RESUMO

Objective:To investigate the clinicopathological features of missed synchronous multiple early gastric cancer (SMEGC).Methods:Clinical and pathological data of 10 missed SMEGC patients in Beijing Friendship Hospital collected from January 2015 to December 2019 were reviewed for the clinicopathological and endoscopic features.Results:Ten missed SMEGC patients were all over 60 years old, and 6 of them were males. Six patients had family history of tumor and 6 had comorbidity (hypertension, diabetes, dyslipidemia, cardiovascular or cerebrovascular diseases). In terms of endoscopic and pathological manifestations, missed lesions of 6 cases were not smaller than the initial lesions, and more than half of the missed lesions had the same vertical location in the stomach (6/10), infiltration depth (8/10), histological classification (9/10), atrophic (8/10) and intestinal metaplasia (8/10) as the initial lesions.Conclusion:Physicians should be aware of the possibility of missed lesions during the first endoscopic treatment and the follow-up, especially at the same vertical location of the initial lesions in elderly males with family history of tumor and comorbidity.

RESUMO

Huge improvement has been made in the diagnosis and treatment of endoscopic ultrasonography (EUS) and more doctors have begun to focus on this subject with the increased level of EUS. Beijing Friendship Hospital has built a comprehensive system for the teaching method and process which include basic information screening of trainee and standardized training of perceiving and operating ability. There were 3 grades training of computer simulation, animal simulation and operation on real patients. During the first month of training, trainees accepted training with computer simulator for at least 100 hours, and underwent animal simulator during the second month, then underwent EUS on the real patients under supervision of instructors during the third month. By doing this, residents can master the technique of treatment and diagnosis of EUS in a safer and better way.

Objective To analyze the possible influencing factors of postoperative bleeding after endoscopic submucosal dissection and endoscopic mucosal resection ( ESD/EMR) for early gastric cancer. Methods Clinical data of patients receiving ESD/EMR for the diagnosis of early gastric cancer at the Endoscopy Center of Beijing Friendship Hospital from January 2013 to May 2018, including demographic information ( age, gender and history ) , endoscopic lesion characteristics ( tumor size, location and morphology) and postoperative pathological features ( differentiated types and invasive depth) were collected to analyze the effects of these factors on bleeding after ESD/EMR. Results A total of 195 patients with early gastric cancer were included in the study and 9 cases ( 4. 6%) had postoperative bleeding. The medication history of clopidogrel and main lesion sizes were statistically different between postoperative bleeding group and non-bleeding group ( P=0. 018 and P=0. 034 ) . Multivariate analysis showed a history of clopidogrel ( OR=10. 223, 95%CI:1. 143-91. 468, P= 0. 038 ) , multiple lesions ( OR= 6. 412, 95%CI:1. 123-36. 616, P=0. 037) and lesions sizes of larger than 2 cm ( OR=6. 718, 95%CI:1. 130-39. 935, P=0. 036) were possible risk factors for postoperative bleeding. Survival analysis showed of higher postoperative bleeding risks in patients with the history of clopidogrel ( P<0. 001) and lesions sizes of more than 2 cm ( P=0. 022) . Conclusion More attention should be paid to the risk of ESD/EMR postoperative bleeding in early gastric cancer patients with medication history of clopidogrel and multiple large lesions.

<p><b>OBJECTIVE</b>To investigate the correlation between intrahepatic eovalently closed circular (ccc)DNA of hepatitis B virus (HBV) and pathogen-and patient-related parameters.</p><p><b>METHODS</b>Ultrasound-guided liver biopsies were obtained from 60 patients with chronic HBV infection. Levels of intrahepatic HBV cccDNA and serum HBV DNA were measured by quantitative fluorescence PCR. Level of serum hepatitis B surface antigen (HBsAg) was measured by chemiluminescence immunoassay. Clinical parameters, such as alanine aminotransferase (ALT), aspartate aminotransferase (AST), gamma-glutamyl transferase (GGT), alkaline phosphatase (AKP), albumin, globulin (GLO), white blood cell, platelet, prothrombin-international normalized ratio, were measured by standard assay. Demographic information was recorded.The correlation between intrahepatic HBV cccDNA and pathogen-and patient-related parameters was assessed.</p><p><b>RESULTS</b>Intrahepatic HBV cccDNA level was negatively correlated with age, GLO, ALT and grade of necroinflammation. Patients with age of 30 years or more showed significantly higher level of HBV cccDNA level than patients under 30 years-old (7.44±0.58 and 5.66±1.35; t=7.157, P less than 0.001). Intrahepatic HBV cccDNA level was positively correlated with serum HBV DNA level (r=0.916, P less than 0.001) and serum HBsAg level (r=0.727, P less than 0.001). The median ratio of HBV cccDNA to HBV DNA was 1.18, and of HBV cccDNA to HBsAg was 1.67.</p><p><b>CONCLUSION</b>Intrahepatic HBV cccDNA levels decrease with age, level of ALT, level of GLO and grade of liver necroinflammation, but increase with level of serum HBV DNA and level of serum HBsAg. To a certain extent, serum HBV DNA and serum HBsAg levels may be a sufficient marker of intrahepatic HBV cccDNA levels.</p>

Objective To analyze the correlation between covalently closed circular DNA (cccDNA) in the peripheral blood mononuclear cells (PBMC) of hepatitis B virus (HBV)-infected patients and serum HBV DNA,hepatitis B surface antigen (HBsAg),hepatitis B e antigen (HBeAg) and liver histology of hepatitis B patients,and to explore the clinical significance of HBV cccDNA detection in PBMC.Methods One hundred and eight patients with chronic HBV infection were involved in this study.PBMC were extracted using density gradient centrifugation.HBV cccDNA in PBMC and serum HBV DNA were detected by real-time fluorescence quantitative polymerase chain reaction.HBsAg and HBeAg were detected by chemiluminescence immunoassay.Liver biopsy was conducted in 59 out of the 108 patients.Chi-square test was used to compare the categorical variables.Correlation analysis was used to compare quantitative variables.Nonparametric test was used to compare the non-normal distribution parameters.Results In the overall population,HBV cccDNA in PBMC was positive in 59 patients (54.6%).Eleven of the 15 patients with liver failure were found to be HBV cccDNA positive,which was significantly higher than that in the acute hepatitis B group (only 2 of the 8 patients were HBV cccDNA positive; x2 =4.960,P<0.05).One hundred and eight patients were categorized into three groups according to their serum HBV DNA levels,with group A:>5 lg copy/mL,group B:3-5 lg copy/mL and group C:<3 lg copy/mL.The proportions of HBV cccDNA positivity in PBMC in three groups were 76.1% (51/67),5/18 and 13.0% (3/23),respectively.Comparing with patients with lower HBV DNA (group B and C),the proportion of HBV cccDNA positivity was higher in patients with higher HBV DNA (group A; x2=14.751,P<0.05 and x2 =28.384,P<0.05,resepectively).The HBV cccDNA quantitation in PBMC was positively correlated with the serum HBV DNA level and HBsAg quantification (r=0.554,P<0.05 and r=0.497,P<0.05,respectively).The proportion of HBV cccDNA positivity in PBMC of patients with liver histology ≥G2 and/or ≥S2 was significantly higher than that in patients with liver histology < G2/S2 (x2 =9.159,P<0.05).Conclusions HBV cccDNA exists in PBMC of hepatitis B patients.The HBV cccDNA quantitation in PBMC is positively correlated with the serum level of HBV DNA and HBsAg quantification,and is also associated with liver histology injury.

Objective To investigate the regulation role of gastric cancer related miR-148a on gastrin receptor CCKBR expression, and find the correct binding sites of miR-148a in CCKBR 3’UTR.Methods The potential binding sites of miR-148a in the CCKBR 3’UTR were predicted with the bioinformatic tools;The miR-148a expressing plasmid was constructed by PCR, and miR-148a expression was verified by Northern Blot;The luciferase report plasmids containing the wild type and mutated binding sites of CCKBR 3’ UTR were constructed, and were used to study the regulation mechanism and identify the binding sites of miR-148a by luciferase activity analysis; The regulation effect of miR-148a on CCKBR protein expression was checked by Western Blot.Results Three potential binding sites of miR-148a in the CCKBR 3’ UTR were found; The miR-148a expressing plasmid was constructed successfully, and highly expressed miR-148a after transfected to gastric cancer cells;The inhibitory effect of miR-148a on CCKBR protein expression was checked by Western Blot.Over-expression of miR-148a inhibited CCKBR expression by directly binding to the binding site in CCKBR 3’UTR 423bp.Conclusion CCKBR is a target of miR-148a, and its expression is inhibited by the binding of miR-148a on its 3’ UTR, indicating that miR-148a may participates in the progression of gastric cancer by regulating CCKBR expression.

ObjectiveTo investigate whether there was inhibitory effect of angiotensin-Ⅰ converting enzyme inhibitors (ACEI) on esophageal carcinoma cell line.Methods The highest expression of vascular endothelial growth factor (VEGF) at mRNA level was screened in esophageal squamous cell carcinoma cell lines KYSE30,TE-1,EC109 and EC9706 by reverse transcriptionpolymerase chain reaction (RT-PCR) and which was transplanted and established nude mice xenografts model.After tumor formed,the nude mice were evenly divided into control group,perindopril group and benazepril group (n=6) of which 0.9% saline,4 mg/kg perindopril and 6 mg/kg benazepril was given respectively to nude mice in each group.The tumor size and the tumor inhibitory rate were measured.The expression of CD31 in xenograft tumor was tested by immunohistochemistry and the microvessel density (MVD) was assessed.ResultsThe highest expression of VEGF at mRNA level was in EC9706 cell line.At second and third week,there was no significant difference in xenograft tumor size between each group. At fourth and fifth week,there was no significant difference in xenograft tumor size between perindopril group and control group,and the tumor inhibitory rate was 24.6 % and 21.1 %.There was significant difference between benazepril group and control group (t=-2.450 and -3.120,P=0.035 and 0.008),and the tumor inhibitory rate was 33.1% and 45.4%.After treated with perindopril and benazepril,the expression of CD31 in xenograft tumor of nude mice decreased.The MVD of benazepril group was significantly lower than that of control group (10.98±1.18 vs 13.98 ± 1.76; t =-3.732,P =0.002),and there was no significant difference between perindopril group (12.41±1.15) and control group (t=-2.053,P=0.07).ConclusionsBenazepril (6 mg/kg) could significantly inhibit the growth of EC9706 cell line formed xenograft tumor of nude mice.One of the possible tumor inhibitory mechanisms was inhibiting new vessel forming in tumor.Perindopril (4 mg/kg) may have the similar effect.

ObjectiveTo explore the effects of enteral nutrition (EN) on intestinal permeability in patients with active ulcerative colitis (AUC). MethodsTwenty-four A UC patients were randomly divided into two groups:routine treatment group (n =11 ) and routine treatment plus EN group (n =13). Patients in routine treatment group were treated with mesalazine as well as low-residue diet, while patients in routine treatment plus EN group received mesalazine and short peptide EN for 14 days. The ratio of lactulose to mannitol in urine (L/M) before and after treatment was detected by high-performance liquid chromatography. ResultsThe L/M ratio was 0. 039 ± 0. 025 in routine treatment group and 0.072 ± 0.019 in routine treatment plus EN group (P =0.069). After 2 weeks of treatment, the L/M ratio of routine treatment plus EN group (0.038 ± 0.012 ) was significantly lower than the pretreatment level (P =0.043 ), while the L/M ratio of routine treatment group between before and after treatment had no significant difference (0.039 ± 0.025 vs. 0.032 ± 0.022, P =0.730). ConclusionEN can effectively improve the intestinal permeability in AUC patients.

RESUMO

Objective To investigate the effects of docosahexaenoic acid (DHA) on the growth of human pancreatic cancer cell lines. Methods Human pancreatic cancer cell lines Patu8988 and SW1990 were treated with DHA. The cell proliferation was evaluated by MTT assay. Cell cycle, apoptosis, and cyclooxygenase-2 ex-pression were evaluated by flow cytometry. Results After incubation of pancreatic cancer cell with DHA for 24 to 72 hours, cell proliferation significantly decreased (P<0. 01) and apoptosis increased, both of which were time- dependent and dose-dependent After incubation with 50μg/ml DHA for 24 hours, cyclooxygenase-2 expression of pancreatic cancer cell lines significantly decreased (P < 0. 05). Conclusions DHA can inhibit the growth of hu- man pancreatic cancer cells via decreasing cell proliferation and inducing apoptosis. These effects may be associat- ed with the decrease of cyclooxygenase-2 expression.
