RESUMO

In this study, a combination of metabolomics and network pharmacology was used to study the pharmacodynamic substances and mechanism of action of Yiyi Fuzi powder (YYFZ) on rheumatoid arthritis (RA) rats. The animal experiments were conducted in accordance with the requirements of the Experimental Animal Ethics Committee of Tianjin University of Traditional Chinese Medicine (approval number: TCM-LAEC2021241). The metabolomic analysis using UPLC-Q-TOF/MS technique identified 22 metabolites, including arachidonic acid, tryptophan, linoleic acid, phenylalanine, as significant biomarkers for the treatment of RA with YYFZ, and they were significantly regressed after YYFZ treatment. The analysis of YYFZ blood components also revealed that 11 blood components, including hypaconitine, benzoylhypaconitine, and deoxyaconitine, may be the components that exert direct pharmacological effects in YYFZ in vivo, and further network pharmacological analysis of blood components obtained that YYFZ may exhibit anti-inflammatory effects through acting on PI3K/Akt signaling pathway, estrogen signaling pathway, vascular endothelial growth factor (VEGF) signaling pathway. The results of this study provide implications for the clinical application of YYFZ.

RESUMO

Long non-coding RNA (lncRNA) is not "transcriptional noise". It can regulate gene expression at pre-transcriptional, post-transcriptional and epigenetic level and participate in the occurrence and development of diseases. A large number of studies have shown that the abnormal expression of lncRNA plays an important role in the occurrence and development of acute myeloid leukemia (AML) and drug resistance. LncRNA can participate in the occurrence, development and drug resistance of AML by acting on target genes and regulating related signal pathways. Detection of its expression has a certain prognostic value. Therefore, this article briefly discusses the research progress of lncRNA in AML, hoping to provide ideas for clinical diagnosis and targeted therapy.

However, choices of fluid resuscitation strategies and fluid input volumes remain a thorny problem. Our study aimed to elucidate the relationship between fluid balance and prognosis of ARDS patients secondary to sepsis. METHODS: Our study included 322 sepsis patients from Ruijin Hospital between 2014 and 2018, and 84 patients were diagnosed as ARDS within 72 hours after onset of sepsis according to Berlin ARDS Definition. RESULTS: Among the 322 sepsis patients, 84 (26.1%) were complicated with ARDS within 72 hours. ARDS patients had a lower oxygenation index (PaO2/FiO2 166.4±71.0 vs. 255.0±91.2, P<0.05), longer duration of mechanical ventilation (11 [6-24] days vs. 0 [0-0] days, P<0.05) than those without ARDS. Sepsis patients with ARDS showed daily positive net fluid balance during seven days compared with those without ARDS who showed daily negative net fluid balance since the second day with significant statistical differences. Among the 84 sepsis patients with ARDS, 58 (69.0%) died. Mean daily fluid input volumes were much lower in survivors than in non-survivors (43.2±16.7 mL/kg vs. 51.0±25.2 mL/kg, P<0.05) while output volumes were much higher in survivors (45.2±19.8 mL/kg vs. 40.2±22.7 mL/kg, P<0.05). Using binary logistic regression analysis, we found that the mean daily fluid balance was independently associated with mortality of sepsis patients complicating with ARDS (P<0.05). CONCLUSIONS: Early negative fluid balance is independently associated with a better prognosis of sepsis patients complicated with ARDS.

Cervical spondylotic myelopathy is the most serious subtype of cervical spondylosis and the most common cause of spinal cord injury. At present, it is considered that mechanical compression and ischemic changes caused by spinal stenosis constitute the pathophysiological basis of spinal stenosis, and the dynamic instability of cervical spine is also an important factor causing spinal cord injury. Its clinical manifestations and physical examinations are complex and varied, and often need to be differentiated from some geriatric diseases. In addition to the abnormal long bundle sign caused by upper motor neuron injury, about 51.9% of cervical spondylotic myelopathy patients also have root lesions. Magnetic resonance imaging is the preferred method of examination in patients with suspected cervical spondylotic myelopathy. Compared with magnetic resonance imaging, diffusion tensor imaging is much more sensitive in detecting early patients. It is suggested that, patients with moderate to severe cervical spondylotic myelopathy should receive surgery, and patients with mild cervical spondylotic myelopathy should consider conservative treatment within three years from the beginning of diagnosis.

In recent years,the development and application of classical famous prescriptions have attracted much attention. However,the differences between ancient and modern conditions lead to difficulties in carrying out practical work. In this paper,with Houpu Wenzhong Decoction as an example,the key technologies of boiling granularity,water addition,boiling time and sample pretreatment methods were investigated on the basis of sufficient literature research. The experimental results showed that there was no significant difference in the concentration of index components between those with different granularity( 2 mm and 3-5 mm) and different decocting time( 30 min and 60 min),but the extraction rate of index components was relatively high when the granularity of powder was 2 mm and decocting time was 30 min. With the increase of water content,the concentration of index components and the extraction rate were increased in varying degrees. A certain proportion of methanol aqueous solution was used as the resolvent before content determination of the reference sample of Houpu Wenzhong Decoction,which could take into account both the spectral information of water-soluble components and fat-soluble components in the prescription,and help to display the overall information of the prescription' s chemical components more comprehensively. At the same time,the boiling and dispersing classical prescriptions in the Catalogue of Ancient Classical Prescriptions( the first batch) were collected and summarized in this study; the key influencing factors of decocting process were analyzed from different angles,and preliminary research suggestions were put forward,so as to provide a certain direction and reference for the establishment of quality standard of Houpu Wenzhong Decoction,as well as for the development,research and clinical application of boiling and dispersing classical prescriptions.

Objective Xuanwei, Yunnan, is one of the areas with the highest incidence of lung cancer in China and in the world as well. This study was to explore the differential expressions of lncRNAs and mRNAs, establish a competing endogenous RNA (ceRNA) coexpression network, and find some new ideas for the diagnosis and treatment of lung cancer in the Xuanwei area.Methods Using the gene chip of the lncRNA + mRNA expression profile, we examined 10 pairs of samples of lung cancer and adjacent normal tissues from the patients in the Xuanwei area and analyzed differentially expressed mRNAs with such bioinformatics Methods as gene ontology (GO), Kyoto Encyclopedia Genomes (KEGG) pathway and network analyses.Results By comparison between the lung cancer and adjacent normal tissues, 384 differentially expressed lncRNAs were identified (fold change≥2.0, P<0.05), in which 78 were upregulated, with abParts as the most differentially expressed (fold change=7.522, and 306 downregulated, with XLOG-012046 as the most differentially expressed (fold change=9.001). Among the 946 differentially expressed mRNAs identified (fold change≥2.0, P<0.05), SPP1 was the most significantly upregulated (fold change=16.500) and AGER was the most significantly downregulated (fold change=14.377). Both up- and downregulated expressions of lncRNA and mRNA were found in the lung cancer tissue of the patients, the upregulated expression of mRNA involved mainly the calcium ion signaling pathway, phagosomes and hematopoietic cell lineage, while the downregulated expression of mRNA chiefly involved in the hematopoietic cell lineage and calcium ion signaling pathway. lncRNAs were correlated with mRNAs in a one-to-one or one-to-many manner, with the coexpression of p14245 (LINC00472) with Q9H8W2 and XLOG-005764. The expressions of SEMA5A and SEMA6A regulated by ENSG00000244513.2 (ceRNA score=0.461) and ENSG00000257424.1 (ceRNA score=0.372) were both decreased in the mRNA expression profile. SEMA5A participated mainly in cell adhesion, bioadhesion, and vasculature development, while SEMA6A chiefly in cell-surface receptor signal transduction, cell movement, and cytoskeleton.Conclusion The expression profile of differentially expressed lncRNAs was identified by microarray in the lung cancer tissue of the patients in Xuanwei, which may become a novel biomarker or therapeutic target of lung cancer in this area.

Proper physical activities may improve strength and endurance, and improve flexibility and coordination, which contrib-ute to improving the quality of life and independence in patients with post-polio syndrome (PPS). Patients should exer-cise in a controlled and sensible manner, avoiding intensive training and excessive muscle use, for fear that exacerbated symptoms and further loss of muscle strength appear. Exercises should not be performed until the patients have been checked and evaluated by experts, followed closely and with certain conditions. The exercise intensity should be adjust-ed and not exceed the patient's fatigue threshold. Individualized exercise prescription should be carried out after disease, surgery or injury.

Falls and fall-related injuries are common and important issues among the patients with poliomyelitis sequelae and post-polio syndrome(PPS).They have the same risk factors of falls as general elderly;in addition,fatigue,muscle and joint pain caused by poliomyelitis sequelae and PPS,and the inability to tolerate the cold can also lead to falls.Falls of-ten result in physical injury,further disability,fear of falling,and then avoidance of activity.In order to reduce falls of the patients with poliomyelitis sequela and PPS,we should evaluate the fall carefully,correct and eliminate the patient's misconceptions,analyze the cause and the mechanism,and reduce the risk factors,give the multi-disciplinary compre-hensive management plan and intervention measures,in order to maintain a healthy lifestyle,and prevent falls and avoid further disability.

New onset of fatigue,weakness,muscle and joint pain occurs in patients with post-polio syndrome(PPS).These symp-toms will lead patients to action and activities difficulty,such as functional limitations,which are serious,and needed to deal with,in order to avoid the emergence of injury.Adequate intensity of aerobic exercise can help the patient maintain the body's activity level,but should be individualized to avoid the burden of muscles and joints.Scientific weight con-trol is also a problem that patients with PPS should pay attention to.The PPS specialized clinic can comprehensively as-sess the patient's health status,functional limitation,and give specific recommendations for treatment,exercise,diet,and orthosis.

<p><b>OBJECTIVE</b>To evaluate the efficacy and safety of the magnesium isoglycyrrhizinate plus nucleoside analogues (MGL + NA) combination therapy in patients with chronic hepatitis B using a meta-analysis approach.</p><p><b>METHODS</b>The Chinese Biochemical literature on Disc (CBMDisc) and the Chinese Scientific Journal database, CNKI, were searched for randomized controlled trials (RCTs) of MGL+NA in patients with chronic hepatitis B published between 1995 and 2013. Data related to treatment type (combination therapy vs. mono-therapy) and outcome (markers of efficacy and safety, including levels of hepatitis B virus (HBV) DNA, hepatitis B e antigen (HBeAg), alanine aminotransferase (ALT) and aspartate aminotransferase (AST)). Weighted mean differences (WMD) were calculated and the Peto method was used to determine the relative risk (RR), both with 95% confidence intervals (CIs).</p><p><b>RESULTS</b>Meta-analysis of the six included RCTs of MGL + NA, representing a 704 patients with chronic hepatitis B, showed WMDs for ALT of -12.98 (95% CI: -18.24 to -7.71, P less than 0.01) and for AST of -9.49 (95% CI: -14.53 to -4.45, P = 0.0002) and RRs for HBeAg of 1.79 (95% CI: 1.17 to 2.76, P = 0.008) and for HBV DNA of 1.35 (95% CI: 1.05 to 1.74, P = 0.02). The therapeutic efficacy of MGL+NA combination therapy was better than that of NA mono-therapy (P less than 0.01).</p><p><b>CONCLUSION</b>For patients with chronic hepatitis B, MGL combination therapy may enhance the antiviral efficacy of NA treatment and help to improve liver function during treatment.</p>

Nowadays, there is a trend of HIV prevalence transmitting from high-risk group to average-risk group in China. Rural China is the weak link of HIV prevention, and rural areas of Henan province which is one of the most high-risk regions in China have more than 60% of the AIDS patients in the province. Thus, improving the HIV awareness and implementing health education become the top-priority of HIV/AIDS control and prevention. A multistage sampling was designed to draw 1129 people living with HIV/AIDS (PLWHAs) and 1168 non-PLWHAs in 4 prevalence counties of Henan province. A health promoting and social-psychological support model was constructed to improve the health knowledge of participants. Chi-square tests and unconditional logistic regression were performed to determine the intervention effect and influencing factors. All groups had misunderstandings towards the basic medical knowledge and the AIDS transmission mode. Before the intervention, 59.3% of the HIV/AIDS patients and 74.6% of the healthy people had negative attitudes towards the disease. There was statistically significant difference in the improvement of knowledge, attitude and action with regards to HIV prevention before and after intervention (P<0.05). PLWHAs who were males (OR=1.731) and had higher education level (OR=1.910) were found to have better HIV/AIDS health knowledge, whereas older PLWHAs (OR=0.961) were less likely to have better HIV/AIDS health knowledge. However, the intervention effect was associated with the expertise of doctors and supervisors, the content and methods of education, and participants' education level. It was concluded that health education of HIV/AIDS which positively influences the awareness and attitude of HIV prevention is popular in rural areas, therefore, a systematic and long-term program of HIV control and prevention is urgently needed in rural areas.

<p><b>OBJECTIVE</b>To investigate the effect of sea cucumber cerebroside(SCC) and its long-chain base(LCB) on lipid and glucose metabolism in obese mice.</p><p><b>METHODS</b>The mouse obese model was established by feeding high fat diet. The mice were randomly assigned to 4 groups: control group, model group, SCC group and LCB group. After 4 weeks, the glucose tolerance test was undertaken. After 5 weeks, the body fat content, organic indexes, serum lipid level, glycemic index and liver lipid level were determined.</p><p><b>RESULTS</b>Compared with the model group, the glucose tolerance in the SCC group and LCB group was ameliorated significantly (P<0.01, P<0.05); glycemic index (P<0.01, P<0.01), the weight of adipose tissue (P<0.05, P<0.01) and the hepatic TG were reduced significantly (P<0.05, P<0.05).</p><p><b>CONCLUSION</b>Sea cucumber cerebroside and its long-chain base can improve the glucose and lipid metabolism in obese mice.</p>

<p><b>OBJECTIVES</b>To study the evolution of HBV quasispecies under the pressures of lamivudine (LAM) - entecavir (ETV) sequential therapy and its clinical significance.</p><p><b>METHODS</b>Consecutive serum samples from 2 patients underwent LAM-ETV sequential therapy were extensively studied for HBV quasispecies composition and evolution, using PCR-cloning-sequencing method. Maximum likelihood trees were built to analyze the genetic relationship between representative sequences. Correlation between HBV quasispecies evolution and serological/virological data was analyzed to determined the clinical significance of the evolution of HBV quasispecies during prolonged nucleotide analog therapy.</p><p><b>RESULTS</b>Virological breakthrough was observed in both patients. Patient I acquired sustained virological response after switching to ETV rescue therapy, whereas Patient II suffered from virological breakthrough after 72 weeks of ETV therapy. Each virological breakthrough was accompanied with the replacement of previous drug susceptible dominant quasispecies with a drug resistant variant, indicating a close correlation between quasispecies composition and drug susceptibility. The rtL180M+S202G+M204V triple mutant, which was most likely a descendant of the LAM resistant rtL180M+M204V variant, was closely correlated with ETV resistant in Patient II.</p><p><b>CONCLUSION</b>Quasispecies composition of HBV is closely correlated with nucleotide analog susceptibility. ETV resistant variant can emerge from a LAM resistant viral population. Dynamic monitoring of HBV quasispecies composition is of great importance during nucleotide analog therapy.</p>

<p><b>OBJECTIVE</b>To investigate the feasibility of repairing the wounds in the distal feet with dorsal metatarsal artery distal perforator pedicle link-pattern flaps.</p><p><b>METHODS</b>Since January of 2004 to April of 2009, 30 patients with distal wounds in the feet underwent surgical wound repair using by dorsal metatarsal arteries distal perforator pedicle link-pattern flaps. Seventeen patients used the 1, 2 dorsal metatarsal artery distal perforator flaps, and 13 had the 3, 4 dorsal metatarsal artery distal perforator flaps, with the flaps measuring 4 cmx4 cm to 8 cmx7cm and the flap pedicle ranged 3-6 cm in length. The donor sites were repaired with skin grafting.</p><p><b>RESULTS</b>Twenty-nine flaps survived completely and 1 flap showed partial necrosis in the distant part (2.0 cmx1.0 cm). Twenty-one patients were followed up for an average of 11 months, during which the color, texture, and contour of the flaps remained normal without ulcers in the donor sites or the flaps.</p><p><b>CONCLUSION</b>The wounds in the distal feet can be repaired by dorsal metatarsal artery distal perforator pedicle link-pattern flaps, which is a simple, applicable and safe procedure.</p>

<p><b>OBJECTIVE</b>To investigate the effect of sea cucumber cerebroside (SCC) on the lipid metabolism in rats with orotic acid-induced fatty liver.</p><p><b>METHODS</b>The non-alcoholic fatty liver disease (NAFLD) model was established by adding orotic acid to the diets in rats. The rats were randomly assigned to four groups:control group, NAFLD group, NAFLD + low SCC group and NAFLD + high SCC group. After 10 days of feeding, the serum and hepatic lipid concentrations and the aminopherase activities were measured; the composition of hepatic fatty acids was also analyzed.</p><p><b>RESULT</b>The serum TC and TG levels reduced significantly in the NAFLD group as compared with the controls (P<0.05), while the sea cucumber cereborside feeding raised the serum lipid concentrations (P<0.05). The hepatic TC and TG levels dramatically increased in the NAFLD group in comparison with the controls (P<0.05, P<0.01), while the hepatic lipid accumulations decreased in both SCC groups (P<0.05, P<0.01). The ALT and AST activities in the NAFLD group increased markedly when compared with the controls (P<0.05, P <0.01), while the sea cucumber cerebroside feeding attenuated the hepatic injury levels (P<0.05, P<0.01). Compared with the control group, the stearoyl-CoA desaturase (SCD) activity increased significantly in the NAFLD group (P<0.05), but decreased in SCC groups (P<0.05).</p><p><b>CONCLUSION</b>Sea cucumber cerebroside can attenuate the rat fatty liver induced by orotic acid.</p>

RESUMO

<p><b>OBJECTIVE</b>To investigate the effect of reverse saphenous nerve neurocutaneous flaps for skin defects at forepart of feet.</p><p><b>METHODS</b>From January 2004 to October 2008,15 cases of skin defects at forepart of feet were repaired with reverse saphenous nerve neurocutaneous flaps. The flap size ranged from 3.5 cm x 3.0 cm to 8 cm x 5 cm. The wounds at donor site were closed with skin graft.</p><p><b>RESULTS</b>All the flaps survived completely with no ulcer at the donor site. 10 patients were followed up for 1 to approximately 9 months. The skin color and texture were satisfactory. The patients could walk very well.</p><p><b>CONCLUSIONS</b>It is reliable to repair the skin defects at forepart of feet with reverse saphenous nerve neurocutaneous flaps. It is easily performed with less morbidity.</p>

RESUMO

<p><b>OBJECTIVE</b>To investigate the feasibility of fibular artery perforator link-pattern flaps at lateral and posterior part of legs.</p><p><b>METHODS</b>From January 2007 to March 2009, 12 cases of with feet and ankle wounds were treat with fibular artery perforator link-pattern flaps at lateral and posterior part of legs. The flap size ranged from 10 cm x 10 cm to 25 cm x 13 cm. The wounds at the donor sites were closed with skin grafts.</p><p><b>RESULTS</b>All the flaps survived completely. The patients were followed up for 1-12 months (median, 6 months). The color, texture and appearance of the flaps were good.</p><p><b>CONCLUSION</b>The link-pattern flap has reliable blood supply and a large area for repairing defects. The sural nerve is reserved, resulting minimal morbidity to donor site.</p>

<p><b>OBJECTIVE</b>To observe the inhibitory effect of a replication-defective hepatitis B virus (HBV) vector plasmid expressing A3C on HBV replication in vitro.</p><p><b>METHODS</b>The HBV vector plasmisd pCH-LJ3-A3C and pCH-LJ3-hrGFP expressing A3C and hrGFP were constructed using PCR and gene recombination technique. The two recombinant plasmids were separately cotranfected into HepG2 cells along with the wild-type HBV plasmid pCH-3093. The HBV DNA in the cell cytoplasmic lysates and in the cell culture supernatant was extracted for Southern blotting, and the nucleocapsid-associated HBV DNA were amplified by PCR, cloned and sequenced.</p><p><b>RESULTS</b>pCH-LJ3-A3C showed obvious inhibitory effect on HBV DNA in the cytoplasmic lysates and cell culture supernatant, causing a reduction of the HBV DNA by 31% and 40%, respectively. The pCH-LJ3-A3C plasmid was capable of editing the HBV DNA. Among the 50 sequenced clones, 36 clones had G-A mutations, with a total of 982 such mutations.</p><p><b>CONCLUSION</b>pCH-LJ3-A3C can inhibit the replication of HBV primarily by editing HBV DNA. The pCH-LJ3-A3C plasmid may serve as a new antiviral agent against human HBV infection.</p>

<p><b>OBJECTIVE</b>To evaluate the virological, serological and biochemical outcomes of 3 years of entecavir (ETV) treatment in nucleoside-naive chronic hepatitis B patients.</p><p><b>METHODS</b>This study was divided into two stages: Patients receiving either ETV 0.5 mg/d (n = 258) or lamivudine (LAM) 100 mg/d (n = 261) entered the initial 96-week randomized, double blind, controlled efficacy study. Patients not achieving a consolidated response (HBV DNA less than 0.7 MEq/ml, ALT less than 1.25 times*ULN, and if HBeAg-positive at baseline, loss of HBeAg for >or= 24 weeks), or those experienced viral breakthrough or relapse, entered a 48-week entecavir rollover study.</p><p><b>RESULTS</b>96 weeks after the treatment, 79% of ETV treated and 46% of LAM treated patients had HBV DNA less than 300 copies/ml (P < 0.0001), 96% of ETV treated and 92% of LAM treated patients had normalized ALT (P = 0.06). 21% of ETV treated and 23% of LAM treated patients achieved HBeAg seroconversion. Among the 160 patients received continuous ETV for 144 weeks, 89% had undetectable serum HBV DNA, 86% showed ALT normalization, and 27% achieved HBeAg seroconversion. ETV resistance was rare: only 3 patients showed ETV resistance 96 weeks after the treatment, and additional 2 patients developed ETV resistance during the following 48 weeks, genotyping indicated the ETV resistance was caused by gene mutation. Adverse event rates in ETV-treated patients were similar to those in LAM-treated patients, but fewer ALT flares were observed in ETV-treated patients.</p><p><b>CONCLUSIONS</b>This study demonstrates that ETV treatment results in long-term HBV suppression and ALT normalization in Chinese CHB patients, and is associated with low rate of drug resistance.</p>
