RESUMO

Objective:To evaluate the real-world short-term effectiveness of ixekizumab in the treatment of psoriasis, and to investigate factors influencing the effectiveness.Methods:Baseline data and short-term effectiveness evaluation results were retrospectively collected from patients with psoriasis, who received ixekizumab treatment in Department of Dermatology, Xiangya Hospital from November 2019 to September 2021. A descriptive analysis was performed on the baseline characteristics of patients, continuous data were described as median (lower quartile, upper quartile), and categorical data were described as percentages. Comparisons of disease severity scores before and after the treatment with ixekizumab were performed using Wilcoxon signed-rank test or paired McNemar test. Multivariable logistic regression analysis was conducted to explore factors influencing the effectiveness of 4-week ixekizumab treatment.Results:A total of 118 patients with psoriasis were included, including 94 males and 24 females, and their age [ M ( Q1, Q3) ] was 43.4 (32.5, 53.0) years; plaque psoriasis (99 cases, 83.9%) and severe psoriasis (72 cases, 68.6%) predominated among the 118 patients, and skin lesions were mainly located on the scalp (59/116, 50.9%). Among the 49 patients who had received 2-week ixekizumab treatment, 27 (55.1%) achieved a 50% improvement in the psoriasis area and severity index (PASI) score (PASI50) ; after 4-week treatment, 44 (89.8%), 30 (61.2%), 13 (26.5%) and 10 (20.4%) patients achieved PASI50/75/90/100 respectively, and their PASI scores (2.1 [1.1, 7.1]), involved body surface area (3.9% [0.5%, 14.5%]), dermatology life quality index scores (1.0 [0.0, 2.0]) and physician global assessment (PGA) scores (1.0 [1.0, 3.0]) were significantly lower than the corresponding scores at baseline (12.4 [8.8, 23.2], 22.0% [11.3%, 43.4%], 6.0 [3.0, 11.0], 4.0 [3.0, 5.0], respectively; all P < 0.001]. Multivariable logistic regression analysis showed that the baseline body mass index was significantly associated with the PASI75 response rate ( OR = 0.814, 95% CI: 0.659 - 0.958, P = 0.029) and the proportion of patients with PGA0/1 ( OR = 0.743, 95% CI: 0.562 - 0.917, P = 0.017) after 4-week ixekizumab treatment, and the baseline BSA score was significantly associated with the proportion of patients with PGA0/1 after 4-week ixekizumab treatment ( OR = 0.924, 95% CI: 0.870 - 0.968, P = 0.003) . Conclusion:The 4-week ixekizumab treatment significantly decreased the severity of psoriasis, and may be more effective in patients with lower disease severity and lower body mass index at baseline.

Objective: To verify whether the enzymatic activity of kynureninase (KYNU) could be changed by the Arg188Gln (G/A) mutation. Methods: The total RNA of human hepatic tissue was extracted and the KYNU gene cDNA was amplified by RT-PCR. Primers were designed according to the sequences around the site Arg188Gln of KYNU gene and the Arg188Gln (G/A) mutant KYNU cDNA was generated by site-directed mutagenesis. Both the wild-type and mutant-type KYNU genes were subcloned into pcDNA vectors and the recombinant plasmids were constructed. After being transfected into human embryonic kidney 293 (HEK293) cells, the expression of KYNU recombinant plasmids were assessed by Western blot. The enzymatic activities of KYNU were detected by high performance liquid chromatography (HPLC). Results: The KYNU enzyme activities were expressed in both wild and mutant HEK293 cells. Michaelis constants (Km) of the wild and mutant KYNU were (9.833±0.513) μmol/L and (29.900±0.265) μmol/L, respectively (P-1·min-1 and (0.084±0.003) nmol·mg-1·min-1, respectively (PConclusion: Arg188Gln (G/A) mutation can decrease the enzymatic activity of KYNU.

BACKGROUND: At present, the meniscus injury is very common, and can lead to serious injury of the knee joint.Meniscus biomechanics has been reported, but no studies have concerned the biomechanical characteristics of the meniscus in knee buckling during gait under different loading conditions.OBJECTIVE: To simulate and analyze the biomechanical characteristics of the knee joint meniscus under different loading conditions using finite element method.METHODS: Based on the knee magnetic resonance imaging data of normal adult volunteers, three-dimensional finite element model of knee joint was reconstructed. The stress, distribution and changes in the meniscus under different flexion angles were analyzed at 300 N, 400 N and 500 N.RESULTS AND CONCLUSION: (1) Under the respectively loading conditions of 500 N, 400 N and 300 N, the maximum stress point all from the place of posterior horn of medial meniscus anterior surface of tibia moved to the posterior margin of the anterior tibial attachment of the lateral meniscus, and the maximum displacement all from the place of midpoint of the inner edge of the medial meniscus moved to front outer edge of the lateral meniscus during normal dynamic buckling process of 0°, 30°, 60° and 90°, and the range of stress and displacement of lateral meniscus was greater than that of medial meniscus; simultaneously, the distribution range of stress and displacement was proportional to the size of the loading and the angle of the buckling. (2) The three-dimensional finite element model of the knee joint meniscus can be used to effectively dynamically simulate the distribution of force and displacement under different loading conditions, and can provide evidence for speculation of the mechanism of the injury of the meniscus in clinic.

BACKGROUND:At present, although the study of three-dimensional finite element biomechanical analysis of knee joint meniscus has been reported and we have a certain understanding of the biomechanical changes of the meniscus, but the dynamic simulation of the knee meniscus in the same load conditions in the process of biomechanical analysis of the knee meniscus is less reported. OBJECTIVE:To analyze the biomechanical characteristics of the knee joint meniscus under different flexion angle by using analogue simulation of finite element method. METHODS:Based on knee MRI data of the normal adult volunteers, the medicine finite element simulation software Mimics10.01 and reverse engineering software Rapidform XOR3 were utilized to reconstruct three-dimensional finite element model of knee joint meniscus. The advanced finite element analysis software Abaqus6.10-1 was utilized for analogue simulation and for analyzing biomechanical changes during flexion under vertical load of 300 N. RESULTS AND CONCLUSION:(1) While the knee joint flexed at 0°, 30°, 60° and 90°, with the increase of angle, maximum stress point moved from the anterior edge of tibia attachment surface of the medial meniscus posterior angle to the posterior edge of tibia attachment surface of the lateral meniscus anterior angle, and the stress range of lateral meniscus was greater than that of the medial meniscus. (2) The maximal displacement point moved from the midpoint of inner edge of the medial meniscus to the front outer-upper edge of the lateral meniscus at knee flexion of 0°, 30°, 60° and 90°. Moreover, the range of displacement of lateral meniscus was bigger than the medial meniscus. (3) These findings suggest that the meniscus is the major bearing structure in the process of knee flexion. The lateral meniscus injury rate is greater than the medial meniscus in process of exercise, which is associated with large stress and displacement.

RESUMO

Reduced sulfur compounds ( RSCs) are one of the main pollutant species in the atmosphere, so it is of great significance to develop a rapid and on-line approaches for their detection. In this study, a portable time-of-flight mass spectrometer ( TOF-MS) with magnetic field enhanced photoelectron ionization source was designed to detect RSCs. The photoelectron ionization source was induced from vacuum ultraviolet photons which generated from vacuum ultraviolet (VUV) lamp with energy of 10. 6 eV. The energy of photoelectrons was controlled by adjusting the extraction voltage to produce the photoelectron ionization, and an annular magnet was used in the ionization region to improve the ionization efficiency of photoelectrons. From the simulation result by SIMION software, it was found that the introduction of magnet field made the motion trajectroies of electrons in the helical motion increase and the convergence of electron at the ionization source was achieved. Experimental results showed that after introducing the magnet filed, the sensitivity of H2 S, SO2 and CS2 was improved by a factor of 5. 3, 9. 4 and 6. 9, respectively. With a detection time of 50 s, the limits of detection for H2S, SO2 and CS2 were 0. 14, 0. 52 and 0. 31 mg/m3(S/N=3), respectively. It could be concluded that the portable TOF-MS with magnetic field enhanced photoelectron ionization source has great potential to be applied for on-line monitoring of volatile sulfides at the emission source.

Objective:To detect the existence of IFNGR in the stellate ganglion in female goats .Methods:The stellate ganglia were taken from female goats .The expression of IFNGR was detected by PCR method and immunohistochemical SP staining .Results:The results showed that:IFNGR immunoreactive substances were distributed in neurons , supporting cells and passing fiber , and mainly in the cytomembrane and nuclei of neurons .The relative expression of IFNGR was very significantly higher than that of non-neuronal cells (P<0.01).The sequence of IFNGR1 gene amplified by PCR method contained 376 bp, and IFNGR1 gene of goats exhibited the highest homology with sheep(98%),followed by cattle(97%).Conclusion:The results suggested that the IFNGR in the Stellate Gan-glion of female goats mainly expressed and located in sympathetic postganglionic neurons which were provided with the conditions for the role of IFN-γ, which implied that the Stellate Ganglion may act as the critical point to concert the immune regulation of IFN -γand neu-roregulation of autonomic nerve on cardiovascular system .

RESUMO

A new method was established for the direct, rapid and quantitative analysis of pesticide residues, dimethoate chlorothalonil and malathion by low temperature plasma ( LTP) ionization miniature ion trap mass spectrometer. The LTP ionization probe and sample inlet of ion trap mass spectrometry were enclosed in a metal cavity. With non-contact heating, the samples placed on the sample platform were desorbed into gaseous phase and ionized by LTP ionization probe. The results showed that closed ionization had an edge over the opened ionization. The quantitative analysis of 3 pesticides within the range of 0. 5-10 mg/L was realized by optimizing heating time and flow rate of air, and the relative standard deviations of signal intensity is less than 11%. LODs of pesticide, which were obtained within 5 s, were as low as several hundred pictograms. The results showed that the method could be used for the analysis of pesticide residue on green and organic fruits or vegetables.

Objective:To discuss the rule of the prescriptions of professor JIANG Wen-zhao in treating the spleen and stomach ailment. Methods:Use the frequency analysis to analyze the 98 pieces of prescriptions of professor JIANG Wen-zhao in treating the spleen and stomach ailment. Results:To understand medicine state of professor JIANG Wen-zhao in treating the spleen and stomach ailment preliminarily,select the main medicine in treating diseases of the spleen and stomach,and make clear the significance of these medicines to the corresponding diseases. Conclusion:The basic rules of professor JIANG Wen-zhao in treating the spleen and stomach disease are regulating qi,removing dampness and reinforcing asthenia. Qi regulating herbs,diuretics for eliminating dampness,tonifying deficiency herbs,dampness-removing herbs,blood-activating and stasis-removing herbs,heat-clearing herbs are the key herbs.
