RESUMO

<p><b>OBJECTIVE</b>This study aimed to evaluate the vascular endothelial growth factor (VEGF) expression and permeability of vascular endothelial cell under microvibration.</p><p><b>METHODS</b>Human umbilical vein endothelial cell (HUVEC) were cultured, randomly vibrated under low frequency of 0.2, 0.5, 2, 5 Hz, 30 min per day. The VEGF mRNA level was detected by Tagman probe real-time fluorescence quantitative polymerase chain reaction (PCR), and the VEGF protein expression level was detected by Western blot. The permeability of vascular endothelial cell was evaluated.</p><p><b>RESULTS</b>Compared with the blank control group, the mRNA and protein expression level of VEGF were significantly increased under 0.2, 0.5 Hz thelial, and increase the permeability microvibration (P<0.05), and decreased under 2, 5 Hz microvibration (P<0.01). The vascular endothelial permeability in creased under 0.2, 0.5 Hz microvibration (P<0.01), whereas the permeability decreased under 2, 5 Hz microvibration (P<0.01).</p><p><b>CONCLUSION</b>0.2-0.5 Hz microvibration can up-regulate the expression of VEGF mRNA and protein in vascular endothelial, and increase the permeability.</p>

OBJECTIVE@#To explore the clinical anatomy of vagal schwannoma and the methods to preserve the continuity of vagus nerve during tumor resection and postoperative functional rehabilitation methods of the nerve.@*METHOD@#To review 15 cases of vagal schwannoma from 2002.07 to 2011.08 treated in the Department of Otolaryngology, West China Hospital. The operative modality was to strip the tumor while keeping the Schwann membrane almost intact so that to protect the nerve fibers as much as possible. postoperative treatment included glucocorticoids, neurotrophic medication, as well as voice and swallowing rehabilitation.@*RESULT@#Among 15 cases of schwannoma patients, 3 cases (3/15) experienced hoarseness of voice with the ipsilateral vocal cord located at the median position 2 years after operation. 2 cases(2/15) experienced no obvious hoarseness of voice with vocal cords slight vibration when pronouncing and reached normal vocal cord movement 3 months after operation, 10 cases were healed without vocal cord complications, 7 cases (7/15) experienced choking during drinking and the symptom was gradually eliminated 1 3 months thereafter.@*CONCLUSION@#To carefully discern and preserve the vagal fibers during the operation of vagal schwannoma could eliminate postoperative hoarseness and choking. Protection of superior laryngeal nerve should be also brought to the forefront because it could affect the quality of swallowing and speaking.

OBJECTIVE@#To investigate the feasibility of salvage surgery of thyroid carcinoma in advanced stage with common carotid artery involvement.@*METHODS@#Reviewed 4 cases of advanced-stage thyroid carcinoma treated in the Department of Otolaryngology, West China hospital from July 2006 to September 2009. CT revealed that the common carotid artery entrapped in the tumor without obvious anatomical space. 2 cases were proved to be low-differentiated squamous carcinoma pathologically and the rest 2 cases were papillary carcinoma. Total thyroidectomy, total laryngectomy and bilateral neck dissection were performed in 3 cases while in one case, total thyroidectomy, bilateral neck dissection and partial tracheal resection being performed. Adjacent prepared cervico-pectoral flap was used to reconstruct the defect of cervico-thoracic trachea. Rupture of common carotid artery in one case took place and was repaired by mere suture. One case of low differentiated squamous carcinoma was given postoperative chemotherapy and radiotherapy while the rest 3 cases given no adjuvant chemotherapy and radiotherapy. The follow-up time lasted from half a year to 2 years.@*RESULTS@#Half a year postoperatively, the low-differentiated squamous carcinoma patient not receiving postoperative chemoradiotherapy suffered from lumber spinal metastasis, one year after operation he was still alive and half a year later lost follow-up, the other 3 patients were alive from half a year to 2 years postoperatively without any obvious clue of recurrence or remote metastasis.@*CONCLUSION@#Salvage surgery is still feasible in thyroid carcinoma in advanced stage with common carotid artery involvement. The larynx and trachea should be kept intact as possible but the reconstruction of the trachea defect is recommended to be less prior in the situation when carotid artery is involved in.
