RESUMO

@#Objective To investigate the application value of 3.0T magnetic resonance diffusion weighted imaging (DWI) combined with attenuated inversion recovery (FLAIR) sequence and 3D arterial spin labeling imaging (3D ASL) in patients with acute cerebral infarction (ACI) with unknown onset time.Methods 35 patients with acute cerebral infarction who underwent 3.0T magnetic resonance DWI,FLAIR,3D ASL and magnetic resonance angiography (MRA) in our hospital with unknown time window were selected.Patients who had severe stenosis or occlusion of large vessels found in MRA and with DWI high signal without high signal on FLAIR in the corresponding DWI high signal area,DWI high signal area and 3D ASL with decreased perfusion area and>1.2 double mismatch were collected.Meanwhile,35 patients with acute cerebral infarction within a clear onset time window (<4.5 h) were selected for reperfusion therapy.NIHSS score before and 24 h after treatment,mRS score at 90 d and incidence of intracranial hemorrhage were compared between the two groups.Results There was significant difference in NIHSS score at 24 h before and after treatment between double mismatch group and clear time window group (P<0.05).There was no significant difference in mRS at 90 d (P>0.05). Conclusion DWI- FLAIR and DWI-3D ASL double mismatch can be used as an effective method to evaluate the onset time,ischemic penumbra and reperfusion in patients with acute cerebral infarction with unknown onset time,and provide a basis for intravenous thrombolytic therapy,bridging therapy and conservative treatment.

Objective:To explore the application value in cerebral blood perfusion status of dynamic susceptibility contrast enhanced perfusion weighted imaging (DSC-PWI) in ischemic cerebrovascular disease (ICVD).Methods:Retrospective analysis of 31 cases ICVD head magnetic resonance imaging (MRI), diffusion weighted imaging (DWI), magnetic resonance angiography (MRA) and DSC-PWI image in clinical diagnosis, and selectively analyze parameters including relative cerebral blood flow (rCBF), relative cerebral blood volume (rCBV), mean transit time (MTT) and time to peak (TTP) for generalize its characteristics.Results:31 cases of ICVD, 16 cases hypoperfusion, rCBF decreased significantly, rCBV decreased, MTT and TTP prolonged, include 7 cases of large vessel disease and 5 cases of small vessel disease. The collateral circulation formed 9 cases, including 3 cases with good compensation, rCBF normal, rCBV normal, MTT and TTP prolonged, 6 cases with bad compensation, rCBF decreased, rCBV normal or increased, MTT and TTP prolonged. The blood reperfusion in 3 cases, rCBF normal or slightly increased, rCBV increased, MTT shortened or normal, and TTP shortened. The excessive perfusion in 3 cases, rCBF significantly increased and rCBV significantly increased, with MTT and TTP shortened.Conclusions:DSC-PWI can reliably reflect the perfusion state and collateral circulation compensation of ICVD, so as to guide the selection of clinical treatment program and significantly improve the prognosis of patients.

Objective To study the features of CT and MRI diagnosis of intracranial germ cell tumors ,and to improve the un-derstanding of its imaging findings .Methods Fifteen cases of intracranial germ cell tumors proved by pathology and follow-up were retrospectively analyzed .Results Among the 15 cases with intracranial germ cell tumors ,9 cases in pineal body , tumours showed like-round masses,the margin of the masses were well defined .CT was slightly high density,MRI revealed isointense or hypointense signals on T1 weighted and homogeneous hyperintense signals on T 2 weighted image without edema around tumors and homogeneous en-hancement.One cases in CSF spread.Four cases were identified in the saddle-up area, among the 2 cases was cystic,1 cases was sol-id-cystic and 1 cases was solid,CT was isointense or high density in solid,low density in cystic,without edema around tumors.MRI re-vealed slight-hypointense signals on T 1 weighted and hyperintense signals on T 2 weighted image clearly enhanced in solid , cystoid with-out enhance .In the other 2 cases of intracranial germ cell tumors in the basal ganglia region ,CT was inhomogeneous high density ,inho-mogeneous signals on T2 and T1 weighted image, inhomogeneous enhancement .Conclusions Intracranial germ cell tumors show char-acteristic manifestations on CT and MRI,according to location,shape,signal,age characteristic,which is helpful in diagnosis and differ-ential diagnosis and guide treatment plan .

RESUMO

OBJECTIVE:To establish a RP-HPLC method for determination of the content of baicaline in Qingkailing Injection,so as to control the quality of this preparation.METHODS:The column was C18.The mobile phase was methanol-acetic acid-water(45∶1∶55).The detection wavelength was 274nm.RESULTS:There was a good linear relationship between the concentration of baicaline and absorption area value in range of 0.25~2.5?g/ml,r=0.9 992.The average recovery was 98.4%(n=5) with RSD=1.5%(n=5).CONCLUSION:The method was highly repeatable and accurate.
