RESUMO

Objective To explore the safety and feasibility of guiding catheter passing through spasmodic vessels in patients undergoing percutaneous coronary intervention (PCI) via radial artery access by the aid of PCI guiding wire and balloon .Methods The clinical data of 33 coronary artery disease (CAD) patients undergoing PCI via radial artery access with radial artery or (and) brachial artery spasm ( group A ) were retrospectively analyzed .Among all these patients , guiding catheters were delivered through the spasmodic vessels successfully by the aid of PCI guiding wires and balloons .The clinical data of other 38 CAD patients having PCI during the same period performed by other operators via radial artery or ( and ) brachial artery approach and experienced vessel spasm were anlysed as the control ( group B ) .All patients in group B received conventional anti-spasm management during PCI .All vessel spasm was identified by angiography.For patients in group A , a diameter of 0.014 inch guiding wire was chosen to pass through the spasmodic vessel segment carefully and gently .The diameter of balloon should be chosen according to the diameter of guiding catheter .A balloon diameter of 2.0 mm and 2.5 mm was corresponded to 6F and 7F guiding catheter respectively .The balloon was advanced to the tip of guiding catheter , keeping a half in catheter and a half in vessel followed by inflating the balloon with a pressure of 8 atm.The balloon was kept inflated the guiding catheter was pushed in vitro carefully and slowly until the catheter passed through the spasmodic vessel segment .Then the balloon was deflated and pulled out together with PCI guiding wire . Exchanged a diameter of 0.035 inch wire and completed the positioning of guiding catheter .After finishing the PCI, radial or ( and) brachial angiography was performed again to observe if spasm disappeared and to determine if there any contrast medium exudation .For patients in group B , routine approach was applied including administration of nitroglycerine , diltiazem or nitroprusside etc . to relieve vessel spasm. Results The location of vessel spasm was similar in group A and group B ( P=0.150 ) , and the incidence rate of spasm in brachial artery was higher than that in radial artery in both groups .The chance of guiding catheter crossing the spasmodic vessel segment was significantly higher in group A than in group B ( 100%vs.39.5%, P=0.00).In patients whose guiding catheter could pass through the spasmodic vessel segment successfully , time spent in group A was shorter than in group B ( P=0.000 ) .The patient number which time spent was less than five minutes , five to 15 minutes and more than 15 minutes was 30 and 2 ( 90.1%vs.13.3%) , 3 and 7 ( 9.9% vs.46.7%) and 0 and 6 ( 0% vs.40.0%) in group A and in group B respectively.The incidence of forearm hematoma was lower in group A than in group B without statistical difference [6.1%(2/33) vs.18.4%(7/38), P =0.113].Conclusions It is safe and feasible for passing guiding catheter through spasmodic vessels during PCI via radial artery access by the aid of PCI guiding wire and balloon .

Objective: To compare the early and long-term outcomes between on-pump and off-pump coronary artery bypass grafting (CABG) in elder female patients. Methods: A total of 763 female patients elder than 65 years of age received isolated CABG in our hospital from 1999-01 to 2008-12 were retrospectively studied. The patients were divided into 2 groups according to operational method: On-pump group,n=331 and Off-pump group,n=432. The mortality at 30 days post-operation, in-hospital clinical indexes and long term mortality with MACCE as all cause death, myocardial infarction (MI), stroke and repeated revascularization were compared between 2 groups. Results: Compared with On-pump group, the patients in Off-pump group had the elder age (P24h mechanical ventilation (P Conclusion: Off-pump CABG would reduce the mortality at 30 days post-operation, have less application of blood products, shorter post-operative mechanical ventilation, less early post-operative renal failure, pulmonary complications and re-exploration for bleeding. While it could not reduce the long-term mortality and MACCE occurrence.

Objective To investigate the efficacy and safety of lamivudine in the patients of hepatitis B virus-associated glomerulonephritis (HBV-GN) treated by prednisolone with leflunomide. Methods 41 HBV-GN patients treated by prednisolone and leflunomide were enrolled in this study. According to the presence of the indications of antiviral treatment, the patients were divided into the prevention group and the treatment group received lamivudine treatment, and another 15 patients with chronic hepatitis B were treated with lamivudine as the control group. The biochemical, virological and serological responses during the process of the treatment and the serum levels of IFN-γ and IL-4 were determined. Results No significant differences of biochemical, virological and serological responses in the prevention and treatment groups compared with the control group at 48 weeks after treatment. Levels of the IFN-γ and IFN-γ / IL-4 decreased significantly in the prevention group and treatment groups compared with the control group (P < 0.05, respectively). No serious adverse reactions occurred in all patients. Conclusion The antiviral treatment of lamivudine lead to good curative effect and security in HBV-GN patients received hormones combined with leflunomide treatments, but leflunomide with prednisolone inhibited the production of IFN-γ and removal of hepatitis B virus.

RESUMO

Selective courses are different from compulsory courses in teaching objective,students' study demand,teaching content.Different teaching designs should be used according to different teaching objectives in selective courses for graduates.Targeted scientific thoughts and methodological training should be designed with respect to the fact that students will begin their experimental studies after the selective lessons.We analyzed the current status of selective course of neuroanatomy for graduates and proposed some solutions.

In order to select better seeding sludge and promote start-up of Anammox reactors, we studied the start-up performances of three Anammox-EGSB bioreactors inoculated with anaerobic methanogenic sludge (AMS) (R1), Fresh Anammox sludge (FAS) (R2) and stored Anammox sludge (SAS) (R3), respectively. Results showed that these three seeding sludges could start up Anammox-EGSB bioreactors successfully, but the start-up progresses showed different characteristics. The start-up course of R1 could be divided into three phases including autolysis phase (15 d), lag phase (54 d) and activity elevation phase (40 d). However, the start-up courses of R2 and R3 only included lag phase (2 d and 12 d, respectively) and activity elevation phase (15 d and 57 d, respectively). Besides, the performance of R3 was better than that of R1, but worse than that of R2. Furthermore, bathing the Anammox sludge in the effluent of bioreactors was a convenient and effective way to keep the activity of the Anammox sludge. The ammonia removal efficiency, percentage of denitrification and the stoichiometric ratios of NH4(+)-Nr/NO2(-)-Nr and NO3(-)-Np/NH4(+)-Nr could serve as indicators to monitor the start-up of Anammox bioreactors.

RESUMO

Early-warning of compartmentalized anaerobic reactor (CAR) was investigated in lab-scale. The performance stability of CAR at high loading rate was worse than that at common loading rate. At high loading rate, the fluctuation of effluent chemical oxygen demand (COD) concentration and volatile fatty acids (VFA) concentration was larger than that of influent COD concentration. The average relative standard deviation of effluent COD concentration and VFA concentration was 32.95% and 40.46% respectively, while that of influent COD concentration was 8.08%. The saturation of volumetric loading rate (S(VLR)) and VFA (S(VFA)) could be used to alarm the performance of anaerobic reactors. The working performance was good when the CAR was operated at normal organic loading rate (OLR), in which S(VLR) and S(VFA) were below 0.89 and 0.40 respectively. The fluctuation of performance became larger when the CAR was operated at OLR near saturation, in which S(VLR) and S(VFA) were close to 1. The performance of CAR was deteriorated when the S(VLR) and S(VFA) were more than 1.

RESUMO

Germ cells exist in an environment created by Sertoli cells. The differentiation of germ cells from spermatogonia to sperm in the seminiferous epithelium is controlled by many factors such as hormones, growth factors, temperature and interaction with Sertoli cells. Paracrine signaling between these intimately associated cells also regulates the process of germ cell death. Sertoli cells' products play an important role in the process of germ cell differentiation in the testis, in which both the spontaneous and induced germ cell apoptosis often occur.

RESUMO

Objective To study the expression of estrogen receptor beta (ER ?) in the hypothalamic paraventricular nucleus (PVN) of maternal mice during pregnancy and lactation. Methods Nickel ammonium sulfate intensified immunocytochemical method was employed to investigate the expression of ER ? in the postnatal developing PVN of female mice. Results ER ? immunopositive materials were predominantly localized in the magnocellular division of PVN, and sparse positive cells were found in the parvocellular division. Most of the positive materials were found in the whole cell nuclei, but no obvious cytoplasma or process immunopositive cells were detected. In the pregnant female brain, generally, the ER ? level was lower. The lowest levels of expression were found at mid pregnancy, and then peaked at peripartum (from gestational days 18 to postpartum day 1), followed by another decrease to normal adult level from postpartum day 4. Conclusion The above results suggest that during pregnancy and lactation, ER ? may be predominantly involved in the PVN regulation of parturitional process.

RESUMO

Objective To explore the effects of maternal exposure to dibutyl phthalate ( DBP) ,an environmental estrogen on the proliferation and differentiation of neural stem cells ( NSCs) in the neural tube of neonatal rats. Methods A total of 40 male and 40 female SD rates at age of 4 to 5 months,were matched,and the morning when vaginal plugs were found was designated as E0. Then the 40 pregnant rats were randomly divided into 4 groups,3 DBP exposure groups ( 25,75,and 225 mg/kg) and control group. DBP at corresponding doses were dissolved in corn oil,and administrated by intragastrically injection once per day from E0 till delivery,while those of control group were given corn oil. Brain tissue of newborn rats was collected for primary culture of NSCs. Then the cell colony formation was counted to detect the NSCs proliferation. Then 10% fetal bovine serum ( FBS) was added into the culture medium to induce the NSCS to differentiate. Neu-N and GFAP immunofluorescence stainings were used to detect neurons and astrocytes,and their morphological changes were observed. Results The proliferation of NSCs was decreased significantly after DBP administration. Objective To determine the aromatase protein expression in A549 cell and to investigate the effects of 17? estrogen (E 2), testosterone (T), estrogen receptor antagonist tamoxifen (TAM), and aromatase inhibitor DL aminoglutethimide (AMIN) on the growth and proliferation of A549 cells. Methods The expression of aromatase protein was determined by immunohistochemical methods. The changes of cell cycle and cell number before and after treatment with E 2, T, TAM, and AMIN were measured by flow cytometry and tetrazolium method (MTT). Results The aromatase protein was positively expressed in A549 cells. The aromatase inhibitor AMIN and 5?10 -7 mol/L TAM could inhibit the growth of A549 cells and block them in G 0/G 1 phase ( P

RESUMO

Objective To study the effect of ovarian estrogen on the expression of estrogen receptor beta (ER beta) of the basal forebrain of mice. Methods Ovariectomized (OVX) mice were used as animal model, and nickel ammonium sulfate intensified immunohistochemical SP technique was used to detect the changes of ER beta expression in the basal forebrain. Results The expression of ER beta in the basal forebrain of mice decreased dramatically, after OVX decreased to the lowest levels at 3 d after OVX, increased gradually, and recovered to normal level 20 d later. Conclusion Ovarain estrogen can regulate the expression of ER ? of the murine basal forebrain, indicating that estrogen, via ER ?, can regulate the nerous structure and function of the basal forebrain, which may be the mechanism of estrogen replacement therapy on Alzheimer′s brain.
