Acantholytic pityriasis rubra pilaris associated with topical use of imiquimod 5%: case report and literature review

Abstract Topical use of immune response modifiers, such as imiquimod, has increased in dermatology. Although its topical use is well tolerated, it may be associated with exacerbations of generalized cutaneous inflammatory diseases, possibly through the systemic circulation of pro-inflammatory cytokines. This report describes a case of development of pityriasis rubra pilaris, a rare erythematous-papulosquamous dermatosis, in a woman aged 60 years during treatment with imiquimod 5% cream for actinic keratosis. It evolved with erythrodermic conditions and palmoplantar keratoderma, presenting progressive clinical resolution after the introduction of methotrexate. The authors emphasize the importance of recognizing possible systemic reactions associated with the topical use of imiquimod.

Pseudo-acne fulminans associated with oral isotretinoin

Acne fulminans is a rare and serious condition characterized by the sudden onset of nodular and ulcerative acne lesions associated with systemic symptoms. It has been recognized a subset of patients with a sudden worsening of acne, often during treatment with oral isotretinoin, but without the strong presence of systemic involvement. Recognized by some authors as "pseudo-acne fulminans" or " acne fulminans sine fulminans," we report a case with these features in order to draw attention to the recognition and early management of this manifestation.

Laserlipólise: atualização e revisão da literatura / Laserlipolysis: update and literature review

A laserlipólise é modalidade terapêutica utilizada atualmente no tratamento da gordura localizada e retração cutânea. Demonstra vantagens na remoção da gordura em relação à lipoaspiração convencional, com menor tempo de recuperação pós-operatória e menores efeitos adversos. A aplicação do laser diretamente no tecido adiposo promove liquefação da gordura, coagulação de vasos sanguíneos e desnaturação do colágeno. O dano térmico induz neocolagênese e contração da pele. A segurança da técnica é bem documentada. Este estudo propõe-se a revisar a literatura sobre laserlipólise, assim como apresentar o estado atual dessa tecnologia.

Laser lipolysis is a therapeutic modality that is currently used to treat localized fat and promote cutaneous retraction. Compared to conventional liposuction, removing fat using laser lipolysis has a shorter post-operative recovery time and fewer adverse effects. Applying laser directly to adipose tissue causes fat liquefaction, blood vessel coagulation and collagen denaturation. The thermal damage induces neocollagenesis and skin contraction. The technique's safety is well documented. This study reviews the literature on laser lipolysis and describes its current status.

Laser subdérmico no tratamento da hiperidrose axilar / Subdermal laser treatment of axillary hyperhidrosis

A hiperidrose axilar é patologia comum que implica distúrbios na vida social e profissional do paciente, necessitando de terapia adequada. Os tratamentos tradicionais incluem terapias tópicas, sistêmicas, iontoforese, toxina botulínica e procedimentos cirúrgicos. Uma nova técnica utilizando laser subdémico tem apresentado ótimos resultados, poucos efeitos colaterais e alta satisfação do paciente. Relata-se caso de uso de laser subdérmico para hiperidrose axilar com excelente resposta.

Axillary hyperhidrosis is a common pathology that disrupts patients? social and professional lives and requires appropriate therapy. Traditional treatments include topical and systemic therapies, iontophoresis, botulinum toxin, and surgical procedures. An innovative technique that uses subdermal laser has shown excellent results, few side effects and high patient satisfaction. This paper presents a case report of treating axillary hyperhidrosis with subdermal laser with an excellent response.

Experiência em laserlipólise: casuística de 120 casos no período de 2004 a 2010 / Laserlipolysisi experience: findings from 120 cases studied from 2004 to 2010

Introdução: A laserlipólise é técnica emergente no tratamento da gordura localizada e na retração cutânea. Essa opção terapêutica tem demonstrado vantagens em relação à lipoaspiração convencional, com menor tempo de recuperação pós-operatória e menores efeitos adversos. Objetivo: Relatar experiência em laserlipólise e avaliar a técnica quanto a sua segurança e eficácia. Métodos: Estudo retrospectivo de 120 pacientes submetidos a laserlipólise através do uso de dois aparelhos, com diferentes comprimentos de onda, o laser de 1064-nm Nd:YAG e o laser de diodo 924nm/975nm. Resultados: Houve significativas redução da gordura localizada, definição do contorno corporal e retração da pele em todos os pacientes analisados, segundo seguimento clínico e fotográfico. Cento e oito pacientes revelaram satisfação com o uso do método. Não ocorreram casos de cicatrizes atróficas, hiperpigmentação, hipopigmentação ou infecção. Conclusões: A laserlipólise realizada com indicação criteriosa oferece aos médicos e pacientes opção terapêutica segura e com alto grau de satisfação no tratamento da gordu-ra localizada e flacidez da pele

Introduction: Laserlipolysis is an emerging technique in the treatment for localized fat and Skin tightening. This therapy has a shorter post-operative recovery time and fewer adverse effects than conventional liposuction. Objective: This study describes the authors? experience in laserlipolysis and evaluates the technique?s safety and efficacy. Methods: Retrospective study of 120 patients who underwent laserlipolysis, which was administered via two devices with different wavelengths: 1064-nm Nd:YAG laser and 924 nm/975 nm diode laser. Results: According to clinical and photographic follow up, there was a significant improvement in localized fat in the definition of the body?s contour and in the skin tightening in all studied patients; 108 patients were satisfied with the technique. There were no cases of atrophic scars, hyperpigmentation, hypopigmentation or infection.Conclusions: If properly recommended and carefully carried out, laserlipolysis is a safe and highly satisfactory option for the treatment localized fat and sagging skin.
