RESUMO

Objective:To investigate the effect of an improved hydro-colloid dressing sticking method on preventing protective equipment induced facial pressure injuries among medical staff through a prospective, single-arm trial. Methods:According to the size of the edge of N95 mask, four pieces of hydro-colloid dressing with a size of 2 cm×5 cm were cut, and the edges of the mask were tension-free stuck. This improved sticking method was applied to the nurses working in the isolation ward. The follow-up was continued for two weeks, skin condition was reported daily, and the occurrence of pressure ulcers was analyzed after two weeks of follow-up.Results:Twenty percent (4/20) of subjects experienced increase grades of nasal and twenty five percent (5/20) in cheek pressure ulcers during a two-week follow-up. The incidence of the increased stage of cheek pressure ulcer in normal/dry skin was higher than in oily skin. Stage 2 pressure ulcers occurred in only five percent (1/20) of subjects, and no subjects had stage 3 or higher stage pressure ulcers. The incidence of increased cheek pressure ulcer grade in patients with neutral / dry skin was (5/11), higher than 0 in patients with oily skin ( χ2 value was 5.378, P = 0.02). The stage of nasal pressure ulcer was positively correlated with the disappearance time of the indentation of protective equipment ( r value was 0.615, P=0.004). The rate of increase in nasal pressure sore grade was higher in subjects who required more than 120 minutes of indentation recovery time (2/8) than in subjects who required 30 to 60 minutes 1/5 ( χ2 value was 4.382, P<0.05) and 60-120 minutes 1/5 ( χ2 value was 6.472, P<0.05). Conclusions:This improved hydro-colloid dressing sticking method can effectively prevent pressure injuries related to wearing N95 masks for a long time. However, skin evaluation of medical staff should be done properly, moisture protection for normal/dry skin should be emphasized, and skin blood flow recovery after compression should be strengthened. This method can reduce the risk of infection of medical staff, and it is worthy of clinical application.

RESUMO

Ovarian cancer is the fifth leading cause of cancer-related death in women. 75% of ovarian cancer patients were detected at an advanced stage. At present, the disease lacks effective early screening method and the clinical therapy effect is poor, which has become a serious threat to women's health. The use of animal models of ovarian cancer is an important mean to elucidate the pathogenesis of the disease, and to screen effective diagnosis and treatment. The disease models are mainly divided into four types: spontaneous, induced, transplanted and gene intervention type. Mice, rats, hens,Mirotus Fortis and Mongolian gerbil are mainly selected to prepare animal models of ovarian cancer. Based on recent literature reports,we reviewed the preparation method of animal models of ovarian cancer and introduced the evaluation standards and main characteristics of these animal models.

RESUMO

Cardiac arrest (CA) is the most serious clinical emergency situation and cardiopulmonary-cerebral resuscitation (CPCR) performed on site with high quality is the optional therapy for its management. It has been reported that prolonging the resuscitation time after 30-minute failed conventional cardiopulmonary resuscitation (CPR) could improve the in-hospital survival rate of CA patients, and how to improve the out-hospital survival rate and survival quality of these patients is a research hot focus at present. A male patient admitted to Emergency Center of Shihezi People's Hospital reported in Xinjiang in this study had two CAs. In 2002, he experienced Adams-Strokes syndrome due to acute myocardial infarction (AMI) and survived after 35-minute of successful CPR. The criminal vessel was judged to re-canalize clinically 2 hours after thrombolytic therapy with urokinase, and he was cured and discharged from hospital 25 days later. In 2016, the second CA insult him and after the 185-minute CPR, he survived but experienced the post-CA syndrome. As long as 7-day continuous mild hypothermia was performed, the temperature of displacement fluids in continuous blood purification (CBP) was adjusted to 35 ℃ to achieve the goal of brain protection management requirements. He was cured and discharged from hospital 75 days later. During the 9-month follow-up, he did well in activities of daily living and could engage in routine housework. This paper introduces the treatment process of the patient in detail, and provides experience for clinical treatment.

RESUMO

BACKGROUND: Non-adherence to fluid restrictions is common in patients maintained on hemodialysis prior to and after kidney transplantation, which has a profound influence on the development and transfer of illness of the patients.Mental factors have great influences on adherence to fluid restrictions.OBJECTIVE: To investigate the effects of rational-emotive therapy on adherence to fluid restrictions of patients maintained on hemodialysis prior to and after kidney transplantation.METHODS: In total 100 patients maintained on hemodialysis were randomly assigned to an intervention group(n = 50)and a control group(n = 50).The control group received the conventional therapy. At the same time, the intervention group received the conventional therapy combined with rational-emotive therapy. All cases were assessed with the Symptom Checklist 90(SCL-90)and Medical Coping Modes Questionnaire(MCMQ)prior to and after intervention.Clinical indices related to adherence to fluid restrictions were measured three successive times and the average score of each index was calculated.RESULTS AND CONCLUSION: Forty-eight patients from the intervention group and 44 patients from the control group accomplished the study. After intervention, the scores of the two subscales, confronce and avoidance, were significantly higher than those in the control group(P < 0.05).However,some scores were significantly lower than those in the control group, including the score of acceptance/resignation subscale of MCMQ,total average score of SCL-90,the score of somatization,obsessive-compulsive,interpersonal sensitivity, depression, anxiety, hostility, photic anxiety and additional items, and the score of four clinical indices related to adherence to fluid restrictions(the ratio of interdialysis weight gain to dry weight, mean systolic blood pressure, mean diastolic blood pressure, and mean ultrafiltration volume prior to hemodialysis)(P < 0.05).All these findings suggest that rational-emotive therapy can effectively amend the coping modes and the mental states of the patients, and then improve their adherence to fluid restrictions.
