RESUMO

Abstract The objective of the present study was to analyse the bioactive compounds of the leaves of Conocarpus lancifolius (C. lancifolius). The GC-MS analysis of the hot methanolic extract of the leaves (HMEL) of C. lancifolius exhibited the bioactive compounds such as 1-(3-Methoxy-2-nitrobenzyl) iso quinoline, morphin-4-ol-6,7-dione, 1-bromo-N-methyl-, phytol, hexadecanoic acid, 2,3-dihydroxypropyl ester, 2,2':4',2"-terthiophene, ethyl iso-allocholate, caryophyllene oxide, campesterol, epiglobulol, cholestan-3-ol, 2-methylene-, (3á,5à)-, dasycarpidan-1-methanol, acetate (ester) and oleic acid, eicosyl ester. The FT-IR analysis of HMEL of C. lancifolius showed a unique peak at 3184, 2413, 1657 cm-1 representing coumaric acid, chlorogenic acid and ferulic acid. The HMEL of C. lancifolius was actively inhibiting the proliferation of breast cancer cells MCF-7 ATCC at the concentration of 72.66 ± 8.21 µg/ml as IC50 value. The HMEL of C. lancifolius also revealed a good spectrum of activity against Gram-positive and Gram-negative bacterial cultures screened in this work. The activity observed has shown more or less similar effects against screened bacteria. However, the magnitude of potentiality was significantly lesser compared to standard ciprofloxacin disc at p< 0.001 level (99% confidence intervals). Furthermore, the study demonstrating the bioactive compounds can be isolated from the leaves of C. lancifolius.

RESUMO

Aim:Some generics were reported to be counterfeit and inferior quality than the innovators. This study was aimed to make sure about the compliance with standard specifications and evaluation of the quality of different selected brands (generic and innovator),after performing different pharmacopeial quality control tests, of Candesartan cilexetil tablets (16 mg) commercially available in Saudi Arabia for hypertensive patients Study Design:In vitrostudy of tablets.Place and Duration of Study: Collegeof Pharmacy, Jazan University, Jazan, KSA, between September 2018 and May 2019.Methodology:The different generic brands of Candesartan cilexetil (CC) and innovator brand (16 mg) were subjected to weight variation, hardness, friability, assay, and disintegration tests following the established protocols. The purity of active ingredient was authenticated by comparative analysis of FT-IR spectra with pure drug. In vitro bioequivalence was studied after analyzing the results of dissolution summaries in phosphate buffer (pH 6.5) mixed with polysorbate 20 (0.35% v/v).Results:The results of the tests conducted for evaluation of the tablets were found to be in acceptable limits for all the selected brands. After comparative analysis of FT-IR spectra with pure drug, it was inferred that correct active ingredient was used for the preparation of tablets. The drug release profile exhibited 96.89 –101.97% of release of CC from all generic brands, in comparison to 99.4% for innovator brand after 60 min of study. The assessment of difference factor (f1<15) and similarity factor (f2>50) revealed the resemblance of generic brands with that of innovator brand. Furthermore, the dissolution efficiency (DE = ±10% of the innovator value) of all generic brands (73.12 –73.25%) exhibited equivalency with that of innovator brand (70.45%). Conclusion:The selected generics were considered to be biopharmaceutically equivalent to the innovator and maintained their efficacy. As a consequence, these brands can be used interchangeably by the hypertensive patients in Saudi Arabia

RESUMO

BACKGROUND: Cutaneous leishmaniasis is a tropical infection of public health importance. Numerous treatment approaches are in practice with variable degree of success however its management has no universal consensus or practice guidelines to follow. OBJECTIVE: Analyze the management of cutaneous leishmaniasis retrospectively at a central hospital of Jazan Province, Kingdom of Saudi Arabia to identify the current treatment pattern and compare the outcomes. METHODS: This cross-sectional study was conducted based on the hospital records of patients who attended the dermatology clinic for cutaneous leishmaniasis during the year 2012 to 2015. RESULTS: Forty three patients were included in the study. There was a male preponderance (65.1%) among the patients and 60.5% of them were of pediatric age group. Monotherapy was the initial choice for 58.1% of the patients. Intralesional sodium stibogluconate (SS-IL) was the most preferred treatment for initial therapy, as monotherapy and as part of combination therapy. A complete response was achieved in 22 patients (51.2%) with initial therapy. Among the different treatment groups, SS-IL+itraconazole showed significantly higher complete response rate compared to other treatments offered as initial therapy (p<0.01). Initial SS-IL monotherapy provided complete response in 41.2% patients receiving it, while itraconazole monotherapy provided complete response in 75% and 90.9% of the patients receiving initial itraconazole+SS-IL combination therapy with achieved complete response. CONCLUSION: The findings and observations suggest that initial combination therapy with SS-IL+itraconazole significantly improved the complete response rates and thus reduced the need for additional or prolonged therapies.

RESUMO

Meconium ileus equivalent or the Distal Intestinal Obstruction Syndome [DIOS] is an entity that is frequently seen in patients with cystic fibrosis [CF]. In the gastrointestinal tract cyst fibrosis may produces symptoms of malabsorption and constipation. The latter may be associated with complications such as impaction and intestinal perforation. Meconium ileus and DIOS are an emergency and requires to be treated aggressively to ensure that consequences such as perforation do not occur. Traditional modes of management of DIOS have included the use of laxatives, Acetylcysteine and Gastrograffin enemas. We are describing the use of oral gastrograffin in our patient seen at the Aga Khan University Hospital where small bowel obstruction was refractory to treatment. Oral gastrograffin was used once diluted in 4 times the volume of water or fruit juice with half doses given on day 2 and 3. Oral gastrograffin use was followed by relief of obstruction in this patient. Gastrograffin use orally or rectally may be helpful in the treatment of refractions distal intestinal obstruction syndrome in cystic fibrosis

RESUMO

To assess the adherence of physicians and pharmacists in private sector to the guidelines of controlled psychotropic medications [CPM], the practicality of the guidelines and the degree of enforcement and follow-up of these guidelines stipulated by the Ministry of Health. The present study was performed in ten private community pharmacies [PCPs] and six private hospital pharmacies authorized to dispense CPM in Riyadh city. Data collection form was used to assess pharmacy records for CPM, and to collect data on prescription parameters in order to assess whether the prescriptions are properly written and dispensed. A twenty% random sample of prescriptions dispensed in each pharmacy, during January 1999 to September 2001 were collected through a systematic random sampling to assess the degree of physicians and pharmacists compliance to the guidelines laid down by the MOH. The pharmacists' opinions on the practicality of the guidelines and degree of enforcement and follow up were assessed prospectively through questionnaire. An analysis of 2077 prescriptions revealed that 54% of patients were male and the mean age was 37.7 years [ +/- 14.4]. Degree of physicians' adherence to guidelines was poor in documenting patient's full name in the prescription [12,5%], use of generic name of the drug when prescribing [12.1%], duration of treatment [22.4%] and quantity to be dispensed were specified in 52.2% of prescriptions. Moreover, prescribing drugs in class 1 and 2 for maximum of two weeks was observed only in 3.1% of prescriptions. Pharmacists documented the details of prescriptions dispensed in 90.5% and stamped 76.4% of prescriptions dispensed. Furthermore, the pharmacist write his/her name and sign the prescription upon dispensing in 62.7% of situations and in 7% of situations pharmacists dispensed prescriptions after the seven days period. About 69% of pharmacies were inspected only once during the year 2001 and prescriptions were repeated for the same patients within 14 days period in 3% of prescriptions. The most commonly prescribed drugs are alprazolam [41.6%], bromazepam [16.1%], diazepam [9.2%] and chlordiazepoxide [8.8%]. Several recommendations emerged from this study that could help MOH to gain better insight to control CPM. The MOH should review the current guidelines to address the problems raised by pharmacists, distribute the guidelines to all pharmacies, improve the inspection process, introduce electronic prescribing for CPM, and setting up a national computer network. The MOH, pharmacists and physicians should agree on the minimum standards of CPM prescription and the documentation procedures, which must be adhered to by both physicians and pharmacists. Finally, a national survey of psychotropic drugs prescribing should be conducted and consumption trend analyzed and compared with other countries

RESUMO

The aim of this investigation was to study the effect of dentin depth and patient's age on the shear bond strength of one dentin adhesive [ART Bond], using an Instron Universal testing machine at a cross-head speed of 0.5 mm/min. Forty human molar teeth were used and were divided into two main groups, twenty each representing two age groups of patients, i.e., between 20 to 30 years and between 30-40 years of age. Every group was further subdivided into two subgroups of different dentin depths, i.e., superficial and deep. Results were recorded in Kg/Cm[2] and converted into MPa units, then tabulated and statistically analyzed. A Duncan's range statistic test at P

RESUMO

This study was designed to evaluate one of the commonly used scoring systems -Score I- [categories from 0-5] to other two suggested scoring systems -Score II and III- [categories from 0-1 and 0-2 respectively], to test the degree of dye penetration for leakage studies. Class V cavities were prepared on 100 extracted human upper central incisors and filled with light cured composite. Herculite, and high copper amalgam. Tytin. All teeth were immersed in sodium fluorescein dye to test the degree of dye penetration. The results revealed that the suggested score II is very easy, simplified and representitive but not indicative regarding the severity of dye penetration as score I. Score III are indicative and obvious discussion can be obtained rather than score I. For any leakage study the use of non parametric measurement is preferable than parametric ones
