to 15%) and in the lower portion the mean was 5.11% (2.3 to 9.8%), with a significant difference (p=0.0001). In the analysis between the upper portion of the right and left testes (p=0.99) and in the analysis of the lower portion of the right and left testes (p=0.83), we did not observe significant differences. Conclusion: The upper portion of the abdominal testis in human fetuses had a higher concentration of vessels than the lower portion. These results suggest that manipulation of the lower end of the testis during Fowler-Stephens surgery should be avoided in order to preserve the collateral circulation.

ABSTRACT Objective The objective of the present study is to evaluate the anatomy of the inferior hypogastric plexus, correlating it with urological pathologies, imaging exams and surgeries of the female pelvis, especially for treatment of endometriosis. Material and Methods We carried out a review about the anatomy of the inferior hypogastric plexus in the female pelvis. We analyzed papers published in the past 20 years in the databases of Pubmed, Embase and Scielo, and we included only papers in English and excluded case reports, editorials, and opinions of specialists. We also studied two human fixed female corpses and microsurgical dissection material with a stereoscopic magnifying glass with 2.5x magnification. Results Classical anatomical studies provide few details of the morphology of the inferior hypogastric plexus (IHP) or the location and nature of the associated nerves. The fusion of pelvic splanchnic nerves, sacral splanchnic nerves, and superior hypogastric plexus together with visceral afferent fibers form the IHP. The surgeon's precise knowledge of the anatomical relationship between the hypogastric nerve and the uterosacral ligament is essential to reduce the risk of complications and postoperative morbidity of patients surgically treated for deep infiltrative endometriosis involving the uterosacral ligament. Conclusion Accurate knowledge of the innervation of the female pelvis is of fundamental importance for prevention of possible injuries and voiding dysfunctions as well as the evacuation mechanism in the postoperative period. Imaging exams such as nuclear magnetic resonance are interesting tools for more accurate visualization of the distribution of the hypogastric plexus in the female pelvis.

ABSTRACT Introduction: in videolaparoscopic surgery, movements are conducted from a twenty times magnified image of an indirect operative field. The video interface used assumes the need for depth perception using two dimensions instead of three. Objective: to evaluate the effectiveness of training to perform the laparoscopic knot in a silicone model, in 8-hour courses, and to analyze the correlation of learning outcomes with factors such as: sex, age, laterality, previous knowledge in endosuture and medical specialty. Material and Methods: in this prospective and randomized study, 56 students were evaluated, who took 8-hour courses, with groups of up to ten students. We used: a white box with camera, LCD screen and silicone piece. Four exercises were performed on the silicone mold: right hand, left hand, needle at 45° and back hand. Results: 56 students (mean age = 33.28 years). The female group, n=18, mean age 29.61 years, 17 right-handed and 1 left-handed. The male group, n=38, mean age 34.57 years, 35 right-handers, 1 left-handed and 2 ambidextrous. In both groups, no correlation was observed between the analyzes of well performed knots when correlated with the age or sex of the participants. Conclusion: laparoscopic knot training, in silicone molds, in 8 h courses, proved to be effective. Factors such as gender, age, laterality, previous knowledge in endosuture and medical specialty do not interfere with the learning results.


RESUMO Introdução: na cirurgia videolaparoscópica os movimentos são conduzidos a partir de uma imagem magnificada de vinte vezes de um campo operatório indireto. A interface de vídeo utilizada pressupõe a necessidade de percepção de profundidade utilizando duas dimensões ao invés de três. Objetivo: avaliar a eficácia do treinamento de execução do nó laparoscópico em modelo de silicone, em cursos de 8h, e analisar a correlação dos resultados de aprendizagem com fatores como: sexo, idade, lateralidade, conhecimentos prévios em endosutura e especialidade médica. Material e Métodos: Neste estudo, prospectivo e randomizado, foram avaliados 56 alunos, que realizaram cursos de 8h de duração, com turmas de até dez alunos. Foram utilizados: uma caixa branca com câmera, tela de LCD e peça de silicone. No molde de silicone foram executados quatro exercícios: mão direita, mão esquerda, agulha a 45° e back hand. Resultados: 56 alunos (média de idade = 33,28 anos). O grupo feminino, n=18, média de idade 29,61 anos, 17 destras e 1 canhota. O grupo masculino, n=38, média de idade 34,57 anos, 35 destros, 1 canhoto e 2 ambidestros. Em ambos grupos não foram observados nenhuma correlação entre as análises de nós bem executados quando correlacionados com a idade ou sexo dos participantes. Conclusão: o treinamento de nó laparoscópico, em moldes de silicone, em cursos de 8h de duração, mostrou-se eficaz. Fatores como sexo, idade, lateralidade, conhecimentos prévios em endosutura e especialidade médica, não interferem nos resultados do aprendizado.

ABSTRACT Objective: To evaluate the anatomical aspects of the kidney surface in human fetuses during the second gestational trimester. Material and Methods: We studied 108 kidneys obtained from 54 human fetuses (29 males and 25 females). The kidney was dissected and the number of clefts was counted. The renal volume was also assessed. To compare the quantitative data in both sexes, the Students-t-test was used (p < 0.05). Simple linear correlations were calculated for all kidney measurements, according to fetal age. Statistical analysis was performed with the R program (Version 3.5.1). Results: The fetuses ranged in age between 11.4 to 23 weeks post-conception. The renal volume of the right kidney ranged from 0.09 to 2.397 cm (mean=0.8479) and the renal volume of the left kidney ranged from 0.07 to 2.416 cm (mean=0.8036). The mean number of renal clefts in fetuses studied was 15.25 (7 to 28). There was no statistical significant difference in renal clefts between the sides either in males (p = 0.646) or in females (p = 0.698). Also, there was no significant difference in the mean number of renal clefts between male and female fetuses in right kidney (p = 0.948) and in left kidney (p = 0.939). Conclusions: The number of renal clefts has a great variation, weak correlation and no tendency to decrease during the 2nd gestational trimester. The number of clefts in right kidney of total sample and female fetuses has a significant development with age.

ABSTRACT Background: To analyze the incidence of epididymal anomalies (EAs) associated to spermatic obstruction in patients with undescended testis (UT) according to testicular position and age. Materials and Methods: We studied 87 patients (110 testis) with cryptorchidism and analyzed the presence of EAs correlated with the testicular position, age and patency of the processus vaginalis (PV). To analyze the relations between the testis and epididymis we considered three situations: (a) Normal pattern: the epididymis was attached to the testis at the head and tail and epididymis totally attached to the testis; (b) EAs: when the epididymis was attached to the testis only at the head (Figure-1A) and (c) EAs associated to spermatic obstruction: epididymis was attached to the testis only at the tail (Figure-1B) and when there are no visible connection between testis and epididymis (Figure-1C). We used the Wilcoxon-Mann-Whitney test and the Chi-square test for contingency analysis (p <0.05). Results: The mean age of the patients was 5.18 years (SD=2.867). Of 110 testes analyzed, 14 were abdominal (12.72%); 83 inguinal (75.45%) and 13 suprascrotal (11.81%). Normal relationships between testis and epididymis were observed in 54 patients (62.1%) with no significant differences in relation to the patient's age (p=0.666). Epididymal tail disjunction was observed in 23 patients (26.44%), with no significant differences in relation to age (p=0.59). EAs associated to spermatic obstruction were observed in 16 patients (18.4%), also with no significant differences in relation to age (p=0.684). We did not observe significant correlation between the testis position and the incidence of EAs (p=0.119). We did not observe significant correlations between patency of the PV (64.7%) and incidence of EAs (p=0.742). Conclusions: Epididymal anomalies associated with spermatic obstruction are present in almost 20% of undescended testes, without significant correlation with age, testicular position and patency of the PV. This information needs to be correlated to the infertility risk of this congenital anomaly.

Abstract Purpose: To quantify and compare the expression of stromal elements in prostate adenocarcinoma of different Gleason scores with non-tumor area (control). Methods: We obtained 132 specimens from samples of prostate peripheral and transition zone. We analyzed the following elements of the extracellular matrix: collagen fibers, elastic system, smooth muscle fibers and blood vessels. The tumor area and non-tumor area (control) of the TMA (tissue microarray) were photographed and analyzed using the ImageJ software. Results: The comparison between the tumor area and the non-tumor area showed significant differences between stromal prostate elements. There was an increase of collagen fibers in the tumor area, mainly in Gleason 7. Elastic system fibers showed similar result, also from the Gleason 7. Blood vessels showed a significant increase occurred in all analyzed groups. The muscle fibers exhibited a different behavior, with a decrease in relation to the tumor area. Conclusions: There is a significant difference between the extracellular matrix in prostate cancer compared to the non-tumor area (control) especially in Gleason 7. Important modifications of the prostatic stromal elements strongly correlate with different Gleason scores and can contribute to predict the pathological staging of prostate cancer.

ABSTRACT High-fat diet-induced obesity is associated with metabolic disorders. The Brazil nut has bioactive substances and has been used to control the damage caused by obesity in several organs. The work intended to show the damage caused by high-fat diet in the bladder wall and if the Brazil nut oil added to the diet could ameliorate or reverse this effect. Sixty-day-old rats were divided into two groups: C (control, n = 30) and HF (high-fat, n = 30) diets. At 90 days, 10 animals of each group were sacrificed. The others were divided into 4 groups: C and HF (animals that maintained their previous diet, n = 10 for each group) and C / Bno and HF / Bno (animals whose control or high-fat diet was supplemented by Brazil nut oil, n = 10 for each group). Sacrifice occurred at 120 days, and the bladders were removed and analyzed. Epithelial height was increased in the HF compared to the C group. In contrast, the C / Bno had a lower epithelial height compared to the others. The percentage of collagen between the detrusor muscle fibers was significantly greater in C / Bno, HF and HF / Bno than in control group. The HF had a larger muscle fiber diameter than the C group, while the C / Bno presented lower values than the HF and HF / Bno groups. HF diets induced bladder wall damage. These changes in the rat's bladder wall were partially reversed by the Bno.

ABSTRACT Purpose: To evaluate if late hormonal replacement is able to recover the prostatic tissue modified by androgenic deprivation. Materials and Methods: 24 rats were assigned into a Sham group; an androgen deficient group, submitted to bilateral orchiectomy (Orch); and a group submitted to bilateral orchiectomy followed by testosterone replacement therapy (Orch+T). After 60 days from surgery blood was collected for determination of testosterone levels and the ventral prostate was collected for quantitative and qualitative microscopic analysis. The acinar epithelium height, the number of mast cells per field, and the densities of collagen fibers and acinar lumen were analyzed by stereological methods under light microscopy. The muscle fibers and types of collagen fibers were qualitatively assessed by scanning electron microscopy and polarization microscopy. Results: Hormone depletion (in group Orch) and return to normal levels (in group Orch+T) were effective as verified by serum testosterone analysis. The androgen deprivation promoted several alterations in the prostate: the acinar epithelium height diminished from 16.58±0.47 to 11.48±0.29μm; the number of mast cells per field presented increased from 0.45±0.07 to 2.83±0.25; collagen fibers density increased from 5.83±0.92 to 24.70±1.56%; and acinar lumen density decreased from 36.78±2.14 to 16.47±1.31%. Smooth muscle was also increased in Orch animals, and type I collagen fibers became more predominant in these animals. With the exception of the densities of collagen fibers and acinar lumen, in animals receiving testosterone replacement therapy all parameters became statistically similar to Sham. Collagen fibers density became lower and acinar lumen density became higher in Orch+T animals, when compared to Sham. This is the first study to demonstrate a relation between mast cells and testosterone levels in the prostate. This cells have been implicated in prostatic cancer and benign hyperplasia, although its specific role is not understood. Conclusion: Testosterone deprivation promotes major changes in the prostate of rats. The hormonal replacement therapy was effective in reversing these alterations.

Objetivos. Describir la disponibilidad de medicamentos psicotrópicos para el tratamiento de trastornos mentales en el Perú en los establecimientos del Ministerio de Salud. Materiales y métodos. Se analizaron informes mensuales de inventarios de medicamentos disponibles y de consumo en los establecimientos del Ministerio de Salud durante el año 2011. Usando un diseño transversal, se determinó la disponibilidad de siete clases de medicamentos psicotrópicos, distinguiendo el nivel de atención de los establecimientos, y el nivel de autonomía de decisión para la compra de medicamentos. Resultados. Los medicamentos ansiolíticos estuvieron disponibles en los establecimientos de salud de todos los niveles de atención. Los antidepresivos y antipsicóticos se encontraron disponibles en alrededor de dos tercios de los hospitales, y en menos del 20% de centros y puestos de salud. Las otras cuatro clases de medicamentos psicotrópicos (litio, hipnóticos y sedantes, psicoestimulantes/TDAH y drogas antidemencia) solo estuvieron disponibles en hospitales y mas no así en centros y puestos de salud. El 5% de los hospitales contó con suministro suficiente para satisfacer la demanda durante todos los meses del año. Conclusiones. Existe una importante brecha en la disponibilidad de medicamentos psicotrópicos en los establecimientos del Ministerio de Salud del Perú. Se observa esta situación tanto en hospitales como en el nivel primario de atención. Es necesario tomar acciones en la política y gestión de salud, incluyendo más financiamiento y una mayor capacitación en salud mental para el personal de la atención primaria, entre otras iniciativas.


Objectives. Describe the availability of psychotropic medications for the treatment of mental disorders in the health care facilities of the Ministry of Health of Peru. Materials and methods. Monthly inventory reports of the availability and consumption of medications in facilities of the Ministry of Health during 2011 were analyzed. Using a cross sectional design, the availability of seven classes of psychotropic medications was determined. Also, the level of care of the establishments and the level of decisional autonomy to purchase medications were determined. Results. Anti-anxiety medications were available in health facilities of all levels of care. Antidepressants and antipsychotics were available in about two thirds of hospitals and in less than 20% of health centers and small health clinics. The other four classes of psychotropic medications (lithium, hypnotics and sedatives, psychostimulants/ADHD, and anti-dementia drugs) were only available in hospitals and not in health centers and small health clinics. 5% of hospitals had a sufficient supply to meet the demand for the year. Conclusions. There is a significant gap in the availability of psychotropic medications in the health care facilities of the Ministry of Health of Peru. This was observed both in hospitals and in primary care facilities. Actions are needed in health policy and management, including more funding and greater mental health training for staff in primary care, among other initiatives.

Background: The geographical distribution of genes plays a key role in genetic epidemiology. The Chilean population has three major stem groups (Native American, European and African). Aim: To estimate the regional rate of American, European and African admixture of the Chilean population. Subjects and Methods: Forty single nucleotide polymorphisms (SNP´s) which exhibit substantially different frequencies between Amerindian populations (ancestry-informative markers or AIM´s), were genotyped in a sample of 923 Chilean participants to estimate individual genetic ancestry. Results: The American, European and African individual average admixture estimates for the 15 Chilean Regions were relatively homogeneous and not statistically different. However, higher American components were found in northern and southern Chile and higher European components were found in central Chile. A negative correlation between African admixture and latitude was observed. On the average, American and European genetic contributions were similar and significantly higher than the African contribution. Weighted mean American, European and African genetic contributions of 44.34% ± 3 9%, 51.85% ± 5.44% and 3.81% ± 0.45%, were estimated. Fifty two percent of subjects harbor African genes. Individuals with Aymara and Mapuche surnames have an American admixture of 58.64% and 68.33%, respectively. Conclusions: Half of the Chilean population harbors African genes. Participants with Aymara and Mapuche surnames had a higher American genetic contribution than the general Chilean population. These results confirm the usefulness of surnames as a frst approximation to determine genetic ancestry.

PURPOSE: To perform a quantitative assessment of different types of pelvic ureter tissues with chronic dilation in adults, using stereological methods. METHODS: We analyzed fragments of dilated pelvic ureters obtained from 6 patients aged between 35 and 67 years (mean, 46 years) who underwent ureteral reimplantation surgery for different reasons. The obstruction duration ranged from 27 to 180 days (mean, 93 days). The control group included fragments of normal pelvic ureters obtained during nephrectomy in 7 kidney transplant donors (age: range, 25-51 years; mean, 36 years). The volumetric density of collagen in the ureter, elastic fibers, and smooth muscle fibers was assessed. RESULTS: The volumetric density (Vv) of collagen showed no significant difference between the two groups (control: 45.3 ± 6.1; dilated: 40.8 ± 6.9; P = 0.23). A statistically significant increase in Vv of elastic fibers was observed in the dilated ureters (control: 18.4 ± 1.2; dilated: 24.6 ± 5.4; P = 0.03). A statistically significant increase in the Vv of smooth muscle fibers was observed in the dilated ureter (control: 42.0 ± 6.0; dilated: 56.2 ± 6.1; P = 0.001). CONCLUSION: The chronically dilated pelvic ureters had significantly more elastin and smooth muscle contents than the controls. .

PURPOSE: The aim of this study was to evaluate the expression of leptin and its receptor in histological sections of prostate tumors, and their association with prognostic factors. METHODS: A total of 532 surgical specimens from prostate cancer were studied. After histopathological diagnosis, the samples were included in tissue microarrays containing cores from tumor and non-tumor (benign prostatic hyperplasia) areas. These were immunostained with anti-leptin and anti-leptin-receptor antibodies. Objective and subjective analyses were performed. Student's-t-test and ANOVA were used to compare mean values, and linear regression was used to evaluate the correlation between histological results and prognostic indicators. RESULTS: Leptin receptor expression was reduced in tumors with a positive surgical margin, urethral margin involvement, and seminal vesicles invasion. Further, there was a negative correlation between the expression of leptin receptor in tumor areas and the sum of prognostic factors, suggesting that leptin receptor may predict the aggressiveness of disease. CONCLUSION: Our findings suggest that leptin receptor expression is a potential prognostic factor for PCa. Further investigation is needed to support the use of leptin receptor as a novel biomarker, although leptin itself does not seem to predict the aggressiveness of prostate cancer. .

Objetivo: Analisar, em fetos humanos, o crescimento da área do pênis, da túnica albugínea e das estruturas eréteis (corpos cavernosos e corpo esponjoso), bem como o aparecimento e modificações das principais estruturas que compõem estes tecidos (colágeno, músculo liso e fibra elástica) durante o período fetal (13 a 36 semanas pós-concepção), fornecendo padrões normativos de crescimento. Material e Métodos: Foram utilizados 56 fetos humanos do sexo masculino com idade gestacional compreendida entre 13 e 36 semanas pós-concepção (SPC). Foram utilizadas técnicas histoquímicas, imunohistoquímicas, e análises morfométricas, e analisados os seguintes parâmetros: área total do pênis, área do corpo cavernoso, área do corpo esponjoso e a espessura da túnica albugínea na região dorsal e ventral do corpo cavernoso. No corpo cavernoso e no corpo esponjoso, as fibras musculares, o colágeno e as fibras do sistema elástico, foram identificados e quantificados por percentagem, no programa Image J (NIH, Bethesda, EUA).Resultados: Da 13ª à 36ª semana pós-concepção, a área do pênis variou de 0,95mm2 a 24,25mm2. No mesmo período a área do corpo cavernoso variou de 0,28mm2 a 9,12mm2 e a área do corpo esponjoso de 0,14mm2 a 3,99mm2. Objective: To analyze the development of penile area, tunica albuginea thickness and the main components of the erectile tissue, such as collagen, smooth muscle and elastic system fibers, in human fetuses, in order to provide normative parameters of development. Material and Methods: We studied 56 male human fetuses aged 13 to 36 weeks postconception (WPC). We used histochemical, immunohistochemical and morphometric techniques to analyze the following parameters: total penile area, area of the corpora cavernosa, area of the corpus spongiosum and thickness of tunica albuginea in the dorsal and ventral region. In the corpora cavernosa and in the corpus spongiosum, the collagen, smooth muscle fibers and elastic system fibers were identified and quantified as percentage by using the Image J software (NIH, Bethesda, USA).Results: From 13 to 36 WPC, the area of the penis varied from 0.95 mm2 to 24.25 mm2. Also, the area of the corpora cavernosa varied from 0.28 mm2 to 9.12 mm2 and the area of the corpus spongiosum from 0.14 mm2 to 3.99 mm2. In the corpora cavernosa, the amount of collagen, smooth muscle fibers and elastic system fibers varied from 19.88% to 36.60%, from 4.39 % to 29.76 % and from 1.91% to 8.92%, respectively. In the corpus spongiosum, the amount of collagen, smooth muscle fibers and elastic system fibers varied from 34.65% to 45.89%, from 0.60% to 11.90% and from 3.22% to 11.93%, respectively. The elderly population in Brazil has been growing apace, and this demographic transition has increasingly led to a situation where the elderly are dependent upon their relatives. However, the development of a training program could contribute to an improvement in the services provided by these caregivers, enhancing quality of life for the elderly. The aim of this study is to evaluate the effect of a Training Program for Informal Caregivers on the quality of life of elderly people with a deficit in self-care. Home visits were initially made to 15 elderly patients and the SF-36 questionnaire was applied. A multi-professional training program for the caregivers of these elderly people was then developed. The SF-36 questionnaire was again applied after 2 months of the training program to verify its effectiveness on the quality of life of the elderly people. On average a significant increase of scores related to "mental health" and a significant decrease in "limitations of physical aspects" was detected. The training of informal groups of caregivers by professionals from the healthcare area should be encouraged in order to promote awareness, exchange experiences and discuss the best inherent strategies for care.


A proporção de idosos no Brasil vem crescendo consideravelmente e essa transição demográfica traz um quadro em que cada vez mais a sobrevivência deles fica dependente de seus familiares. Para aprimorar a qualidade de vida desse grupo há a proposta da elaboração de um programa de capacitação que contribua para melhorar os serviços prestados a estes indivíduos. O objetivo do presente estudo é avaliar um Programa de Capacitação para Cuidadores Informais na qualidade de vida de idosos. Foram realizadas visitas às residências de 15 idosos com déficit de autocuidado, onde foi aplicado o questionário SF-36. Em seguida realizou-se um programa multiprofissional de capacitação para seus cuidadores. Após 2 meses aplicou-se novamente o questionário para verificar a eficácia do programa de capacitação na qualidade de vida dos idosos. Houve um aumento significativo dos escores relacionados ao domínio "saúde mental" e uma diminuição significativa dos relacionados às "limitações por aspectos físicos". A partir dos dados obtidos concluiu-se que deve ser incentivada a formação de grupos de cuidadores informais, conduzidos por profissionais da área de saúde, para fomentar o conhecimento, a troca de experiências e a discussão sobre estratégias para melhorar o ato de cuidar.

Objetivos. Valorar el efecto predictivo de características claves de pacientes atendidos en salas de emergencia para detectar casos de uso problemático de alcohol. Materiales y Métodos. La muestra de estudio estuvo constituida por 371 personas atendidas en el lapso de siete días completos de enero de 2005 en el servicio de emergencia de un hospital público de Lima, Perú. Se aplicó un cuestionario demográfico, el SIDUC/CICAD para uso reciente de sustancias psicoactivas en salas de emergencias (i.e., uso dentro de las seis horas previas a la atención) y el AUDIT para uso problemático de alcohol en el último año. El análisis de regresión logística simple y multivariada permitió valorar el efecto predictor de la edad, sexo, especialidad del servicio de atención, presencia de daño físico y el uso reciente de alcohol para detectar casos problemáticos de su uso. Resultados. El odds de tener uso problemático de alcohol en los varones es 26 veces el odds de tener dicho problema entre las mujeres (p<0,001). El análisis estratificado por sexo y ajustado por edad y especialidad del servicio de emergencia, mostró que para los varones el uso reciente de alcohol estuvo fuertemente asociado con presentar uso problemático (OR=5,2; IC al 95 por ciento: 2,4-11,5; p<0,001), mientras que para las mujeres no se encontró dicha asociación. Conclusiones. Se debe tamizar uso problemático de alcohol en aquellos varones con uso reciente de alcohol atendidos en salas de emergencia, para poder identificarlos y darles la oportunidad de recibir consejería o un manejo médico adecuado.


Objectives. To assess the predictive effect of key individual-level characteristics to identify cases of alcohol use problems among patients visiting an emergency room. Materials and methods. The study sample was composed of 371 people attending an emergency room in a public hospital in Lima, Peru, during a period of seven complete days in January, 2005. For data gathering, we used a questionnaire for demographic information, the SIDUC/CICAD for recent use (i.e., in the last 6 hours) of psychoactive substances before arriving to the emergency room, and the AUDIT, to identify alcohol use problems in the last year. Univariate and multivariate logistic regression models were used to estimate the predictive effect of age, sex, area of attention in the emergency room, presence of physical injuries and recent use of alcohol. Results. The odds of being a case of alcohol use problem for males is 26 times the odds of having that problem for females (p-value<0,001). Stratified analysis by sex and adjusted by age and area of attention in the emergency room, showed that males who had recent use of alcohol were more likely of having alcohol use problems as compared to those who did not have recent alcohol use (OR=5.2; 95 percent CI: 2.4 - 11.5; p<0,001), while for females such an association did not exist. Conclusions. These results support screening initiatives for those males who have recent alcohol use before arriving to an emergency room in order to identify cases of alcohol-related problems and refer them for an appropriate counseling or medical treatment.

Objetivo: Valorar el consumo reciente de alcohol y otras variables como predictores del uso reciente y consumo de vida de alguna droga ilegal, entre personas atendidas en una sala de emergencia (SE) de un hospital de Lima. Materiales y Métodos: Este estudio es un análisis secundario de datos de un trabajo previo observacional de corte transversal realizado por Mazzotti et al. (2005), cuya muestra estuvo constituida por 371 pacientes, que involucraron a todos los que fueron atendidos en la SE (Medicina y Cirugía) del Hospital Nacional Cayetano Heredia durante siete días completos de enero del año 2005. Se aplicó la encuesta del Sistema Interamericano de Datos sobre el Uso de Drogas (SIDUC) desarrollada por la Comisión Interamericana para el Control del Abuso de Drogas (CICAD) para uso reciente de sustancias psicoactivas. Resultados: El consumo reciente de alguna droga ilegal estuvo fuertemente asociado con el consumo reciente de alcohol (OR=32,21; IC 95%=9,0-114,5; p<0,001) y el sexo masculino (OR=4,52; IC 95%=1,2-15,8; p=0,018), lo cual se mantuvo aún después de ajustar por posibles variables de confusión. Asimismo, estas variables (consumo reciente de alcohol y sexo) mostraron asociación significativa con el consumo de vida de marihuana y cocaína. Conclusiones: El consumo reciente de alcohol y el sexo predicen fuertemente tanto uso reciente como uso de vida de alguna droga ilegal en el contexto de una SE, por lo que estas características pueden ser utilizadas para estrategias de tamizaje dirigidas a la intervención y manejo de problemas de drogas a nivel de servicios hospitalarios.


Objective: To assess recent alcohol consumption and other variables as predictors of recent use and lifetime use of illegal drugs among people visiting an emergency room in a hospital in Lima. Materials and Methods: This study is a secondary analysis of data from a previous study by Mazzotti et al. (2005). It had a cross-sectional observational design, and the sample consisted of 371 patients, involving all patients who were treated in the emergency room (Medicine and Surgery) of the Hospital Nacional Cayetano Heredia for seven full days in January 2005. The Inter-American System of Uniform Drug-Use Data (SIDUC) survey developed by the Inter-American Drug Abuse Control Commission (CICAD) was applied for recent use of psychoactive substances in emergency rooms. Results: We found strong association between recent use of any illegal drugs and both having recent alcohol use (OR=32.21;95% CI=9.0-114.5; p<0.001) and being a male (OR=4.52; 95% CI=1.2-15.8; p=0.018). These associations were kept even after adjustment for meaningful covariates. In addition, these characteristics (recent alcohol consumption and male) showed significant association with lifetime use of marijuana and cocaine. Conclusions: Recent alcohol use and sex were strong predictors of both recent and lifetime use of illegal drugs. Therefore, these individual-level features can be used in hospital-based screening strategies aimed to identify drug-related problems and provide timely help.

Objetivo: Identificar factores asociados al éxito de la investigación en salud mental en América Latina y el Caribe, como base para el fortalecimiento de las capacidades de investigación en la región. Material y métodos: Se envió un cuestionario a 792 investigadores y se recibió respuesta de 216. Los probandos fueron clasificados en base a los montos de financiamiento de sus investigaciones y número de publicaciones, en: [1] investigadores con financiamiento superior a USD 5,000 en el último año y con al menos 2 publicaciones internacionales en los últimos 5 años (n=50), [2] investigadores con menor financiamiento/publicaciones (n=88). Setenta y ocho investigadores fueron excluidos de la clasificación por tener información incompleta en estos parámetros. Resultados: Tanto los investigadores en el grupo 1 (IG1) como aquellos en el grupo 2 (IG2) declararon como filiación principal una institución universitaria (66% y 50%, respectivamente); la segunda filiación fue con institutos de investigación en el caso de los de IG1 (48%), y con hospitales (32%) o el sector privado (30%) para los de IG2. Comparativamente, los miembros de IG1 mostraron mayor participación en la formación de recursos humanos (76 vs 47%), consultorías (58 vs 36%), redes colaborativas (78 vs 51%), edición (86 vs 57%) y revisión científica (80 vs 43%); en cuanto a disponibilidad de recursos para investigación, los de IG1 indicaron tener mayor acceso a recursos de internet (66vs.33%), a revistas especializadas (64 vs 43%); a apoyo en epidemiología/bioestadística (82 vs 67%) y ciencias básicas (80 vs 41%), y un mayor número de estudiantes graduados (84 vs 41%). Los grupos difirieron en su percepción de los retos para lograr una eficiente implementación de la investigación en salud mental: IG1 identificaron la falta de recursos humanos capacitados como el principal reto, mientras que IG2, la falta de cultura de investigación en sus instituciones.


Objective: To identify factors associated to success in Mental Health (MH) research in the Latin America and Caribbean region, in order to use them as a basis for the strengthening of research capacities. Material and methods: A questionnaire was sent to 792 researchers in the region. Respondents (n=216) were classified according to their research success in: Group 1 researchers (G1Rs): funded with more than USD 5,000 in the last year, authors of at least 2 international publications in the past 5 years (n=50). Group 2 researchers (G2Rs): those reporting less funding or publications (n=88). Researchers who showed missing data in either parameter were excluded (n=78). Results: The main affiliation of both G1Rs and G2Rs were universities (66% and 50%, respectively); the second affiliation of G1Rs were research institutes (48%) while G2Rs were affiliated to hospitals (32%) or the private sector (30%). Compared to G2Rs, G1Rs had higher involvement in human resource training (76 vs 47%), consultantions (58 vs 36%), networking with colleagues (78 vs 51%), participation as scientific editors (86 vs 57%) or reviewers (80 vs 43%), more access to paid internet resources (66 vs 33%), specialized journals (64 vs 43%). In terms of research resources, GR1s declared having more support in epidemiology/biostatistics (82 vs 67%) and basic sciences (80 vs 41%); and more graduate students (84 vs 41%). G1Rs and G2Rs also differed in prioritizing the challenges they envisage for implementing mental health research: G1Rs ranked the lack of trained resources as the main challenge, while G2Rs remarked on the lack of research culture in their institutions. Conclusions: Our results identify mental health care institutions as the main focus for strengthening of research capacity . This task could be achieved through an intensive interactive work with already stronger universities and research institutes in the region.

OBJECTIVE: The World Health Organization Atlas Project identified important deficiencies in world mental and neurological health resources. These deficiencies, especially evident in low and middle-income countries, can be overcome by improving research capacity. The objective of this study is to assess the status of mental and neurological research in Latin American countries and identify the main difficulties encountered in conducting research, publishing results, and shaping health policies, interventions, and programs. METHOD: Semi-structured interviews were conducted with 34 key informants from 13 Latin American countries. RESULTS: Participants reported that production of mental and neurological research in Latin American countries is low. Lack of financial and human resources, including lack of support from government agencies, were identified as the main factors contributing to the dearth of local research. The few research projects that do take place in Latin American countries are often funded at researchers' personal expense. Few policies, interventions, or programs are generated from research results. To address these deficiencies, participants called for training in research methodology, mechanisms for identifying funding opportunities, and greater recognition of their research products. CONCLUSIONS: Researchers and stakeholders recognize the need to mobilize local and international efforts aimed at strengthening research capacity and results implementation. This will lead to an overall optimization of mental and neurological research in the region.


OBJETIVO: El proyecto Atlas de la Organización Mundial de la Salud identifica importantes deficiencias en salud mental y neurológica. Estas deficiencias, especialmente evidentes en países de medianos y bajos ingresos, pueden resolverse mejorando las capacidades en investigación. El objetivo de este estudio es evaluar el estado de la investigación en salud mental y neurológica en países Latinoamericanos, e identificar las principales dificultades encontradas al hacer investigación, publicar resultados, y generar políticas, intervenciones, y programas. MÉTODO: Entrevistas semi-estructuradas fueron realizadas a 34 informantes de 13 países Latinoamericanos. RESULTADOS: La producción de investigación en salud mental y neurológica en países Latinoamericanos es escasa, debido principalmente a la carencia de recursos financieros y humanos, incluyendo el casi ausente apoyo de agencias gubernamentales. Los pocos proyectos de investigación que se llevan a cabo son financiados mayormente con recursos propios de los investigadores. Pocas políticas, intervenciones o programas son generados a partir de resultados de investigación. Resolver estas deficiencias requerirá entrenar profesionales en metodología de la investigación, identificar oportunidades de financiación y lograr un mayor reconocimiento de los productos de la investigación. CONCLUSIONES: Hay necesidad de movilizar esfuerzos locales e internacionales orientados a fortalecer las capacidades en investigación y la implementación de resultados. Esto llevará a una optimización general de la investigación.

Objetivo: evaluar el estado de la investigación en ciencias básicas (CB) dentro del ámbito de la salud mental (SM), en países de América Latina y el Caribe (pALC). Material y métodos: Se recopiló información de bases de datos de publicaciones (PubMed/PsycINFO), mediante buscadores de internet y muestreo bola de nieve, para identificar investigadores y otros actores involucrados en la gestión de la salud mental. Los 2555 actores identificados recibieron un cuestionario dirigido a evaluar su capacidad personal/ institucional para realizar investigación en SM. Tanto la base de datos de publicaciones como las respuestas al cuestionario fueron utilizadas para el análisis. Resultados: Se identificó 2397 publicaciones en SM, de las cuales 222 estuvieron vinculadas a las CB. Sólo 9 de los 30 ALC tenían publicaciones en esta área. Uno de cada cuatro investigadores en Sm de la región la mayoría psiquiatras y neurólogos declaró tener entrenamiento formal en Cb. El 41% de los investigadores declaró contar con apoyo técnico para la investigación en CB en sus instituciones. Sólo 10% de los proyectos de investigación realizados en os últimos 5 años estuvieron relacionados a las CB. De estos, un 62% se realizó con colaboración local, y 30% con colaboradores de países desarrollados. La financiación provino principalmente de fundaciones, ONGs y universidades. Las principales motivaciones de los investigadores de SM en el área de las CB fueron el interés personal (77%), y la carga de enfermedad (63%). La investigación en CB no fue considerada una prioridad por la mayor parte de los investigadores (55%) y tomadores de decisión (56%). Los investigadores con entrenamiento formal en CB tuvieron más publicaciones y accedieron a mayores fondos de investigación que el resto de investigadores. Conclusiones: La investigación en CB en el área de SM permanece relegada en la región.


Objective: To evaluate the status of basic science (BSci) research in mental health (MH) in Latin American and the Caribbean (LAC) countries. Material and methods: A questionnaire was sent to 2555 MH researchers and stakeholders identified by a mapping process through publication databases (PubMed/PsycINFO), internet searches of institutions, and snowball sampling. The Questionnaire was designed to obtain information about their capacity for MH research. Both, the publication database and the questionnaire responses were the basis for all analyses. Results: Two hundred twenty-two out of 2397 MH publications were related to BSci. Only 9 LAC countries had MH publications in this area. One out of four researchers in MH in the region most of them psychiatrists and neurologists had formal training in BSci research methods. Forty one percent of them declared to have technical support for BSci research from their institutions. Barely 10% of the research projects in the past 5 year had a BSci approach. From them, 62% were local projects, and% were done in collaborations with developed countries, being funded mainly by NGOs or foundations. The main motivations for research in BSci were personal interest (77%) and burden of disease (63%). BSci research was not considered a priority by most researchers (55%) or stakeholders (56%). BSci researchers published more and had better financing than non-BSci is an integral component of MH research and cannot de set aside when establishing agendas and setting priorities in this health area. Given their characteristics, BSci researchers could effectively contribute to improve MH research capacity in the region.

OBJETIVOS: Validar la Escala de Depresión de Edinburgh (EDS) para mujeres peruanas que no están en el período postparto. METODOS: A 104 mujeres nulíparas o cuyo último hijo tenía más de 1 año se les aplicó la EDS y la sección del SCID para el diagnóstico de depresión según el DSM-IV. RESULTADOS: Las mujeres con depresión mayor o menor tuvieron puntajes de EDS significativamente mayores que las deprimidas. La EDS con un punto de corte de 17.5 tuvo una sensibilidad de 80 por ciento y una especificidad de 88.3 por ciento para la identificación de depresión mayor, y con un punto de corte de 15.5 tuvo una sensibilidad de 80 por ciento y una especificidad de 86.9 por ciento para la detección de depresión menor o mayor. Ninguna mujer con EDS menor de 12.5 tuvo depresión menor o mayor; y las 2 pacientes que tuvieron EDS por encima de 24.5 tuvieron depresión mayor. El puntaje de EDS correlacionó con el número de síntomas depresivos del DSM-IV (r=0.6386, p<0.001). Se halló un coeficiente alfa de Cronbach de 0.8213. CONCLUSIONES: La traducción al español de la EDS es válida para la evaluación de depresión en mujeres que no están en el postparto.
