RESUMO

We report a procedure using a pressure-controllable flute needle to remove subfoveal retention of perfluorocarbon liquid (PFCL) under silicone oil. With a two-port pars plana approach, we used a 27-gauge dental injection needle to create a retinotomy at the farthest edge of the PFCL bubble from the fovea. A 27-gauge flute needle was then inserted into the edge of the subfoveal PFCL to aspirate it with vacuum pressure. Three patients with subfoveal retained PFCL were treated by this procedure within silicone oil tamponade 1 month after the first operation. They promptly underwent successful removal of the PFCL with postoperative retinal reattachment and good visual outcome. This procedure allows safe and early treatment for subfoveal retained PFCL. Many medical institutions around the world could implement this procedure using common dental injection needles and flute needles.

This study aims to evaluate the carbon footprint of raw water from reservoirs for domestic use in Taiwan. It also provides a preliminary measure and reference database for greenhouse gas (GHG) emission of reservoir systems in Taiwan. Four reservoirs, Feitsui (F.T.) and Liyutan (L.Y.T.) in subtropical zone and Nanhua (N.H.) and Tsengwen (T.W.) in tropical zone, were selected as the cases to be examined for carbon footprint inventory, including the GHG emission from the water body and from human activities. Carbon footprint inventory followed PAS 2050 (2011 Specification for the assessment of the life cycle greenhouse gas emissions of goods and services). GHG emission from water body followed the instruction of UNESCO guidelines. The boundary of this inventory covers the water intake works, impoundment region, the dam, the affiliated hydroelectricity power plant, the administration center and other facilities. In this study, the floating chambers with gas chromatography (GC) were chosen to measure the GHG flux from the water body. For the emission of CH4 and N2O from the water body, there are no significantly difference between the fluxes during the daytime and nighttime. For carbon dioxide, the instantaneous flux during the nighttime is higher than the daytime flux. The two reservoirs in tropical zone emit more CO2e from the water body than those in subtropical zone. Summarizing the direct and indirect GHG emission, for the four reservoirs, the annual emission quantities ranged from 653 ton of CO2e to 23,146 ton of CO2e. The carbon footprint of water supply for domestic use ranged from 0.002 kg CO2e/m3 to 0.028 kg CO2e/m3. Roughly speaking, the total GHG emission quantity of the 24 main reservoirs in Taiwan was estimated to be around 121,800 ton of CO2e with the total yield of 4.35 billion m3 of water annually using the highest carbon footprint 0.028 kg CO2e/m3.

Transitional cell carcinoma (TCC) of the urothelium is often multifocal and subsequent tumors may occur anywhere in the urinary tract after the treatment of a primary carcinoma. Patients initially presenting a bladder cancer are at significant risk of developing metachronous tumors in the upper urinary tract (UUT). We evaluated the prognostic factors of primary invasive bladder cancer that may predict a metachronous UUT TCC after radical cystectomy. The records of 476 patients who underwent radical cystectomy for primary invasive bladder TCC from 1989 to 2001 were reviewed retrospectively. The prognostic factors of UUT TCC were determined by multivariate analysis using the COX proportional hazards regression model. Kaplan-Meier analysis was also used to assess the variable incidence of UUT TCC according to different risk factors. Twenty-two patients (4.6 percent). developed metachronous UUT TCC. Multiplicity, prostatic urethral involvement by the bladder cancer and the associated carcinoma in situ (CIS) were significant and independent factors affecting the occurrence of metachronous UUT TCC (P = 0.0425, 0.0082, and 0.0006, respectively). These results were supported, to some extent, by analysis of the UUT TCC disease-free rate by the Kaplan-Meier method, whereby patients with prostatic urethral involvement or with associated CIS demonstrated a significantly lower metachronous UUT TCC disease-free rate than patients without prostatic urethral involvement or without associated CIS (log-rank test, P = 0.0116 and 0.0075, respectively). Multiple tumors, prostatic urethral involvement and associated CIS were risk factors for metachronous UUT TCC, a conclusion that may be useful for designing follow-up strategies for primary invasive bladder cancer after radical cystectomy.

The ability of purified plasma membrane glycoconjugates to inhibit the EDTA-resistant agglutination between aggregation-stage cells of Dictyostelium discoideum has suggested that receptor binding of these glycoconjugates provides a basis for cell-cell cohesion during aggregation. This has been tested by analysis of a series of mutants with different defects in the assembly of N-linked oligosaccharides. Mutant HL241 lacks outer branch components of N-linked oligosaccharides and fails to aggregate or express EDTA-resistant cohesion. HL244 makes unsulphated but otherwise normal N-linked oligosaccharides, generates multiple tips on aggregated cell mounds in some clones, and shows abnormally strong EDTA-resistant cohesion. Two mutants that are temperature-sensitive for complete processing of N-linked oligosaccharides are also temperature-sensitive for expression of both aggregation ability and EDTA-resistant cohesion. A revertant that recovered essentially normal N-linked oligosaccharide processing at the restrictive temperature has also recovered its ability to aggregate and to agglutinate in EDTA.
