RESUMO

A three-month-old, intact male Maltese dog was presented to the hospital with lethargy after taking human medication, Motilitone. Physical examination, including a neurological examination, revealed no remarkable findings, but cholinergic crisis symptoms appeared gradually. Blood and radiological examinations showed no remarkable findings. The dog was tentatively diagnosed with a cholinergic crisis associated with Motilitone intake. Treatment included intravenous administration of atropine (0.02 mg/kg) every 30 minutes and supportive fluid therapy. After 12 hours of treatment, the patient’s clinical signs were resolved. This is the first case report describing Motilitone toxicity in a dog.

Background@#Atopic dermatitis (AD) is a common chronic inflammatory skin disease. To understand AD, there have been many trials establishing AD animal models. Although various trials to establish AD animal models have been existed, even the mechanisms of AD in animal models are not enough clarified. @*Objectives@#This study assessed AD characteristics induced in Nishiki-nezumi Cinnamon/Nagoya (Nc/Nga) mice following trinitrochlorobenzene (TNCB) treatment for different periods and house dust mite (HDM) treatment to compare each model's immunological patterns, especially with cytokine antibody array tool. @*Methods@#In this study, we exposed Nc/Nga mice to TNCB or HDM extract to induce AD. Nc/Nga mice were divided into 4 groups: control, TNCB 2 weeks-treated, TNCB 8 weeks-treated, and HDM-treated groups. After AD induction, all mice were evaluated by serum immunoglobulin E (IgE) concentration and serum cytokine antibody assays, scoring of skin lesions, scoring of scratching frequency, and histological analysis. @*Results@#The results showed significant differences between groups in serum IgE concentration, skin lesion scores, and scratching frequency. The analysis results for serum cytokine antibody arrays showed that in the TNCB 8 weeks- and HDM-treated groups, but not in the TNCB 2 weeks-treated group, expressions of genes related to the immune response were enriched. Among the histological results, the skin lesions in the HDM-treated group were most similar to those of AD. @*Conclusions@#We confirmed that immunological pattern of AD mice was markedly different between HDM and TNCB treated groups. In addition, the immunological pattern was quietly different dependent on TNCB treated duration.

Despite significant advances in neuroscience research over the past several decades, the exact cause of AD has not yet fully understood. The metabolic hypothesis as well as the amyloid and tau hypotheses have been proposed to be associated with AD pathogenesis. In order to identify metabolome signatures from the postmortem brains of sporadic AD patients and control subjects, we performed ultra performance liquid chromatography coupled with linear ion trap-Orbitrap mass spectrometer (UPLC-LTQ–Orbitrap-MS). Not only our study identified new metabolome signatures but also verified previously known metabolome profiles in the brain. Statistical modeling of the analytical data and validation of the structural assignments discovered metabolic biomarkers associated with the AD pathogenesis. Interestingly, hypotaurin, myo-inositol and oxo-proline levels were markedly elevated in AD while lutamate and N-acetyl-aspartate were decreased in the postmortem brain tissue of AD patients. In addition, neurosteroid level such as cortisol was significantly increased in AD. Together, our data indicate that impaired amino acid metabolism is associated with AD pathogenesis and the altered amino acid signatures can be useful diagnostic biomarkers of AD. Thus, modulation of amino acid metabolism may be a possible therapeutic approach to treat AD.

RESUMO

Large cell carcinoma with a rhabdoid phenotype is very rare. We report the case of a 53-year-old man who presented with multiple nodules on the scalp, face, and trunk. A skin biopsy revealed large cell carcinoma with a rhabdoid phenotype throughout the dermis. Tumor cells had abundant cytoplasm, eccentric nuclei, and prominent nucleoli and did not adhere to each other. Immunohistochemical tests showed positive reactions for vimentin, epithelial membrane antigen (EMA), and thyroid transcription factor-1 (TTF-1) and weakly focal reactions for pan-CK, CK7, and p63. Imaging studies and a percutaneous lung biopsy were performed and the results were consistent with a large cell lung carcinoma with a rhabdoid phenotype. Based on these clinical and histopathological findings, we concluded that his condition was a cutaneous metastasis from a large cell lung carcinoma with a rhabdoid phenotype, which occurs very rarely.

BACKGROUND: Ultrasonography is an important diagnostic tool for many aspects of clinical medicine. It is a non-invasive method of examination that provides high-resolution images in real time. However, some dermatologists may be anxious about the efficacy of ultrasonography. OBJECTIVE: To evaluate the diagnostic accuracy and to assess the utility of diagnostic ultrasonography for subcutaneous benign lesions. METHODS: The study included 96 patients with subcutaneous benign lesions who underwent ultrasonography and then received a pathologic diagnosis after biopsy or surgery at our hospital from July 2010 to December 2012. This study evaluated the number of cases in which the preoperative diagnosis after ultrasonography or just after physical examination agreed to the pathologic diagnosis. RESULTS: Ultrasonography significantly increased the diagnostic yield of subcutaneous benign lesions (after physical examination, 54.2%; after ultrasonography, 84.4%; p<0.001). The sensitivity for the diagnosis of lipoma (after physical examination, 70%; after ultrasonography, 92.5%; p<0.001) and the specificity for the diagnosis of epidermal cyst (after physical examination, 84.1%; after ultrasonography, 94.2%; p<0.001) significantly increased after ultrasonography. The sensitivity for the diagnosis of epidermal cyst and pilomatricoma also tended to increase after ultrasonography. CONCLUSION: The study results suggested that ultrasonography is useful for diagnosis and/or preoperative examination of subcutaneous benign lesions.

RESUMO

DRESS syndrome is an uncommon but potentially life-threatening subset of severe drug eruption characterized by fever, rash, leukocytosis with eosinophilia or atypical lymphocytosis, lymphadenopathy, and liver or renal dysfunction. The aromatic anticonvulsants (phenytoin, phenobarbital, carbamazepine) and sulfonamides are the most common cause of DRESS syndrome. Valproic acid, which is a widely used non-aromatic anticonvulsant, rarely causes DRESS syndrome. We report a case of DRESS syndrome in 47-year-old man caused by the use of valproic acid.

Pyoderma gangrenosum (PG) is a rare and painful skin disease, frequently associated with underlying systemic conditions, such as inflammatory bowel disease or hematological malignancy. PG is generally classified into 4 types: classic (ulcerative), bullous, pustular, and vegetative. Pustular PG is an uncommon variant of PG that the pustules do not progress to form ulcers. We report a case of pustular PG, which was successfully treated with infliximab and topical tacrolimus, in a 28 year-old Korean man with ulcerative colitis.

RESUMO

Syringomas are common benign adnexal tumors, probably of the eccrine origin. They are more common in women and usually found around the eyelids. An exclusively acral site of the lesions is very rare. We present a case of acral syringomas in a 44 year-old woman, involving only the dorsum of the fingers of both hands. It is important to include syringomas in the clinical differential diagnosis of papular lesions of the hands.

Completely neurotized melanocytic nevi are often clinically and histologically indistinguishable from neurofibromas. We report a 23-year-old female who presented with a dark brown plaque with an underlying soft blue subcutaneous nodule in the left infra-axillary area. The skin biopsy revealed markedly increased melanocytic proliferation and nests of nevus cells within dermo-epidermal junction and upper dermis. Circumscribed, non-encapsulated tumor of the dermis was composed of loosely-spaced spindle cells and wavy stroma. Spindle cells in the lower dermis showed staining for S-100, but failed to express glial fibrillary acidic protein and Leu-7. We present an interesting case because the lesion presented as a soft blue subcutaneous nodule suggested an underlying hemangioma or neurofibroma initially, but was shown to be a neurotized melanocytic nevus based on immunohistochemical examination.

PURPOSE: To evaluate the liver parenchyma according to the echo patterns of CSS (compound spatial sonography), and to correlate them with the extent of hepatic fibrosis and the serum aminotransferase level. MATERIALS AND METHODS: The CSS images were classified into the following three echo patterns: type I, a normallooking echo; type II, hyperechoic or hypoechoic nodules scattered in a normal-looking echo; type III, a severely heterogeneous echogenic or hypoechoic honeycomb-like echo. The CSS findings were correlated with the histopathology findings in 63 patients with HBV. The serum aminotransferase levels and the occurrence of acute exacerbation in 168 patients with HBV, with and without a progressed parenchymal echo pattern, and who were followed up more than 1-year period, were compared. The interobserver agreement between the two radiologists for assessing the parenchymal echo pattern was scored. RESULTS: The correlation between the CSS pattern and hepatic fibrosis was statistically significant (correlation coefficient = 0.58, p < 0.05). The baseline serum aminotransferase level was not significantly different between the patients with and without a progressed parenchymal echo pattern. However, the rate of acute exacerbation was higher in patients with a progressed parenchymal echo pattern (p < 0.05). The interobserver agreement was good (k statistic = 0.63, 0.78). CONCLUSION: The liver parenchymal pattern based on the 5-12 MHz CSS is a useful and objective tool for diagnosing and monitoring HBV related chronic liver disease.

Lingual thyroid is a rare clinical entity that is due to the failure of the thyroid gland to descend early in the course of embryogenesis. It may be present with symptoms of dysphagia and upper airway obstruction. We report here on the case of a 63-year-old female who was scheduled for an operation for lumbar disc herniation and she could not be intubated. The cause of the airway obstruction was an ectopic thyroid at the base of the tongue, which made visualization of the glottis impossible. Several attempts at endotracheal intubation were unsuccessful. Fortunately, the patient was mask ventilated. We awakened the patient and consulted an otolaryngologist, and she was diagnosed with lingual thyroid. After a week, she was operated on using spinal anesthesia.

BACKGROUND/AIMS: Cases of small hepatocellular carcinoma (HCC) have been increasing with the progress of diagnostic methods . In this study the screening methods for early diagnosis of HCC were re-evaluated, and comparative therapeutic analyses were perfomed. METHODS: A total of 110 patients with small HCC (< 5 cm and < 4 nodules ) were retrospectively analyzed. The patients were divided into four treatment groups ; unt reated group (No T x, n=12), transarterial-oily-chemoembolization group (TOCE, n=43), combined treatment group of percutaneous ethanol injection and TOCE (CEI, n=22), OP group ( OP, n=33). RESULTS: Small HCC occupied 22.6% of total HCC cases. Only one third of small HCC cases were detected during the regular screening. In this group, Alpha-fet oprotein as say provided a diagnostic clue in 50% of cases, ultras onography in 71%, and the combination of both in 88%. Five year survival rate and 5-year non-recurrence rate in small HCC was 29% and 37% respectively. Comparative therapeutic analys es showed t hat CEI and OP gave a better survival than TOCE in Child grade A. CEI prolonged survival in Child grade B wher eas TOCE did not. Only TOCE was tried and did not improve the survival in Child grade C. CONCLUSION: 1) A more strict screening is needed in high risk group of HCC. 2) As a first line of treatment in small HCC, OP or CEI can be selected in Child grade A, and CEI in Child grade B. In Child grade C, a less invasive treatment (PEIT , microwave coagulat ion therapy) should be investigated.

Renal oncocytoma is an epithelial tumor of the kidney with a particularly good prognosis and is composed entirely of uniform packets of eosinophilic granular cells so-called 'Oncocytic cells' The cytoplasm of the oncocytoma cells is packed with mitochondria and contains few other organelles. At present time 4 cases have been reported in Korean literature without any identifiable angioinvasive lesions. We report a case of renal oncocytoma associated with angioinvasion that was suspected preoperatively and confirmed by pathologic examination including electron microscopy postoperatively. The pathology, diagnosis and treatment of this uncomrnon entity is discussed.
