RESUMO

【Objective】 To investigate the expression of Toll-like receptor 9 (TLR9) in B cells in the peripheral blood of patients with allergic rhinitis (AR), allergic asthma (AA), AR combined with AA (ARA) and the blood or lung tissue of sensitized mice, as well as the effect of allergens on its expression. 【Methods】 A total of 100 volunteers from The First Affiliated Hospital of Jinzhou Medical University were recruited for outpatient and acute inpatient attacks, consisting of 19 healthy people (HC) with negative prick test result, 40 AR patients, 26 AA patients, and 15 ARA patients with positive prick test result. The expression of TLR9 in the peripheral blood B cells of the patients before and after stimulation by house dust mite allergen extract (HDME), Artemisia sieversiana wild allergen extract (ASWE), and Platanus pollen allergen extract (PPE) was detected by flow cytometry. AR and AA sensitization models were established in WT mice and FcεRI-KO mice to detect the effects of allergens and FcεRI on the expression of TLR9 in B cells. 【Results】 The expression and mean fluorescence intensity (MFI) of TLR9 in peripheral blood B cells of unstimulated AR, AA and ARA patients were higher than those of HC. After allergen stimulation, the expression of TLR9 and its MFI in blood B cells of AR and AA patients increased (P<0.05). In WT mice and FcεRI-KO mice, compared with NS control mice, MFI was increased in almost each group. Compared with the NS control group, there was no significant difference in the expression of TLR9+ in B cells in the lung tissues of AA mice with FcεRI-KO after allergen challenge, but their MFI increased. FcεRI-KO mice had lower TLR9+ MFI in B cells after allergen challenge compared with WT mice. 【Conclusion】 TLR9 in B cells may be involved in the occurrence of AR and AA, and detecting the expression of TLR9 in B cells may be a new direction for the diagnosis of AR and AA.

RESUMO

Objective:To explore the impact of hypophosphatemia on the occurrence and prognosis of critically ill patient. Methods:The clinical data of critically ill patients admitted to the intensive care unit (ICU) of Tianjin First Central Hospital from October 2021 to April 2022 were retrospectively analyzed. Patients were divided into hypophosphatemia group (serum phosphorus level < 0.80 mmol/L) and non-hypophosphatemia group (serum phosphorus level ≥ 0.80 mmol/L) when they were admitted to the ICU. The following variables were also collected, including gender, age, acute physiology and chronic health evaluationⅡ(APACHE Ⅱ), sequential organ failure assessment (SOFA), serum phosphorus level, serum calcium level, serum magnesium level, white blood cell count (WBC), neutrophil count (NEU), lymphocyte count (LYM), neutrophil/lymphocyte ratio (NLR), C-reactive protein (CRP), presence of infection and infection site, length of hospital stay, ICU stay, 28-day mortality, and mechanical ventilation time. Multivariate Logistic regression analysis was used to evaluate the relationship between each variable and the 28-day mortality. The receiver operator characteristic curve (ROC curve) was drawn, and the area under the ROC curve (AUC) and 95% confidence interval (95% CI) were calculated to evaluate the predictive value of serum phosphorus levels for the prognosis of ICU patients. Results:A total of 263 patients were enrolled, including 54 patients with hypophosphatemia and 209 patients without. The SOFA score, LYM level and the infection rate of patients in the hypophosphatemia group were significantly higher than those in the non-hypophosphatemia group [SOFA score: 6.70±3.17 vs. 5.64±3.59, LYM (×10 9/L): 0.99±0.54 vs. 0.77±0.54, infection rate: 77.78% (42/54) vs. 59.33% (124/209), all P < 0.05], the NLR was significantly lower than that of the non-hypophosphatemia group [10.67 (7.08, 18.02) vs. 12.25 (7.25, 21.68), P < 0.05]. The length of hospital stay, ICU stay, and mechanical ventilation duration in the hypophosphatemia group were significantly longer than those in the non-hypophosphatemia group [length of hospital stay (days): 15 (11, 28) vs. 12 (6, 21), length of ICU stay (days): 10.35±7.80 vs 7.15±6.61, mechanical ventilation duration (days): 3 (0, 12) vs. 2 (0, 5), all P < 0.05]. There was no significant difference in the 28-day mortality between the hypophosphatemia group and the non-hypophosphatemia group [9.26% (5/54) vs. 11.00% (23/209), P > 0.05]. Multivariate Logistic regression analysis showed that APACHE Ⅱ score [odds ratio ( OR) = 1.188, 95% CI was 1.110-1.271], CRP ( OR = 1.016, 95% CI was 1.007-1.026), and NLR ( OR = 1.002, 95% CI was 0.996-1.008) were independent risk factors affecting the 28-day mortality of critically ill patients in ICU (all P < 0.05). ROC curve analysis showed that the AUC of serum phosphorus levels for predicting the length of hospital stay of critically ill patients in ICU > 10 days, ICU stay > 5 days, and mechanical ventilation duration > 5 days were 0.701 (95% CI was 0.632-0.770), 0.771 (95% CI was 0.691-0.852), 0.617 (95% CI was 0.541-0.692), respectively, all P < 0.01. Conclusion:Hypophosphatemia has some predictive value for the length of hospital and ICU stay and mechanical ventilation time in critically ill patients, but it cannot predict the 28-day mortality.

RESUMO

Aim To observe the effect of gypenosides ( GP) on the expression of receptor for advanced gly-cated endproducts ( RAGE ) and transforming growth factor-β1 ( TGF-β1 ) in human mesangial cells( HMCs) induced by AGEs. Methods HMCs were cultured in DMEM of low glucose containing 15% fetal bovine ser-um in vitro, which were divided into four groups: the normal group, model group, GP group and positive control group. In addition to the normal group, the other groups were stimulated by AGEs ( 200 mg · L-1 );furthermore, GP group was intervened with dif-ferent concentrations(25,75,175 mg·L-1) of GP, while control group was given 10 mmol · L-1 of amin-oguanidine hydrochloride. The expression of RAGE and TGF-β1 protein of each group was detected by Western blot; the expression of RAGE and TGF-β1 mRNA of each group was detected by RT-PCR. Re-sults The expression of RAGE, TGF-β1 protein and mRNA in HMCs induced by AGEs in the model group was significantly higher than that of the normal group ( P<0. 01 );compared with the positive control group ( P<0. 01 ) , GP could obviously reduce the expression of RAGE, TGF-β1 protein and mRNA in a dose-de-pendent manner. Conclusion GP can reduce the ex-pression of RAGE in HMCs induced by AGEs, block AGEs-RAGE signaling pathway and decrease the ex-pression of the downstream factor TGF-β1 , therefore, it plays the role in the resistance of rennal fibrosis in DN.

RESUMO

This paper was aimed to analyze the theoretical connotation with liver bodyyin with function yang, and to explain the physiological and pathological mechanism of body yin and function yang, respectively. The mechanism of its corresponding syndrome and symptoms were described and the composing principle of classical prescription for liver regulation was analyzed. The clinical significance of liver body yin with function yangwas confirmed by clinical cases. This paper argued that the liver ascending characteristics, which is a necessary guarantee for ascendingqi in the human body to adapt to the ascending gas of the world, is the key point to maintain balance of qi ascending and descending in the body. The liver ascending disorder mainly manifested for the excessive ascending and ascending deficiency, which analyzed the pathogenesis characteristic and clinical manifestation of the liver qi ascending disorder. This paper put forward a new view that the liver storing blood referred to the convergence and the conservation properties of the liver, which demonstrated the relationship between liver storing blood and controlling dispersion. It suggested that liver blood convergences liverqi so as not to make liverqi too excessive. The liver storing blood can raise spirit. Liver blood is the source of menstruation, and etc. This paper finally put forward the clinical prescription viewpoint of soothing liver and dropping inverse should raiseyin and gentle liver necessarily, releasing liver should raise liver and nourish blood and clearing away the liver-fire should cool blood and nourishyin. This research had certain clinical value.

RESUMO

Objective To investigate the potential influence of TNF on the expression of protease activated receptor (PAR)-1,2,3 and 4 by using P815 mast cells. Methods After being challenged with various concentrations of TNF for 2 h, 6 h and 16 h, the P815 mast cells were treated with or without Triton X-100 and the PAR expressions were detected by flow cytometry and immunofluorescent microscopy. Results Compared with the corresponding controls, TNF concentration-dependently upregulated expressions of PAR-2 and PAR-4 both in Triton X-100-treated and the untreated groups, but had no significant effect on the expression of PAR-1 and PAR-3. Moreover, no significantly different expressions of TNF-induced PAR-1, 2, 3 were observed between Triton X-100-treated and the untreated groups, whereas Triton X-100-treated PAR-4 expressions were significantly enhanced by TNF compared with the Triton X-100-untreated ones. Conclusion TNF can up-regulate PAR-2, 4 expression of P815 mast cells but has little effect on the expression of PAR-1, 3 correspondingly. And Triton X-100 treatment had no significant effect on TNF-modulated expression of PARs in P815 mast cells.

Objective To explore the behavioral dimensions and validity of the light-dark box(LDB),as an model of state anxiety in Kunming mice. Methods The behavior of adult, male,Kunming mice in LDB was recorded twice, for five minutes, with a one-week inter-trial interval. The following parameters were evaluated by factor analysis:latence for the first entry into the dark area(Latence);percentage of time exploring in the light area (Ltime%);percentage of squares crossing in the light area(Lcross%);percentage of rears in the light area (Lrear%);transitions between two areas(Transition);total number of squares crossing in the whole apparatus (Cross);total number of rears in the whole apparatus(Rear);and number of fecal boli(FB).Results There was a good intercorrelation both among Ltime%,Lcross%,Lrear%,and among Transition,Cross,Rear.A good correlation also was found between test and retest for Transition,Rear and FB.Ltime%,Lcress%and Lrear%had significant loadings on Anxiety factor(Loading%:39.54%,35.40%);Transition loaded on Locomotor factor(Loading%:23.78%,19.66%),combined with Cross;Rear loaded on Exploration factor(Loading%:13.75%,22.60%);and FB loaded on Emotionality factor(Loading%:14.42%,13.02%);Latence was not stable. Conclusion Light-dark box,as an animal model of state anxiety in Kunming mice, comprised three behavioral dimensions:anxiety factor,LDB-activity factor(locomotor factor and exploration factor)and emotionality factor.

RESUMO

<p><b>OBJECTIVE</b>To explore the effects of Baixiangdan capsule (BXD), a Chinese medicinal formula, on the premenstrual syndrome (PMS) rats with Liver-qi invasion and the possible underlying micro-mechanisms.</p><p><b>METHOD</b>Wistar rats were randomly assigned to two groups, nomal group and BXD group After evaluated by macro-behavior observation and open-field test, MTT assay and the whole-cell patch clamp recording were performed respectively to evaluate the effects of serum from BXD capsule-treated rats on the viability and GABA(A) R-induced currents of cortical neurons in vitro.</p><p><b>RESULT</b>In the open-field test, the crossing score, rearing score and total score of BXD rats decreased significantly (P < 0.05), compared by the normal rats. Compared with cells exposed to serum of normal rats, the viability values of those incubated with serum of BXD group for 24 h and 48 h significantly increased (P < 0.05), measured by MTT assay. The results of whole-cell patch clamp recording showed that concentration-response relationship curves revealed an EC50 value of (29.0 +/- 4.4) micromol x L(-1) and a Hill coefficient of 1.07 for normal-exposed cultures, (63.5 +/- 8.2) micromol x L(-1), 1.04 for BXD-exposed cultures after incubation for 24 h. Furthermore, the difference in EC50 values was statistically significant (P < 0.01), that in the Hill coefficient was not obvious.</p><p><b>CONCLUSION</b>BXD capsule could significantly decrease crossing score and total score of open-field test and effectively enhance the neuron viability and GABA(A) receptor activity in rat cortex. Paeonimetabolins I and paeonol may play a significant role in treating PMS model rats with Liver-qi invasion by BXD capsule, and paeonol may target at GABA(A) receptor, especially.</p>
