RESUMO

@#Tuberous Sclerosis Complex (TSC) is a genetic disorder that presents in a myriad of clinical manifestations affecting the different organ systems. These manifestations emerge at different times in a patient’s lifespan and diagnosis early in the disease can be challenging. Majority of patients with TSC develop epilepsy and is often one of the most difficult to manage. We report two neonates with drug resistant epilepsy with seizure onset on the first day of life and were subsequently diagnosed with Tuberous Sclerosis. These two neonates exemplified the diverse phenotypic expression of TSC.

BACKGROUND@#Anti-N-Methyl-D-Aspartate receptor (anti-NMDAR) Encephalitis is the most common type of autoimmune encephalitis that affects children, adolescents and young adults. Since its discovery in 2007, there is still a paucity of data on the disease and factors affecting its outcome.@*OBJECTIVES@#To describe the clinical characteristics of children and adolescents with anti-NMDAR encephalitis and to analyze factors that may affect its outcome.@*METHODS@#Forty-three patient records of diagnosed anti-NMDAR Encephalitis were included. The outcome was evaluated using the modified Rankin Scale (mRS), and Clinical Assessment Scale for autoimmune Encephalitis (CASE). @*RESULTS@#Ages ranged from 2 years to 18 years old, majority in the 12-18 years age range. Sixty percent were female. First line treatment using immunotherapy was given to all patients: 37% as monotherapy and 84% combination therapy (MPT only 23%, IVIg only 4%, MPT + IVIg or TPE 21-26%, and MPT + IVIg + TPE 16%). Clinical outcomes on discharge and on follow-up were assessed using the mRS and CASE. On discharge the proportion of the patients who had mild impairment (mRS<2, CASE<9) was more than 50%. On median duration follow-up of 31 weeks (range 24-40 weeks), 96.8% had significant improvement (mRS<2, CASE<9). Among the possible factors that were assessed to affect outcome, only severity of the illness at the start of the treatment influenced clinical outcome.@*CONCLUSION@#Early diagnosis and initiation of treatment before the progression of the disease will promote faster recovery and more optimal clinical outcome. CASE may be used as an additional tool in assessing response to treatment.

OBJECTIVE@#To compare the safety and efficacy of buccal midazolam as opposed to rectal diazepam in the treatment of acute seizures in children less than 18 years old.@*METHODOLOGY@#This is a meta-analysis of randomized controlled trials comparing the use of buccal midazolam and rectal diazepam as treatment for acute seizures in children less than 18 years old. The total population of each study are as follows: 330 patients (Mpimbaza, 2008), 43 patients (Baysun, 2005), and 17 patients, all less than 18 years old, coming into the emergency department due to seizures.@*RESULTS@#There is no significant difference in the mean duration of seizure in minutes and seizure cessation in ten minutes between the buccal midazolam and rectal diazepam groups (Mean difference 0.39; 95% Confidence interval [CI] -0.18 to 0.96; p=0.17; Risk ratio [RR] 0.99; 95% CI 0.83 to 1,19, p=0.2). There is no significant difference in the risk of respiratory depression between buccal midazolam and rectal diazepam (RR 0.96; 95% CI 0.22 to 4.13; p=0.61).@*CONCLUSION@#The administration of buccal midazolam and rectal diazepam are similar in terms of efficacy and safety in terms of time to seizure cessation, termination of seizure within ten minutes, and risk of respiratory depression.

En Afrique noire; la place de l'infection par le virus de l'hepatite C (VHC) dans la survenue des hepatopathies chroniques reste discutee; en particulier au Senegal ou la prevalence du VHC est moderee. C'est pourquoi une etude cas-temoins a ete menee a l'Hopital Principal de Dakar; en 1995; incluant 73 malades et 73 temoins. Les malades etaient repartis en 2 hepatites chroniques; 25 cirrhoses et 46 carcinomes hepatocellulaires. Les malades et les temoins ont fait l'objet d'etude serologique vis-a-vis du VHC (test ELISA de depistage; test ELISA de 3e generation en cas de positivite; puis confirmation par immunoblot) avec determination du serotype du VHC par methode immunoenzymatique; une recherche d'infection par le virus de l'hepatite B et une recherche d'anticorps anti-delta ont complete l'etude. Seulement 2 malades etaient porteurs d'anticorps anti-VHC (3 p. 100) et la serologie etait douteuse chez 2 autres ; le serotype 2 a ete mis en evidence chez l'un de ces malades ; aucun temoin n'etait positif vis-a-vis du VHC. Cinquante-quatre malades (74 p. 100) et 15 temoins (21 p. 100) etaient porteurs de l'antigene HBs parmi lesquels 13 malades (24 p. 100) et 1 temoin (7 p. 100) etaient porteurs d'anticorps anti-delta. Cette etude montre le role actuellement negligeable du VHC dans la survenue des hepatopathies chroniques en milieu hospitalier au Senegal et confirme le role predominant du virus de l'hepatite B et aggravant du virus de l'hepatite delta. Ces resultats sont confrontes aux donnees de la litterature concernant les pays d'Afrique noire. Au Senegal; l'impact du VHC parait inferieur a celui qui est oberve en Afrique centrale

RESUMO

La presencia de un tumor mixto en laringe constituye una rareza. Se reporta el caso de una paciente femenina de 63 años de edad, con disfagia, voz gangosa y perdida de 10 libras de peso. Una biopsia de la laringe mostró la presencia de un adenoma pleomórfico. Al revisar la literatura mundial se encuentran 13 casos reportados hasta la fecha, pudiendo ser este el número 14 y el primero en la República Dominicana

RESUMO

El carcinoma secretorio de la mama es uno de los menos frecuentes. Fue reportado por primera vez en niñas en 1966. En este trabajo reportamos el caso de una niña de 12 años de edad que se presentó con una masa en su mama derecha que fue extirpada quirurgicamente y al examen anatomopatológico resultó ser un carcinoma secretorio. Hicimos una revisión de la literatura mundial y nacional sobre este tipo de tumor. Al revisar la literatura nacional constatamos la posibilidad de que este sea el primer caso reportado en la República Dominicana

RESUMO

La invaginación intestinal del intestino delgado en un adulto, es algo llamativo. Se reporta el caso de una paciente femenina de 41 años de edad, con invaginación intestinal del yeyuno producida por un pólipo fibroide inflamatorio. Al revisar la literatura mundial se encuentran unos 90 casos reportados hasta 1986. No se encuentra un caso similar publicado en la República Dominicana

Se reporta en una mujer de 35 años un tercer ovario legitimamente supernumerario con formación de Quiste Luteinico. Nuestro caso es probablemente el No.33 de la literatura internacional. Su localización en el mesoapendice lo sitúa como el primero encontrado a ese nivel en la literatura mundial

RESUMO

En este trabajo se consideran los resultados del tratamiento con decanoato de flufenazina en un grupo de 112 pacientes psiquiatricos, controlados por personal profesional de enfermeria. De acuerdo con la evaluacion de dicho tratamiento, podria indicarse su efecto positivo, como tambien una marcada ventaja, al permitir que se reduzcan los dias de hospitalizacion y consultas medicas, en la mayor parte de los pacientes considerados
