RESUMO

Sclerosing stromal tumor (SST) is an uncommon benign tumor of the ovary, representing 2~6% of all stromal ovarian tumors. It is a distinct benign neoplasm that differs from fibromas, thecomas, luteinized tumors and steroid cell tumors. It presents most often with non-specific symptoms and tumor markers usually remain normal. However massive ascites is a rare symptom. We report a case of sclerosing stromal tumor with massive ascites and elevated CA 125 with brief review of literature.

RESUMO

Steroid cell tumors, not otherwise specified (NOS), are infrequently encountered ovarian neoplasms. They constitute <0.1% of all ovarian tumors. They usually occur in younger individuals (mean age, 43 years) and, in contrast to other steroid cell tumors, occasionally occur before puberty. The majority of these tumors produce steroids with testosterone being the most common. Various virilizing symptoms such as hirsutism, temporal baldness, and amenorrhea are common in these patients; however massive ascites is an infrequent symptom. We report a case of steroid cell tumor, not otherwise specified, with hypertension, obesity, ascites and elevated CA 125 with brief review of literature.

RESUMO

Clear cell carcinoma of the uterine cervix is rare type of the uterine cervical adenocarcinoma. Although uterine cervical adenocarcinomas presently represent 20% to 30% of cervical cancers in the industrialized countries, the clear cell carcinoma of uterine cervix is very rare malignancy that accounts for 4% to 9% of the adenocarcinoma of uterine cervix. This malignancy occurs in two distinct age groups; those younger than 24 years and those older than 45 years. In younger patients, most of these malignancies are mainly related to prenatal diethylstilbestrol (DES) exposure, but older patients are unrelated to in utero DES-exposure. We experienced a case of clear cell carcinoma of the uterine cervix of 34 years old housewife who was not related to prenatal DES-exposure. We presented a case with a brief review of related literatures.

RESUMO

The squamous cell carcinoma of ovary is very rare in the ovarian malignancy. The majority of squamous cell carcinomas of ovary are considered to be associated with mature cystic teratoma, Brenner tumor and malignant degenerated endometriosis. Therefore, pure ovarian squamous cell carcinoma is extremely rare. The pathogenesis, prognosis and proper treatment guideline of this malignancy are still unknown, because of it's rarity. Recently we experienced a case of primary squamous cell carcinoma without previously commented disease condition. After nearby cytoreductive surgery, postoperative chemotherapy with radiotherapy has been undergoing. We report this malignancy with a brief review of literature.

RESUMO

Carcinomas of Bartholin's gland are rare tumor that account for less than 0.001% of all gynecological malignancy, and account for 2-7% of vulvar malignancy. The treatment of Bartholin's gland carcinoma is not established yet. So it should be individualized to the patient by the resectability and the status of resection margin involvement. We experienced a case of squamous cell carcinoma of the Bartholin's gland treated by wide local excision and post-operative radiation therapy. We report this malignancy with a brief review of literature.

OBJECTIVE: The purpose of this study was to assess the impact of vascular endothelial growth factor (VEGF) in pregnancies with mild and severe preeclampsia. METHODS: From January 1999 to June 2001, we studied the severity for pregnant women with pregnancy induced hypertension between 28 and 40 weeks gestation. In the mild (n=46) and severe preeclamptic women (n=28), the laboratory evaluation included liver function test, platelet counts, and serum creatinine. The systolic/diastolic (S/D) ratio of the fetal umbilical artery flow for placental resistance was measured by ultrasonographic doppler velocimetry. To detect the damage of vascular endothelial cells in all preeclamptic women, serum concentrations of VEGF were measured. RESULTS: Severe preeclampsia had more elevated liver enzymes, thrombocytopenia, high creatinine than mild preeclampsia. HELLP syndrome (hemolysis, elevated liver enzymes, and low platelets) was encountered in 8/28 (28.6%) of severe preeclampsia. Fifteen out of twenty-eight cases (53.6%) in severe preeclampsia had elevated S/D ratio from 3.0 to 4.5 including 6 cases with absent end diastolic velocity, whereas 4/46 (8.7%) was elevated SD ratio (>3.0) in mild preeclampsia. Serum concentrations of VEGF were elevated in both mild (7.5+/-4.9 ng/mL, p<0.05) and severe preeclampsia (19.3+/-8.8 ng/mL, p<0.05) compared to normal pregnancy (0.5~2.1 ng/mL). CONCLUSION: The higher serum concentration of VEGF and elevated S/D ratio of umbilical artery were responsible for the changes of the resistance of placental blood flow in severe preeclampsia. Furthermore, elevated S/D ratio of umbilical artery velocity was essential as a surveillance method of fetal health status with IUGR (Intrauterine growth restriction) by vascular declination of placenta.

Baicalin is flavonoid and major component of PC-SPES. Flavonoids including baicalin have been reported to not only function as anti-oxidant but also cause cytotoxic effect. Baicalin hydrate has been reported to induce cell death, however the mechanism by which baicalin hydrate induces the apoptosis of cancer cells is still unclear. To evaluate the mechanistic insights of apoptosis by baicalin hydrate, we tested the activities of apoptosis signaling pathway in HeLa cells. The viability of HeLa and HeLa s3 cells was markedly decreased by baicalin hydrate in a dose- and time- dependent method. Baicalin hydrate induced the apoptotic death of HeLa cells, which was characterized by the chromatin condensation of the nuclei and phosphorylation of histone H2AX. Baicalin hydrate increased the sub-G1 DNA content of HeLa cell lines. Baicalin hydrate digested Bid protein, increased Bak protein level and also, induced mitochondrial dysfunction disrupted as shown as the mitochondrial membrane potential. It activated caspase-3, thereby resulted in cleavage of poly (ADP) ribose polymerase (PARP).

Cis-diamminedichloroplatinum II (cisplatin) has been reported to induce cell death. However, the mechanism by which cisplatin is induced the apoptosis of cancer cells is still unclear. To evaluate the mechanistic insights of apoptosis by cisplatin, we tested the activities of apoptotic signaling pathway in HeLa cells. Apoptotic death of HeLa cells by cisplatin was confirmed by ladder-pattern fragmentation of genomic DNA. Cisplatin induced the activation of caspase-3 and 9 proteases in a time dependent manner. The caspase-3 protease activation and the cleavage of poly (ADP-ribose) polymerase (PARP) and procaspase-3 was demonstrated. We also showed that the expression of Bcl-2 and Bcl-xL was markedly decreased by the addition of cisplatin in HeLa cells. Moreover, expression of Fas and FasL proteins was increased by cisplatin. These data suggest that cisplatin triggers the activation of apoptotic signaling pathway in human cervical cancer, HeLa cells, via affects the expression of Fas, FasL and Bcl-2 families as well as triggers the activation of caspase-3 and -9 proteases.

Angiomyofibroblastoma of the vulva is a recently described mesenchymal tumor. It can be distinguished from an aggressive angiomyxoma by its circumscribed borders, higher cellularity, more numerous blood vessels, the frequent presence of stromal cells, minimal stromal mucin, and rarity of erythrocyte extravasation. We experienced a case of angiomyofibroblastoma of the vulva occurring in a 52-year-old woman. The lesion was a well-circumscribed mass, measuring 4x5 cm in size. Histologically, the mass was characterized by alternating hypercellular and hypocellular edematous zones in which abundant blood vessels were irregularly distributed. The stromal cells were concentrated around the blood vessels, or loosely dispersed in the hypocellular area. Immunohistochemically, the stromal cells were diffusely reactive for vimentin, focally reactive for desmin, but not for smooth muscle actin.

OBJECTIVE: To determine whether oxidants are formed as part of the cisplatin-induced apoptotic process, intracellular markers of oxidative stress were examined. METHODS: Apoptotic death of HeLa cells by cisplatin was confirmed by flow cytometry. RESULTS: The pre-treatment with glutathione (GSH) significantly attenuated cisplatin-induced apoptosis through the reduction of reactive oxygen species (ROS) accumulation and diminished caspases-3 and 9 protease activity. Furthermore, z-VAD-fmk, an inhibitor of pan-caspase, effectively inhibited the activation of caspases and prevented apoptosis by cisplatin, although cisplatin-induced ROS generation was not attenuated. CONCLUSION: These data indicate that ROS may play a role as an upstream mediator of caspases. Taken together, our results suggest that oxidative stress mediates cisplatin-induced apoptosis in HeLa cells.

Fetal teratomas are histologically diverse masses containing variety of tissues foreign to the anatomic site in which they reside. Sacrococcygeal teratomas are the most common and account for 50% of neonatal teratoma. Faciocervical teratomas are rare and accounts for only 5.5% of all neonatal teratoma, having an incidence of only 20,000:1 to 40,000:1 of live births. Immature teratoma was composed of poor differentiated or embryonal tissue which coexisted mature tissue and immature tissue was originated from neuroepithelium. We have experienced a large faciocervical teratoma and present this case with a brief review of literatures.

RESUMO

The Sertoli-Leydig cell tumor is a gonadal tumor of sex-cord type, similar to that seen in the various phase of testicular development in the male. It is the most common type of all virilizing ovarian tumors and account for less than 0.5% of all ovarian tumors. This case has been presented of a 16 years old girl who had Sertoli-Leydig cell tumor of the right ovary. The clinical manifestation included deepening voice and hirsutism. The removal of the tumor, which proved to be a Sertoli-Leydig cell tumor, so we present it with a brief review of literature.

OBJECTIVE: The aim of this study was to identify human papillomavirus (HPV) in cervical intraepithelial neoplasia (CIN) and to evaluate the persistence of HPV after loop electrosurgical excision precedure (LEEP). METHODS: Records of 138 patients with LEEP performed due to cervicitis, CIN I, CIN II and CIN III were reviewed and persistence of HPV were followed up at 3, 6, 9 and 12 month after LEEP treatment from January 1996 to December 1999. RESULTS: Among 202 patients, 138 cases (68.3%) were showed HPV positive before LEEP and histologic findings were showed as cervicitis 20 cases (14.5%), CIN I 23 cases (16.7%), CIN II 31 cases (22.5%) and CIN III 64 cases (46.3%). Persistence of HPV after LEEP treatment were 10.9% (15/138) at 3 month and 2.9% (4/138) at 6 month. However, among 15 cases of persistent HPV, only 5 cases (33.3%) were showed an abnormal Pap smear at 3 month after LEEP. CONCLUSION: Human papillomavirus (HPV) was eliminated over 90% at 3 month after LEEP treatment in CIN. Detection of HPV was helpful to estimate therapeutic effect and to predict the recurrence or persistence of CIN after LEEP treatment.

The association of carcinoma and tuberculosis in any organ is rare and in the endometrium is extremely rare. Only a small number of cases of endometrial tuberculosis with coexisting endometrial adenocarcinoma have been reported in the literature. We experienced a case of endometrial tuberculosis with coexisting endometrial adenocarcinoma and report with a brief review of literature.

RESUMO

Primary transitional cell carcinoma of the ovary is recently recognized, as one of the main types of ovarian carcinoma. Histologically, it is distinguished from malignant Brenner tumor only in the abscence of benign or proliferative Brenner tumor component. primary transitional cell carcinomas are more aggressive than malignant Brenner tumors. However, Primary transitional cell carcinomas have a better response to chemotherapy than other types of ovarian carcinomas. We report a case of primary transitional cell carcinoma presenting as both ovarian masses that developed in a 60-year-old woman with a brief review of literature.

RESUMO

Placental site trophoblastic tumor(PSTT) is a rare form of gestational trophoblastic disease. This rare tumor has the potential for metastasis and death. The average age at onset of PSTT is 28 years, and the second believed to be postmenopausal at the time of diagnosis. The most common presenting complaint is amenorrhea or abnormal vaginal bleeding and rare complaints are virilization, nephrotic syndrome, uterine rupture, etc. The treatment of choice is surgical removal of the tumor, that is total abdominal hysterectomy with both salpingo-oophorectomy. We present a case of placental site trophoblastic tumor associated with spontaneous uterine rupture that is rare complication with a brief review of literature.

OBJECTIVE: To evaluate the effect of serum obtained before and after treatment for endometriosis on in vitro fertilization and development of two cell mouse embryo. Design: Pretreatment and posttreatment comparoson of fertilization of mouse oocyte and embryo development in serum supplement form patients with endometriosis; result were compared using Stuent T-test analysis. METHOD: Infertility Clinic, Department of Obstetrics and Gynecology, Collage of Medicine, Won Kwang university, Korea. Patients was chosed eleven consecutive women with endometriosis. Interventions was all patient underwent laparoscopic or conservative surgery. This was followed by a 6-month course of burserelin acetate 900 microgram/d. Main outcome was measured total number of fertilization and embryo that was fertilization after 24 hours and reached blastocyst stage after 72 hours of incubation were compared before and after treatment. RESULT: Before treatment, 47% of the oocyte were fertilized and 31% of the embryo reached blastocyst stage. After treatment, Significantly more fertilized and Significantly more embryo developed to blastocyst on the stage I and II of endometriosis. CONCLUSION: The fertilization and embryo toxicity of serum samples from patients with endometriosis is lost after treatment.
