RESUMO

This paper reports a case of atypical cholesterol side chain enzyme deficiency caused possibly by CYP11A1p.A359V heterozygous mutation. The patient was diagnosed as congenital adrenal hyperplasia because he was born with gonadal dysplasia and there were manifestations of salt losing and pigment changing, combined with high level of plasma ACTH. Further genetic screening revealed CYP11A1p.A359V heterozygous mutations in patients with a final diagnosis of cholesterol side chain enzyme deficiency which was treated effectively to glucocorticoid.

RESUMO

Objective To investigate the application of semi-quantitative analysis of dual energy lung perfusion imaging(DEPI)in patients with acute pulmonary embolism(PE)before and after treatment. Methods 25 patients with acute PE diagnosed by dual source CT were recruited consecutively. All of them underwent CT pulmonary angiography(CTPA)and DEPI before and after treatment. Radiologists visually evaluated the PE numbers, its distribution(central or peripheral)and its nature(occlusive or non-occlusive)on CTPA.DEPI visual analysis and semi-quantitative analysis before and after treatment in different types of PE evaluated by CTPA were analyzed statistically. Results Four types of PE(central occlusive PE,central non-occlusive PE,peripheral occlusive PE and peripheral non-occlusive PE)were classified according to embolic sites and the extent of embolization.Comparing to CTPA as a standard reference, the positive rates of DEPI visual analysis were 100.00%,70.54%,87.23%,76.19% before treatment,100.00%, 76.67%,86.67%,77.78% after treatment, respectively.Significant differences were found in central occlusive PE,central non-occlusive PE and peripheral occlusive PE with mean difference of CT values before and after treatment[(70.7 ± 10.1)HU vs(29.8 ± 6.0)HU,P<0.01;(50.1 ± 9.6)HU vs(29.3 ± 7.8)HU,P< 0.01;(53.4 ± 11.4)HU vs(27.4 ± 8.9)HU,P<0.01,respectively].Although(24.5 ± 8.2)HU (after treatment)were lower than(28.5 ± 9.0)HU(before treatment),no significant difference was found in the type of peripheral non-occlusive PE(P> 0.05).Conclusion The combination of CTPA and DEPI can offer more comprehensive information of the acute PE therapeutic evaluation before and after treatment.

RESUMO

Objective To identify various arteriographic manifestations and the curative effects of emergency embolization of renal hemorrhage after percutaneous nephrolithotomy. Methods Emergency arteriography was performed in 21 patients of renal hemor-rhage after percutaneous nephrolithotomy.Emergency embolotherapy was conducted by using different embolic agents such as gelat-in sponge, polyvinyl alcohol (PVA)particles and coils after bleeding arteries were identified. The arteriographic manifestations and the curative effects of the treatment for renal hemorrhage were retrospectively analyzed. Results Contrast extravasations were con-firmed in 21 patients, among them,4 contrast extravasations with arteriovenous fistulas(AVF)of renal artery;3 pseudoaneurysms of renal artery. The hemorrhage completely stopped after emergent embolotherapy in 21 cases.During follow-up,no rebleedings and severe complications occurred. Conclusion Emergency arterial embolization for renal hemorrhage after percutaneous nephrolithotomy is a safe and effective treatment. The key to successful treatment is to select embolization agent reasonably and embolize the bleeding arteries thoroughly and completely.

RESUMO

Succinic acid production by fermentation from biomass, especially the lignocellulosic hydrolysate, is an alternative to chemical synthesis. Many studies report the inhibition of cell growth and succinic acid production from lignocellulosic hydrolysate, hardly is known about the actual kinetic and mechanism of the inhibition of individual factors. In this study, we studied inhibition effects of furfurals and 5-hydroxymethylfurfural (5-HMF) on cell growth and succinic acid production of engineered E. coli. Cell growth and succinic acid titer were severely inhibited by furfural and HMF with both concentrations higher than 0.8 g/L. Cell growth was totally inhibited when the concentration of furfural was above 6.4 g/L, or the concentration of HMF was above 12.8 g/L. At the concentration of maximum toleration, which was 3.2 g/L, furfural decreased the cell mass by 77.8% and the succinic acid titer by 36.1%. HMF decreased the cell mass by 13.6% and the succinic acid titer by 18.3%. Activity measurements of key enzymes revealed that phosphoenolpyruvate carboxylase, malate dehydrogenase, fumarate reductase all were inhibited by furfural and HMF. This study gave a quantitative view to the succinic acid production under the inhibition of lignocellulose degradation products and will help overcome the difficulties of the lignocellulosic hydrolysate fermentation.
