RESUMO

Objective:To investigate the efficacy of proximal femoral intramedullary nail antirotation fixation in the treatment of intertrochanteric fracture of the femur and its effects on Harris hip scores.Methods:Sixty-eight patients with intertrochanteric fracture of the femur who received treatment in Cixi People's Hospital from April 2018 to October 2019 were included in this study. They were randomly assigned to receive dynamic hip screw fixation (control group, n = 34) or proximal femoral intramedullary nail antirotation fixation (observation group, n = 34). Clinical efficacy, Harris score, surgical indicators, and the incidence of complications were compared between the two groups. Results:The response rate was significantly higher in the observation group than in the control group [94.12% (32/34) vs. 76.47% (26/34), χ2 = 5.81, P < 0.05]. The excellent and good rate of hip function as evaluated by Harris hip scores was significantly higher in the observation group than in the control group [91.18% (31/34) vs. 73.53% (25/34), χ2 = 6.05, P < 0.05]. The operative time, blood loss, incision length, and fracture healing time in the observation group were (51.66 ± 10.52) minutes, (120.26 ± 12.29) mL, (8.09 ± 2.62) cm, and (9.86 ± 2.67) weeks respectively, and those in the control group were (78.32 ± 12.23) minutes, (238.45 ± 17.85) mL, (12.95 ± 3.29) cm, and (13.65 ± 3.46) weeks, respectively. There were significant differences in these indices between the two groups ( t = 14.55, 14.03, 14.85, 14.60, all P < 0.05). The incidence of complications was significantly lower in the observation group than in the control group [5.88% (2/34) vs. 23.53% (8/34), χ2 = 6.51, P < 0.05]. Conclusion:Proximal femoral intramedullary nail antirotation fixation is superior to dynamic hip screw fixation in the treatment of intertrochanteric fracture of the femur. The former increases Harris hip score, decreases the incidence of complications, and is of great clinical innovation.

RESUMO

【Objective】 To establish a regional hemovigilance system of adverse reactions to blood donation(ARBD) and to evaluate its operation effect. 【Methods】 A regional monitoring working group was established to improve the ARBD monitoring system in two phases [phase-ⅠJanuary-June, 2019 and phase-Ⅱ July-December, 2019). The suitability of monitoring standards and reporting approaches, as well as the running effect of this system were evaluated by comparing the accuracy of ARBD classification and severity grading, timely report processing and the contents of reports submitted in two phases. The incidence of ARBD and the number of different types of ARBD cases in the two phases were analyzed to compare the monitoring effect. 【Results】 7 and 17 blood banks joined the regional working groups in phase-Ⅰand phase-Ⅱ, respectively. In phase-Ⅰ, 565 cases of ARBD were reported, 72.04%(407/565)of them was accurately sorted and 80.95%(34/42)were timely reported. Over 99% of reports covered such monitoring items as blood donors′ gender, type of blood donation, place of blood donation, organization mode of blood donation, symptom of ARBD, type, severity grading and other related factors, and less than 20% of reports recorded the details concerning the treatment following ARBD. In phase-Ⅱ, 1098 cases of ARBD were reported, 94.54%(1 038/1 098)of them was accurately sorted and 96.08%(98/102)were timely reported. Over 97% of reports covered the monitoring items mentioned above, still less than 30%, though elevated in comparison with phase-Ⅰ, of reports recorded the details concerning the treatment following ARBD, and less than 20% tracked the ARBD. The overall incidence of ARBD reported was 0.87%, while 0.93%(1 098/118 565)in phase-Ⅱ and 0.77%(565/73 267)in phase-Ⅰ, with statistically significant differences (P<0.01). 【Conclusion】 The regional monitoring system for ARBD in Chongqing, with Blood Safety Hemovigilance Guidelines(T/CSBT 001-2019)as the standard and the Blood Safety Monitoring Management Information System as the reporting approach, has been basically established through two phases of development, and the reporting contents need to be further managed at specific levels.

RESUMO

Objective@#To compare the efficacies of two training protocols, i.e. the multimedia instruction and the conventional method, in periodontal surgery teaching for undergraduate students.@*Methods@#One hundred and twenty-three dental undergraduates in their pre-clinical training course were recruited and divided into two groups according to the learning grade matching principle: the experimental group (multimedia instruction, 60 students) and the control group (conventional method, 63 students). The teaching aim was to train the students gingivectomy and periodontal flap surgery by using the pig jaws. The conventional teaching method of teacher-demonstrating and student-practicing was used in the control group, and the practice time of the students′ for each surgery was 45 minutes. A standardized teaching video combined with the teachers′ explanations of the key steps was used in the experimental group. The students′ practice time for each surgery was 60 minutes. The efficacy of teaching protocol was evaluated by the teachers according to the scoring criteria set by the teaching group.@*Results@#In gingivectomy training, accuracy of fixed points, angle of postoperative gingival margin and morphology of gingiva of the experimental group were better than those of the control group. The experimental group also had more complete and continuous excised gingivae and more thorough adjacent gingival removal. The total scores of the experimental group were significantly higher than those of the control group (92.8±2.6 vs. 89.9±3.7, P<0.05). In periodontal flap surgery training, the experimental group operated the blade around the shape of the tooth better in internal bevel incision than the control group. Additionally, the incision position of internal bevel incision, periosteal integrity after flapping and flap depth of the experimental group were better than those of the control group. Besides, the experimental group had smoother flap edge and more thorough debridement. The total scores of the experimental group were significantly higher than those of the control group (92.2±4.1 vs. 89.2±4.4, P<0.05).@*Conclusions@#The teaching efficacy of multimedia instruction was better than that of the conventional method. Its value needs to be further tested in future teaching practice.

RESUMO

To provide references and recommendations about periodontal professionals training for our country by introducing American training mode of clinical postgraduate students of periodontology. Based on ground visits and surveys of University of Washington School of Dentistry's postdoctoral program in periodontics, the professional training mode and its characteristics which adapt to the development trend of American periodontology were summarized and concluded. The discussion is in the following three parts: the overall status of postgraduate education of periodontology in the United States, the training mode of the clinical postgraduate students of periodontology at University of Washington School of Dentistry, and the observation of the American Academy of Periodontology's annual conferences. This kind of training mode supervised by the American Dental Association, giving priority to clinical practice and emphasizing cultivating the multidisciplinary comprehensive ability are worth learning. Meanwhile, the development trend of periodontology in the United States which pay increasing attention to carry out implant therapy with periodontal features and the adaptive training mode of clinical postgraduate students of periodontology also deserves our deep reflection and can be used for reference.

RESUMO

Objective@#To explore the training pattern of periodontal probing practice so as to improve the accuracy of probing depth measurements of undergraduate students.@*Methods@#Sixty-two undergraduate dental students in their pre-clinical training course were divided into two groups (31 students in each group): the reform group and the control group. The students in reform group learned and practiced periodontal probing by using transparent gingiva models for 30 minutes, and using conventional periodontal probing models for further 90 minutes in two sessions. The total practicing time was 2 hours. The students in control group learned and practiced periodontal probing using conventional periodontal probing models only for 1 hour of practicing time. After the training, new conventional periodontal probing models were used to evaluate the accuracy of probing depth measurements of the students in the two groups. The probing depths of Ramfjord index teeth were recorded by the students and were compared with the results of an experienced teacher's measurements.@*Results@#The percentage of absolutely consistent sites in the reform group (63.4% [708/1 116]) was significantly higher than that of the control group (60.0% [670/1 116]) (P=0.012). In both reform group and control group, the percentages of absolutely consistent sites of the incisors (reform group: 69.1% [257/372], control group: 65.9% [245/372]) were significantly higher than those of the molars (reform group 58.9% [219/372], control group 54.3% [202/372])(P< 0.01), meanwhile, the percentages of absolutely consistent sites of the buccal-lingual sites (reform group: 88.7% [330/372], control group: 80.4% [299/372]) were significantly higher than those of the interproximal sites (reform group: 50.8% [378/744], control group: 49.9% [371/744])(P<0.01).@*Conclusions@#The comprehensive reform of periodontal probing training could improve the accuracy of probing depth measurement of the undergraduate students. During the pre-clinical practice, probing training in the molar areas and the interproximal sites should be reinforced.

RESUMO

Objective@#To observe the long-term clinical treatment outcome and the influencing factors of the outcome for the teeth receiving modified crown lengthening surgery combined with root canal treatment and post-core crown restoration. To summarize the clinical guidelines of modified crown lengthening surgery in selection of indications and for mulation of treatment planning.@*Methods@#Fifty-seven patients with a total of 67 teeth receiving modified crown lengthening surgery combined with root canal treatment and post-core crown restoration for at least a 6 months' follow-up period between July 2004 and July 2013 were recruited in this retrospective study by phone call interviews. The patients' clinical outcomes were evaluated by the combination of clinical examination, radiograph and questionnaire regarding patient-reported outcome of the last follow up (≥9 months post modified crown lengthening surgery and ≥6 months after definite crown restorations). All of the treated teeth were classified into two groups, group A (teeth with good clinical treatment outcome) and group B (teeth with poor clinical treatment outcome), based on the defined criteria including patients' satisfaction with the function and esthetics of the teeth and absence of periodontal, endodontic and prosthodontic complications. The potential influencing factors of clinical treatment outcome were also determined by Logistic regression analysis.@*Results@#Vertical root fracture in 1 tooth was found on its periapical film and the tooth was deemed hopeless. Thus, the survival rate is 99% (66/67) for the multidisciplinary treatment approach. Seventy-two percent (48/67) of the teeth achieved good clinical treatment outcome and 28% (19/67) of the teeth developed one or several complications. In group B (teeth with poor clinical treatment), 16 out of teeth exhibited periodontal complications with bleeding on probing (BOP) positive mostly found. Logistic regression analysis demonstrated that plaque control (OR=21.392, P=0.014), edge form (OR=7.610, P=0.011), and smoking experience (OR=7.315, P=0.018) were the risk factors influencing the clinical treatment outcome of modified crown lengthening surgery combined with root canal treatment and post-core restoration.@*Conclusions@#Modified crown lengthening surgery combined with root canal treatment and post-core restoration has a good and stable clinical effect in the observational time of 6-114 months. Plaque control, smoking status and edge form of the tooth appeared to be the influencing factors of this multidisciplinary treatment approach.

RESUMO

BACKGROUND:As the existence of tumor stem cel s, it is difficult to completely eliminate tumors in clinic. OBJECTIVE:To explore the tumor-kil ing effect of gastric cancer stem cel s as antigen to stimulate dendritic cel s combined with cytokine-induced kil er cel s. METHODS:Side population cel s from human gastric cancer cel lines were isolated, and tumor antigen was prepared by freeze thawing method. After coculture with dendritic cel s, dendritic cel s combined with cytokine-induced kil er cel s, gastric cancer cel antigen, and gastric cancer stem cel antigen, kil ing rates of gastric cancer cel s were detected using MTT assay. Expression rate of CD83, a mature dendritic cel surface marker, was also detected. RESULTS AND CONCLUSION:The CD83 expression level and kil ing rate of gastric cancer cel s were both significantly lower in the gastric cancer stem cel antigen group than the other groups (both P<0.05). These results indicate that gastric cancer stem cel s as antigen to stimulate dendritic cel s combined with cytokine-induced kil er cel s can promote the proliferation of gastric cancer cel s and elevate the ability to kil ing gastric cancer cel s.

RESUMO

BACKGROUND:Interleukin-8 is an important inflammatory chemokine that plays an important role in the regulation of tumor cel proliferation and angiogenesis. OBJECTIVE:To investigate the effect of interleukin-8 on the invasion and metastasis of CD133+hepatocel ular carcinoma stem cel s. METHODS:After isolation and culture of MHCC97-H cel lines, CD133+/CD133-MHCC97-H cel s were sorted using immunomagnetic beads. CD133 expression was detected using flow cytometry, and interleukin-8 level in supernatant was measured using ELISA method. Cloning efficiency, tumorigenic capacity, cel migration and invasion ability were detected through colony formation assay, tumorigenesis experiment in nude mice, and Transwel detection. Additional y, other cel s were neutralized using interleukin-8 neutralizing antibody. Measurement results were compared between cel s undergoing different treatments. RESULTS AND CONCLUSION:The CD133 level, interleukin-8 level, cloning efficiency and cel membrane permeability of CD133+MHCC97-H cel s were significantly higher than those of CD133-MHCC97-H cel s (P0.05). These results show that interleukin-8 could be specifical y involved in the invasion and metastasis of CD133+MHCC97-H cel s.

RESUMO

Objective:To explore a crown form classification method for upper central incisor which is more objective and scientific than traditional classification method based on the standardized photography technique. To analyze the relationship between crown form of upper central incisors and papilla filling in periodontally healthy Chinese Han-nationality youth. Methods:In the study,180 periodontally healthy Chinese youth (75 males, and 105 females )aged 20 -30 (24.3 ±4.5)years were included. With the standardized upper central incisor photography technique, pictures of 360 upper central incisors were ob-tained.Each tooth was classified as triangular, ovoid or square by 13 experienced specialist majors in prothodontics independently and the final classification result was decided by most evaluators in order to ensure objectivity. The standardized digital photo was also used to evaluate the gingival papilla filling situation. The papilla filling result was recorded as present or absent according to naked eye observation. The papilla filling rates of different crown forms were analyzed.Statistical analyses were performed with SPSS 19.0.Results:The proportions of triangle, ovoid and square forms of upper central incisor in Chinese Han-nationality youth were 31.4% (113 /360),37.2% (134 /360)and 31.4% (113 /360 ), respectively ,and no statistical difference was found between the males and females.Average κvalue be-tween each two evaluators was 0.381.Average κvalue was raised up to 0.563 when compared with the final classification result. In the study,24 upper central incisors without contact were excluded, and the papilla filling rates of triangle, ovoid and square crown were 56.4% (62 /110),69.6% (87 /125 ), 76.2% (77 /101)separately.The papilla filling rate of square form was higher (P =0.007 ).Conclu-sion:The proportion of clinical crown form of upper central incisor in Chinese Han-nationality youth is obtained.Compared with triangle form, square form is found to favor a gingival papilla that fills the inter-proximal embrasure space. The consistency of the present classification method for upper central incisor is not satisfying, which indicates that a new classification method, more scientific and objective than the present one,is to be found.

RESUMO

Objective:To construct the finite element models of maxillary central incisor and the simu-lations with crown lengthening surgery and post-core restoration in management of different crown-root fracture types, to investigate the stress intensity and distributions of these models mentioned above, and to analyze the indications of crown lengthening from the point of view of mechanics. Methods:An extrac-ted maxillary central incisor and alveolar bone plaster model were scanned by Micro-CT and dental impression scanner ( 3shape D700 ) respectively. Then the 3D finite element models of the maxillary central incisor and 9 simulations with crown lengthening surgery and post-core restoration were construc-ted by Mimics 10. 0, Geomagic studio 9. 0 and ANSYS 14. 0 software. The oblique static force (100 N) was applied to the palatal surface ( the junctional area of the incisal 1/3 and middle 1/3 ) , at 45 degrees to the longitudinal axis, then the von Mises stress of dentin, periodontal ligament, alveolar bone, post and core, as well as the periodontal ligament area, were calculated. Results:A total of 10 high-precision three-dimensional finite element models of maxillary central incisor were established. The von Mises stress of models: post >dentin >alveolar bone >core >periodontal ligament, and the von Mises stress increased linearly with the augmentation of fracture degree ( besides the core) . The periodontal ligament area of the crown lengthening was reduced by 12% to 33%. The von Mises stress of periodontal ligament of the B2L2c, B2L3c, B3L1c, B3L2c, B3L3c models exceeded their threshold limit value, respective-ly. Conclusion: The maxillary central incisors with the labial fracture greater than three-quarter crown length and the palatal fracture deeper than 1 mm below the alveolar crest are not the ideal indications of the crown lengthening surgery.

RESUMO

SUMMARY In this article , different methods to deal with teeth fractures were discussed by presenting a case of traumatic crown-root fracture in the anterior esthetic zone .The traumatic crown-root fracture is a common problem in clinic .When a fracture line locates in close proximity to or below the alveolar bone crest , the fracture most likely involve the junctional epithelium and the connective tissue attachment .This type of fracture becomes a challenge for restorative dentists because it involves biologic , functional , and es-thetic considerations , especially when the fracture occurs in an esthetic area .In this case , a young patient presented with two fractured upper anterior teeth to the Department of Periodontics , Peking University School and Hospital of Stomatology .After the comprehensive clinical evaluation , the right central incisor was decided to extract for implant therapy and the right lateral incisor was decided to retain by one modified crown lengthening surgery .The most common technique applied to save a retained root is a clinical crown lengthening procedure .However , the aggressive alveolar bone resection of both target and adjacent teeth to reestablish the bone width and periodontal health may compromise functional and esthetic outcomes .To re-duce loss of excessive osseous tissue during osteotomy procedure , the modified crown lengthening of the right lateral incisor was performed , including minor bone resection and root reshaping .Regarding the right central incisor , the retained root was all located below the alveolar bone crest .The extraction and implant procedure , combined with guided bone graft were performed to avoid the damage to neighbor teeth during traditional restorative therapy and to reshape a preferable buccal contour .At the last visit , the patient was recalled with healthy periodontium , normal tooth function and favorable esthetic results .

Objective To study the effect of anterolateral calf flap with extra-territorial blood supply in repairing large area of foot skin tissue injury. Methods 40 cases of arge area of foot skin tissue injury were divided into the observation group which were given anterolateral crural flap transplantation and the control group which were given anterolateral thigh flap transplantation. The wound healing condition and walking ability were observed. Results The level of wound healing, Holden walking ability and FAC walking ability were better than control group (P<0. 05). In the observation group, wound healing time was (15. 2 ± 6. 2) d and donor skin crusting time was (9. 4 ± 1. 5) d, which were less than the control group(P<0. 05). Conclusion Anterolateral calf flap with extra-territorial blood supply has positive clinical value as it can improve wound healing and walking function.

RESUMO

Objective To investigate the effects of ω-3 fatty acids on inflammation and coagulation function in hepatocellular carcinoma (HCC) patients after partial hepatectomy. Methods In this prospective randomized controlled trial,80 HCC patients were randomly divided into two groups based on the parenteral nutrition (PN) scheme that was provided 5 days after partial hepatectomy:fish oil group:20% fish oil and 80% mediumlong fatty acid; and control group:100% medium-long fatty acid. The inflammation, liver function, blood coagulation indicators, and prognosis were compared between two groups. Results After 3 days of hepatectomy, the serum C-reactive protein levels in the control and fish oil groups were (119.6 ± 57.3) mg/L and (97.1 ± 46.1) mg/L(P =0.016),respectively; and on day 6 after surgery,C-reactive protein levels of two groups were (54.9 ± 26.1)and (40.7 ±21.8) mg/L,respectively (P =0.018).The plasma fibrinogen level in the fish oil group was significantly lower than that in the control group 3 days [(3.4 ± 1.1) vs.(4.1 ± 1.3) g/L,P < 0.001] and 6 days [(3.3 ± 0.9) vs.(3.8 ± 1.2) g/L,P < 0.001] after surgery. The retention rate of indocyanine green at 15 minutes on the 7th postoperative day were 8.6% ±4.9% and 9.2% ±7.3% in the control and fish oil groups, respectively (P=0.179).The recurrent rate (40.63% vs.44.12%,x2 =0.082,P =0.774) and survival rate in the fish oil and control groups (90.62% vs.88.24%,x2 <0.001,P =1.000) also showed no significant difference after 6 months of follow-up.Conclusion ω-3 fish oil emulsion can reduce postoperative inflammatory response and prevent thrombosis.
