RESUMO

Background@#Guselkumab is a monoclonal antibody that selectively blocks the p19 subunit of interleukin-23. It has shown good efficacy and safety profile in several clinical trials of plaque psoriasis. However, studies on the efficacy of guselkumab in patients treated with other biologics are lacking. @*Objective@#We aimed to investigate the efficacy and safety profile of guselkumab in patients with moderate-to-severe plaque psoriasis. We also compared the efficacy of guselkumab between biologic-naïve (Bio-Naïve) and biologicexperienced (Bio-Ex) patients. @*Methods@#This multicenter, retrospective study included 72 patients treated with guselkumab. The patients’ clinical characteristics and psoriasis area and severity index (PASI) scores were recorded at each visit. The PASI90 and PASI100 responses and mean PASI scores were compared between the Bio-Naïve and Bio-Ex groups. @*Results@#Fifty-five Bio-Naïve patients and 17 Bio-Ex patients were included in the study. At week 20, there were no significant differences in the PASI90 (64.2% vs. 53.8%) and PASI100 (28.3% vs. 15.4%) responses between the groups. However, at weeks 36 and 44, the PASI90 response (week 36: 89.2% vs. 36.4% and week 44: 97.8% vs. 63.6%) and the PASI100 response (week 36: 64.9% vs. 18.2% and week 44: 68.9% vs. 27.3%) were significantly higher in the Bio-Naïve group (p<0.05). There were no differences in PASI90 and PASI100 responses between the groups in terms of other clinical characteristics and comorbidities at week 20. @*Conclusion@#The efficacy of guselkumab remained consistent among patients in whom other biologics had failed. However, the efficacy was slightly lower in the Bio-Ex group than in the Bio-Naïve group.

RESUMO

Desmoplastic Spitz nevus is a rare variant of Spitz nevus characterized by predominantly spindle-shaped or epithelioid nevus cells within the fibrotic stroma that can be confused with fibrous lesions. A 43-year-old woman presented with a 1-cm-sized dome-shaped papule on the dorsum of her left foot. The lesion showed histopathological features of a desmoplastic Spitz nevus with structures that resemble adenoma. Immunohistochemical staining was positive for S-100 protein, Melan-A, and SOX-10. Herein, we report this case because desmoplastic Spitz nevus is rare and can lead to confusion regarding the diagnosis of adnexal neoplasms.

Background@#Risankizumab is a humanized immunoglobulin G1 monoclonal antibody that selectively binds to the p19 subunit of interleukin-23. Risankizumab has demonstrated rapid and excellent therapeutic effects in several clinical trials. Although a growing number of studies have reported data on the real-world efficacy and safety of risankizumab for the treatment of psoriasis, no such study has been conducted in Korea. @*Objective@#We evaluated the real-world efficacy and safety of risankizumab for the treatment of moderate-to-severe plaque psoriasis in Korean patients. @*Methods@#This was a retrospective single-center study. A total of 33 patients treated with risankizumab, for at least 16 weeks, were enrolled. Based on electronic medical records, the clinical characteristics, psoriasis area and severity index (PASI) score, body surface area, and adverse events were investigated. @*Results@#The mean PASI score was significantly reduced at 4 weeks of risankizumab treatment (3.27±2.15) and gradually reduced at week 16 (1.06±0.97) and week 52 (0.24±0.63) (p<0.05). At week 16, all patients achieved a PASI 75 response, and 66.7% and 27.3% of patients achieved PASI 90 and PASI 100 responses, respectively. Obese patients (body mass index, BMI≥25 kg/m2 ) showed a lower PASI 90 response than non-obese patients (BMI<25 kg/m2 ) at week 16. Older patients (age≥65 years) showed significantly higher PASI scores than younger patients (age<65 years) at week 16. Mild to moderate adverse events were reported in four patients; however, no patient discontinued treatment. @*Conclusion@#Risankizumab was very effective in a real-world clinical practice with a favorable safety profile in Korean patients with moderate-to-severe psoriasis.

Sarcoidosis is a systemic granulomatous disorder of unknown etiology characterized by granuloma formation. Due to the limited incidence of sarcoidosis in pediatric patients, little is known about the clinical course of this disease. A combination of clinical, radiologic, and pathologic examination is necessary to exclude other differential diagnoses (i.e., infection and granulomatous inflammatory disorder) and establish a diagnosis of sarcoidosis. Here, we report a case of histologically confirmed sarcoidosis initially misdiagnosed as hepatosplenic abscesses in an 11-year-old male. Treatment with corticosteroids improved his symptoms and resolved his skin and hepatosplenic lesions. A three-year follow-up was uneventful. This study emphasizes the importance of considering sarcoidosis in children presenting with findings of multi-organ involvement in the presence of histologic evidence of granuloma.

OBJECTIVE: To investigate the image quality, radiation dose, and intermodality agreement of cervical spine CT using spectral shaping at 140 kVp by a tin filter (Sn140-kVp) in comparison with those of conventional CT at 120 kVp. MATERIALS AND METHODS: Patients who had undergone cervical spine CT with Sn140-kVp (n = 58) and conventional 120 kVp (n = 49) were included. Qualitative image quality was analyzed using a 5-point Likert scale. Quantitative image quality was assessed by measuring the noise and attenuation within the central spinal canals at C3/4, C6/7, and C7/T1 levels. Radiation doses received by patients were estimated. The intermodality agreement for disc morphology between CT and MRI was assessed at C3/4, C5/6, C6/7, and C7/T1 levels in 75 patients who had undergone cervical spine MRI as well as CT. RESULTS: Qualitative image quality was significantly superior in Sn140-kVp scans than in the conventional scans (p < 0.001). At C7/T1 level, the noise was significantly lower and the decrease in attenuation was significantly less in Sn140-kVp scans, than in the conventional scans (p < 0.001). Radiation doses were significantly reduced in Sn140-kVp scans by 50% (effective dose: 1.0 ± 0.1 mSv vs. 2.0 ± 0.4 mSv; p < 0.001). Intermodality agreement in the lower cervical spine region tended to be better in Sn140-kVp acquisitions than in the conventional acquisitions. CONCLUSION: Cervical spine CT using Sn140-kVp improves image quality of the lower cervical region without increasing the radiation dose. Thus, this protocol can be helpful to overcome the artifacts in the lower cervical spine CT images.

BACKGROUND: Korean National Health Insurance reimburses factor VIII (FVIII) and factor IX (FIX) clotting factor concentrate (CFC) infusions to discrepant activity levels, allowing elevation of FVIII activity to 60 IU/dL and FIX to 40 IU/dL. We aimed to assess hemostatic response to these target levels using global hemostatic assays. METHODS: We enrolled 34 normal healthy men, 34 patients with hemophilia A, and 36 with hemophilia B, with residual factor activity of 3 IU/dL or less and without inhibitors. Patients with hemophilia A and B received injected CFCs according to reimbursement guidelines. Fifteen minutes after injection, we assessed hemostatic response with global hemostatic assays: thrombin generation assay (TGA), thromboelastography (TEG), and clot waveform analysis (CWA). RESULTS: Normal healthy men and patients with hemophilia A and B were 36.7, 37.2, and 35.1 years old, respectively. FVIII and recombinant FIX concentrate doses were 28.8 IU/kg and 43.6 IU/kg. Post-infusion FVIII activity rose from 0.5 IU/dL to 69.4 IU/dL, while FIX activity rose from 1.4 IU/dL to 46.8 IU/dL. Post-infusion peak thrombin concentrations in hemophilia A and B were 116.6 nM/L and 76.4 nM/L (P < 0.001). Post-infusion endogenous thrombin potential (ETP) in hemophilia A and B was 1349.8 nM/min and 915.6 nM (P < 0.001). TEG index of hemophilia A and B was 0.11 and −0.51 (P=0.006). CONCLUSION: Current reimbursed doses for FIX concentrates are insufficient to achieve hemostatic responses comparable to those after reimbursed doses for FVIII concentrates in terms of peak thrombin concentration, ETP, and TEG index.

PURPOSE: The purpose of this study was to examine the relationship among emotional clarity in emotional intelligence, maternal identity, and fetal attachment to measure how emotional clarity and maternal identity impact on fetal attachment and to determine mediating effects of maternal identity in pregnant women at the time of diagnosis with gestational diabetes mellitus (GDM). METHODS: This study used a correlational survey design. 88 pregnant women with GDM completed a study questionnaire of emotional clarity, maternal identity, and fetal attachment immediately after the diagnosis of GDM. Data were analyzed Mann-Whitney U test, and ANOVA with Duncan test, Pearson correlation, three-step regressions to test mediating effect, and Sobel test. RESULTS: The emotional clarity was positively related with maternal identity and fetal attachment. It affected maternal identity with 21.9% of explained variance. The emotional clarity and the maternal identity were significant predictors of fetal attachment by 57.7% of explained variance. The maternal identity mediated the relationship between emotional clarity and fetal attachment. CONCLUSION: The results suggest that a nursing program to enhance the emotional clarity and the maternal identity needs to be developed as an effective strategy to improve fetal attachment.

PURPOSE: The present study aims at researching the subjective satisfaction of patients who have experienced both conventional impression taking and digital impression taking to measure the possibility of wide clinical application of digital impression. MATERIALS AND METHODS: The study surveyed 170 adult patients over the age of 20, between October 2015 and April 2016, who voluntarily consented to participation and who experienced both conventional impression and digital impression at five dental hospitals that use intraoral digital impression. A total of 128 surveys were used for data analysis, involving frequency analysis, multiple response frequency analysis, descriptive statistics, and contingency table analysis, with the significance level set at 0.05. RESULTS: Responses on the reason for taking impressions using the digital method appeared in the order of 'for implant treatment' (43.8%), 'for crown treatment' (30.5%), and 'for inlay treatment' (15.6%). Patients satisfaction was higher for digital impression taking than conventional impression taking (P<.05). As the preferred choice of impression, digital impression (60.2%) was higher than conventional impression (11.7%). Responses on the reason for choosing digital impression taking appeared in the order of 'no vomiting reflex' (35.1%), 'reliability of 3D digital scanning' (33.8%), and 'short time' (33.8%). CONCLUSION: The patients preferred digital impression taking to conventional impression taking in terms of satisfaction.

PURPOSE: Inflammation-based prognostic scores including neutrophil-to-lymphocyte ratio (NLR), platelet-to-lymphocyte ratio (PLR), and lymphocyte-to-monocyte ratio (LMR) are associated with oncologic outcomes in diverse malignancies. We evaluated the predictive value of pretreatment prognostic scores in differentiating nonmuscle invasive (NMIBC) and muscle invasive bladder cancer (MIBC). MATERIALS AND METHODS: Consecutive transurethral resection of bladder tumour (TURBT) cases from January 2011 to December 2013 were analysed retrospectively. Patient demographics, tumour characteristics and prognostic scores results were recorded. Receiver operating characteristics curves were used to determine prognostic score cutoffs. Univariate and multivariate binomial logistic regression analysis was performed to evaluate the association between variables and MIBC. RESULTS: A total of 226 patients were included, with 175 and 51 having NMIBC (stages Ta and T1) and MIBC (stage T2+) groups, respectively. Median age was 75 years and 174 patients were male. The NLR cutoff was 3.89 and had the greatest area under the curve (AUC) of 0.710, followed by LMR (cutoff218; AUC, 0.642). Full blood count samples were taken a median of 12 days prior to TURBT surgery. Multivariate logistic regression analysis identified tumour grade G3 (odds ration [OR], 32.848; 95% confidence interval [CI], 9.818-109.902; p=0.000), tumour size> or =3 cm (OR, 3.353; 95% CI, 1.347-8.345; p=0.009) and NLR> or =3.89 (OR, 8.244; 95% CI, 2.488-27.316; p=0.001) as independent predictors of MIBC. CONCLUSIONS: NLR may provide a simple, cost-effective and easily measured marker for MIBC. It can be performed at the time of diagnostic flexible cystoscopy, thereby assisting in the planning of further treatment.

<p><b>INTRODUCTION</b>This study reports our experience of audit and critical incidents observed by paediatric anaesthetics from 2000 to 2010 at a paediatric teaching hospital in Singapore.</p><p><b>METHODS</b>Data pertaining to patient demographics, practices and critical incidents during anaesthesia and in the perioperative period were prospectively collected via an audit form and retrospectively analysed thereafter.</p><p><b>RESULTS</b>A total of 2,519 incidents were noted at the 75,331 anaesthetics performed during the study period. There were nine deaths reported. The majority of incidents reported were respiratory critical incidents (n = 1,757, 69.8%), followed by cardiovascular incidents (n = 238, 9.5%). Risk factors for critical incidents included age less than one year, and preterm and former preterm children.</p><p><b>CONCLUSION</b>Critical incident reporting has value, as it provides insights into the system and helps to identify active and system errors, thus enabling the formulation of effective preventive strategies. By creating and maintaining an environment that encourages reporting, we have maintained a high and consistent reporting rate through the years. The teaching of analysis of critical incidents should be regarded by all clinicians as an important tool for improving patient safety.</p>

RESUMO

We investigated the inhibitory effects of hesperidin on melanogenesis. To find melanosome transport inhibitor from natural products, we collected the structural information of natural products from Korea Food and Drug Administration (KFDA) and performed pharmacophore-based in silico screening for Rab27A and melanophilin (MLPH). Hesperidin did not inhibit melanin production in B16F10 murine melanoma cells stimulated with alpha-melanocyte stimulating hormone (alpha-MSH), and also did not affect the catalytic activity of tyrosinase. But, hesperidin inhibited melanosome transport in melanocyte and showed skin lightening effect in pigmented reconstructed epidermis model. Therefore, we suggest that hesperidin is a useful inhibitor of melanosome transport and it might be applied to whitening agent.

RESUMO

Angular pregnancy refers to implantation of the embryo just medial to uterotubal junction in the lateral angle of the uterine cavity. This is differentiated from interstitial pregnancy in which the gestational sac is located within intramural portion of the tube. Angular pregnancy is categorized as intrauterine pregnancy and the outcome is known favorable. However, there have been few reports about adverse perinatal complications such as abortion, retained placenta, placenta accreta, placenta percreta and uterine rupture. We report a case of angular pregnancy complicated with preterm labor at 25 gestational weeks, placenta accreta and postpartum endometritis. This case suggests that angular pregnancy should be differentiated from normal intrauterine pregnancy because of its potential risk of adverse outcome.

PURPOSE: This study was undertaken to observe the pattern of methylation of the p16INK4a and human telomerase reverse transcriptase (hTERT) genes and the p16 and hTERT protein expressions in invasive ductal carcinoma of the breast. In addition, we evaluated the relationship between the methylation status of the two genes and their protein expressions. METHODS: We performed methylation-specific PCR (MSP) and immunohistochemical staining in 63 breast cancer specimens. RESULTS: There was no statistical association between p16INK4a gene methylation and the histological grade (tumor grade, tumor size and lymph node status). Methylation of the hTERT promoter did show significant differences according to the histological tumor grade and tumor size, but there was no clinical significance. Methylation of the p16INK4a and hTERT genes was found in 22.2% and 31.8% of the specimens, respectively. A negative p16 protein expression (0-10% expression rate) was observed in 38.1% of the specimens (24 of 63). A positive hTERT expression (more than a 25% expression rate) was observed in 73.0% of the specimens (46 of 63). There was no statistical significance in the relationship between the methylation status and the protein expression. CONCLUSION: Our data suggest that methylation of the p16 and hTERT genes is not associated with their protein expressions according to Immunohistochemisty. There seemed to be another complicated mechanism for p16 inactivation and hTERT activation in breast cancer.

PURPOSE: We investigated to identify factors related to thrombocytosis and clinical data for thrombopoiesis in children with iron deficiency anemia (IDA). METHODS: We retrospectively analyzed clinical and laboratory data for 85 children admitted for acute infection or inflammation. Seventy patients of 85 children were diagnosed as IDA. The others were clinically suspected as IDA but they were not diagnosed. We divided three groups: group 1 included severe anemia below hemoglobin (Hb) 8.0 g/dL, group 2 mild to moderate anemia (Hb: 8.0~10.0 g/dL), and group 3 (control) were clinically suspected but without IDA. RESULTS: There are no differences among groups except age at diagnosis. The age at diagnosis in group 1 are higher than other groups. In control group, there are not any factors correlated with thrombocytosis. In group 1, the white blood cell and lymphocyte counts are significantly related to the platelet counts. However, serum iron level is only correlated with platelets in group 2. In multiple regression analysis, we found significantly correlation between white blood cell counts and serum iron level and thrombocytosis in IDA including group 1 and 2. CONCLUSION: We suggest that white blood cell counts and serum iron level in IDA may be related with increased platelet counts, as a reactive thrombocytosis. We need further study for correlation between acute phase reactants and thrombocytosis in IDA.

OBJECTIVE: The purpose of this study is to see whether the incidence of RDS (respiratory distress syndrome) in premature twin is higher than in premature singleton, and to see the clinical difference between the groups. METHODS: Seventy seven premature twins with respiratory distress syndrome who admitted to the department of Pediatrics, Catholic University Medical Center and Chonnam University Hospital for 5 years from Jan. 1998 to Dec. 2002 were enrolled. They were classified with group A in which both of twins have RDS (A1=25, A2=25), group B1 (B1=14) in which first twin has the RDS and group B2 (B2=13) in which second twin has the RDS. Incidence rate of RDS, birth order, gender, delivery mode, gestational week, birth weight, Apgar score, use of surfactant, and mortality rate between groups were also compared. RESULTS: Premature twins have higher incidence of RDS than premature singletons. The RDS is more often than in female. The average gestational week was 30.5 in A, 32.6 in B1, 32.1 in B2, and the average birth weight (gm) was 1,470 (A1), 1,464 (A2), 1,803 (B1), 1,687 (B2). Gender, delivery mode, Apgar score at 1 minute and 5 minute, use of surfactant, and mortality rate and morbidity showed no significant difference. CONCLUSION: Premature twins are more prevalent to RDS than premature singletons. The second twin don't likely to have the RDS than the first baby, but the special care is required because their mortality rate and morbidity were more or less higher.
